A case of inflammatory myoglandular polyp (IMGP) causing hematochezia is reported. The patient was a 33-year-old man who visited our hospital for further evaluation of hematochezia. Colonoscopy revealed a red, hard, spherical peduncular polyp with erosion and mucous exudation, about 20 mm in diameter, in the descending colon. Excluding the polyp, there was no lesion in the colorectum. Endoscopic polypectomy was performed. Histological examination of the specimen revealed inflammatory granulation tissue in the lamina propria, proliferation of smooth muscle, and hyperplastic glands with variable cystic changes. This polyp was diagnosed as an IMGP. The symptom of hematochezia was resolved after endoscopic resection. Our case shows that treatment is necessary for IMGP if intestinal bleeding occurs and endoscopists should be aware of the endoscopic characteristics of IMGP.

Use of Analogy by Public Health Nurses in Problem Solving for Individual Consultations in Japan: A Multiple Case Study

Aim: Nurses must develop good problem-solving skills (PSS) to work in the complex health care environment. This study explored PHNs’ use of analogy in PSS development. The purposes of the study were to clarify how PSS developed in one area (i.e., mental health) could be applied to another area (i.e., maternal health) and whether new PSS could develop in response to PSS gained in another area. Methods: A multiple case study was conducted using interviews. We interviewed 27 consultations from eight Japanese PHNs who consulted in mental health departments before transferring to maternal health departments. The data on how PHNs applied PSS in the selection, mapping, evaluation, and learning stages of the analogy process were extracted from transcribed interviewed data and compared. Results: PHNs provided 59 PSS used in 27 consultations. All PHNs applied past mental health PSS to solve new problems in maternal health. They tended to select past PSS based on structural similarity and to apply PSS via low-level abstraction in serious situations or preventively to avoid causing the current situation to worsen. Notably, PHNs developed maternal health PSS by using past mental health PSS;these new PSS were derived through analogy from various failures and successes. Conclusions: PSS developed in one area can be applied in another area, and new PSS can develop through applying these previous PSS. Identification of structural similarities and preventive analogies must be included in nursing education, especially for nurses working in public health fields.

The stable isotopic ratio of water vapor (δO and δD) was measured near July-1st Glacier (39.25N, 97.75E, 4250 m) in Northwest China. From 11 to 17 August 2003, we collected and then analyzed atmospheric samples at July-1st Glacier (Qiyi) and a base camp (BC), that was 4km from Qiyi. Then the temporal change of the isotopic composition was compared with the atmospheric circulation fields using and European Centre for Medium-range Weather Forecasts (ECMWF) objective analysis dataset.

It has been proposed that 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1), which activates glucocorticoids, plays a role in chronic inflammatory diseases including metabolic diseases, rheumatoid arthritis, and ulcerative colitis. We have recently reported that the expression of 11β-HSD1 is increased in the gingiva of patients with chronic periodontitis and in that of rats with ligature-induced periodontitis. In this study, to further demonstrate the involvement of 11β-HSD1 in chronic periodontitis, the expression of 11β-HSD1 was investigated in another rat model of experimental periodontitis induced by intragingival injection of lipopolysaccharide from Porphyromonas gingivalis (LPS-PG). Alveolar bone loss was observed two weeks after intragingival injection of LPS-PG. The level of 11β-HSD1 mRNA assessed by real-time reverse transcriptase-polymerase chain reaction was significantly elevated in LPS-PG-induced periodontitis compared with controls. The expression of 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2), which inactivates glucocorticoids, was not significantly different between control and LPS-PG-induced periodontitis. The expression of 11β-HSD1 was significantly correlated with that of TNF in LPS-PG-induced periodontitis. The increased expression of 11β-HSD1 protein in LPS-PG-induced periodontitis was confirmed by immunohistochemistry using anti-11β-HSD1 antibody. These results further suggest a role for 11β-HSD1 in the pathogenesis of chronic periodontitis.

The Direction of Research on Active Aging and Healthy Life Expectancy in Japan

＜正＞ Soils generated as byproducts of construction are used in other projects. When such soils are to be used for embankments but are unsuitable for such use, they must be solidified first. When solidifying agents are added, the gain in strength appears shortly thereafter, but there are cases where banking cannot be done within the prescribed time after mixing with solidifying agent, due to sudden changes of work schedule or weather. The solidified soil must then be crushed and compacted before use. The strength characteristics of such soil are unclear. To clarify the strength characteristics of soil that has been mixed with solidifying agents, cured for a certain period, crushed and then compacted （"crushed solidified soil"）, we conducted several laboratory tests in which the time elapsed between the addition of solidifying agent to soil and crushing was varied. The lest results showed that the strength of crushed solidified soil could be estimated from the strength of solidified soil. Also, the tests showed that the cone index of crushed solidified soil is greatly affected by the time elapsed between the addition of solidifying agents and crushing. However, unconfined compressive strength, cohesion and California Bearing Ratio （CBR） of crushed solidified soil did not greatly change from one day of elapsed time onward.

In Japan, 18.1% of the population known as baby-boomers will become the late-stage elderly in 2025, thereby needing a foundation to support this change. The Japanese Ministry of Health, Labour and Welfare is promoting the development of a regional comprehensive system allowing the elderly to continue living in their familiar surroundings. However, a care shortage is inevitable unless elders are able to age in good health, regardless of the system’s level of enhancement. This study aims to review the literature on active aging, clarify trends in clinical operations undertakings and research in Japan, and consider relevant research issues. After combining the search results of “active aging” and “healthy life expectancy,” we used a text mining technique to analyze the abstracts of 120 original articles and 213 reviews, commentaries, and features. Eight categories were extracted from the original articles: health statistics, gender, age, etc. From the reviews, commentaries, and features, 16 categories were extracted: orientation, disease, and living, etc. Cerebrovascular disease and osteoporosis were the most common diseases covered in the original articles;there has been a substantial amount of research on “active aging” and “healthy life expectancy” because they can easily lead to being bedridden and to a decrease in QOL. In the reviews, commentaries, and features, lifestyle-related diseases and menopause rather than cerebrovascular disease and osteoporosis, were extracted. The categorical differences found in the original articles may be due to the possibility that Japanese researchers are publishing their research abroad rather than in Japan or they submit research on topics that are guaranteed to be published at home or abroad. Little research has been conducted using the terms, “active aging” and “healthy life expectancy,” evidenced by the small number of studies generated. Background: The north-east (NE) region of Sri Lanka observed a critical health workers’ shortage after the long-lasting armed conflict. This study aimed to explore medical students’ attitudes towards working in the NE and to identify factors determining such attitudes. Methods: A semi-structured, self-administered questionnaire survey was conducted in two medical schools, one in the NE and the other near the capital, in October 2004. Data were qualitatively analysed using the framework approach. Results: Three main themes were identified: 1) Professional motives and career plans;2) Students’ perceptions of the healthcare situation in the NE;and 3) Students’ choice of the NE as a future practice location. It was found that familiarity with the difficulties faced by the NE people was a major motivation for medical students to work in the NE in the future. For NE students, familiarity was linked to their sense of belonging. For non-NE students, their personal experience of the NE familiarized them with the difficult situation there, which positively influenced their willingness to work there. Demotivations to work in the NE were poor working and living conditions, fewer opportunities for postgraduate education, language differences, insecurity, and fear of an unpleasant social response from the NE communities. Conclusions: NE local medical students had a sense of belonging to the NE and compassion for the Tamil people as members of the ethnic group. They were willing to work in the NE if their concerns about difficult working and living conditions and postgraduate education could be solved. Non-NE students who were familiar with the NE situation through their personal experience also showed a willingness to work there;thus, early exposure programmes in medical education might help to increase the health workforce in the NE.
