Effectiveness of Aromatherapy Treatment in Alleviating Fatigue and Promoting Relaxation of Mothers during the Early Postpartum Period

Background: This study aimed to determine the effectiveness of aroma hand treatment in alleviating fatigue and promoting relaxation of mothers in their early postpartum period. Methods: This is a quasi-experimental study with a two-group pretest-post-test design which used purposive sampling and non-random assignment of 242 consenting Japanese early postpartum mothers who were patients from a hospital maternity unit in Tokyo, Japan. The intervention involved a 20-minute aroma hand treatment using an effleurage method. The participants selected from 5 essential oils: pure lavender, ylang-ylang, citron, rosewood, and sweet orange. Relaxation promotion and fatigue alleviation as the main outcomes were measured using self-administered valid and reliable questionnaires. Statistical analysis was performed using the two-sample t-test, two-way factorial ANOVA, and simple main effect test. SPSS ver. 23.0 (SPSS, Chicago, IL, USA) was used for data analysis at a 5% significance level. Results: Of 242 participants (intervention group, n = 122;comparison group, n = 120), 229 participants (intervention group, n = 115;comparison group, n = 114) were analyzed per protocol. The intervention group showed a significant improvement in relaxation (t = 6.43, p < 0.001). A significant difference in the simple main effect test of relaxation (F = 37.58, p < 0.001) was found. However, there was no significant reduction in fatigue. The majority (88.4%) of the participants indicated high satisfaction with the aroma hand treatment, and 90.5% evaluated the length of intervention time as appropriate. Conclusions: The aroma hand treatment effectively promoted relaxation of early postpartum mothers, but was less effective in alleviating their fatigue. Based on their high satisfaction rate, aroma hand treatment using essential oils may be considered effective in promoting relaxation of early postpartum mothers.

Background: Hemodialysis (HD) patients have high rates of cardiovascular (CV) mortality. Although structural and functional echocardiographic alterations in HD patients have been the subject of several survival analysis studies, the prognostic value of these alterations is not well established. The aim of this study was to determine the prognostic value of echocardiographic parameters in chronic HD patients. Objectives: One hundred eighteen HD patients were clinically evaluated and underwent Doppler echocardiography, being followed for 45.7 ± 13.6 months. The outcome measures were CV mortality. The predictive value of echocardiographic variables was evaluated by Cox regression model and survival curves were constructed using the Kaplan-Meier method and log-rank test to compare them. Results: CV diseases accounted for 46.4% of all deaths during the follow-up period. We found that the event-free survival rates in one and two years were 96.5% and 83.0%, respectively. Diabetes and E/e' ratio were predictors of CV outcome by multivariate analyses. Conclusion: Diabetes and diastolic dysfunction are independent predictors of CV mortality in chronic HD patients.

Nasolabial cysts are uncommonly diagnosed non-odontogenic soft tissue lesions occurring close to the nasal alar region of the face. Patients usually present with a slowly enlarging asymptomatic swelling. Diagnosis is usually made in the early stages because of the esthetic effects. Histologically, the lesion is lined with non-keratinized squamous epithelium or, more frequently, pseudostratified columnar epithelium with goblet cells. These cysts are most often diagnosed in the fourth decade of life. However, we report a case of nasolabial cyst in an 80-year-old woman, and discuss the diagnosis, differential diagnosis, and treatment with reference to the literature.

在轻型载重汽车市场,以柴油机为动力的越野四轮驱动客货两用汽车为主和由此派生的厢式货车,由于燃油经济性好,赢得了用户的青睐。特别是在日本市场,这类汽车的销量迅速增长,比过去5年的总销售量增加约2倍,其中柴油机稳占90%左右。在这样的情况下,用户对柴油汽车的要求变得越来越高,主要是要求高功率、低噪声、低振动和排烟少,与对汽油机的要求类似。另一方面,柴油机的废气排放法规也逐渐变得严格起来,且在不久的将来会变得很严格。三菱汽车公司已在生产一种排量为2.5L的4缸柴油机(4D56系列),用于轻型载重汽车。但考虑到上述要求,又研制了4M40系列。这种全新设计的2.8L的4缸柴油机于1993年7月发表,装在"NEW PAJERO"汽车上。本文介绍4 M40系列柴油机的设计特点。

Severe Hyperphosphatemia Resulting in Acute Renal Failure and Ischemic Encephalopathy in a Patient with Infantile Leukemia

Tumor lysis syndrome (TLS), hyperleukocytosis, and disseminated intravascular coagulation (DIC) are representative oncological emergencies that overlap mutually at the beginning of therapy for aggressive leukemia. Lately recombinant urate oxidase (rUO) enables to control uric acid level and its crystallization, the most frequent risk factor for clinical TLS;therefore, hyperphosphatemia appears to be the main risk in the rUO era. We here report an infantile leukemia patient who developed severe hyperphosphatemia, resulting in acute renal failure and ischemic encephalopathy. A 9-month-old female baby was adynamic with a bulging anterior fontanel, and was diagnosed as infantile acute lymphoblastic leukemia with a mixed lineage leukemia gene rearrangement. A laboratory examination revealed leukocytosis, bicytopenia, hyperuricemia, a prolonged prothrombin time, activated partial thromboplastin time, and elevated lactate dehydrogenase level. Soon after a reduced dose of prednisolone was administered, she developed hypoxia caused by systemic inflammatory response syndrome and heart failure. Her white blood cell count decreased sharply, leading to acute renal failure due to hyperphosphatemia, which required continuous hemodiafiltration for 48 hours. Although renal function subsequently recovered, severe ischemic encephalopathy remained. She achieved morphological remission once, however, relapsed and passed away soon after. We have to pay attention to the progression of hyperphosphatemia, hyperkakemia and DIC, although hyperuricemia was controlled using rUO. Changes in electrolyte levels must be continuously monitored, and TLS, DIC and/or hyperleukocytosis should be promptly managed especially in patients who are sensitive to therapy.

Amenorrhea as a rare drug-related adverse event associated with everolimus for pancreatic neuroendocrine tumors

The patient was an asymptomatic 43-year-old woman. Abdominal ultrasonography and enhanced computed tomography showed a tumor lesion accompanied by multiple cystic changes in the liver and the pancreatic tail. Endoscopic ultrasound-fine needle aspiration was performed on the pancreatic tumor lesion and revealed pancreatic neuroendocrine tumor(PNET). As it was unresectable due to multiple liver metastases, the decision was made to initiate treatment with everolimus and transcatheter arterial chemoembolization. The patient ceased menstruating after the start of everolimus administration. When the administration was discontinued due to interstitial lung disease, menstruation resumed, but then again stopped with everolimus resumption. An association between everolimus and amenorrhea was highly suspected. Amenorrhea occurred as a rare adverse event of everolimus. As the younger women might be included in PNETs patients, we should put this adverse event into consideration.

Aim: To compare diets between obese and non-obese in children. Methods: Thirty-four obese and ten non-obese school children were recruited and their habitual factors of obesity were asked. Intakes of food in the obesity and non-obesity groups were checked using a model nutritional balance chart (MNBC). Results: Average intake ratio of food relative to ideal food intake was significantly higher in the non-obesity group than the obesity group. The relationship between obesity and exercise was significant but not significant for intake ratio of food, times watching TV and playing games. Conclusion: Food intake is not a primary factor of obesity but exercise is a key factor for obesity in school children. Since the effect of diet intervention in obese children was slight, exercise habit would be a more important strategy to reduce obesity than diet in school children.

The Al2O3-supported PtAg catalysts were prepared and evaluated for the dehydrogenation of n-butane at 550°C in the presence of H2. The PtAg/Al2O3 catalyst prepared by an impregnation method using the Cl- removing Pt/Al2O3 and AgNO3 showed a higher activity and selectivity to butenes and 1,3-butadiene compared to the Pt/Al2O3 catalyst, but a large amount of coke (about 30 wt% versus the catalyst weight) was formed during the dehydrogenation. The free Ag metal on the prepared catalyst dramatically promoted the coke formation, because the dehydrogenation of 1-butene over the Ag/Al2O3 catalyst produced a large amount of coke. The Cl- addition to the Cl- free Pt/Al2O3 catalyst decreased the coke formation by the reaction of the free Ag particles and Cl to form AgCl which was inactive for the coke formation. The highest initial conversion (50.3%) was obtained with the selectivity to butenes and 1,3-butadiene (butenes = 80.2% and 1,3-butadiene = 5.9%) when the PtAg/Al2O3 catalyst modified with Cl- was used.
