Evaluation of preferable insertion routes for esophagogastroduodenoscopy using ultrathin endoscopes

AIM:To evaluate the discomfort associated with esophagogastroduodenoscopy(EGD)using an ultrathin endoscope through different insertion routes.METHODS:This study(January 2012-March 2013)included 1971 consecutive patients[male/female(M/F),1158/813,57.5±11.9 years]who visited a single institute for annual health checkups.Transnasal EGD was performed in 1394 patients and transoral EGD in 577.EGD-associated discomfort was assessed using a visual analog scale score(VAS score:0-10).RESULTS:Multivariate analysis revealed gender(M vs F:4.02±2.15 vs 5.06±2.43)as the only independent predictor of the VAS score in 180 patients who underwent EGD for the first time;whereas it revealed gender(M vs F 3.60±2.20 vs 4.84±2.37),operator,age group(A:70 years;A/B/C/D/E:4.99±2.32/4.34±2.49/4.19±2.31/3.99±2.27/3.63±2.31),and type of insertion as independent predictors in the remaining patients.Subanalysis for gender,age group,and insertion route revealed that the VAS score decreased with age regardless of gender and insertion route,was high in female patients regardless of age and insertion route,and was low in males aged over60 years who underwent transoral insertion.CONCLUSION:Although comprehensive analysis revealed that the insertion route may not be an independent predictor of the VAS score,transoral insertion may reduce EGD-associated discomfort in elderly patients.

Coastal and demersal chondrichthyans(sharks,rays,and skates)are expected to exhibit high levels of genetic differentiation in areas of complex geomorphology.Population genetic studies investigating the extent to which demographic history shapes the genetic structure of theseﬁshes are rare.Here,we combined mitochondrial DNA(Cytb and ND2)and 8 nuclear microsatellite loci from 244 individuals to examine the population genetic structure and demographic history of the 3 Indo-West Paciﬁc species of sharpnose rays(Telatrygon zugei,Telatrygon biasa,and Trygon crozieri).High levels of genetic variation both within and between species were identiﬁed.Phylogenetic analysis partitioned haplotypes into 2 lineages supporting divergence of T.zugei from T.crozieri and T.biasa during the Pleistocene.Furthermore,microsatellite-based clustering analyses identiﬁed 4 genetic groups(i.e.T.zugei from Japan,T.zugei from coastal China,T.biasa from Gulf of Thailand,and T.crozieri from the Andaman Sea).Measurements of genetic differentiation also support these 4 groups.Additionally,Pleistocene demographic expansions were examined in all genetic groups.The climate oscillations and current hydrologic cycles in the Indo-West Paciﬁc appear to coincide with the hypothesis regarding speciation and the observed demographic history trends of the sharpnose rays.Considering the species group has,until recently,been thought to be one species,these results are critical for deﬁning management units and guiding conservation efforts to preserve stingray biodiversity.

Role of animal models in glaucoma research

Glaucoma is a neurodegenerative disease of the eye,and it presents with visual field defects accompanied by progressive degeneration of the optic nerve and retinal ganglion cells(RGCs).It is one of the major causes of blindness worldwide and affects 1 in 20 people over the age of 40.Glaucoma is caused by multiple factors,but it is usually associated with elevated intraocular pressure(IOP).Extensive studies have been carried out to discover therapeutic targets and to develop new drugs to treat ocular hypertension using experimental models of rodents(rats and mice)and non-human primates(e.g.,cynomologus,rhesus,or marmoset monkeys).

High-temperature formation phases and crystal structure of hot-pressed thermoelectric compounds with chalcopyrite-type structure

In this study, we introduced the temperaturedependent formation phases and crystallographic parameters of hot-pressed silver gallium telluride AgGaTe2 and copper gallium telluride CuGaTe2 with chalcopyrite structure from 300 to 800 K. These two compounds are potential thermoelectric materials in the intermediate temperature range; however, the temperature-dependent formation phases and crystallographic parameters of hotpressed samples have not yet been analyzed in detail. The crystal structure analysis based on synchrotron X-ray diffraction （SXRD） measurements clarifies that impurity phases such as Te and Ag2Te in the AgGaTe2 matrix and Te and CuTe in the CuGaTe2 matrix appear at some temperature regions above 300 K. The existence of such impurity phases could be correlated with the increases of the electrical resistivity and Seebeck coefficient of the samples after multiple measurement cycles of the temperature-dependent transport properties from 300 to 800 K. The tetragonal lattice parameters a and c, tetragonal lattice volume, thermal expansion coefficients, tetragonal distortion, anion displacement parameter, and isotropic displacement parameter of the hot-pressed AgGaTe2 and CuGaTe2 were also analyzed. These crystallographic parameters are expected to substantially affect the thermoelectric properties of AgGaTe2 and CuGaTe2. Our results provide prospect of the long-term high-temperature stability and clues of the detailed analysis on the transport properties of hot-pressed AgGaTe2 and CuGaTe2, which should aid their development for thermoelectric applications.

Dietary di/tripeptides elicit preventive effects against lifestyle-related diseases such as hypertension,and hypercholesterolemia,etc.Although there have been evidential reports that the intake of protein hydrolysate improved impaired memory in human,limited studies on bioavailability,in particular,beyond the blood-brain barrier(BBB)of candidates in hydrolysate may prevent their extensive physiological studies.Thus,this review discusses the updated studies on BBB transport of peptides showing improved cognitive decline.Furthermore,their accumulation in the brain cerebral parenchyma is also introduced.

Prevalence of nonalcoholic fatty liver disease and its association with age in patients with type 2 diabetes mellitus

BACKGROUND Type 2 diabetes mellitus(T2DM)is a risk factor for nonalcoholic fatty liver disease(NAFLD).AIM To determine the prevalence and clinical correlates of NAFLD in a large cohort of patients with T2DM.METHODS Four hundred thirty-seven participants with T2DM who consulted at Meijo Hospital from April 2019 to September 2020 and underwent computed tomography(CT)were assessed.The mean age was 74±13 years,and 269 were men.Hepatic attenuation minus splenic attenuation(CTL−S)less than 1 Hounsfield unit was considered fatty liver.NAFLD was defined as fatty liver in the absence of significant alcohol consumption and hepatitis virus infection.A multiple logistic regression was used to assess the independent factors associated with NAFLD.RESULTS NAFLD was identified in 25.2%of the participants.Young age(odds ratio[OR]=−0.945;95%confidence interval[CI]:0.922–0.969),higher hemoglobin levels(OR=1.501,95%CI:1.278–1.764),lower high-density lipoprotein(HDL)cholesterol levels(OR=0.971,95%CI:0.953–0.989),and the absence of dialysis(OR=0.109,95%CI:0.014–0.856)were independent predictors of NAFLD.CONCLUSION NAFLD was detected with CT in 25.2%of the participants.NAFLD was associated with younger age,higher hemoglobin levels,lower HDL cholesterol levels,and an absence of dialysis.

Effects of tomato juice on the pharmacokinetics of CYP3A4-substrate drugs

We previously demonstrated that tomato juice(TJ) contains potent mechanism-based inhibitor(s) of CYP3A4. In this study, we investigated the effects of TJ and grapefruit juice(GFJ)on the pharmacokinetics of the CYP3A4-substrate drugs, nifedipine(NFP) and midazolam(MDZ), in male Wistar rats. Oral administration of GFJ 90 min before the intraduodenal administration of NFP or MDZ increased the area under the concentration–time curves(AUCs)of NFP and MDZ by 32.4% and 89.4%, respectively. TJ increased MDZ blood concentrations and AUC after intraduodenal MDZ administration;however, it had no effect on NFP. When MDZ and NFP were intravenously administered, GFJ significantly increased the AUC of MDZ,but only slightly increased that of NFP. In contrast, TJ only slightly increased the AUC of MDZ.These results suggest that, similar to GFJ, TJ influences the pharmacokinetics of CYP3A4-substrate drugs;however, it may be a drug-dependent partial effect.

INTRODUCTION Sharks,skates and rays originated 400 Ma,with a further radiation throughout every ocean,and play a vital role in maintaining the balance of marine ecosystems as top predators(Weigmann 2016).There are more than 1000 species of sharks,skates,rays and chimaeras in the world,244 of which are distributed in Chinese seas.That is,Chinese chondrichthyan fauna make up at least one-fifth of the world’s extant species.Recent habitat degradation and overexploitation have caused sharp declines in many populations of elasmobranchs(White 2007;Dulvy et al.2017).Environmental changes and loss of oceanic apex predators due to overfishing could affect the migration routes and distribution of batoids(rays),resulting in community restructuring in the coastal ecosystem(Yamaguchi et al.2005;Myers et al.2007).Despite the combination of known high biodiversity and heavy exploitation of elasmobranchs in China,there is little reliable information on the population genetics of many shark and ray species.Consequently,clarification of the population structure is crucial for conservation and management,especially for endangered elasmobranchs.

Learning from the Film “Mitorishi (Transition Doula)”—Application to Understanding the End-of-Life in a Gerontological Nursing Practice Course in Japan

Purpose: This study aims to detail what nursing students learned from watching the film “Mitorishi”, which was used among the teaching materials in a gerontological nursing practice course in nursing education in Japan, and demonstrate the applicability of using the film as an element relevant to the teaching of end-of-life care in Japan. Methods: The participants were fourth-year nursing students at University A who had watched the film “Mitorishi” during the course, had submitted a report on what they learned, and provided informed consent to the study participation. The reports submitted by the participants were used as data and analyzed qualitatively using the content analysis approach. Results: The analysis identified the following five categories (number of codes): Characteristics of life during the final days (9), Importance of views on life and death (3), Importance of end-of-life care (3), Assistance in preparation for the final days of life (13), and Understanding the profession of transition doula (9). Discussion: The five categories show that the students have learned about the “role of nurses at the time of end-of-life care” while “visualizing end-of-life care” and learning “preparedness for end-of-life care”. These findings suggest that using the film “Mitorishi” in the teaching curriculum is effective for learning about end-of-life care for older people.

Intraoperative frozen section diagnosis of bile duct margin for extrahepatic cholangiocarcinoma

AIM To evaluate the usefulness of frozen section diagnosis(FSD) of bile duct margins during surgery for extrahepatic cholangiocarcinoma(CCA). METHODS We retrospectively analyzed 74 consecutive patients who underwent surgery for extrahepatic CCA from 2012 to 2017, during which FSD of bile duct margins was performed. They consisted of 40 distant and 34 perihilar CCAs(45 and 55 bile duct margins, respectively). The diagnosis was classified into three categories: negative, borderline(biliary intraepithelial neoplasia-1 and 2, and indefinite for neoplasia), or positive. FSD in the epithelial layer, subepithelial layer, and total layer was compared with corresponding permanent section diagnosis(PSD) postoperatively.Then, association between FSD and local recurrence was analyzed with special reference to borderline.RESULTS Analysis of 100 duct margins revealed that concordance rate between FSD and PSD was 68.0% in the total layer, 69.0% in the epithelial layer, and 98.0% in the subepithelial layer. The extent of remaining biliary epithelium was comparable between FSD and PSD, and more than half of the margins lost > 50% of the entire epithelium, suggesting low quality of the samples. In FSD, the rate of negative margins decreased and that of borderline and positive margins increased according to the extent of the remaining epithelium. Diagnostic discordance between FSD and PSD was observed in 31 epithelial layers and two subepithelial layers. Alteration from borderline to negative was the most frequent(20 of the 31 epithelial layers). Patients with positive margin in the total and epithelial layers by FSD demonstrated a significantly worse local recurrence-free survival(RFS) compared with patients with borderline and negative margins, which revealed comparable local RFS. Patients with borderline and negative margins in the epithelial layer by PSD also revealed comparable local RFS.
