Role of lipid-lowering agents in the management of diabetic retinopathy

Diabetic retinopathy affects a substantial proportion of patients with diabetes mellitus(DM) and is the leading cause of blindness in working-aged adults. Even though the incidence of diabetic retinopathy has declined in the last decades, its prevalence increased and is expected to rise further as a result of the increasing incidence of type 2 DM(T2DM) and the longer life expectancy of patients with DM. The pathogenesis of diabetic retinopathy is multifactorial. Some observational studies suggested an association between dyslipidemia and the development and progression of retinopathy in patients with DM but others did not confirm this association. Regarding lipid-lowering agents, studies that evaluated the role of statins in the management of these patients are mostly small and yielded discrepant results. Large randomized studies with statins in patients with T2DM showed no benefit of these agents on diabetic retinopathy but were not designed to address this effect. In contrast, both preclinical data and two large randomized controlled studies, the FIELD and the ACCORD trial, showed that fenofibrate delays the progression of diabetic retinopathy. Even though the mechanisms underpinning this favorable effect are not entirely clear, these findings suggest that fenofibrate might represent a useful tool for the management of diabetic retinopathy.

An Alternative Method for Incorporating Fiber Meshes in Complete Upper Dentures

The most commonly used material for constructing complete dentures is polymethyl methacrylate (PMMA). However, the strength characteristics of PMMA, such as impact strength and fatigue strength, are poor, and fracturing of PMMA dentures is a common problem in prosthodontic practice. Reinforcing PMMA with various materials, such as carbon fibers, glass fibers (fiberglass), and ultrahigh modulus polyethylene fibers, has been suggested to strengthen the denture-base material. A common problem encountered when packing the resin on these specimens is fiber slippage beyond the denture edges. The present study proposes an alternative method of incorporating fiber meshes into complete dentures, whereby a thin filament of self-polymerizing resin at the perimeter of the fiber mesh is produced, giving a clear and stable shape to the mesh that fits the upper jaw cast. During placement of the shaped mesh on the cast, a positive-negative relationship is created between the mesh and cast, which immobilizes the mesh during the incorporation process.

Scleredema adultorum is a rare sclerotic disorder characterized by diffuse swelling and nonpitting induration of the skin. Its occurrence has been documented in association with infections, diabetes mellitus, paraproteinemia, multiple myeloma, and monoclonal gammopathy. We report an unusual case of a 48- year- old man with an asymptomatic bilateral eyelid edema of sudden onset. During a period of 6 months, the condition slowly progressed to extensive nonpitting edematous swelling restricted to the periorbital sites. The presumptive diagnosis of scleredema adultorum was confirmed by the presence of typical histologic findings. This case is unique in that the periorbital swelling remained as the sole clinical manifestation of scleredema during the 5- year follow-up and was complicated with partial vision blockage.

结节性乳房畸形是一种罕见的乳房畸形，可导致年轻女性双侧或单侧乳房异常。其特征是乳房基底部环状收缩，导致乳房实质组织水平和垂直方向发育不良，伴或不伴有乳头乳晕复合体处乳房实质组织疝出和乳晕扩大。许多学者已提出许多矫正畸形的方法，但是由于这些方法大多没有解决收缩环的问题，进而导致矫正效果不能令人满意。本研究介绍一种新的治疗方法，即采取隆乳术入路，并通过分离收缩环重置乳房的实质组织，进而创建了2个乳房支柱。支柱的作用是上提乳房。如果乳房体积不足，可将硅胶假体置入乳腺下腔隙。如果乳头乳晕复合体较大，利用双环切口矫正。本组共成功矫治了11例（21侧）患者，且美学效果均较好。
