Efficacy and safety of endoscopic papillary balloon dilation for the removal of bile duct stones: Data from a “real-life” multicenter study on Dilation-Assisted Stone Extraction

AIM To report data on Dilation-Assisted Stone Extraction(DASE) use in clinical practice and its efficacy and safety trough three Italian referral centers for biliopancreatic diseases treatment. METHODS From January 2011 to December 2015 we collected data on 120 patients treated with DASE. Technical success was obtained when the endoscopist was able to place the balloon trough the papilla inflating the balloon until the final diameter for an adequate time(at least 30 s). Clinical success was obtained after complete stone removal(no remaining stones were visible at the cholangiogram).RESULTS Forty-nine male(40.8%) and 71 female(59%) were enrolled. The mean age was 67.8 years ± 15.7. The mean common bile duct(CBD) dilation was 19.2 mm± 3.9 and the mean size of stones 15.8 ± 2.9. DASE was applied as first approach in 38%(62% after initial failure of stones extraction). Technical and clinical success was of 91% and 87% respectively. In those in which DASE failed alternative treatment were adopted. After DASE 18% of patients experienced a complication(bleeding 9%, pancreatitis 8%, perforation 0.8%). At univariable analysis, elective endoscopic retrograde cholangiopancreatography(P = 0.031), DASE as first approach(P = 0.032), and cannulation of major papilla followed by guidewire insertion(P = 0.004) were related to low risk of complications. Pre-cut was related to an increased risk of complications(P = 0.01). CONCLUSION DASE allowed a higher first-session success rate and can be consider a valid alternative to endoscopic sphincterotomy not only for bigger CBD stones.

Structural health monitoring of bridges in the State of Connecticut

A joint effort between the Connecticut Department of Transportation and the University of Connecticut has been underway for more than 20 years to utilize various structural monitoring approaches to assess different bridges in Connecticut. This has been done to determine the performance of existing bridges, refine techniques needed to evaluate different bridge components, and develop approaches that can be used to provide a continuous status of a bridge＇s structural integrity. This paper briefly introduces the background of these studies, with emphasis on recent research and the development of structural health monitoring concepts. This paper presents the results from three different bridge types： a post-tensioned curved concrete box girder bridge, a curved steel box-girder bridge, and a steel multi-girder bridge. The structural health monitoring approaches to be discussed have been successfully tested using field data collected during multi-year monitoring periods, and are based on vibrations, rotations and strains. The goal has been to develop cost-effective strategies to provide critical information needed to manage the State of Connecticut＇s bridge infrastructure.

基于气象数据,采用Theil-Sen方法和改良的Mann-Kendall非参数趋势检测法,对1952—2012年长江流域地区气温和降水的时空变化特征进行分析。改良后的方法将数据中所有显著的自相关结构纳入考虑范围,以此来消除水文时间序列的自相关影响。结果表明:长江流域整体增温趋势明显,年降水量减少趋势并不明显,而气温与降水的变化趋势均表现出显著的季节性和地域性特征。这一现象可以归因于长江流域广阔的地域、复杂的地形和天气条件相互作用,共同影响着气温和降水的变化模式。此外,地表植被和地表水资源对气温和降水的变化也有相应的响应特征,具体的响应模式值得进一步的阶段性和相关性分析。

Basic Earth＇s Parameters as estimated from VLBI observations

The global Very Long Baseline Interferometry observation for measuring the Earth rotation＇s parameters was launched around 1970 s. Since then the precision of the measurements is continuously improving by taking into account various instrumental and environmental effects. The MHB2000 nutation model was introduced in 2002, which is constructed based on a revised nutation series derived from 20 years VLBI observations（1980-1999）. In this work, we firstly estimated the amplitudes of all nutation terms from the IERS-EOP-C04 VLBI global solutions w.r.t. IAU1980, then we further inferred the BEPs（Basic Earth＇s Parameters） by fitting the major nutation terms. Meanwhile, the BEPs were obtained from the same nutation time series using a BI（Bayesian Inversion）. The corrections to the precession rate and the estimated BEPs are in an agreement, independent of which methods have been applied.

Oleuropein-Specific-β-Glucosidase Activity Marks the Early Response of Olive Fruits (Olea europaea) to Mimed Insect Attack

Olive fruits are seriously deteriorated by pre and postharvest damage due to the attack of insects, such as Bactrocera olaea, which strongly alters the quality of olives. Defence response in olive fruits injured both by pathogens and by mechanical damages has been associated with the enzyme β-glucosidase, which specifically hydrolyses oleuropein, producing highly reactive aldehyde molecules. In situ detection of ^-glucosidase activity in olive fruit tissues following injury, which simulates Bactrocera oleae punctures, is reported. The assay was performed in two cultivars showing different degrees of susceptibilities to fly infestation. In both cultivars, the histochemical assay for β-glucosidase showed that within 20 min after the injury, a strong ^-glucosidase activity could be observed in the damaged tissues. Thereafter a progressive enzyme inactivation occurred starting from tissues around the boundary of the injury with decrease of the enzyme activity and stopped after 3 h. Whereas the mass of active cells reached a distance of （300±50） μm from the edge of the injury. Biochemical analyses showed that in extracts of the injured fruit, β-glucosidase activity rapidly increased within 20 min from injury, thereafter decreasing and reaching values comparable with those in intact fruits. Following puncture, the oleuropein contents did not change significantly in the high susceptibility cultivar, whereas it rapidly decreased in the cultivar showing low susceptibility. The results strongly suggest that olive fruits susceptible towards fly infestation could be related to the ability of the oleuropein-degrading-β-glucosidase to produce the highly reactive molecules in the damaged tissues. As a consequence of puncture, high level of peroxidase activity was detected. This feature also suggested that this enzyme could play a key role in the defence response against insect injuries.

Protocol liver biopsies in long-term management of patients transplanted for hepatitis B-related liver disease

AIM：To evaluate the long-term histological outcome of patients transplanted for HBV-related liver disease and given HBIg prophylaxis indefinitely after LT. METHODS： Forty-two consecutive patients transplanted for hepatitis B were prospectively studied. HBsAg, HBVDNA and liver function tests were evaluated in the serum 3, 6 and 12 mo after LT and then yearly. LB was obtained 6 and 12 mo after LT and yearly thereafter. Chronic hepatitis （CH） B after LT was classified as minimal, mild, moderate or severe. RESULTS： HBV recurred in 7/42 （16.6%） patients after 6-96 mo of follow-up. A hundred and eightyseven LB were evaluated. Four of 7 patients with graft reinfection, all with unknown HBV DNA status before LT, developed cirrhosis at 12-36 mo of follow-up. Of the 122 LB obtained from 28 HBsAg＋/HCV- recipients with no HBV recurrence after LT, all biopsies were completely normal in only 2 patients （7.1%）, minimal/non-specific changes were observed in 18 （64.2%）, and at least 1 biopsy showed CH in the remaining 8 （28.5%）. Twentynine LB obtained from 7 patients transplanted for HBVHCV cirrhosis and remaining HBsAg- after LT revealed recurrent CH-C. Actuarial survival was similar in patients with HBsAg＋ or HBsAg- liver ***： Though protocol biopsies may enable the detection of graft dysfunction at an early stage, the risk of progression and the clinical significance of these findings remains to be determined.

Relational Capital and Open Innovation： Two Cases of Successful Italian Companies

Praxis and doctrine agree on the importance of innovation for the competitiveness and profitability of the firm. Innovation is considered as a leverage to start a virtuous circle that improves company performance and, consequently, reduce financial constraints for its development. Being creative is definitely the first mechanism that leads to innovation. Creativity spreads from both the individuals that work in the firm and from the social processes and contextual factors that characterize individual relationships within the company. Relational capital is the second mechanism that brings innovation. Relationships with clients, suppliers, competitors, producers of complementary products, public and private research institutions provide creative elements that, once screened, give rise to innovation. The first mechanism requires the firm to be creative on its own as innovations come directly from within the organization （＂closed innovation＂ model）. In the second, the firm must get in touch with potentially innovative idea~ coming from outside （＂open innovation＂ model）. Doctrine and praxis point out that firms, which do not stop at the first mechanism of innovation, turn out to be more innovative. The paper is structured as follows. In the first part, a theoretical framework is provided, describing the main findings, in the literature, in terms of relationships among innovation, creativity and relational capital. The second part is based on the analysis of two case studies. The first case （Coswell）, the very different degree of success （in terms of generated income） of its products seems to be tightly related on how much ＂relational capital＂ is ＂inside＂ the innovation. In the second case （Pastificio Rana） shows that innovation resulting from relational capital allows the company to excel against its far bigger and financially superior competitors.

Voltage-controlled reverse filament growth boosts resistive switching memory

Nonvolatile memory devices based on filamentary resistance switching （KS） are among the frontrunners to fuel future devices and sensors of the internet of things （IoT） era. The capability of many two distinctive resistive states in response to an external electrical stimulus has been demonstrated. Through years of selection, cells based on the drift of metal ions, namely conductive-bridge memory devices, have shown a wide range of applications with nanosecond switching speeds, nanometer scalability, high-density, and low power-consumption. However, for low （sub-10-~A） current operation, a critical challenge is still represented by programming variability and by the stability of the conductive filament over time. Here, by introducing the concept of reverse filament growth （RFG）, we managed to control the structural reconfiguration of the conductive filament inside a memory cell with significant enhancements of each of the aforementioned properties. A first-in-class Cu-based switching device is demonstrated, with a dedicated stack that enabled us to systematically trigger RFG, thus tuning the device＇s properties. Along with nanosecond switching speeds, we achieved an endurance of up to 106 cycles with a 102 read window, with outstanding disturb immunity and optimal stability of the filament over time. Furthermore, by tuning the filament＇s shape, an excellent control of multi-level bit operations was achieved. Thus, this device offers high flexibility in memory applications.

Dear editor,A new infectious disease outbreak,caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),is now spreading all around the world.[1]One of the most critical understandings of the current pandemic of the coronavirus disease 2019(COVID-19)focuses on an imbalanced coagulation *** on the current evidence,we can divide the COVID-19-related hypercoagulable state into two categories[2]:patients without other pre-existing indications for anticoagulant therapy who develop a state of hypercoagulability linked to SARS-CoV-2 infection especially during the cytokine storm phase,and patients already on anticoagulation in whom we have to reevaluate their drug choices,dosages,and *** the same time,recent literature recommends the use of heparin in severe SARS-CoV-2 infection,both as thromboembolic prophylaxis and as an anticoagulant therapy,in light of possible anti-infl ammatory and antiviral mechanisms and less pharmacological interferences.[3]This indication should be adopted in all patients except for those with prosthetic heart valves,in whom vitamin K antagonists continue to remain the drug of choice.[4]In this subgroup of patients,strict monitoring of PT-INR is required to maintain the therapeutic *** purpose of this article is to underline the need for good quality evidence regarding the management of this high-risk category of the patient population.
