The objective of this study was to evaluate the expression of estrogen receptors （ER（（α ） and ER（β）） and androgen receptors （ARs） as prognostic factors for biochemical recurrence, disease progression and survival in patients with pT3NOMO prostate cancer （PCa） in an urban Greek population. A total of 100 consecutive patients with pT3NOMO PCa treated with radical prostatectomy participated in the study. The mean age and follow-up were 64.2 and 6 years, respectively. The HSCORE was used for semi-quantitative analysis of the immunoreactivity of the receptors. The prognostic value of the ER（（α） and ER（β） and AR was assessed in terms of recurrence, progression, and survival. AR expression was not associated with any of the above parameters; however, both ERs correlated with the prognosis. A univariate Cox regression analysis showed that ER（α） positive staining was significantly associated with a greater hazard for all outcomes. Increased ER（β） staining was significantly associated with a lower hazard for all outcomes in the univariate analysis. When both ER HSCORES were used for the analysis, it was found that patients with high ER（α） or low ER（β） HSCORES compared with patients with negatively stained ER（α） and 〉1.7 hSCORE ER（β） had 6.03, 10.93, and 10.53 times greater hazard for biochemical disease recurrence, progression of disease and death, respectively. Multiple Cox proportional hazard analyses showed that the age, preoperative prostate specific antigen, Gleason score and ERs were independent predictors of all outcomes. ER expression is an important prognosticator after radical prostatectomy in patients with pT3NOMO PCa. By contrast, AR expression has limited prognostic value.

Purpose: To evaluate the rate of any type of anticoagulant drug use in urological inpatients and patients awareness of their effect on coagulation. Material and methods: This observational study was conducted prospectively in a cohort of 193 consecutive urological inpatients who were asked to state the medications they were taking and following that, were specifically asked whether they were taking aspirin or other antiplatelet/anticoagulant agents. In case they did so, they were further asked why they were taking them, whether they knew their effect on coagulation and who had informed them on the matter. Results: Forty-seven patients received some kind of antithrombotic treatment. Twenty-nine per cent of aspirin users had to be specifically prompted in order to state its use, in comparison to 35.7% and 25% of other antiplatelets and warfarin users, respectively. Half of patients receiving warfarin were not aware of its effect on coagulation in comparison to 32.3% and 21.4% of those taking aspirin and other antiplatelets, respectively. Conclusion: Urologists should be aware of the high use of such agents by their patients and that not all patients are aware of their effect on coagulation, while some, even fail to report their use and have to be specifically prompted.

Aim: Aim of our study was to evaluate the effects of interval exercise training (IT) programs, regarding whole body oxygen uptake and peripheral tissue oxygenation, in Chronic Heart Failure (CHF) patients during recovery. Methods: Twenty-six CHF patients (21 males/5 females), mean age of 49 ± 12 years, participated in the study. Fifteen patients were assigned to IT and 11 patients were assigned to IT followed by strength training. All patients were trained for 40 minutes per session, 3 times per week, for 12 weeks. They performed a symptom-limited cardiopulmonary exercise testing (CPET), before and after the completion of the program. Muscle tissue oxygen saturation (StO2) of quadriceps femoris was continuously measured by Near Infrared Spectroscopy (NIRS) during CPET and during the recovery period after the end of exercise. Results: No differences were noted between the two patient groups regarding whole body and peripheral tissue oxygenation indices and, therefore, data from all patients were pooled. After training, an increase in peak oxygen uptake (17 ± 4.5 to 19 ± 5.5 ml/kg/min, p < 0.05), gas exchange threshold (11 ± 3.5 to 12.5 ± 3.5 ml/kg/min, p < 0.05), peak work rate achieved (105 ± 29 to 124 ± 37 Watt, p < 0.05) and the first degree slope of VO2 at the first minute of recovery (0.45 ± 0.2 to 0.61 ±0.3 L/min2, p ) was noted. In addition, tissue re-oxygenation time constant was decreased (65 ± 25 to 52 ± 28 sec, p < 0.001). Conclusion: In conclusion, interval exercise training accelerates oxygen uptake and peripheral tissue oxygenation during recovery from exercise in CHF patients.

The aim of this study is to estimate externalities created round a cultural heritage preservation site.A research was conducted concerning the ruins of an ancient‘Sacred Way’(Iera Odos)located in Attica,*** sample of the research was 200 citizens(interviewees).It is used the CVM(Contingent Valuation Method)and Logit model of the regression *** preservation of cultural heritage is entailing excessive cost(paid by people through taxation)while is a source of additional income for both,the State and the people,due to *** the evaluation of this good cannot be in market terms,authors apply a modified version of the CVM(Contingent Valuation Method).The findings show there is strong evidence that at the 5%significance level,WTP(against WTA)is a better preferred course of action i.e.,leaving the ruins situation as it is,performing only the necessary remediation,proceeding with radical *** statistical processing of answers,obtained through a properly designed/circulated questionnaire,was carried out by Logit model regression *** model gave significant(at 0.05 levels)dependence of WTPar(Willingness to Participation)on preferred course of action(i.e.,leaving the ruins situation as is,performing only the necessary remediation,proceeding with radical restoration).

Background: Intermittent dobutamine infusions(IDI) combined with oral amiodarone improve the survival of patients with end-stage congestive heart failure(CHF). The purpose of the present study was to evaluate whether the response to longterm treatment with IDI + amiodarone is different in patients with ischemic heart disease(IHD) versus idiopathic dilated cardiomyopathy(IDC).Methods: The prospective study population consisted of 21 patients with IHD(the IHD Group) and 16 patients with IDC(the IDC Group) who presented with decompensated CHF despite optimal medical therapy, and were successfully weaned from an initial 72-h infusion of dobutamine. They were placed on a regimen of oral amiodarone, 400 mg/day and weekly IDI, 10 μ g/kg/min, for 8 h. Results: There were no differences in baseline clinical and hemodynamic characteristics between the 2 groups. The probability of 2-year survival was 44% in the IDC Group versus 5% in the IHD Group(long-rank, P=0.004). Patients with IDC had a 77% relative risk reduction in death from all causes compared to patients with IHD(odd ratio 0.27, 95% confidence interval 0.13 to 0.70, P=0.007). In contrast, no underlying disease-related difference in outcomes was observed in a retrospectively analyzed historical Comparison Group of 29 patients with end stage CHF treated by standard methods. Conclusions: Patients with end stage CHF due to IDC derived a greater survival benefit from IDI and oral amiodarone than patients with IHD.
