Psychological Factors Affecting the Family’s Satisfaction of Patients in an Intensive Care Unit: The Dominant Role of Self-Compassion and Resilience

Background: Family members of Intensive Care Unit (ICU) patients are subject to a higher risk of depression, anxiety, and stress-related disorders. This psychological distress inevitably affects their perception of the quality of care that their relative receives. The aim of this study is to enlighten the effect of psychopathology, resilience, and self-compassion on the satisfaction of family members of ICU patients and examine the role of self-compassion and resilience as explaining variables of the psychopathology and satisfaction relation. Methods: One hundred and seventy-six family members participated in the study. Each participant completed the Symptom Check List 90 (SCL-90-R), the Family Satisfaction in the Intensive Care Unit (FS-ICU) questionnaire, the Connor-Davidson Mental Endurance Scale (CD-RISC), and the Self-Compassion scale (SCS). Analysis of variance and path analysis was applied in order to test the research hypotheses. Results: The patient’s age, patient’s gender, type of relation, and age of the family member did not have a statistically significant direct or interaction effect on the satisfaction of the family members. The psychopathology had a significant negative correlation with the total satisfaction of the family members. The self-compassion was found to have a significant direct effect on psychopathology and a significant indirect effect on satisfaction via resilience, while both resilience and self-compassion had significant direct or indirect effects on satisfaction. Self-compassion was found to eliminate the mediation role of resilience on psychopathology and the resilience and psychopathology effect on satisfaction. Conclusions: Self-compassion emerged as the most important personality characteristic concerning the satisfaction of family members of ICU patients. Why there is a need to discuss pulmonary hypertension other than pulmonary arterial hypertension?

Pulmonary hypertension(PH) is a condition characterized by the elevation of the mean pulmonary artery pressure above 25 mm Hg and the pulmonary vascular resistance above 3 wood units. Pulmonary arterial hypertension(PAH) is an uncommon conditionwith severe morbidity and mortality, needing early recognition and appropriate and specific treatment. PH is frequently associated with hypoxemia, mainly chronic obstructive pulmonary disease and DPLD and/or left heart diseases(LHD), mainly heart failure with reduced or preserved ejection fraction. Although in the majority of patients with PH the cause is not PAH, a significant number of published studies are still in regard to group Ⅰ PH, leading to a logical assumption that PH due to other causes is not such an important issue. So, is there a reason to discuss PH other than PAH? Chronic lung diseases, mainly chronic obstructive lung disease and DPLD, are associated with a high incidence of PH which is linked to exercise limitations and a worse prognosis. Although pathophysiological studies suggest that specific PAH therapy may benefit such patients, the results presented from small studies in regard to the safety and effectiveness of the specific PAH therapy are discouraging. PH is a common complication of left heart disease and is related to disease severity, especially in patients with reduced ejection fraction. There are two types of PH related to LHD based on diastolic pressure difference(DPD, defined as diastolic pulmonary artery pressure- mean PAWP): Isolated post-capillary PH, defined as PAWP > 15 mm Hg and DPD 15 mm Hg and DPD ≥ 7 mm Hg. The potential use of PAH therapies in patients with PH related to left heart disease is based on a logical pathobiological rationale. Efficacy of prone position in acute respiratory distress syndrome patients: A pathophysiology-based review

Acute respiratory distress syndrome(ARDS) is a syndrome with heterogeneous underlying pathological processes. It represents a common clinical problem in intensive care unit patients and it is characterized by high mortality. The mainstay of treatment for ARDS is lung protective ventilation with low tidal volumes and positive end-expiratory pressure sufficient for alveolar recruitment. Prone positioning is a supplementary strategy available in managing patients with ARDS. It was first described 40 years ago and it proves to be in alignment with two major ARDS pathophysiological lung models; the "sponge lung"- and the "shape matching"-model. Current evidence strongly supports that prone positioning has beneficial effects on gas exchange, respiratory mechanics, lung protection and hemodynamics as it redistributes transpulmonary pressure, stress and strain throughout the lung and unloads the right ventricle. The factors that individually influence the time course of alveolar recruitment and the improvement in oxygenation during prone positioning have not been well characterized. Although patients' response to prone positioning is quite variable and hard to predict, large randomized trials and recent meta-analyses show that prone position in conjunction with a lung-protective strategy, when performed early and in sufficient duration, may improve survival in patients with ARDS. This pathophysiology-based review and recent clinical evidence strongly support the use of prone positioning in the early management of severe ARDS systematically and not as a rescue maneuver or a last-ditch effort.

Plant-to-plant variability is a crop stability *** objective of this study in maize(Zea mays L.)was to test the validity of the theoretical background of the hypothesis that the coefficient of variation(CV)for grain yield per plant and mean yield are connected exponentially,following the Taylor’s Power Law(TPL)*** experimentation was conducted across two sites,two seasons,and two planting *** were the main plots,corresponding to the typical practice of 8.89 plants m–2(TCD)and the low-input optimal of 5.33 plants m–2(LCD),while hybrids were the *** from 26 subplots in the first site averaged CV values of 22.6%at the TCD and 21.9%at the LCD,and mean yields of 19.1 and 13.9 t ha–1,respectively,following the TPL *** same was true for the second site,with average CVs and means of 41.5%and 14.3 t ha–1 at the TCD and 36.8%and 11.5 t ha–1 at the LCD.A test was performed on the simple correlation between the logarithms of variances and their respective means to investigate whether there is a systematic variance dependence on mean,thus questioning the reliability of *** validity of TPL was verified in the first ***,there was a systematic dependence of yield variance on mean yield in the second site,implying that the CV-yield correlation might be not biologically *** of the variance to remove its dependence on the mean did not validate the CV-yield negative relationship,meaning that caution is needed when interpreting the CV as a stability index for intra-crop *** the determinant factor of invalidity of TPL was the extensive intra-crop variation in the lower yielding second site can be assessed in future research.

The Barrier Properties of Flake-Filled Composites with Precise Control of Flake Orientation

Additive manufacturing, especially in the form of 3D printing, offers the exciting possibility of generating heterogeneous articles with precisely controlled internal microstructure. One area in which this feature can be of significant advantage is in diffusion control, specifically in the design and fabrication of microstructures which optimize the rate of transport of a solute to and from a contained fluid. In this work we focus on the use of flakes as diffusion-control agents and study computationally and theoretically the effect of orientation on the barrier properties of flake-filled composites. We conducted over 1500 simulations in two-dimensional, doubly-periodic unit cells each containing up to 3000 individual flake cross-sections which are randomly placed and with their axes forming an angle () with the direction of macroscopic diffusion. We consider long-flake systems of aspect ratio () 100 and 1000, from the dilute () and into the concentrated () regime. Based on the rotation properties of the diffusivity tensor, we derive a model which is capable of accurately reproducing all computational results ( and ). The model requires as inputs the two principal diffusivities of the composite, normal and parallel to the flake axis. In this respect, we find the models of Lape et al. [1] and Nielsen [2] form an excellent combination. Both our model and our computational data predict that at the quadratic dependence of the Barrier Improvement Factor (BIF) on () is lost, with the BIF approaching a plateau at higher values of (). This plateau is lower as () increases. We derive analytical estimates of this maximum achievable BIF at each level of misalignment;these are also shown to be in excellent agreement with the computational results. Finally we show that our computational results and model are in agreement with experimental evidence at small values of ().

Incidental finding of Zinner syndrome in a Greek military recruit: a case report of a rare clinical entity

Background: Zinner syndrome represents a rare congenital malformation of the urinary tract. It comprises a constellation of Wolffian duct anomalies and is almost exclusively encountered as a classic triad of seminal vesicle cysts, ejaculatory duct obstruction and renal agenesis. Patients can be either asymptomatic or symptomatic. Recently, minimally invasive surgical techniques have emerged, superseding traditional surgery for select symptomatic cases. Our case highlights the finding of a rare clinical syndrome that was incidentally detected during a routine mass screening of military recruits in the Greek Armed *** presentation: Herein, we present a case of a 19-year-old male who reported having a solitary right kidney when examined in a military training center of Northern Greece. No additional clinical information was available;thus, referral to a tertiary urology department for further investigation ensued. Imaging studies, namely, computed tomography and magnetic resonance imaging, revealed left renal aplasia, multiple left seminal vesicle cysts, and ejaculatory duct obstruction. Laboratory values and urinalysis were within normal range. Semen analysis was significant for cryptozoospermia. Our patient remained asymptomatic during the entire hospitalization. Long-term follow-up was recommended. Nevertheless, he declined further investigation and sought treatment in a private practice ***: This article aims to present the incidental diagnosis of a rare syndrome in a military setting. Population screening conducted in the armed forces permits the identification of undiagnosed diseases that warrant further investigation. To the best of our knowledge, this was the first report of Zinner syndrome in a military recruit and the second case cited of a Greek patient in the published literature. Regular follow-up is the key to timely intervention in conservatively managed cases.

Pain is a common symptom in humans, which differs significantly depending on their status of health. This review lists the specific features of pain that are necessary for its evaluation and treatment by the health professionals. Extensive review of the recent literature was conducted in electronic databases （PubMed, Medline and Iatrotek） and scientific journals in Greek and English. Pain is perceived as any uncomfortable feeling, and emotional experience that occurs anywhere in the body when there is some kind of injury. The assessment of pain in humans is a complex and demanding process. Pain should be treated early with appropriate pharmaceutical or other treatment. Each pain has a different cause and treatment. The objective of health professionals＇ interventions is to cure or at least reduce pain and to relieve those who are in pain.

Background: Patients with mitral stenosis(MS) and heart failure(HF) are characterized by changes in the left atrial(LA)-function and activation of the apoptotic process. The purpose of the present study was the evaluation of the effect of mitral valve replacement on the LA function, on inflammatory process and apoptotic markers in patients with MS and HF. Methods: We studied 30 patients with MS and HF(15 in NYHA III-IV and 15 in NYHA IV) in sinus rhythm(mean age 56.2±4.6 years), and 20 age and gender matched healthy volunteers. Blood samples were obtained before and 6 months after surgical mitral valve replacement, and plasma levels of soluble Fas/APO-1 receptor(sFas), tumor necrosis factor-alpha(TNF-a) and interleukin-6(IL-6) were measured. Echocardiographically, LA volumes were measured at mitral valve opening(Vmax), at the onset of left atrial systole(P wave of the electrocardiogram, Vp) and at the mitral valve closure(Vmin). LA contractile function was assessed by the LA active emptying fraction(ACTEF). Results: After mitral valve replacement, TNF-a, IL-6 and sFas levels, as well as the Vmax LA volume, were significantly reduced(p< 0.05). ACTEF showed a significant postoperative decrease(0.29±0.09 vs. 0.23±0.06, p< 0.01) and it was signifi cantly correlated with sFas(r=-0.88, p=0.001), TNF-a(r=-0.81, p=0.001) and IL-6(r=-0.74, p=0.001) levels. Conclusion: The present findings indicate that mitral valve replacement in patients with mitral valve stenosis, reduces the size of the left atrium, improves left atrial contractile function and depresses inflammatory and apoptotic process.
