The frequency of digitally controlled iodine stabilized He-Ne laser locked to a hyperfine component in ^127I2 was measured using the National Metrology Institute of Japan＇s reference iodine stabilized He-Ne laser. This laser is operated under the conditions of the practical implementation and its frequency with respect to the International Committee for Weights and Measure recommended value is known from international comparisons. Adopting a sampling rate of 120 kHz for the control system of an iodine stabilized He-Ne laser enables frequency deviation of the test laser from the reference laser by ＋ 5 kHz （relative accuracy of 1 ×10^11） thus limited only by the reproducibility of the iodine stabilized He-Ne laser itself.

Under the JI-EPA （Japan-Indonesia Economic Partnership Agreement）, approximately one thousand nurse and certified care worker candidates have been migrated to Japan since 2008. Migration has been known as a risk factor of mental well-being. Little is known about how the in pre-migration stage may affect the migrant＇s mental health. The study examines the mental health predictor in pre-migration stage using cross sectional design. Gender, Age, EPA course, educational degree, working background, language proficiency, motivation for migration and knowledge about destination country selected as mental health predictor. The results showed that the variable knowledge about living environment is the only independent variable which is correlate with GHQ in the model, β = -0.20, P 〈 0.05. The finding may assume that the knowledge as part of cognitive preparation plays important role in explaining the mental health of the EPA candidates in pre-migration stage.

Primary malignant tumors of the small intestine are rare, comprising less than 2% of all gastrointestinal tumors. An 85-year-old woman was admitted with fever of 40 °C and marked abdominal distension. Her medical history was unremarkable, but blood examination showed elevated inflammatory markers. Abdominal computed tomography showed a giant tumor with central necrosis, extending from the epigastrium to the pelvic cavity. Giant gastrointestinal stromal tumor of the small intestine communicating with the gastrointestinal tract or with superimposed infection was suspected. Because no improvement occurred in response to antibiotics, surgery was performed. Laparotomy revealed giant hemorrhagic tumor adherent to the small intestine and occupying the peritoneal cavity. The giant tumor was a solid tumor weighing 3490 g, measuring 24 cm × 17.5 cm × 18 cm and showing marked necrosis. Histologically, the tumor comprised spindle-shaped cells with anaplastic large nuclei. Immunohistochemical studies showed tumor cells positive for vimentin, CD31, and factor VIII-related antigen, but negative for c-kit and CD34. Angiosarcoma was diagnosed. Although no postoperative complications occurred, the patient experienced enlargement of multiple metastatic tumors in the abdominal cavity and died 42 d postoperatively. The prognosis of small intestinal angiosarcoma is very poor, even after volume-reducing palliative surgery.

Basket pattern blood flow signals discovered in a case of splenic hamartoma by power Doppler ultrasonography

＜正＞ The progress of fire research in Japan is summarized. A systematic modern fire research started on early years of the 20th Century. Through the period for a half century including the Great Kanto Earth Quake and the World War Ⅱ, the leaders dealing with improvement of fire safety technology had gradually understood the importance of fire research and established the Japan Association for Fire Science and Engineering on 1950. Thereafter, the Japan Association for Fire Science and Engineering has been closely related with the progress of fire research in Japan. Recent progress of fire research in Japan has been under influence of fire research performed in other countries and recent serious fires. Basic and applied studies for fire safety in Japan are briefly reviewed and international activities of Japanese researchers are also presented.

We present the gray-scale ultrasonography （GSUS）, power Doppler ultrasonography （PDUS）, abdominal computed tomography （CT）, and magnetic resonance imaging （MRI） findings for a case of splenic hamartoma in a 27-year-old man, showing a φ 50 mm homogeneous, iso- and hypo-echoic splenic mass with evidence of a small plural cystic lesion. This splenic hamartoma showed increased vascularity on power Doppler sonograms. PDUS showed multiple circular blood flow signals inside the mass （i.e. a basket pattern）, which was consistent with the small plural cystic lesion shown by GSUS. Spectral analysis also confirmed arterial and venous flow. CT scans showed that the mass had lowdensity relative to the normal spleen and MRI showed that the mass was isodense, relative to the normal spleen. Therefore, CT and MRI are not useful for the diagnosis of splenic hamartoma. Ultrasonography can be used to diagnose splenic hamartoma without administration of a contrast matedal and therefore is an indispensable method for the diagnosis of splenic hamartoma.

AIM: To clarify the clinical significance of high serum IgE in autoimmune pancreatitis (AIP). METHODS: Forty-two AIP patients, whose IgE was measured before steroid treatment, were analyzed. To evaluate the relationship between IgE levels and the disease activity of AIP, we examined (1) Frequency of high IgE (> 170 IU/mL) and concomitant allergic dis-eases requiring treatment; (2) Correlations between IgG, IgG4, and IgE; (3) Relationship between the presence of extrapancreatic lesions and IgE; (4) Re-lationship between clinical relapse and IgE in patients treated with steroids, and (5) Transition of IgE before and after steroid treatment. RESULTS: IgE was elevated in 36/42 (86%) *** allergic disease was observed in seven patients (allergic rhinitis in three, bronchial asthma in three, and urticaria in one). There were no signifi-cant correlations between IgG, IgG4, and IgE (r = -0.168 for IgG, and r = -0.188 for IgG4). There was no significant difference in IgE in the patients with and without extrapancreatic lesions (526 ± 531 IU/mL vs 819 ± 768 IU/mL, P = 0.163), with and without clini-cal relapse (457 ± 346 IU/mL vs 784 ± 786 IU/mL, P = 0.374). There was no significant difference in IgE between before and after steroid treatment (723 ± 744 IU/mL vs 673 ± 660 IU/mL, P = 0.633). CONCLUSION: Although IgE does not necessarily reflect the disease activity, IgE might be useful for the diagnosis of AIP in an inactive stage.

A method for the identification of the double bond positional isomers of hexadecenyl acetate has been established by analysing similarity of the mass spectra patterns on a fuzzy classification, in which the intensity ratios of six diagnostic pairs of the predominant ions were selected as standard parameters for the characterization of the double bond position. The procedure was tested with △2 to △15-isomers of chemically unmodified hexadecenyl acetate, and the original double bond position in the acetates was located unambiguously.

Development of a Social Activities Scale for Community-Dwelling Older Men Requiring Support in Japan

Aim: We developed a scale to measure the social activities of community-dwelling older men requiring support. Methods: The participants were a group of 134 men, ≥65 years old, who required support and were living in Hokkaido, Japan. An anonymous questionnaire was administered through individual interviews. Valid responses were obtained from 121/134 interviewees. The construct validity of the resulting scale was assessed by exploratory factor analysis (EFA). Criterion-related validity was tested with Spearman’s rank correlation test based on the Social Activities Index for Elderly People (SAI-E). Reliability was assessed by Cronbach’s alpha. Results: A Social Activities Scale for Community-dwelling Older Men Requiring Support (SASOMS) scale was created, comprised of the following three subscales: daily interactions with familiar people;intimate relationships with family members;and interactions with others through activity programs (e.g., exercise, games, recreation, etc.). The created SASOMS scale correlated with the SAI-E (r = 0.557), and its criterion-related validity was confirmed. The alpha coefficient of the new scale was 0.791, and its internal consistency was confirmed. Conclusions: The reliability and validity of the developed SASOMS scale was confirmed, demonstrating that it can be used to assess social activities specifically in older men requiring support. Our results suggest that the scale can be used effectively by care providers who support older men requiring care. The usability of the SASOMS should continue to be improved, and it is necessary to verify its validity in longitudinal studies.

Characteristics of Body Composition and Relationship between Muscle Mass and Muscle Strength among Elderly Women in Different Age Groups

Age-related changes in the body composition of older adults differ among age groups. The purpose of the present study was to clarify the characteristics of body compositions in young (age, 65 - 74 years;yE group) and old (age, ≥75 years;oE group) elderly women, and compare the relationship between muscle mass and strength in elderly women with that in young women for preventing motor function loss in older adults. A total of 30 elderly and 45 young women aged ≥ 65 and 19 - 22 years, respectively, participated in this study. The participants underwent body composition measurement via bioelectrical impedance analysis and examinations of handgrip and leg muscle strength. The age-related body composition changes varied among age groups. Compared with young women, fat-free mass (FFM) in the yE group did not decrease significantly;however, fat mass (FM) and waist-hip ratio (WHR) were significantly greater. Compared with the yE group, decreases in FFM, FM, and WHR in the oE group were significant;furthermore, the decrease in FM measurements was attributed to the loss of FM in the trunk and limbs (upper and lower). The measurement results suggested that the greater FM decrease in the oE group was characterized by decreases in both visceral and subcutaneous fat. In the yE group, the muscle mass was comparable to that in young women;however, there was a remarkable reduction in the lower-limb muscle mass (9% - 10%). In the oE group, muscle mass was reduced in all body parts, including upper and lower limbs and trunk. In young women, significant positive correlations between muscle mass and muscle strength in the upper and lower limbs were observed. No such correlations in the lower limbs were found in elderly women, indicating that muscle mass is not proportionally reflected in muscle strength. In conclusion, for improving and maintaining the health of elderly women, especially those above the age of 75 years, it is important to maintain muscle mass, including muscle strength,
