AIM:To evaluate the effectiveness of a short-type single-balloon-enteroscope(SBE) for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a reconstructed ***:Short-type SBE was developed to perform ERCP in postoperative patients with a reconstructed ***-type SBE is a direct-viewing endoscope with the following specifications:working length,1520 mm;total length,1840 mm;channel diameter,3.2 *** addition,short-type SBE has a water-jet *** study group comprised 22 patients who underwent 31 sessions of short-type SBE-assisted ERCP from June 2011 through May *** was performed by Billroth-Ⅱ(B-Ⅱ) gastrectomy in 6 patients(8 sessions),Roux-en-Y(R-Y) gastrectomy in 14 patients(21 sessions),and R-Y hepaticojejunostomy in 2 patients(2 sessions).We retrospectively studied the rate of reaching the blind end(papilla of Vater or choledochojejunal anastomosis),mean time required to reach the blind end,diagnostic success rate(defined as the rate of successfully imaging the bile and pancreatic ducts),therapeutic success rate(defined as the rate of successfully completing endoscopic treatment),mean procedure time,and ***:Among the 31 sessions of ERCP,the rate of reaching the blind end was 88% in B-Ⅱ gastrectomy,91% in R-Y gastrectomy,and 100% in R-Y *** mean time required to reach the papilla was 18.3 min in B-Ⅱ gastrectomy,21.1 min in R-Y gastrectomy,and 32.5 min in R-Y *** diagnostic success rates in all patients and those with an intact papilla were respectively 86% and 86% in B-Ⅱ gastrectomy,90% and 87% in R-Y gastrectomy,and 100% in R-Y *** therapeutic success rates in all patients and those with an intact papilla were respectively 100% and 100% in B-Ⅱ gastrectomy,94% and 92% in R-Y gastrectomy,and 100% in R-Y *** the channel diameter was 3.2 mm,stone extraction could be performed with a wire-guided basket in 12

Organic agriculture is increasingly used as an alternative to conventional agriculture due to its positive impact on the health of ecosystems and agroecosystems. However, the outcome of organic agriculture in terms of the production of various crops remains uncertain due to the influence of many variables, rising questions about its advantages over conventional agriculture. This study assessed the impacts of organic agricultural system on arbuscular mycorrhizal (AM) fungi diversity in soil and green tomato (Physalis ixocarpa Brot. ex Horn) crop production. A field experiment was conducted using a random block design with five repetitions of the following treatments: a) Control (no fertilization, NF);b) Vermicompost use (OTV);c) OTV with vermicompost leaching (OTH);and d) Inorganic fertilization (CST). Throughout the crop cycle, soil samples were analyzed chemically, the relative growth rate (RGR) of the plants was measured, and the colonization and diversity of AM fungi were quantified in roots and soil;finally, above-ground, root biomass, and fruit production were measured. Organic fertilization (OTV, OTH) increased (p p > 0.05). It is concluded that organic agriculture management is essential to promote a greater AM fungi diversity and fungi root colonization. Plant-AM fungi interaction increases growth rates and it allows a similar tomato production compared with conventional agriculture.

Olmesartan for non-alcoholic steatohepatitis complicated with hypertension: An open-label study

Aim: We evaluated the long-term effects of olmesartan, an angiotensin type 1 receptor blocker, in patients with non-alcoholic steatohepatitis (NASH) complicated with hypertension. Methods: All patients were given a standard calorie diet and exercise counseling more than 3 months before the treatment. Seven patients with NASH received olmesartan treatment for 1 year. Liver biopsy, clinical parameters and blood markers of hepatic fibrosis, including serum hyaluronic acid, type IV collagen, and procollagen III N-terminal propeptide levels, were also examined at the beginning and the end of the study. Results: The median dose of the final administration was 20 mg (range, 10 - 40 mg). Olmesartan reduced MAP by –11.3 ± 13.0% (P = 0.046) after 1 year. In the laboratory data, serum AST, ALT, and ferritin significantly decreased after a year of administration (AST, 62 ± 24 vs. 39 ± 20 IU/L, P = 0.018;ALT, 106 ± 79 vs. 55 ± 35 IU/L, P = 0.043;ferritin, 323.8 ± 252.8 vs. 202.3 ± 194.1 ng/ml, P = 0.028). Furthermore, fasting glucose significantly decreased. However, transforming growth factor-beta1, the serum concentration of the fibrosis markers, and all histological features were unchanged at the end of the study. No side effects of the treatment were noted at any time during the study. Conclusion: Olmesartan significantly reduced blood pressure, fasting glucose, aminotransferase, and serum ferritin but could not suppress the hepatic fibrosis markers or histological features after 1 year. Therefore, olmesartan is advisable only for its anti-inflammatory effect in patients with NASH-complicated hypertension.

AIM:To study the relationship between adverse events(AEs),efficacy,and nursing intervention for sorafenibtherapy in patients with hepatocellular carcinoma(HCC).METHODS:We enrolled 37 consecutive patients withadvanced HCC who received sorafenib *** among baseline characteristics as well as AEoccurrence and tumor response,overall survival(OS),and treatment duration were *** nursingintervention program consisted of education regardingself-monitoring and AEs management,and telephoneRESULTS:A total of 37 patients were enrolled in the study,comprising 30 males(81%) with a median age of 71 *** disease control rate at 3 mo was 41%,and the median OS and treatment duration were 259 and 108 d,*** intervention was given to 24 patients(65%).Every patient exhibited some kinds of AEs,but no patients experienced G4 *** observed AEs > G2 included anorexia(57%),skin toxicity(57%),and fatigue(54%).Factors significantly associated with longer OS in multivariate analysis demonstrated that age ≤ 70 years,presence of > G2 skin toxicity,and absence of > G2 *** disease control rate in patients with > G2 skin toxicity was 13/20(65%),which was significantly higher compared with that in patients with no or G1 skin *** analysis revealed that nursing intervention and > G2 skin toxicity were independent significant predictors for longer treatment ***:Skin toxicity was associated with favorable outcomes with sorafenib therapy for advanced *** intervention contributed to better adher-ence,which may improve the efficacy of sorafenib.

BACKGROUND Post-endoscopic retrograde cholangiopancreatography(ERCP)pancreatitis(PEP)is new onset acute pancreatitis after *** complication is sometimes *** such,PEP should be diagnosed early so that therapeutic interventions can be carried *** lipase(s-Lip)is useful for diagnosing acute ***,its usefulness for diagnosing PEP has not been sufficiently *** This study aimed to retrospectively examine the usefulness of s-Lip for the early diagnosis of *** We retrospectively examined 4192 patients who underwent ERCP at our two hospitals over the last 5 *** primary outcomes were a comparison of the areas under the receiver operating characteristic(ROC)curves(AUCs)of s-Lip and serum amylase(s-Amy),s-Lip and s-Amy cutoff values based on the presence or absence of PEP in the early stage after ERCP via ROC curves,and the diagnostic properties[sensitivities,specificities,positive predictive values(PPV),and negative predictive value(NPV)]of these cutoff values for PEP *** Based on the eligibility and exclusion criteria,804 cases were *** the entire course,PEP occurred in 78 patients(9.7%).It occurred in the early stage after ERCP in 40 patients(51.3%)and in the late stage after ERCP in 38 patients(48.7%).The AUCs were 0.908 for s-Lip[95%confidence interval(CI):0.880-0.940,P<0.001]and 0.880 for s-Amy(95%CI:0.846-0.915,P<0.001),indicating both are useful for early *** comparing the AUCs,s-Lip was found to be significantly more useful for the early diagnosis of PEP than s-Amy(P=0.023).The optimal cutoff values calculated from the ROC curves were 342 U/L for s-Lip(sensitivity,0.859;specificity,0.867;PPV,0.405;NPV,0.981)and 171 U/L for s-Amy(sensitivity,0.859;specificity,0.763;PPV,0.277;NPV,0.979).CONCLUSION S-Lip was significantly more useful for the early diagnosis of *** s-Lip after ERCP could help diagnose PEP earlier;hence,therapeutic interventions can be provided earlier.

In order to measure CO2 flux in wintertime arctic ecosystems, CO2 gas was sampled from various snow-covered grounds by using a closed chamber method during the First China Arctic Scientific Expedition from March to May in 1995. The CO2 gas samples were measured by using an infra-red analyzer （IRGA）. The results showed that （ⅰ） CO2 emission was detected from all kinds of the snow-covered grounds, which provides direct evidence that the arctic tundra is functioning as a source of atmospheric CO2; （ⅱ） CO2 release was also detected from the permanent ice profile and icecap, and （ⅲ） CO2 evolution from terrestrial ecosystems in higher latitudes increased with an increase of surface temperature in accordance with the exponential function. This indicates a close coincidence with that under normal temperature conditions, and provides a useful method for predicting change in CO2 flux in the arctic ecosystems with the global climate change.
