Respiratory Changes in the Superior Vena Cava Area on Inspiratory and Expiratory Chest CT: Comparison between Patients with COPD and with Bronchial Asthma

Purpose: In patients with obstructive lung diseases, alteration of intrathoracic pressure is closely related to hyperinflation and leads to hemodynamic impairments. Both intrathoracic pressure and hemodynamics are affected by the respiratory phase. In fact, a previous study showed that respiratory phasic variations in the superior vena cava (SVC) area significantly correlated with the extent of emphysema. Chronic obstructive pulmonary disease (COPD) and bronchial asthma manifest different pathophysiological changes in hyperinflation and hemodynamics. The current study was conducted to evaluate differences in respiratory variations in the cross-sectional area of the SVC between patients with COPD and with asthma. Materials and Methods: We measured the SVC area and calculated the ratio of the SVC area on inspiratory and expiratory scans (i/e-SVC ratio) in 66 patients with COPD and 16 patients with asthma. The cor-relations of the i/e-SVC ratios with airflow limitation, pulmonary small vessels less than 5 mm2 (%cross-sectional area [%CSA] < 5), and respiratory changes in lung volume (i/e-LV) obtained by inspiratory and expiratory computed tomography (CT) images were evaluated. Results: There was no significant difference in i/e-SVC ratio between COPD and asthma groups. In COPD patients, the i/e-SVC ratio significantly correlated with the %CSA ﹣0.433, P = 0.003), i/e-LV ratio (ρ = ﹣0.371, P = 0.011), and percent of predicted forced expiratory volume in 1 second (FEV1% predicted) (ρ = ﹣0.474, P = 0.001), whereas in asthma patients, there were no significant correlations between the i/e-SVC ratio and those functional parameters. Conclusion: There would be differences in the relationship between intrathoracic pressure and cardiopulmonary hemodynamics between COPD and asthma patients.

Implementation and Evaluation of Dual-Purpose Normal and Disaster Situations System Based on Delay and Disruption Tolerant Bluetooth MANETs

Dual-purpose systems for both normal and disaster situations are necessary for providing continuous services from normal situations to disaster situations. We have been developing the dual-purposed systems based on the assurance network design principle. The assurance network design principle makes the dual-purpose systems work stably in both normal and disaster situations. This paper proposes a connectivity-dependent data propagation scheme, in which each terminal transfers data adaptively by wireless multi-hop data transfer or store-and-forward data transfer depending on whether the terminal has connections to its neighboring terminals. To verify the resilience against disconnection among neighboring terminals, we show field experimental results on data propagation time. Also we propose the dual-purpose system, in which there are two types of graphical user interface (GUI) for both situations. Whenever each terminal receives a special packet in disaster situations, the GUI automatically switches from one type for normal situations to another type for disaster situations. We have unified these two types of GUI so that users can understand how to use them even when GUI is automatically switched. To validate feasibility of the dual-purpose normal and disaster situations system, we show experimental results on dissemination of assessment information and automatical switching of GUIs.

Food Intake Characteristics during Early Pregnancy in Women with Gestational Diabetes Mellitus

To investigate the characteristic food intake during early pregnancy in women with gestational diabetes mellitus (GDM) in a rural city in Aomori Prefecture, Japan, one hundred and twenty-one women were recruited and queried about their habitual dietary intake. Food intake of patients was assessed using the model nutritional balance chart at 12 - 16, 24 - 28, and 34 - 36 weeks of gestation. Of the 121 pregnant women examined, 19 were obese. During early pregnancy, food intake ratios of the obese women were significantly lower than those of the non-obese women for the categories of milk (p < 0.001) and sugar (p < 0.05). GDM group was 7 women among 19 women in obesity group during mid-pregnancy. During early pregnancy, women with GDM had significantly higher sugar intake ratios than women without GDM (p < 0.05). These results suggested that obese pregnant women are able to prevent GDM by limiting their sugar intake during early pregnancy.

Analysis of Residual Solvents in Annatto Extracts Using a Static Headspace Gas Chromatography Method

An analytical method for the quantification of residual solvents in annatto extracts, natural food colorants, was established using a static headspace gas chromatography (HSGC) coupled with a flame ionization detector (FID). As a sample diluent in a headspace sampling, dimethylformamide (DMF) was selected owing to its high capacity for dissolving both bixin-based and norbixin-based annatto extracts. The quantification of residual solvents was performed using the external standard method. The linearity of the calibration curves was assured with relative coefficients (R2) that were greater than 0.999. The recoveries of all standard solvents spiked in the annatto extracts were in the range from 95.1% to 107.1% to verify the accuracy and the relative standard deviation (RSD%) values (n = 3) were in the range from 0.57% to 3.31%. The quantification limits (QL) were sufficiently lower than the limits specified by Joint FAO/WHO Expert Committee on Food Additives (JECFA). With the established HSGC method, six residual solvents (methanol, ethanol, 2-propanol, acetone, ethyl acetate, and hexane) in 23 commercial annatto-extract products that consist of seven bixin-based and 16 norbixin-based products were quantified. The levels of residual ethyl acetate and hexane in all products were lower than the specified limits of JECFA. However, three samples of bixin-based products showed higher levels of residual 2-propanol (approximately 313.9 - 427.7 ppm) than the specified limit. Other bixin products also showed higher concentrations of residual methanol (approximately 166.6 - 394.7 ppm) and residual acetone (approximately 75.2 - 179.8 ppm) than the limits of JECFA. In the case of norbixin-based products, nine samples showed higher levels of residual acetone (approximately 42.6 - 139.5 ppm) than the limits of JECFA. This is the first survey of residual solvents in annatto extracts using the validated HSGC method.

Influence of smoking on postpartum depression in Japan

Aim: To investigate the influence of smoking on postpartum depression. Methods: One thousand fifty-one women, in a rural city in Aomori Prefecture, Japan, prospectively fulfilled the selection criteria and completed self-reporting questionnaires on postnatal depression at 5-6 days, 1 month, 4 months, 7 months and 12 months after childbirth, using the Edinburgh Postnatal Depression Scale (EPDS) and a life and social events scales including smoking habits. Results: Seven hundred seventy-seven women were non-smokers. Among two hundred seventy-four women who were smokers before becoming pregnant (26% of pregnant women), 241 women quit smoking during pregnancy and 33 women continued smoking. Smoking habits were significantly associated with sociopsychological states and we found that EPDS scores of smokers were significantly higher than EPDS of non-smokers. The EPDS scores of both non-smokers and smokers were higher at 5-6 days, but were stable from 1 month to 12 months, after childbirth. Fifty-one women who quit smoking after childbirth resumed smoking during he 1-12 month periods after childbirth. The EPDS scores of 51 women who resumed smoking were significantly reduced after they resumed smoking. Conclusion: The EPDS scores of smokers were higher than those of non-smokers and a smoking habit may help to alleviate postpartum depression.

The energy analysis development in this study contributes to the understanding of the dynamics of the organic coffee productive system, in particular to assess the independence of this system with respect to the use of industrialized input products. Thus, it provides information about the sustainability of that production system. Technical itineraries used in this study consist of energy expenditure made with coffee cultivation, according to the type, source and form of energy inputs, agricultural machines, equipment and labor force used in that production system. The energy expenditure, converted into energy units, quantified the input energy. And the organic coffee production, measured in kilograms of processed coffee beans, was the output energy. Primary data used in this study were obtained from organic coffee producers in the Southern region of Minas Gerais State, Brazil, in 2011. Energy balance identified was positive, since the estimated output energy was 626.465 M J/ha and the energy expenditure was 112.998 M J/ha, during the useful life of the crop.

Neoadjuvant hepatic arterial infusion chemotherapy for resectable hepatocellular carcinomas

Aim: To evaluate the effect of neoadjuvant hepatic arterial infusion chemotherapy (HAIC) on the survival of patients with resectable hepatocellular carcinoma (HCC). Methods: Between January 2003 and January 2014, 80 patients underwent hepatic resection for HCC. Of these patients, we evaluated 49 patients who met the following inclusion criteria: (1) preserved liver function (Child-Pugh A);(2) resectable HCC (≤ 3 nodules, regardless of the size);and (3) HCC with high-grade malignant potential. Among them, 13 patients underwent neoadjuvant HAIC and curative hepatectomy (treatment group). The remaining 36 patients underwent curative hepatic resection without neoadjuvant therapy (control group). Survival after hepatic resection was compared retrospectively between the groups. Results: During follow-up, 2 (15.4%) patients in the treatment group and 25 (69.4%) patients in the control group developed recurrence. The 1-, 3-, and 5-year disease-free rates (100%, 78.6%, and 78.6%, respectivelyvs. 65.8%, 33.7%, and 26.6%, respectively;P = 0.003) and overall survival rates (100%, 100%, and 100%, respectively vs. 91.7%, 77.8%, and 55.3%, respectively;P = 0.037) were significantly better in the treatment group than in the control group. Conclusion: Neoadjuvant HAIC decreased the risk of recurrence and improved survival in patients with HCC with high malignant potential.

Diets of obese and non-obese older subjects

Aim: To compare diets between obese and non-obese in healthy older subjects. Methods: Forty-five obese and eighty-seven non-obese older subjects were recruited and their habitual factors that may contribute to obesity were assessed. Intakes of food by food-group in the obesity and non-obesity groups were checked using a visual type presentation of model nutriational balance chart (MNBC). Results: Average intake ratio of food relative to ideal food intake was significantly higher in the obesity group than the non-obesity group. The relationship of obesity and exercise or habitual activities was not significant. Conclusion: Food intake is a primary factor of obesity but regular exercise or habitual activities is not a key factor for obesity in older subjects. Since exercise habit is difficult to achieve in older subjects, particularly those who are obese, food control using the present visualtype MNBC would be one strategy forthe management of obesity.

Improvement of the Assay Method for Steviol Glycosides in the JECFA Specifications

Steviol glycosides are natural sweetener constituents found in the leaves of Stevia rebaudiana Bertoni (Asteraceae). The specifications for steviol glycosides were established by the Joint FAO/WHO Expert Committee on Food Additives (JECFA) in 2008, although there was a call in the following year for the modification of this assay method to enable the determination of nine steviol glycosides rather than just seven. In response, based on a proposed method by the Japan Stevia Association, we developed an improved method by changing the HPLC conditions and including the use of an octadecylsilyl column instead of an amino-bonded column to enable the rapid and reliable determination of the nine steviol glycosides by an isocratic HPLC-UV method. With the developed method, the nine steviol glycosides can be separately determined, and identified using individual reference chemicals as standards, unlike the previous identification method, which was based on the relative retention times. In addition, the single stevioside quantification standard was replaced with both stevioside and rebaudioside A quantification standards. Importantly, the validation of the developed method was successful. The limits of quantification for the nine steviol glycosides were between 0.2% and 0.6%. The developed assay method for the nine steviol glycosides was proposed to JECFA and adopted as the revised assay method for the steviol glycosides specifications at its 73rd meeting in 2010.
