Introduction: Birth spacing to prevent closely-spaced pregnancies, which are a source of maternal and neonatal morbidity and mortality, is not always respected by our women, as recommended by the World Health Organization (WHO). The objective was to study the factors associated with closely-spaced pregnancies among women followed at the Departmental Hospital Center du Zou et du Colline (DHC/Z-C) in 2022. study Method: This was a descriptive, analytical cross-sectional study with prospective data collection from 20 May to 30 June 2022. Results: At the end of the study, 83 of the 117 women surveyed had close pregnancies, a frequency of 70.94%. They had an average age of 27.0 ± 6.3 years and were of Fon ethnicity and related in 91.57% of cases. The majority were married (96.60%). The factors significantly associated with these closely spaced pregnancies were age between 25 and 30 years, household occupation, age at first birth, age at marriage, women’s perception of contraceptive method (CM), women’s knowledge of the existence of a center providing family planning services and non-use of MC. Conclusion: Close pregnancies are still very common in the commune of Abomey. It is therefore important to increase the use of contraceptive methods in this part of the country.

Background: Maternal death is a major public health problem worldwide, particularly in sub-saharan Africa. Objective: This study sought to investigate dysfunctions in the management of patients whose outcome was classified as “maternal death” in the Gynaecology-Obstetrics section of the Departmental University Teaching Hospital of Borgou Alibori (CHUD-BA) from 2017 to 2021. Method: This was a retrospective cross-sectional study with descriptive and analytical purposes. The study population consisted of pregnant women, parturients and puerperas admitted into the CHUD-BA maternity ward from 2017 to 2021. Result: A total of 2011 patients were included in this study. The in-hospital maternal mortality ratio was 1526 per 100,000 live births. The dysfunctions identified were the amount of time spent in the referring center (more than 48 hours) (p = 0.001), delay of more than 2 hours between referral and admission into the referral center (p s of transport (motorcycle or public transport) (p compliance to the protocol for emergency obstetric and neonatal care (sONU) (p s in the etiological management of pregnant women (p Conclusion: Particular attention should be paid to the management of pregnant women in our healthcare system if we are looking forward to reducing maternal mortality.

Introduction: Uterine fibroids are the most common gynecological tumors in women of childbearing age and have multiple clinical manifestations. Objective: To determine factors associated with uterine fibroids in the Borgou department of Benin in 2022. Method: Descriptive, analytical cross-sectional study from August 22 to september 22, 2022. The sample consisted of women consulting for gynecological problems in 6 hospitals in the Borgou department in the North of Benin. Results: Of the 744 women surveyed, 10.1% were diagnosed with uterine fibroids. Women with uterine fibroids were often married, paucigesture, and nulliparous. Their main reasons for consultation were the desire for motherhood (50.7%), pelvic pain (46.7%), and genital hemorrhage (14.7%). Age, history of miscarriage, high blood pressure, and diabetes were significantly associated with the presence of uterine fibroids (Conclusion: Uterine fibroids are common in hospitals in northern Benin. There were factors associated with them that should be taken into account in management strategies.

Introduction: Breast cancer mortality remains high in most developing countries. Benin Republic does not yet have a technical platform required for the diagnosis and effective management of breast cancer “according to the recommendations”. Objective: To study the epidemiological, diagnostic and therapeutic aspects of locally advanced and/or metastatic breast cancers at the Centre Hospitalier Universitaire-de la Mère et de l’Enfant Lagune (CHU-MEL) of Cotonou from 2018 to 2021. study Methods: This was a retrospective cross-sectional study with a descriptive purpose, carried out from January 1, 2018 to september 30, 2021 in the Gynaecology-Obstetrics section of the CHU-MEL of Cotonou. The study population was patients diagnosed with locally advanced and/or metastatic breast cancers. Results: At the end of the study, 63 cases of locally advanced and/or metastatic breast cancer were recorded in 80 cases of breast cancer;a 74.11% prevalence of locally advanced and metastatic breast cancer. The mean age of the patients was 42.6 years. The main reason for consultation was the discovery of a mass in the breast (65.1%). Clinically, an orange peel was noted in 39.6%. Mammography (47.6%) and breast ultrasonography (42.9%) were the reference imaging workups for breast cancer diagnosis. Histopathologically, infiltrating ductal carcinoma (94.1%) was the predominant type with scarff-Bloom and Richardson (sBR) grade II found in 35.3%. Progesterone and estrogen receptors were present in 42.9%. Mastectomy with axillary node dissection was the surgical treatment performed (80%) after neoadjuvant chemotherapy (39.6%). Only one patient had received radiotherapy. No patient received hormone therapy. Conclusion: Locally advanced and metastatic cancers remain a real public health problem. It is essential that major investments be made in order to improve both their diagnosis and management.

Background: Obstetric Fistula (OF) is a breach of the birth canal due to obstructed labor. It is a major public health problem in developing countries. Objective: study the epidemiology and treatment of obstetric fistulas at the Departmental University Hospital Centre of Borgou and Alibori (CHUD-B/A) in Parakou, Benin. study Method: This was a descriptive and analytical cross-sectional study with prospective data collection on women treated during OF care missions from February 2020 to April 2021 in CHUD-B/A in Parakou, Benin. The data concerned their socio-demographic characteristics, obstetric history, the clinic and therapeutic characteristics of their obstetric fistulas. Results: In total 97 patients with OF were treated during the 14-month period. The average age of the patients was 36.42 ± 11.75 years old. The patients were married (51.54%), without professional occupation (54.64%) and had no formal education (73.20%). The vesico-vaginal variety was predominating (54.64%). The fistulas were of Type I (46.39%), Type II (20.62%) and Type III (32.99%) according to the classification of Waaldjik Kees. The patients were operated on under spinal anesthesia (94.79%), and vaginally (55.67%) according to the principle of Chassar Moir (68, 60%). The associated procedures were lengthening urethroplasty (17.53%), and interposition of the Martius flap (1.03%). The overall success rate without stress Urinary Incontinence was 71.13%. The factors associated with the failure of the fistula repair were: the nutritional status of the patients (p = 0.004), the previous repair failure (p = 0.001), the high size of the fistula (p = 0.007), the fistula severity (p and a urethral reconstruction (p Conclusion: OF is a relatively frequent pathology in Benin. Vesico-vaginal fistula is the most common form. The success rate of the surgical treatment is satisfactory, but depends on some factors.

Background: Breast cancer mortality remains high in the majority of developing countries. The Ministry of Health has established two population-based cancer registries in Benin: the first one in Cotonou in 2014 and the second one in Parakou in 2017. However, there is a scarcity of data on breast cancer survival and prognosis in Benin Republic. Objective: This study sought to investigate epidemiological, diagnostic, and survival aspects of breast cancer in Parakou, based on data from its population-based cancer registry from 2017 to 2021. Method: For descriptive and analytical purposes, we used a retrospective cohort design. From January 24, 2022 to August 31, 2022, data were collected in all health facilities covered by the Parakou population-based cancer registry using an individual questionnaire. survival and prognosis analysis were performed using KAPLAN MEIER method and David COX proportional hazard model respectively. Result: A total of 81 patients have been included in this study. The incidence rate of breast cancer in Parakou was 17.5 per 100,000 person-years with a mortality rate of 2.76 per 100,000 person-years. The median age at diagnosis was 44.50 years with extremes ranging from 19 to 76 years and a predominance of 40 - 50 years age group. The median survival time was estimated at 30 months with an overall 5-year survival of 47%. Young age at diagnosis (p-value = 0.002) and advanced stage at diagnosis (p-value = 0.000) had a negative impact on survival in women. The combination of surgery and chemotherapy improved survival (p-value = 0.018). Conclusion: Breast cancer is still a public health issue in Parakou. It comes out mandatory that resources be made available to make screening, early diagnosis and appropriate treatment of breast cancer affordable.

Introduction: Peripartum cardiomyopathy (PPCM) is a dilated cardiomyopathy occurring in the last month of pregnancy or the first five months postpartum without pre-existing cardiovascular pathology. It is a major cause of pregnancy-related heart failure with high morbidity and mortality. In severe forms (10% to 15% of cases), thrombo-embolic complications are the main cause. The initial hemodynamic evolution is totally unpredictable and sometimes extremely brutal and fatal. The objective of this work was to show the often pejorative evolution of PPCM in our country. Methods: We report in this work three serious clinical cases revealing the complications of this PPCM among patients admitted to the cardiology department of the CHUD-B/A in 2022 for heart failure. The data were collected according to the Declaration of Helsinki. Patients and Observations: The first case was a PPCM with severe left ventricular (LV) systolic dysfunction complicated by spontaneous left intraventricular contrast and right superficial sylvian ischemic stroke. The second case reports a global cardiac decompensation of a PPCM with severe LV systolic dysfunction complicated by an apical thrombus. The third case is that of a state of cardiogenic shock complicating a PPCM with severe LV systolic dysfunction. Among our 03 patients presenting these severe forms of PPCM, the evolution, in spite of the symptomatic and prognostic treatments of the heart failure, and even of the complications, was unfavourable with death in two of them. Conclusion: Complications of PPCM are frequent and fatal in Benin.

Maternal Hospital Mortality in Cotonou: Incidence, Care-Associated Adverse Events and Causes

Introduction: Despite the many efforts made to combat preventable maternal deaths, these still remain high in Benin. It was therefore important to revisit the causes but especially the adverse events associated with care (EIs) in maternity. Objective: Determine the incidence, adverse events associated with care and causes of intra hospital maternal mortality in cotonou. Methods: This was a cross-sectional maternal mortality review study with retrospective data collection. It covered all cases of maternal death recorded between 2017 and 2021 in two (2) reference university maternities in Cotonou. Adverse events associated with care and the patient were analyzed using the maternal death audit grid validated for Benin. sPss.26 software was used for data analysis. Results: The in-hospital maternal mortality ratio in Cotonou was 2028 maternal deaths per 100,000 live births in 2021. Only 7.2% (n = 36) of deaths were audited. The deceased mothers were 29.8 ± 7.4 years old, with no fixed monthly income in 82.7% (n = 420). serious adverse events associated with care were: delay in decision to refer in 37% (n = 188), non-medical referral in 85.8% (n = 436) of cases, inadequate pre-referral treatment in 25.7% of cases. In receiving maternities, delay in diagnosis and inappropriate treatment at the receiving maternity were noted respectively in 22.9% and 28.6% of cases of maternal death. The direct causes of maternal death were dominated by serious obstetric hemorrhage in 43.9% (n = 223). As for the indirect causes, they were dominated by anemia excluding obstetric hemorrhages in 21.5% (n = 109). Conclusion: The in-hospital maternal mortality ratio was very high in Cotonou. The main cause was severe obstetric hemorrhage. There were several serious healthcare-associated adverse events whose correct management would significantly reduce the incidence of maternal deaths.

Introduction: Gynaecological cancers are the deadliest of the women’s cancers in the Republic of Benin. Late diagnosis is the most common reason. Objective: This paper aims to describe the epidemiological characteristics, and clinical and pathological signs of gynaecological cancers treated in the Republic of Benin between 2018 and 2022. Patients and Methods: This was a cross-sectional, descriptive, retrospectively collected study of patient data treated between 2018 and 2022 in two university gynaecology departments in Cotonou. All gynaecological cancers that have histological evidence were included. The epidemiological, clinical and pathological characteristics of the cancers were assessed. Results: Cervical, endometrial and ovarian cancers were the most common in the proportions of 62.0%, 24.1%, 12.0% and 1.8% respectively. The mean age at diagnosis was 54 years. The victims were uneducated and had low economic power in 81% and 85% of cases, respectively. The consultation was late in 82.1% of cases. Metrorrhagia, postmenopausal metrorrhagia and pelvic cluster headache were the common reasons for consultation for cervical, endometrial and ovarian cancer, respectively. Diagnosis was late in 66.7% (n = 71). The most common histological types were squamous cell carcinoma, endometrioid adenocarcinoma, and serous cystadenocarcinoma for cervical, endometrial, and ovarian cancers, respectively. Conclusion: Gynaecological cancers were common and their consultation time was delayed. The diagnosis was made at the advanced stage and there were several reasons for this.
