Glioblastoma(GBM,WHO grade Ⅳ) is the most common and the most malignant primary brain tumor occurring during adulthood,with an annual incidence of 5 cases per 100 *** involves surgical resection,followed by radiotherapy and concomitant and adjuvant *** multimodality treatment,the median survival time is 15 *** we discuss the value of neuroimaging in differentiating GBM from other types of brain tumors,in guiding tumor biopsy,in making non-invasive assessment of tumor's aggressiveness,in estimating overall prognosis,in differentiating treatment-induced brain necrosis from tumor recurrence and in assessing response to treatment.

Intoduction: The adenocarcinomas of the appendix are rare tumors of the gastrointestinal truct that often reveal no clear clinical symptoms and are not diagnosed in time. In this study, we analyze a rare case of an incidentally discovered adenocarcinoma of the appendix after appendicectomy. Case presentation: A Greek male patient of 37 years old patient presented to our hospital with signs of acute appendicitis and peritonitis. He underwent appendicectomy as therapeutic treatment. Post operational histological examination revealed in situ carcinoma of the appendix without high hematological tumor markers or visible metastasis in CT scan analysis. We decided to proceed to right hemicolectomy. The patient was discharged without complications and without displaying other symptoms in a monitoring interval of two months after surgery. Conclusion: Adenocarcinomas of the appendix are generally difficult to detect in the early stage leading to significant morbidity and mortality rates. Therefore, it is necessary to execute a very careful histopathological examination and perform a detailed intraperitoneal cleaning during operation, so that any indications of appendix adenocarcinomas in cases of appendectomy won’t be missed or misjudged.

Aim:To evaluate the diagnostic value of technetium Tc99m-tetrofosmin(^(99m)Tc-TF)in primary cancers of the head and ***:Single photon emission computer tomography with planar imaging of the neck for primary site evaluation and whole body scanning for assessment of metastases in 12 patients with newly diagnosed head and neck ***-to-background index(T/Bg)was derived in patients with positive findings(tumor or lymph nodes).Results:The tomographic images showed increased tracer uptake in pathological sites(primary tumor or lymph node)in 9 patients(overall sensitivity 75%).Primary tumor was visualised in 7 patients(sensitivity 58%)and infiltrated lymph nodes in 4 out 7 patients(sensitivity 57%).Mean values for T/Bg index were 5.44±1.28 for primary tumor and 4.25±1.67 for lymph *** values for T/Bg index were 4.5±0.71 for patients with in situ and grade I carcinoma and 6.68±0.36 for patients with tumor grade II and III(P=0.034,Mann-Whitney U test).Conclusion:The present study demonstrates that ^(99m)Tc-TF is a valuable radiotracer for head and neck cancer *** determine the potential role of this imaging protocol in clinical practice will require a larger sample size.

Aim: The inflammatory response induced by perinatal infections and asphyxia is considered to participate in neonatal brain damage. Inflammatory responses are characterized by the expression of chemokines. Although chemokine levels have been investigated in healthy newborns, their role during neonatal pathological conditions has not been studied. The aim of our study was to examine chemokine serum levels in asphyxiated and infected neonates. Methods: Peripheral blood samples were obtained from perinatally asphyxiated and infected neonates during the first days of life and from neonates who developed nosocomial infections. Serum levels of interleukin-8 (IL-8), interferon-γ-inducible protein-10 (IP-10), monocyte chemoattractant protein-1 (MCP-1), macrophage inflammatory protein-1α(MIP-1α), and regulated upon activation, normal T cells expressed and secreted (RANTES) were determined. Results: In perinatally asphyxiated neonates, IL-8 levels were significantly elevated on the 1st day of life. In perinatally infected neonates, IL-8 and IP-10 levels were significantly increased on the 1st day of life, while RANTES levels were significantly lower and remained so until the 4th day. In nosocomially infected neonates, IL-8, IP-10 and MIP-1αlevels were significantly increased on diagnosis of infection. Conclusion: The neonatal immune system is able to produce chemokines for the induction of an inflammatory response during perinatal asphyxia and perinatal or nosocomial infections. Blockade of inflammatory chemokines could possibly contribute to the prevention of brain damage.

Increased endogenous nitric oxide production has been proposed as an important mediator of the peripheral arterial vasodilation and the hyperdynamic circulation in cirrhosis, whereas a decreased intrahepatic production of nitric oxide has been implicated in the pathogenesis of portal hypertension. The present study investigated the possible beneficial effects of methylene blue, which is a potent inhibitor of guanylate cyclase and nitric oxide synthase, on hyperdynamic circulation and renal function in cirrhotic patients with ascites together with the effects on portal hemodynamics. Twenty patients were evaluated at baseline and during 2 consecutive 4-hr periods after the administration of methylene blue at a dose of 3 mg/kg (10 patients) or placebo (10 patients). Mean arterial pressure, heart rate, cardiac output, systemic vascular resistance, plasma active renin, plasma aldosterone, plasma antidiuretic hormone, serum urea, serum creatinine, serum sodium, urinary flow rate, glomerular filtration rate, effective renal plasma flow, portal flow volume, and portal vein velocity were not modified by methylene blue or placebo. Urinary sodium excretion, fractional sodium excretion and serum nitric oxide levels were significantly decreased 4 hr after methylene blue administration (P < 0.05), to return toward basal levels over a further 4-hr period. It is concluded that methylene blue, at the dose used in the present study, has no effect on systemic and portal hemodynamics in cirrhotic patients with ascites. The reduction in renal sodium excretion, in the absence of changes in renal function and hemodynamics, suggests, at least partly, a direct antinatriuretic effect of methylene blue.

Dietary sodium restriction and diuretic treatment have been shown to be effective in the treatment of ascites in the majority of cirrhotic patients. However, approximately 5 to 10%of patients develop refractory ascites, which is defined as ascites that does not respond to intensive diuretic therapy (diuretic-resistant) or ascites that cannot be controlled because the patient develops diuretic-induced complications that prevent the use of an effective diuretic dose (diuretic-intractable). Current therapeutic approaches for refractory ascites include repeated large-volume paracentesis and transjugular intrahepatic portosystemic shunting. In the present report, subcutaneous octreotide treatment improved renal function and hemodynamics and diuretic response in two patients with refractory ascites in line with a marked decrease in renin and aldosterone secretion. We consider that octreotide could be of value in the management of refractory ascites in cirrhotic patients.
