BACKGROUND Endoscopic papillectomy(EP) for benign ampullary neoplasms could be a lessinvasive alternative to pancreatoduodenectomy(PD). There are some problems and limitations with EP. The post-EP resection margins of ampullary tumors are often positive or uncertain because of the burning effect of EP. The clinical outcomes of resected margin positive or uncertain cases after EP remain *** To investigate the clinical outcomes of resected margin positive or uncertain cases after *** Between January 2007 and October 2018, all patients with ampullary tumors who underwent EP at Kobe University Hospital were included in this study. The indications for EP were as follows: adenoma, as determined by preoperative endoscopic biopsy, without bile/pancreatic duct extension, according to endoscopic ultrasound or intraductal ultrasound. The clinical outcomes of resected margin positive or uncertain cases after EP were retrospectively *** Of the 45 patients, 29 were male, and 16 were female. The mean age of the patients was 65 years old. Forty-one patients(89.5%) underwent en bloc resection,and 4 patients(10.5%) underwent piecemeal resection. After EP, 33 tumors were histopathologically diagnosed as adenoma, and 12 were diagnosed as adenocarcinoma. The resected margins were positive or uncertain in 24 patients(53.3%). Of these cases, 15 and 9 were diagnosed as adenoma and adenocarcinoma, respectively. Follow-up observation was selected for all adenomas and 5 adenocarcinomas. In the remaining 4 adenocarcinoma cases,additional PD was performed. Additional PD was performed in 4 cases, and residual carcinoma was found after the additional PD in 1 of these cases. In the follow-up period, local tumor recurrence was detected in 3 cases. Two of these cases involved primary EP-diagnosed adenoma. The recurrent tumors were also adenomas detected by biopsy. AIM To elucidate the factors associated with residual gastroesophageal reflux disease(GERD) symptoms in patients receiving proton pump inhibitor(PPI) maintenance therapy in clinical *** The study included 39 GERD patients receiving maintenance PPI therapy. Residual symptoms were assessed using the Frequency Scale for Symptoms of GERD(FSSG) questionnaire and the Gastrointestinal Symptom Rating Scale(GSRS). The relationships between the FSSG score and patient background factors, including the CYP2C19 genotype, were *** The FSSG scores ranged from 1 to 28 points(median score: 7.5 points), and 19 patients(48.7%) had a score of 8 points or more. The patients' GSRS scores were significantly correlated with their FSSG scores(correlation coefficient = 0.47, P < 0.005). In erosive esophagitis patients, the FSSG scores of the CYP2C19 rapid metabolizers(RMs) were significantly higher than the scores of the poor metabolizers and intermediate metabolizers(total scores: 16.7 ± 8.6 vs 7.8 ± 5.4, P < 0.05; acid reflux-related symptom scores: 12 ± 1.9 vs 2.5 ± 0.8, P < 0.005). In contrast, the FSSG scores of the CYP2C19 RMs in the non-erosive reflux disease patients were significantly lower than those of the other patients(total scores: 5.5 ± 1.0 vs 11.8 ± 6.3, P < 0.05; dysmotility symptom-related scores: 1.0 ± 0.4 vs 6.0 ± 0.8, P < 0.01). CONCLUSION Approximately half of the GERD patients receiving maintenance PPI therapy had residual symptoms associated with a lower quality of life, and the CYP2C19 genotype appeared to be associated with these residual symptoms.

The authors give several new criteria to judge whether a simple convex polytope in a Euclidean space is combinatorially equivalent to a product of simplices. These criteria are mixtures of combinatorial, geometrical and topological conditions that are inspired by the ideas from toric topology. Carbon nanofiber(CNF)-reinforced aluminum-matrix composites were fabricated via ball milling and spark plasma sintering(SPS), SPS followed by hot extrusion and powder extrusion. Two mixing conditions of CNF and aluminum powder were adopted: milling at 90 rpm and milling at 200 rpm. After milling at 90 rpm, the mixed powder was sintered using SPS at 560 °C. The composite was then extruded at 500 °C at an extrusion ratio of 9. Composites were also fabricated via powder extrusion of powder milled at 200 rpm and 550 °C with an extrusion ratio of 9(R9) or 16(R16). The thermal conductivity and tensile properties of the resultant composites were evaluated. Anisotropic thermal conductivity was observed even in the sintered products. The anisotropy could be controlled via hot extrusion. The thermal conductivity of composites fabricated via powder extrusion was higher than those fabricated using other methods. However, in the case of specimens with a CNF volume fraction of 4.0%, the thermal conductivity of the composite fabricated via SPS and hot extrusion was the highest. The highest thermal conductivity of 4.0% CNF-reinforced composite is attributable to networking and percolation of CNFs. The effect of the fabrication route on the tensile strength and ductility was also investigated. Tensile strengths of the R9 composites were the highest. By contrast, the R16 composites prepared under long heating duration exhibited high ductility at CNF volume fractions of 2.0% and 5.0%. The microstructures of composites and fracture surfaces were observed in detail, and fracture process was elucidated. The results revealed that controlling the heating and plastic deformation during extrusion will yield strong and ductile composites.

The authors study torsion in the integral cohomology of a certain family of2 n-dimensional orbifolds X with actions of the n-dimensional compact torus. Compact simplicial toric varieties are in our family. For a prime number p, the authors find a necessary condition for the integral cohomology of X to have no p-torsion. Then it is proved that the necessary condition is sufficient in some cases. The authors also give an example of X which shows that the necessary condition is not sufficient in general.
