Adult Onset Still’s Disease in Tropical Area: Illustration of Diagnostic and Therapeutic Difficulties from 3 Senegaleses Observations

Introduction: The Adult Onset Still’s Disease (ASD) is a systemic auto-inflammatory affection of unknown cause seldom described in sub-Saharan Africa. We report 3 observations of ASD illustrating the diagnostic and therapeutic difficulties of this affection in our areas. Observation 1: Our first patient is a 56 years old schoolteacher presenting an ASD in its chronic articular form. She had been followed for an inflammatory arthralgia for 10 years and of the pharyngal pains without exact diagnosis. She presented ASD’s criteria of Yamaguchi and of Fautrel. The prednisone was begun at the dose of 0.8 mg/kg/day with fast appearance of a progressive muscular weakness. Use of methotrexate at a rate of 15 mg per week, associated with low dose of prednisone was effective in long-term without any flare of the disease so far. Observation 2: Our second patient is a 30 years old dressmaker presenting an ASD in its complicated systemic form of lymphohistiocytic activation syndrome. She validated the criteria of Yamaguchi and Fautrel for ASD. She also presented resistance to corticosteroid therapy. The evolution was marked by a hospital-acquired septicemia and a multi-organ failure leading to death. The diagnosis was retrospectively confirmed after that, with the low level of the glycosylated ferritin serum value. Observation 3: The third patient is a 22 years old Guinean student who presented prolonged fever and inflammatory polyarthralgia without articular deformation. He had been misdiagnosed for ASD with diagnostic wandering of several months. He was treated successfully with prednisone after set up of ASD diagnosis according common criteria. Corticosteroid therapy was stopped after 8 months without any relapse noted so far. Conclusion: Caring for ASD is difficult in our context mainly because of high cost of several explorations needed to set up its exact diagnosis while making differential one. Introduction: We assess the severity of the rheumatoid arthritis in a Senegalese African black population. Patients and methods: It is a retrospective study achieved in the service of Internal Medicine of Aristide Le Dantec teaching hospital of Dakar between January 2005 and December 2016 in patients suffering from rheumatoid arthritis. We specified for every patient the predictive data of severity of the rheumatoid arthritis. Results: Four hundred and three patients have been gathered (39 men and 364 women), with the mean age of 45.8 years. An active tobacco addiction was noticed in 10 patients. The diagnostic delay was on average of 72 months. Characteristic articular deformations were noticed in 215 patients (53.3%). They were correlated to male (p = 0.038), to age (p = 0.001) and to the activity of the rheumatoid arthritis (p = 0.0445). Systemic manifestations have been observed in 213 cases (52.9%), particularly anemia (50.8%). They were correlated to the anti-CCP antibodies (p = 0.047). The ESR was increased at the first hour in 84.4% of cases (median: 43 mm;extreme: 1and 160). CRP was elevated in 63.71% of cases (median of 12 mg/l;extreme: 1 and 384). The rheumatoid factor was positive in 57.6% of the cases. The anti-CCP antibodies were present in 89.2% of the cases. Articular erosions were objectified in all cases. A DAS 28 superior to 5.1 was noticed in 71% of cases. Conclusion: The rheumatoid arthritis was severe in our study.

Introduction: Immunosenescence contributes to the development of auto-antibodies. However, while the prevalence of some autoantibodies increases with age, the incidence of connective tissue diseases decreases with age. This study aims to determine the clinical and paraclinical characteristics of connective tissue diseases in the elderly population. Materials and Methods: We conducted a retrospective and descriptive study, from March 2010 to March 2021, in the Internal Medicine Department of Aristide Le Dantec Hospital, including all the medical records of patients followed up for connective tissue disease and whose symptomatology began at an age greater than or equal to 65 years. Epidemiological, clinical, paraclinical, therapeutic and evolutionary data were collected from a pre-established survey form and then processed using SPSS software. Results: Overall, 22 cases were collected out of 275 seen on consultation. They involved 16 women and 6 men, aged between 65 and 85 years with a median of 70 years. The mean duration of the clinical picture was 15.7 months with extremes of 1 and 96 months. The clinical symptoms were joint pain in 21 patients associated with dry eye and mouth syndrome in seven cases and one patient underwent isolated dry eye and mouth syndrome. Joint involvement was deforming in 13 patients. Poor general condition was noted in 20 patients. Biology showed anemia in 14 patients, hyperleukocytosis (2 patients) and thrombocytosis (5 patients). The sedimentation rate (SR) was accelerated in 9 patients and the C-reactive protein (CRP) was positive in 12 patients. Immunology showed a positive Latex test (3 patients/6), Waaler-Rose reaction (8 patients/13), anti-cyclic citrullinated peptides (anti-CCP) antibodies (11 patients/11), anti-nuclear antibodies (2 patients/4). Dynamics of Land Salinization in the Commune of Fimela (Fatick, Senegal) from 1973 to 2020

In Senegal, the agricultural sector remains a major component of the economy and national growth. As the main subsistence activity for 60% of the population, agricultural activity is essential to reduce poverty and ensure food security for the population. In this context, land degradation is a major constraint. In the Fatick region, in the commune of Fimela, land salinization is a worrying environmental problem. The purpose of this study is to understand the dynamics of soil salinization in Fimela in the context of climate change that tends to modify the evolution of landscapes. It required direct observations in the field, socio-economic surveys based on a questionnaire administered in six (6) villages. The processing of the data from these surveys was carried out with Sphinx software for the extraction of data in numerical form and SPSS to carry out the correlations between the collected variables. Excel was also used to perform calculations and make tables and graphs. In addition, the acquisition and processing of Landsat multi-spectral satellite images from 1973, 1988 and 2020 allowed us to observe the evolution of landscape units according to determining climatic events such as drought. The areas of tans have experienced a positive evolution during the period 1973-2020 with an increase of 163.11 hectares or an evolution rate of 7.47%. The localities most affected are Ndangane, Fimela, Djilor, Simal and the villages of the Mar Islands. The overall dynamics of cultivable land are marked by a decline with a rate of change of −18%. Despite the multiple reforestation campaigns, the mangrove has recorded a continuous decrease of 54.58% or a loss estimated at 5335.59 ha during the period 1973-2020. Finally, the analysis of the results of our study shows that land salinization is a determining element of the dynamics of land use and deteriorates the already precarious living conditions of rural populations and compromises the future of the agropastoral production syste

Management of Multiple Endocrine Neoplasia Type 2A (MEN 2A): Diagnostic and Therapeutic Concerns with the First Documented Senegalese Family

Introduction: Multiple endocrine neoplasia (MEN) type 2A is a multiglandular tumor condition inherited in an autosomal dominant manner. It is related to proto-oncogene RET mutation whose analysis is the best technique for family screening. It features in a variable way medullary thyroid cancer (MTC), primary hyperparathyroidism (HPT) and pheochromocytoma. The revealing manifestations of these tumors are often neglected for a long time and the screening should be systematic particularly in a known family context. Methods: After a family tree establishment of a MEN 2A index case, a family survey allowed to diagnose other cases in the family by means of biological, radiological and/or genetic examinations. Results: We report a family form of MEN 2A in a family of three households. In this family 13 people (index case included) were probed out of 34 members. The average age of our patients was 43.54. The sex ratio men/women was 0.85. The simultaneous diagnosis of a primary HPT and a MTC was carried out in our index case and constituted the circumstance of discovery of MEN 2A. The time limit of MEN 2A diagnosis on the other family members was on average 7.7 years. A MTC was recorded in 7 patients. It was asymptomatic in overall cases. A pheochromocytoma was present in only one patient. Primary HPT was found in four patients. Renal lithiasis with recurrent unilateral or bilateral nephritic colic attacks was the main manifestation. Besides the index case, 11 patients had a genetic testing. In 7 patients, a mutation on proto-oncogene RET located on the codon 634 was noted. A surgical care was carried out on 6 patients. We recorded three patients lost to follow-up. A patient died before surgery. In the index case, biological and radiological monitoring found a locoregional residual disease that indicated surgical revision and radiotherapy. Poisson Process and Its Application to the Storm Water Overflows

The homogenous Poisson process is often used to describe the event arrivals. Such Poisson process has been applied in various areas. This study focuses on the arrival pattern of storm water overflows. A set of overflow data was obtained from the storm water pipeline of a municipality. The aim is to verify the overflow arrival pattern and check whether the Poisson process can be applied. The adopted method is the analysis over the inter-arrival times. The exponential distribution test is conducted on the annual data set as well as the entire data set. The results show that all data sets follow the exponential distribution. With the verification of Poisson process, specific examples are also given to show how the Poisson process properties can be used in the management of storm water pipeline management. For other data that are featured with various heterogeneities, the homogenous Poisson process might not be able to be verified and used. Under such circumstances, non-homogenous survival model can be used to simulate the arrival process.

Study of HIV Prevalence and Behaviors in Rural Areas of Goudomp Health District (Senegal)

This study aims to determine the factors related to HIV-positive status in people aged 15 to 49 years of age in the commune of Niagha in Goudomp. The study was descriptive and analytically cross-cutting and had taken place in a release of 2018. The data was collected on anonymous questionnaires and then analyzed using R software after entering with Epi 7 software. Bivariate analyses and logistical modelling have been used to investigate the factors associated with HIV status. The sample included 400 people, and a participation rate of 100% was obtained in relation to the administration of the questionnaire. For the detection of HIV infection, 318 people agreed to do so, a proportion of 79.7%. The average age of the respondents was 27.8 years with a standard deviation of 9.1 and extremes of 15 to 49 years. 21.5% of respondents were under 20 years of age, the median was 26 years. With 58.8% of women, the sex ratio (M/F) was 0.7. In this study, 11.5% of respondents did not believe in the existence of HIV infection, 92.0% were sexually active, and 40.5% had sex with casual partners. The main places for casual sex were cultural events (84.0%), weekly markets (53.1%), religious events (27.8%) and during travel (22.8%). Among them, only 20 respondents, or 12.3%, declared that they systematically protect themselves with a condom during these occasional sexual relations. This wearing of condoms was found in 20.7% of singles and 10.7% of married couples having casual sex. In this study, 60.0% of the respondents declared that they had paid for sex, although this practice is not cited in income-generating activities. Non-consensual sex was found in 6.5% of the respondents. In this study, 319 people agreed to be tested for HIV, or 79.7% of the sample. HIV infection was found in 19 people, 6.0% seroprevalence, with 16 cases of HIV 1 infection and three cases of HIV 2 infection. Several risk behaviors were found in this study. The Scalp Involvements in the Connective Tissue Diseases

Background: The involvements of the scalp over the connective tissue diseases (lupus, sclerodermia and dermatomyositis) are often the present complaint due to the disfigurement they cause. The aim of our study was to identify the various involvements of the scalp over the connective tissue diseases. Method: We conducted a descriptive study over a period of seven months of every instance of involvement of the scalp and hair during connective tissue diseases received in the department. A tensile test, a Trichogram and mycological sampling were performed. Results: 25/35 involvements of the scalp were noted (71.42%);all of them were female and the mean age was 32 years. It was a lupus in 17 cases, sclerodermia in 6 cases, dermatomyositis and a sclerodermatomyositis. They were the present complaint in 48% of cases within 3 years and preceding the other lesions in 32% of cases. We noted fragile and dry hair in 10 cases, straightened hair in 8 cases and hypochromic spots fleck in 5 cases. Conclusion: Connective tissue diseases cause severe injuries and often reveal a risk of permanent cicaticial alopecia and fungal superinfection. The cicatricial alopecia and straightened hair are secondary to immune disorders.

INTRODUCTION: An adenopathy is a pathological hypertrophy of a lymph node of various etiologies requiring a rigorous approach. Thus we proposed in this work to study the etiological particularities of patients admitted for adenopathy in order to identify their specificities. METHODS: This was an observational study of a transverse and descriptive type, which took place from 1 July 2015 to 30 June 2016 in the internal medicine department of the Aristide Le Dantec national hospital in Dakar. RESULTS: We identified 84 patients, consisted of 46 male and 38 female. The sex ratio was 1.2. The mean age was 37.5 years. The consultation period was on average 2 months with extremes of 1 month and 1 year. The mean hospital stay was 29. 2 days with extremes of 1 week and 2 months. The etiologies found were classified into 6 groups. Hemopathies (30.9%) were dominated by acute leukemia in 8 patients (9.52%), non-Hodgkin’s lymphoma in 9 patients (9.5%), Hodgkin’s disease in 5 patients (5.9%). Chronic lymphocytic leukemia was retained in 3 patients (3.5%) and Castleman disease in 1 patient. Systemic diseases (11.9%) were divided into lupus disease in 4 patients and rheumatoid arthritis in 4 patients, one case of Sj?gren’s syndrome and one case of sarcoidosis. Infections were very frequent in our study, found in 34 patients (40.4%). Tuberculosis was more frequent (27.3%). The association with HIV was noted in 4 patients. The other infections were pyogenic in 3 patients (3.5%), HIV in 6 patients (7.1%), Borrelia and toxoplasma in 1 patient respectively. Cancers (8.3%) were metastatic of primary tumors including the esophagus, lung, prostate, cavum, stomach, pancreas and breast, one case each. One patient presented lymphadenopathy with inguinal localization, the etiology of which was found to be thrombophlebitis of the right lower limb.
