Comparison of visual and topographic outcomes of deep-anterior lamellar keratoplasty and penetrating keratoplasty in keratoconus

AIM：To compare visual,surgical and topographic outcomes of deep anterior lamellar keratoplasty（DALK）and penetrating keratoplasty（PK）for keratoconus（KC）.METHODS：In this multicenter,prospective,randomized clinical trial 76 eyes of 71 KC patients operated between January 2011 and July 2014 in 2 tertiary referral hospitals were included. Consecutive patients were alternately selected to receive one of the two surgical methods. Thirty eight eyes underwent DALK with the big-bubble technique and 38 eyes underwent ***：Mean best spectacle corrected visual acuity（BSCVA）at the first postoperative week（P=0.012）and the first postoperative month（P〈0.001）was statistically significantly higher in DALK group. The mean BSCVA at12 mo was not significantly different for DALK（0.30±1.99 log MAR）versus PK（0.40±0.33 log MAR）（P=0.104）. The76.3% of the eyes had a BSCVA over 0.5 in DALK and 47.4%in PK group（P=0.009）. The 7.9% of the eyes had a BSCVA of 1.0 in DALK and 5.3% in PK group（P=0.644）. Mean spherical equivalent was-2.94 D in DALK and-3.09 D in PK *** topographic astigmatism was 4.62 D and 4.18 D respectively. Regular topographic patterns were observed in 31（81.6%）of DALK and 29（76.3%）of PK（P=0.574）. The most frequent topographic pattern was oblate asymmetric bow tie,seen in 39.5% in DALK and 23.7% in PK. CONCLUSION：Big bubble DALK provides an earlier visual improvement compare to PK. However,visual and topographic outcomes are similar to those in PK at 1y. Postoperative complications including rejection and intraocular pressure elevation are more frequent in PK. DALK is a safer alternative to PK for KC. However,intraoperativeperforation of the Descemet＇s membrane is a significant complication.

Relationship between raised intraocular pressure and ischemia-modified albumin in serum and humor aqueous: a pilot study in rabbits

AIM:To evaluate the relationship between increased intraocular pressure(IOP),is chemia-modified albumin levels in serum(IMA-s)and in humor aqueous(IMA-HA)in ***:Twenty-five albino New Zealand rabbits weighing between 2.0 and 2.8 kg were used in this pilot *** permission from Canakkale Onsekiz Mart University Animal Ethics Committee,the IOP of both eyes of each rabbit were recorded with a Tonopen(Tono-Pen XL,Reichart Inc.,Depew,NY,USA)after the application of topical proparacaine 0.5%HCl ***(4 mL)was collected from the marginal ear vein and an intracameral injection of 2.3 mg/mL sodium hyaluronate and subconjunctival dexamethasone was given in the right *** chamber aqueous fluid was obtained using a limbal approach with a 27 gauge needle from both *** left eyes were used as *** was measured on the 1st,3rdand 10thday after the initial injection,with Tonopen,IMA-s levels and IMA-HA examined ***:Beforetheinjections,IOPwas11.4±3.0mmHg in the right eye and 11.3±3.1 mm Hg in the left eye(P>0.05).There was a statistically significant difference between IMA-s levels before the IOP increase(IMA-s0)and IMA-s levels on the 1stand 3rddays after the increase in IOP(P=0.012 and P=0.01,respectively).No difference was observed between IMA-s0and serum IMA levels on the 10thday(IMA-s10)after IOP increase(P=0.989).IMA-HA in the right eye in the first day after the injection was positively correlated with IOP(r=0.748;P=0.02).No othercorrelation is found between any other parameter with IMA-HAlevels at any test time.A statistically significant positive correlation was observed between IMA-s values and IOP on the 1stand 3rddays(r=0.398,P=0.04 and r=0.382,P=0.04,respectively).There was no correlation between IMA-s levels and increased IOP on the 10thday after IOP increase(r=0.026,P=0.902).CONCLUSION:IMA may be an important indicator of acute damage caused by diseases involving ischemic damage to the eye,especially

The selection of optimum chock (support) capacity is very crucial for a successful longwall mining. The selection of chock capacity depends on the site-specific geotechnical parameters, constraints and longwall panel geometry, which are generally not known in detail in priority. Hence, based on the field and laboratory data, various possible combinations should be analyzed to cater for the unforeseeable mining conditions. This paper discusses the use of numerical model for selecting an appropriate chock capacity based on the site-specific geological and geotechnical information and longwall panel geometry. The fracture mechanisms of immediate and main roofs are also discussed for various panel widths and support capacities. For the models considered, the chock convergence is predicted to increase by about 33% due to the increase in face width from 100 to 260 m. Similarly, the massive roof strata are found to yield higher chock convergence compared to bedded strata.

Impacts of Land Use, Fertilizer and Manure Application on the Stream Nutrient Loadings in the Salmon River Watershed, South-Central British Columbia, Canada

Manure and fertilizer applications can increase soil productivity and land economic values, but the controversial result can be a decline of water quality due to the increased nutrient exports from soils to the streams. The impacts of landuse, manure and fertilizer application on nutrient exports from soils to the streams were analyzed using the SWAT (Soil Water Assessment Tool) model for the Salmon River watershed in south-central British Columbia, Canada. The results showed that the animal farms had the highest rates of nutrient exports from soils to the streams and the natural forested lands had the lowest. It was estimated that the whole Salmon River watershed would export approximately 11.52 t·yr-1 of organic nitrogen (ON), 8.05 t·yr-1 of nitrate nitrogen (NO3-N), 2.30 t·yr-1 of organic phosphorus (OP) and 1.36 t·yr-1 of soluble reactive phosphorus (SRP) if the whole watershed was covered by natural vegetation without human disturbance. Current landuse changes, by converting natural vegetation lands to agriculture and animal farms and associated manure and fertilizer applications, have in-creased approximately 53.30 t·yr-1 of ON, 9.68 t·yr-1 of NO3-N, 22.69 t·y-1 of OP and 6.23 t·y-1 of SRP exports to the streams in the whole watershed. The SWAT model predicted that a daily 100 kg·ha-1 of fresh manure deposit from grazing cows during grazing season from later spring to later fall could increase 2.57 kg·ha-1·yr-1 of ON, 0.39 kg·ha-1·yr-1 of NO3-N, 2.35 kg·ha-1·yr-1 of OP and 0.48 kg·ha-1·yr-1 of SRP export to the streams. Fertilization could increase 1.57 kg ha-1 yr-1 of ON and 4.02 kg·ha-1·yr-1 of NO3-N export to the streams if 100 kg·ha-1·yr-1 of nitrogen (NH4NO3) fertilizer was applied in spring. Also fertilization could increase 1.18 kg·ha-1·yr-1 of OP and 0.20 kg·ha-1·yr-1 of SRP export to the streams if 100 kg·ha-1 phosphorus (P2O5) fertilizer was applied in spring.

Experimental and Theoretical Analysis of the Impinging Stream Drying

The experiments of one-stage semi-circular and two-stage semi-circular impinging stream drying as well as the vertical and semi-cricular combined impinging stream drying were carried *** velocity distribution and the mean residence time of particles,and the influence of various factors on drying characteristics were studied.A mathematical model of granular material drying in a semi-circular impinging stream dryer was proposed,in which the flow characteristics as well as the heat and mass transfer mechanisms were *** numerical methods were used to solve the *** various conditons,the calculated results of drying rate and moisture content versus time were *** results indicate that constant drying rate period does not exist in a semi-circular impinging stream *** semi-cricular stage number and curvature radius,flow-rate ratio,air velocity,and higher inlet air temperature should be used for enhancing the drying process.

comparative evaluation of semen parameters in pre-and post-Hurricane Katrina human population

A natural disaster leading to accumulation of environmental contaminants may have substantial effects on the male reproductive system. Our aim was to compare and assess semen parameters in a normospermic population residing in the Southern Louisiana, USA area pre- and post-Hurricane Katrina. We retrospectively evaluated semen analyses data （n = 3452） of 1855 patients who attended the Tulane University Andrology/Fertility Clinic between 1999 and 2013. The study inclusion criteria were men whose semen analyses showed -〉 1.5 ml volume; _〉15 million ml1 sperm concentration; _〉39 million total sperm count; _〉40% motility; 〉30% morphology, with an abstinence interval of 2-7 days. After the inclusion criteria applied to the population, 367 normospermic patients were included in the study. Descriptive statistics and group-based analyses were performed to interpret the differences between the pre-Katrina （Group 1, 1999-2005） and the post-Katrina （Group 2, 2006-2013） populations. There were significant differences in motility, morphology, number of white blood cell, immature germ cell count, pH and presence of sperm agglutination, but surprisingly there were no significant differences in sperm count between the two populations. This long-term comparative analysis further documents that a major natural disaster with its accompanied environmental issues can influence certain semen oarameters （e.g.. motility and morphology） and, bv extension, fertility potential of the population of such areas.

The effect of protective genital care protocol on preventing diaper dermatit development in 0 - 18 month old children using antibiotics

Diaper dermatit is among the widely seen inflamatory skin diseases in neonatals and small 0 - 18 month old children. Control of causative agents as well as providing good genital care may prevent or reduce the risk of the disease. The aim of this study is find out the effect of protective genital care on reducing the diaper dermatit development in 0 - 18 months old inpatient 0 - 18 month old children who are using antibiotic. The study was conducted as prospective half-experimetal using control-experimental groups. Our samples comprise of 0 - 18 month old children admitted at Dokuz Eylul University Practical and Research Hospital, pediatric clinic between January and May 2009, 41 0 - 18 month old children as a control group were not under protective genital care, and 42 0 - 18 month old children as an experimental group were under the protecion and using vaseline. The mothers of the experimental group 0 - 18 month old children were given education. All 0 - 18 month old children were followed-up 3 - 7 days and the collected data were evaluated in computer using Pearson’s chisquare, Fisher’s exact chi-square and Yatest-corrected chi-square tests. The diaper dermatit develop- ment ratio was 34.1% in the control group and 7.1% in the experimental group
