The impact of chemotherapy-naïve open radical cystectomy delay and perioperative transfusion on the recurrence-free survival: A perioperative parameters-based nomogram

Objective: To develop and internally validate a nomogram to predict recurrence-free survival (RFS) including the time to radical cystectomy (RC) and perioperative blood transfusion (PBT) as potential ***: Patients who underwent open RC and ileal conduit between January 1996 to December 2016 were split into developing (n=948) and validating (n=237) cohorts. The time to radical cystectomy (TTC) was defined as the interval between the onset of symptoms and RC. The regression coefficients of the independent predictors obtained by Cox regression were used to construct the nomogram. Discrimination, validation, and clinical usefulness in the validation cohort were assessed by the area under the curve, the calibration plot, and decision curve ***: In the developing dataset, the 1-, 5-, and 10-year RFS were 83.0%, 47.2%, and 44.4%, respectively. On multivariate analysis, independent predictors were TTC (hazards ratio [HR] 1.07, 95% confidence interval [CI] 1.05-1.08, p<0.001), PBT (one unit: HR 1.40, 95% CI 1.03-1.90, p=0.03;two or more units: HR 1.72, 95% CI 1.29-2.29, p<0.001), bilateral hydronephrosis (HR 1.54, 95% CI 1.21-1.97, p<0.001), squamous cell carcinoma (HR 0.60, 95% CI 0.45-0.81, p=0.001), pT3-T4 (HR 1.77, 95% CI 1.41-2.22, p<0.001), lymph node status (HR 1.53, 95% CI 1.21-1.95, p<0.001), and lymphovascular invasion (HR 1.28, 95% CI 1.01-1.62, p=0.044). The areas under the curve in the validation dataset were 79.3%, 69.6%, and 76.2%, for 1-, 5-, and 10-year RFS, respectively. Calibration plots showed considerable correspondence between predicted and actual survival probabilities. The decision curve analysis revealed a better net benefit of the ***: A nomogram with good discrimination, validation, and clinical utility was constructed utilizing TTC and PBT in addition to standard pathological criteria.

Evaluation of CD64 Index in HBV and in Chronic HCV Infections

Background and aims: CD64 [Fc gamma receptor 1 (FcγRI)] is a promising biomarker used in predicting severe bacterial infection. The study was designed to assess their level in all stages of HBV infection and in chronic HCV infection before and after treatment with direct acting antiviral therapy as a possible biomarker of inflammation. Patients and methods: A case-control study was conducted, 50 patients with different disease stages of HBV infection (10 acute, 15 chronic hepatitis, 15 liver cirrhosis (LC) and 10 with hepatocellular carcinoma (HCC)), twenty patients with chronic HCV and 15 as a control group. Laboratory and imaging studies were evaluated. The levels of CD64 expressions in peripheral blood and CD64 Index were measured for all patients by flowcytometry using fluorescein isothiocyanate (FITC)-conjugated anti-CD64 monoclonal antibody. Results: The levels of CD64 expressions in peripheral blood and CD64 index were significantly higher in patients with HBV and HCV than in control group (P value = 0.01, 0.01 and 0.000, 0.000 respectively). They were increased significantly with disease progression in patients with HBV infection, acute hepatitis B infection showed the highest values. Their levels were significantly decreased in patients with HCV infection post treatment than before treatment. Conclusions: The levels of CD64 expressions in peripheral blood and CD64 index are considered good biomarkers of inflammation in viral hepatitis both B and C and could detect disease progression and also suppression of inflammation after antiviral therapy.

Egyptian heavy vacuum gas oil hydrotreating over Co-Mo/CNT and Co-Mo/γ-Al2O3 catalysts

The catalytic activity of CoMoS /CNT towards the Egyptian heavy vacuum gas oil hydrotreating was studied. The delivered CNT was functionalized with 6 mol /L HNO_3. The CNT were loaded with 12% MoO_3( by weight) and 0.7 Co /Mo atomic ratio with impregnation methods. The γ-Al_2O_3 catalyst was also prepared by impregnation method to compare both catalysts *** analysis tools such XRD,Raman spectroscopy,TEM,and BET were used to characterize the catalysts. The autoclave reactor was used to operate the hydrotreating experiments. The hydrotreating reactions were tested at various operating conditions of temperature 325-375 ℃,pressure 2-6 MPa,time 2-6 h,and catalyst /oil ratio( by weight) of 1 ∶75,1 ∶33 and 1 ∶10. The results revealed that the CoMoS /CNT was highly efficient for the hydrotreating more than the CoMoS /γ-Al_2O_3. Also, the hydrodesulfurization( HDS) increased with increasing catalyst /oil ratio. Additionally,results showed that the optimum condition was temperature 350℃,pressure 4 MPa,catalyst /oil ratio of 1 ∶75 for 2 h. Furthermore,even at low CoMoS /CNT catalyst /oil ratio of 1 ∶75,an acceptable HDS of 77.1% was achieved.

Bio-Inspired of Pyramidical Concrete Barrier Walls against the Effects of Explosion Waves

Biomimicry is a technique that inspired solutions to engineering problems through the study of natural systems, designs and processes. Recently, the engineering Biomimicry technique has been used to inspire a protection system against the blast waves and that can mitigate, absorb, and reflect the blast waves. Hence, this work studies the effect of different geometrical configurations of concrete wall barriers inspired by nature against blast waves. The non-linear 3d numerical model is used to model the proposed configurations. The finite element modeling is validated with referenced experimental works. The response of the proposed structural configurations of the wall barrier is analyzed. The results showed that the new bio-inspired pyramidical structure configuration (PCBW) has a notable effect on the mitigation of blast hazards which gave the best performance for the protection of the area behind the wall barrier with 19.5% with respect to traditional concrete barrier wall (TCBW). Also, it is concluded that nature inspiration has a great effect on designing new protection systems against the effect of blast waves.

A secure image steganography algorithm based on least significant bit and integer wavelet transform

The rapid development of data communication in modern era demands secure exchange of information. Steganography is an established method for hiding secret data from an unauthorized access into a cover object in such a way that it is invisible to human eyes. The cover object can be image, text, audio,or video. This paper proposes a secure steganography algorithm that hides a bitstream of the secret text into the least significant bits（LSBs） of the approximation coefficients of the integer wavelet transform（IWT） of grayscale images as well as each component of color images to form stego-images. The embedding and extracting phases of the proposed steganography algorithms are performed using the MATLAB software. Invisibility, payload capacity, and security in terms of peak signal to noise ratio（PSNR） and robustness are the key challenges to steganography. The statistical distortion between the cover images and the stego-images is measured by using the mean square error（MSE） and the PSNR, while the degree of closeness between them is evaluated using the normalized cross correlation（NCC）. The experimental results show that, the proposed algorithms can hide the secret text with a large payload capacity with a high level of security and a higher invisibility. Furthermore, the proposed technique is computationally efficient and better results for both PSNR and NCC are achieved compared with the previous algorithms.

Evaluation of Washington Navel Orange Economic Indicators

In Egypt, oranges are the most important citrus crops for local consumption or export. It represents approximately 72.2% of the total Egyptian citrus production in 2019/2020. The main cultivated variety of orange is the Washington navel orange and it is more popular because of its delicious taste, nutrition, and its seedless besides being rich in vitamin C. Washington navel orange is susceptible to fruit cracking, and it is a common physiological disorder that takes place during fruit development. This work aims to investigate the negative effect of fruit cracking in reducing the yield and quality of Washington navel orange produced in Egypt by studying major economic indicators and estimating an economical evaluation of using treatment of Washington navel orange during the period time of 2007/2008 until 2019/2020. Results showed that fruit areas and production of Washington navel orange represented about 52.9% and 53.2% of fruit areas and production of orange during the study period. However, the yield was unstable with a significant annual growth rate of about 1.1%. Also, the last mentioned treatment achieved a high price for farmers by about 5.1% and total revenue of about 3.2% during the study period. Nowadays, Egypt has become the largest exporter of oranges in the world within the last year and the achieved earnings reached $661 million in 2019/2020. In addition, results showed that evaluation of using a treatment to reduce the rate of Washington navel orange cracking was economical to be applied.

Bloodstream Bacterial Infection in Neutropenic Acute Leukemia Patients

Objective: The rapid detection of blood stream infection has the potential to not only improve patient outcomes due to quicker administration of appropriate antibiotics but also improve antibiotic stewardship by reducing patient exposures to ineffective or unnecessary broad-spectrum antibiotics. Methods: In this study blood samples from acute leukemia neutropenic patients (samples under study) and non neutropenic patients (control) were tested using cultural and non-cultural based techniques via blood culture, C-reactive protein (CRP), Erythrocyte sedimentation rate (ESR) and Molecular techniques (bacterial 16S rDNA analysis). Results: About 22% & 15% were shown positive in blood culturing;94% & 85% were positive for CRP;86% and 75% were presented by elevation in ESR rate for the first hour;94% and 80% were presented by elevation in ESR rate for second hour. Treatment response of the positive cases by blood culture only was found to be 72.7% and 100% not in remission for samples under study and control respectively. In cases under study there is statistically significant correlation between culture growth sensitivity and PCR technique with P value 0.005.

Laboratory efficacy of mycoparasitic fungi(Aspergillus tubingensis and Trichoderma harzianum) against tropical bed bugs(Cimex hemipterus)(Hemiptera:Cimicidae)

Objective: To test the effectiveness of conidial spore formulations [Aspergillus tubingensis(***) and Trichoderma harzianum(***)] against tropical bed bugs, Cimex ***: Spore formulations were made from two fungal strains, *** and *** bed bugs were exposed to the conidial spores placed soaked onto a fabric cloth for 1 h and the mortality counts were recorded daily until 14 ***: Mean survival times based on Kaplan–Meier survival analysis showed no significant differences between all the concentrations in both the fungal isolates:*** and ***, the evaluation of both the isolates in terms of virulence resulted in low lethal hours in all the concentrations except for the high concentration of ***(LT_(50)= 44.629 h) at the conidial exposure of 1 × 10~6 spores/*** mortality of the bed bugs was observed from Day 6 to Day 12, ranging from 13% to 90% in all three concentrations of *** reference to the *** exposure, the concentration of 1 × 10~4 spores/mL displayed a gradual increase in the percentage mortality of 90 on Day ***: Approaches to the bed bugs treatment should be explored in-depth using a natural biological agent like fungus especially *** to reduce this pest population, in order to replace chemical methods.

Assessment of Standard Criteria Implementation in Heamodialysis Water Treatment Units in Sharkia Governorate

Introduction: Haemodialysis is the most well-established form of treatment for ESRD. Method: To evaluate the implementation of standard criteria in heamodialysis water treatment units in Sharkia governorate and to determine the weak points in application of standard criteria, and reach the optimal standards to improve pt. outcomes, across the sectional study was conducted at 30 heamodialysis units of Sharkia governorate, using a modified questionnaire was developed based on MOH protocol and international guidelines such as CARI guidelines, AAMI guidelines and others by the researchers. All data were collected, tabulated and statistically analyzed using SPSS 22.0 for windows (SPSS Inc., Chicago, IL, USA). Results: Of the 30 units, the majority more than 80% of the units achieved the infrastructure and schematic structure, contain water purification devices, good infection control policies, proper chemical disinfection, good monitoring and quality control, accepted maintenance technician evaluation and collected processed water samples results matched decree of 63 for 1996. Conclusion: Most of the studied units nearly fulfilled the standard specifications of both MOH and AAMI. Ensuring that water quality meets AAMI standards and recommendations will minimize patient exposure to potential contaminants such as chemical hazards and endotoxemia associated with the use of the treated water for HD.

Prevalence of Chronic Kidney Disease in Relatives of Chronic Hemodialysis Patients

Objectives: To estimate the prevalence of chronic kidney disease (CKD) in first and second degree relatives of Hemodialysis patients. Background: Early detection and intervention of chronic kidney disease (CKD) may prevent or delay the progression and achieve improved patient outcomes. Family members of end-stage renal disease (ESRD) patients are considered as a high-risk population for CKD. The aim of this work is to screen and estimate the prevalence of chronic kidney disease in first and second degree relatives of hemodialysis patients. Methods: This is an observational prospective study carried out in Nephrology Unit, Internal Medicine Department, Menoufia University Hospital during January 2019 to January 2020. First and second degree relatives of ESRD were included. Relatives with known CKD risk factors or relative to ESRD with known hereditary disease were excluded. Serum creatinine and estimated eGFR, albumin creatinine ratio (ACR) and abdominal ultrasound were done twice with 3 months apart to screen for CKD. Results: 321 persons from first and second degree relatives of chronic hemodialysis patients were included. CKD prevalence was 5.6% Comparison between CKD group and non CKD showed no statistically significant difference as regard age, Gender, Smoking, BMI, and degree of relatives. There were highly significant difference between the studied groups as regard serum creatinine, bloodurea, eGFR and ACR. Relatives of ESRD patients of unknown etiology showed highly significant difference to develop CKD (44.4%) compared to non CKD group 11.5% with P value Conclusions: Prevalence of CKD in relatives—without any CKD risk factors—to ESRD was 5.6%. Family members of ESRD should be screened for CKD, especially relatives to ESRD of unknown etiology.
