Usefulness of liver infiltrating CD86-positive mononuclear cells for diagnosis of autoimmune hepatitis

AIM： Although the pathogenic mechanism underlying autoimmune hepatitis （AIH） remains unclear, the immune system is thought to be critical for the progression of the disease. Cellular immune responses may be linked to the hepatocellular damage in AIH. Recently, much attention has been focused on the critical functions of costimulatory molecules expressed on mononuclear cells in the generation of effective T cell-mediated immune responses. Analysis of costimulatory molecule expressed on mononuclear cells from the patients with AIH may give us insight into the pathogenic mechanism of hepatocellular damage in AIH. METHODS： Peripheral blood mononuclear cells （PBMC） were taken from the patients with AIH （34 cases） and healthy controls （25 cases）. Uver infiltrating mononuclear cells （LIMCs） were taken from the patients with AIH （18 cases）, the patient with chronic hepatitis C （CH-C）（13 cases） and the patients with fatty liver （2 cases）. Using flow cytometry, the cells were analyzed for the expression of costimulatory molecules, such as CD80, CD86, and CD152 （CTLA-4）. The results were compared with clinical data such as the level of gammaglobulin, histological grade, presence or absence of corticosteroids administration and the response to corticosteroids. RESULTS： The levels of CD80＋, CD86＋ and CD152＋ PBMC were significantly reduced in the patients with AIH as compared with healthy controls. By contrast, those cells were significantly higher in LIMC than in PBMC of the patients with AIH. Especially, the level of CD86＋ LIMC showed a marked increase irrespective of the degree of disease activity in the patients with AIH,although CD86＋ cells were rarely present in PBMC. The levels of CD86＋ cells were present in significantly higher frequency in patients with AIH than in the patients with CH-C. Furthermore, the patients with AIH with high levels of CD86＋ LIMC showed good responses to corticosteroids, whereas 2 cases of AIH with low levels of CD86＋ LIMC did not respond well. C

Changes in Water Soluble Carbohydrates Content of Timothy (<i>Phleum pretense</i>L.) in Pastures of TMR Centers over 4 Years

Timothy (Phleum pretense L.) pastures of 3 Total mixed ration (TMR) centers in Hokkaido were selected, the first cutting grasses were continuously collected from the same pastures on different harvest days over 4 years (2011-2014), and changes in the water soluble carbohydrates (WSC) content were analyzed to investigate the relationship with weather conditions. The harvest days in the 4-year period were June 17-22, June 24-26, June 27-30, and July 2-5, and grasses on these dates were designated as the harvest date groups. The weather data during the cutting period in each year were utilized. The WSC content tended to change yearly and it was significantly lower in the 4 groups (67.3 - 82.1 g/kg DM) in 2011 than in the groups (101.5 - 130.3 g/kg DM) in 2013 excluding the June 24-26 group (P < 0.05, P < 0.01). The WSC content tended to increase as the harvest date progressed in 2011-2013 and it significantly increased even though the harvest date was delayed only in 2013 (P < 0.01). It was suggested that differences in the WSC content among the years were related to the weather condition and dry matter yield of the grass.

A rare case of hyaline-type Castleman disease in the liver

Castleman disease often develops in the neck, mediastinum and pulmonary hilum. Its onset in the peritoneal cavity is very rare. The patient, a woman in her 70s, was referred to our department for a detailed examination of an abdominal mass. On abdominal ultrasonography, computed tomography scan, magnetic resonance imaging and positron emission tomography, a mass approximately 15 mm in diameter was noted in the hepatic S6. We attempted radical treatment and conducted a laparoscope-assisted right lobectomy. On the basis of histopathological findings, the patient was diagnosed as having hyaline type Castleman disease in the liver, a very rare condition.

Aim: To evaluate the effect of neoadjuvant hepatic arterial infusion chemotherapy (HAIC) on the survival of patients with resectable hepatocellular carcinoma (HCC). Methods: Between January 2003 and January 2014, 80 patients underwent hepatic resection for HCC. Of these patients, we evaluated 49 patients who met the following inclusion criteria: (1) preserved liver function (Child-Pugh A);(2) resectable HCC (≤ 3 nodules, regardless of the size);and (3) HCC with high-grade malignant potential. Among them, 13 patients underwent neoadjuvant HAIC and curative hepatectomy (treatment group). The remaining 36 patients underwent curative hepatic resection without neoadjuvant therapy (control group). Survival after hepatic resection was compared retrospectively between the groups. Results: During follow-up, 2 (15.4%) patients in the treatment group and 25 (69.4%) patients in the control group developed recurrence. The 1-, 3-, and 5-year disease-free rates (100%, 78.6%, and 78.6%, respectivelyvs. 65.8%, 33.7%, and 26.6%, respectively;P = 0.003) and overall survival rates (100%, 100%, and 100%, respectively vs. 91.7%, 77.8%, and 55.3%, respectively;P = 0.037) were significantly better in the treatment group than in the control group. Conclusion: Neoadjuvant HAIC decreased the risk of recurrence and improved survival in patients with HCC with high malignant potential.

Further Stabilization and Power Density Improvement of Stack-Type Thermoelectric Power Generating Module with Biphasic Medium by Using Various Flexible Metals as Electrodes

In order to realize further stability of a stack-type thermoelectric power generating module (i.e. no electrical connections inside), flexible materials of metal springs and/or rods having restoring forces were installed between lower-temperature-sides of thermoelectric elements. These flexible materials were expected to play three important roles of interpolating different thermal expansions of the module components, enlarging heat removal area and penetration of any media through themselves. Then, a low-boiling-point medium (i.e. NOVEC manufactured by 3M Japan Ltd.) was also applied for a high-speed direct heat removal via its phase change from the lower-temperature-sides of the thermoelectric elements in the proposing stack-type thermoelectric power generating module. No electrical disconnections inside the module were confirmed for more than 9 years of use, indicating further module stability. The power generating density was improved to about 120 mW·m-2 with SUS304 springs having 0.7 mm diameter. Increasing power generating density can be expected in terms of suitable selection of flexible metal with high Vickers hardness, cavities control on the spring surface, more vigorous multiphase flow with adding powders to the medium and optimization of the module configurations according to numerical simulations.

Laser-induced breakdown spectroscopy(LIBS)is a capable technique for elementary analysis,while LIBS quantitation is still under *** quantitation,precise laser focusing plays an important role because it ensures the distance between the laser and *** the present work,we employed spectral intensity as a direct way to assist laser focusing in LIBS quantitation for copper *** is found that both the air emission and the copper line could be used to determine the position of the sample surface by referencing the intensity ***,the fine quantitation was only realized at the position where the air emission(e.g.O(I)777.4 nm)reached intensity maximum,and also in this way,a repeatable quantitation was successfully achieved even after 120 *** results suggested that the LIBS quantitation was highly dependent on the focusing position of the laser,and spectra-assisted focusing could be a simple way to find the identical condition for different samples’*** the future,this method might be applicable in field measurements for LIBS analysis of solids.

A combination therapy of ethanol injection and radiofrequency ablation under general anesthesia for the treatment of hepatocellular carcinoma

AIM: To summarize the effects of laparoscopic ethanol injection and radiofrequency ablation (L-EI-RFA), thora- coscopic (T-EI-RFA) and open-surgery assisted EI-RFA (O-EI-RFA) under general anesthesia for the treatment of hepatocellular carcinoma (HCC). METHODS: Time-lag performance of RFA after ethanol injection (Time-lag PEI-RFA) was performed in all cases. The volume of coagulated necrosis and the applied en- ergy for total and per unit volume coagulated necrosis were examined in the groups treated under general (group G) or local anesthesia (group L). RESULTS: The results showed that the total applied energy and the applied energy per unit volume of whole and marginal, coagulated necrosis were significantly larger in group G than those in the group L, resulting in a larger volume of coagulated necrosis in the group G. The rate of local tumor recurrence within one year was extremely low in group G. CONCLUSION: These results suggest that EI-RFA, un- der general anesthesia, may be effective for the treat- ment of HCC because a larger quantity of ethanol and energy could be applied during treatment under pain- free condition for the patients.

基于Matveev和Culick提出的涡脱落引起的热声不稳定性一维简化模型,对涡脱落引起的热声振荡中的典型非线性现象进行研究,着重研究了系统的初值敏感性、关键参数对热声振荡的影响规律及涡声锁频现象.首先,采用Galerkin方法将控制方程中压力和速度波动在基函数下展开,使偏微分方程组转化为一簇常微分方程;然后,数值求解得到了不同系统参数下声场的压力和速度波动,并详细分析了系统在不同初始条件下的热声不稳定性,同时研究了不同稳态流动速度对系统热声振荡的影响规律,以及在不同稳态流动速度下热声振荡过程中出现的涡声锁频现象.结果表明:该涡脱落热声振荡系统对初值极为敏感,是典型的非线性系统;随着稳态流动速度增大,压力波动的振幅总体有增大趋势,但在几个速度区间内却重复出现振幅先减小后增大的相似结构;系统最终以涡撞击频率(fs)的整数(fp/fs)倍做周期振荡,呈现转数为fp/fs的涡声锁频,该涡声锁频可以作为周期性燃烧振荡的重要特征.

Disparity in clinical care for patients with inflammatory bowel disease between specialists and non-specialists

Background: Although inflammatory bowel disease (IBD) patients have been increasing and new thera-peutic options for IBD have been developed, there are relatively few clinicians who specialize in IBD. Patients treated by a non-specialist of IBD may not receive appropriate treatment. This study aimed to compare disease and medication status between IBD patients treated by a specialist and those treated by a non-specialist. Methods: Medical charts of ambulating IBD patients in two hospitals were examined. All patients in one hospital were treated by one of the IBD specialists, while in the other hospital, patients were treated by one of the gastroenterologists who was a non-specialist of IBD. Results: The numbers of IBD patients were 255 (hospital with specialists) and 74 (hospital without specialists), respectively. Disease activity of the patients was not well-controlled in the hospital without specialists compared to in the hospi- tal with specialists (ulcerative colitis (UC): p = 0.0006 and Crohn’s disease: p = 0.012, respectively). The proportion of UC patients who received an insufficient dose of mesalazine (Pentasa < 3 g/day or Asacol < 3.6 g/day) was higher in the hospital without specialists (47% vs. 15%, p < 0.0001). In the hospital without specialists, more patients received long-term corticosteroids (UC: 23% vs. 5%, p < 0.0001), while fewer patients received immunomodulators (UC: 8% vs. 46%, p < 0.0001). Conclusions: IBD patients of the hospital without specialists were not well-controlled and were not prescribed appropriately with thera-peutic drugs. Fostering and placement of the specialist of IBD is an urgent problem.

We report a case of hypovascular advanced hepa-tocellular carcinoma (HCC) successfully treated with a novel combination therapy of percutaneous ethanol-lipiodol injection (PELI) and intervention radiology (IVR),lipiodol-targetting IVR (Lipi-IVR). The present case had a hypovascular HCC (3 cm in diameter) located in the S6 region of the liver. Although the tumor was not detectable at all by both of early and late phase of helical dynamic computed tomography (CT),it could be detected by ultrasonography (US) as a low echoic space occupying lesion (SOL) beside the gallbladder and right kidney. Serum levels of alpha fetoprotein (AFP) and AFP-L3 were extremely high. Combination therapy of PELI,firstly reported in our department,and IVR (PELI and IVR,lipiodol-targetting IVR) was performed twice for the treatment. PELI could effectively visualize the location of the tumor for IVR treatment and show the presence of a thin blood vessel branching from the right hepatic artery flowing into the lipiodol deposit. After treatment,the serum levels of AFP and AFP-L3 were rapidly decreased to normal and maintained for more than eight months. Thus,this case expressing the tremendous effect might give us insight into the effectiveness of the novel combination therapy of PELI and IVR for the treatment of hypovascular HCC.
