Tracheal Stenosis Caused by Thoracic Malignancy: Correlation between CT-Based Tracheal Measurement and Spirometric Values before and after Tracheal Stenting

Background: The correlation between computed tomography (CT)-based tracheal size and spirometry values is unknown in patients with tracheal stenosis by thoracic malignancy, which is often treated by tracheal stenting. Objectives: To evaluate the correlation between the tracheal cross-sectional area (CSA) and spirometric values before and after tracheal stenting, and to confirm that greater improvement in tracheal CSA leads to a larger improvement in spirometry values. Methods: A total of 32 patients with malignant tracheal stenosis underwent tracheal stenting. Before (n = 32) and after (n = 27) treatment, patients underwent chest CT, measuring mean and minimum tracheal CSA values, and spirometry. The correlation between tracheal CSA and each spirometric value was evaluated using Spearman rank correlation analysis. Differences in the pre- and posttreatment tracheal CSA and spirometric values were evaluated using the Wilcoxon matched-pairs test. Results: Significant improvement in the minimum tracheal CSA and in spirometric values was observed after stenting (P P P Conclusions: The tracheal size measured on chest CT correlates with patients’ spirometric values, particularly at the prestenting examination, in patients with malignant tracheal stenosis. The increase in the minimum tracheal CSA after stenting on CT is a predictor for improved spirometric values, which is first demonstrated by this study.

The special alloy steel which is widely used for the bearing and structural parts for a car and construction equipment *** great needs for improving the function and quality,because it affects the processing characteristic and heat treatability at machining and decides the reliability and durability of the final ***,various countermeasures have been taken for improving surface quality and internal quality of bloom（*** and internal crack,inclusions,center segregation） in the continuous caster for a long *** STEEL ＆ SUMIKIN ENGINEERING CO.,Ltd.（NSENGI） has developed many innovative technologies for improving the quality of the large section bloom caster from 1970’ s and put them into commercial use,such as NS plasma heater in tundish,NS M-EMS（In-Mold Electro-Magnetic Stirrer）,NS Soft ***,NSENGI aimed at no internal defects in the special bar quality and co-developed the large reduction technology for bloom continuous caster（NS bloom large reduction） with a special steel ***,NSENGI succeeded in reducing center porosity of cast *** addition,NSENGI firstly succeeded in putting it into commercial use in the world and have contributed to improving the yield ratio at the various steel *** the development,NSENGI did the numerical analysis and a rolling mill test by using the casting piece processed the artificial porosity in it,and gained a better understanding of the reduction characteristic of the ***,NSENGI realized the compact equipment which could reduce center porosity by high efficient bloom reduction and be installed into a little space of multi-strand bloom continuous *** this paper,the principle,the solved technological problems,the improved quality effect and the feature of bloom large reduction equipment are stated.

Aim： To conduct a double-blind, placebo-controlled randomized and dose-ranging study to evaluate the safety and efficacy of the extract of Ganoderma lucidum （G. lucidum） in men with lower urinary tract symptoms （LUTS）. Methods： We enrolled male volunteers （〉 50 years） with an International Prostate Symptom Score （IPSS; questions 1-7）〉 5 and a prostate-specific antigen （PSA） value 〈 4 ng/mL. Volunteers were randomized into groups of placebo （n = 12）, G. lucidum of 0.6 mg （n = 12）, 6 mg （n = 12） or 60 mg （n = 14）, administered once daily. Efficacy was measured as a change from baseline in IPSS and the peak urine flow rate （Qmax）. Prostate volume and residual urine were estimated by ultrasonography, and blood tests, including PSA levels, were measured at baseline and at the end of the treatment. Results： The overall administration was well tolerated, with no major adverse effects. Statistical significances in the magnitude of changes between the experimental groups were observed at weeks 4 and 8. No changes were observed with respect to Qmax, residual urine, prostate volume or PSA levels. Conclusion： The extract of G. lucidum was well tolerated and an improvement in IPSS was observed. The recommended dose of the extract of G. lucidum is 6 mg in men with LUTS.

Aim： To evaluate the safety and efficacy of an extract of Ganoderma lucidum that shows the strongest 5α-reductase inhibitory activity among the extracts of 19 edible and medicinal mushrooms by a double-blind, placebo-controlled, randomized and dose-ranging study in men with lower urinary tract symptoms （LUTS）. Methods： In this trial, we randomly assigned 88 men over the age of 49 years who had slight-to-moderate LUTS to 12 weeks of treatment with G. lucidum extract （6 mg once a day） or placebo. The primary outcome measures were changes in the International Prostate Symptom Score （IPSS） and variables of uroflowmetry. Secondary outcome measures included changes in prostate size, residual urinary volume after voiding, laboratory values and the reported adverse effects. Results： G. lucidum was effective and significantly superior to placebo for improving total IPSS with 2.1 points decreasing at the end of treatment （mean difference, -1.18 points; 95% confidence interval, -1.74 to -0.62; P 〈 0.0001）. No changes were observed with respect to quality of life scores, peak urinary flow, mean urinary flow, residual urine, prostate volume, serum prostate-specific antigen or testosterone levels. Overall treatment was well tolerated with no severe adverse effects. Conclusion： The extract of G. lucidum was well tolerated and improved IPSS scores. These results encouraged a further, large-scale evaluation of phytotherapy for a long duration using the extract of G. lucidum on men with LUTS.

本文说明绝热机用活塞陶瓷与金属的连接方法和该种活塞的潜力。虽然研究出了多种陶瓷和金属的连接方法,但是化学结合的方法,如钎焊和扩散压合不仅连接过程复杂和不耐高温,而且也不能得到陶瓷所需的196MPa 的连接强度。例如,常用的机械方法,是加撑来连接陶瓷与金属,但这种连接会由于金属塑性变形后剪切力加大而损坏陶瓷的接合面。因此,有必要减小陶瓷与金属材料之间的剪切力和改进金属的塑性。采用这种连接方法,陶瓷与金属相接触后,金属瞬时受热硬度就降低,具有韧性,因而金属在外力的作用下将具有陶瓷的形状。预期陶瓷连接面加槽可以进一步改进连接强度。这种连接方法的特点是:无需选择陶瓷材料、缩短连接时间和高温寿命长。上述方法制出的陶瓷绝热活塞,仅在与燃气直接接触的活塞顶部采用陶瓷材料,裙部是用廉价的韧性金属制成的,活塞顶部和裙部连接面之间用隔热材料衬垫。因此,制成了很简单而寿命长的隔热陶瓷活塞。全世界开发绝热柴油机的速度似乎稍放慢,有人对它是否能改进性能和提高燃油经济性持怀疑态度。然而,作者所研究的、且经3000h 以上耐久试验的陶瓷绝热涡轮复合发动机,最后已进入了性能试验的阶段。起初,绝热机开始预定的试验前,就由于陶瓷零件缺乏足够的机械强度和耐热强度而损坏,迫使发动机停车,这就需要花费许多时间和精力来改进陶瓷零件的强度。即便解决了陶瓷的强度问题,陶瓷发动机的许多其他问题,例如绝热性能、陶瓷与金属的连接方法、成本、可靠性以及改进性能和提高燃油经济性的潜力等仍有待于解决。其中,制造陶瓷与金属的连接件对于将来解决所有绝热发动机的问题是首要条件。绝热活塞分为陶瓷顶部和金属裙部,必须把这两部分完全可靠地连接起来。本文说明可经受高温和高机械应力的陶瓷与金属的连接件以及活塞用陶瓷与金属的新连接方法。

Respiratory Changes in the Superior Vena Cava Area on Inspiratory and Expiratory Chest CT: Comparison between Patients with COPD and with Bronchial Asthma

Purpose: In patients with obstructive lung diseases, alteration of intrathoracic pressure is closely related to hyperinflation and leads to hemodynamic impairments. Both intrathoracic pressure and hemodynamics are affected by the respiratory phase. In fact, a previous study showed that respiratory phasic variations in the superior vena cava (SVC) area significantly correlated with the extent of emphysema. Chronic obstructive pulmonary disease (COPD) and bronchial asthma manifest different pathophysiological changes in hyperinflation and hemodynamics. The current study was conducted to evaluate differences in respiratory variations in the cross-sectional area of the SVC between patients with COPD and with asthma. Materials and Methods: We measured the SVC area and calculated the ratio of the SVC area on inspiratory and expiratory scans (i/e-SVC ratio) in 66 patients with COPD and 16 patients with asthma. The cor-relations of the i/e-SVC ratios with airflow limitation, pulmonary small vessels less than 5 mm2 (%cross-sectional area [%CSA] < 5), and respiratory changes in lung volume (i/e-LV) obtained by inspiratory and expiratory computed tomography (CT) images were evaluated. Results: There was no significant difference in i/e-SVC ratio between COPD and asthma groups. In COPD patients, the i/e-SVC ratio significantly correlated with the %CSA ﹣0.433, P = 0.003), i/e-LV ratio (ρ = ﹣0.371, P = 0.011), and percent of predicted forced expiratory volume in 1 second (FEV1% predicted) (ρ = ﹣0.474, P = 0.001), whereas in asthma patients, there were no significant correlations between the i/e-SVC ratio and those functional parameters. Conclusion: There would be differences in the relationship between intrathoracic pressure and cardiopulmonary hemodynamics between COPD and asthma patients.

Factors Predicting the Relapse of Radiation-Induced Organizing Pneumonia after Breast-Conserving Therapy

We investigated the factors predicting radiation-induced organizing pneumonia (RIOP) relapse after tangential breast irradiation. The participants included 23 patients diagnosed with RIOP at the St. Marianna University School of Medicine Hospital between January 2008 and March 2015. Relapse was defined as the appearance of new lesions on diagnostic images during follow-up or after commencing treatment. The relapse-free survival rate and the following 9 parameters were compared between patients with and without RIOP relapse: 1) age (less than vs. equal to or more than the median);2) white blood cell count (less than vs. equal to or more than the median);3) C-reactive protein (CRP) level at the time of RIOP diagnosis (less than normal, more than normal/ less than borderline, and more than borderline);4) boost irradiation (yes vs. no);5) maximum lung depth on linacgraphy (less than vs. equal to or more than the median);6) hormone therapy (yes vs. no);7) chemotherapy (yes vs. no);8) RIOP ratio in the whole lung (less than vs. equal to or more than the median) at the time of RIOP diagnosis;and 9) use of corticosteroids (yes vs. no). The Kaplan-Meier method was used for statistical analysis, with relapse as the cutoff. The follow-up period spanned the date of RIOP onset to May 30, 2015. The level of significance for 2-sided tests was p < 0.05. Relapse was evident in 14 patients (60.8%). The relapse-free survival rate was significantly greater in the normal CRP group (less than 0.30 mg/dl) than in the abnormal CRP group (more than 0.36 mg/dl) (p = 0.044) and in the normal/borderline CRP group (less than 0.36 mg/dl) than in the high CRP group (more than 0.70 mg/dl) (p < 0.01). A retrospective analysis of the diagnostic laboratory submissions from 2016 to 2020 was performed to assess the antibiotic resistance of Staphylococcus pseudintermedius and determine locations in the hospital which might be acting as environmental source(s). Previous studies have identified a significant increase in infections with S. pseudintermedius. Samples were taken from the hospital environment by swabbing areas in the intensive care unit and anesthesia preparation room and bacterial species identified. Isolates obtained from patients were then examined regarding the ability to form biofilm, an important phenotype on hospital-related infection. In addition, veterinary hospital associated strains of S. pseudintermedius were used to determine the bactericidal effect of the used disinfectant, applying the hospital current protocol, by comparing the efficacy against S. pseudintermediius and a strain of Staphylococcus aureus from a dog. The isolates identified were resistant to commonly used antibiotics such as enrofloxacin and cephalosporins, and 45% percent of those were methicillin resistant. The environmental survey in the hospital identified S. pseudintermedius in the pre-anesthesia area, although the isolate was killed by the current used disinfection protocol. A few disease associated bacteria were evaluated for biofilm formation in comparison to a dog isolate of Staphylococcus aureus, demonstrating strong ability to form biofilms.
