The impact of alpha-fetoprotein (AFP), child-turcotte-pugh (CTP) score and disease staging on the survival of hepatocellular carcinoma (HCC) patients: a retrospective cohort from single oncology center

Background:Hepatocellular carcinoma (HCC) is the most common cause of cancer-related death in Saudi Arabia. Our study aimed to investigate the patterns of HCC and the effect of TNm staging, alfa-fetoprotein (AFP), and Child-Turcotte Pugh (CTP) on patients' overall survival (OS).methods:A retrospective analysis was conducted on 43 HCC patients at a single oncology center in Saudi Arabia from 2015 to 2020. all patients had to fulfill one of the following criteria: (a) a liver lesion reported as definitive HCC on dynamic imaging and/or (b) a biopsy-confirmed diagnosis.Results:The mean patient age of all HCC cases was 66.8 with a male-to-female ratio of 3.3:1. all patients were stratified into two groups: viral HCC (n = 22, 51%) and non-viral HCC (n = 21, 49%). Among viral-HCC patients, 55% were due to HBV and 45% due to HCV. Cirrhosis was diagnosed in 79% of cases. Age and sex did not significantly statistically differ in OS among viral and non-viral HCC patients (-value > 0.05). About 65% of patients had tumor size >5 cm during the diagnosis, with a significant statistical difference in OS (-value = 0.027). AFP was >400 ng/ml in 45% of the patients. There was a statistically significant difference in the OS in terms of AFP levels (-value = 0.021). A statistically significant difference was also observed between the CTP score and OS (-value = 0.02). CTP class B had the longest survival. BSC was the most common treatment provided to HCC patients followed by sorafenib therapy. There was a significant statistical difference in OS among viral and non-viral HCC patients (-value = 0.008).Conclusions:The most common predictors for OS were the underlying cause of HCC, AFP, and tumor size. Being having non-viral etiology, a tumor size >5 cm, an AFP > 400 ng/mL, and a CTP score class C were all negatively associated with OS.

Nanosecond pulse and dual-wavelength Q-switched 1.5 μm fiber laser using mAX phase Cr2TialC2 saturable absorber

We demonstrated a new type of mAX phase material, chromium titanium aluminum carbide（Cr2TialC2） polymer film, to generate a passively Q-switched erbium-doped fiber laser（EDFL）. The film thickness was measured to be around 45 μm, which was fabricated using the embedding method with polyvinyl alcohol（PVA） polymer as hoster. The saturable absorber（SA） film demonstrates a dual-wavelength passively Q-switched EDFL which operates at 1 531 nm and 1 560.19 nm, respectively. The Q-switching pulse duration could be varied from 2.46 μs to 770 ns, while the repetition rate varied from 92.76 kHz to 106.6 kHz with an increasing input pumping range from 154 mW to 300 mW. The maximum output power and pulse energy of 15.05 mW and 141.18 nJ were obtained at the maximum input power of 300 mW, respectively.

Rapid sequence induction(RSI) in trauma patients: Insights from healthcare providers

BACKGROUND: We aimed to describe the current practice of emergency physicians and anaesthesiologists in the selection of drugs for rapid-sequence induction(RSI) among trauma ***: A prospective survey audit was conducted based on a self-administered questionnaire among two intubating specialties. The preferred type and dose of hypnotics, opioids, and muscle relaxants used for RSI in trauma patients were sought in the questionnaire. Data were compared for the use of induction agent, opioid use and muscle relaxant among stable and unstable trauma patients by the intubating ***: A total of 102 participants were included; 47 were anaesthetists and 55 were emergency physicians. Propofol(74.5%) and Etomidate(50.0%) were the most frequently used induction agents. Significantly higher proportion of anesthesiologist used Propofol whereas, Etomidate was commonly used by emergency physicians in stable patients(P=0.001). Emergency physicians preferred Etomidate(63.6%) and Ketamine(20.0%) in unstable patients. The two groups were comparable for opioid use for stable patients. In unstable patients, use of opioid differed significantly by intubating specialties. The relation between rocuronium and suxamethonium use did change among the anaesthetists. Emergency physicians used more suxamethonium(55.6% vs. 27.7%, P=0.01) in stable as well as unstable(43.4 % vs. 27.7%, P=0.08) ***: There is variability in the use of drugs for RSI in trauma patients amongst emergency physicians and anaesthesiologists. There is a need to develop an RSI protocol using standardized types and dose of these agents to deliver an effective airway management for trauma patients.

Analytical wave solutions of an electronically and biologically important model via two efficient schemes

We search for analytical wave solutions of an electronically and biologically important model named as the Fitzhugh–Nagumo model with truncated m-fractional derivative, in which the expafunction and extended sinh-Gordon equation expansion(ESh GEE) schemes are utilized. The solutions obtained include dark, bright, dark-bright, periodic and other kinds of solitons. These analytical wave solutions are gained and verified with the use of mathematica software. These solutions do not exist in literature. Some of the solutions are demonstrated by 2D, 3D and contour graphs. This model is mostly used in circuit theory, transmission of nerve impulses, and population genetics. Finally, both the schemes are more applicable, reliable and significant to deal with the fractional nonlinear partial differential equations.

Predisposing factors, clinical assessment, management and outcomes of agitation in the trauma intensive care unit

BACKGROUND: Agitation occurs frequently among critically ill patients admitted to the intensive care unit(ICU). We aimed to evaluate the frequency, predisposing factors and outcomes of agitation in trauma ICU. mETHODS: A retrospective analysis was conducted to include patients who were admitted to the trauma ICU between April 2014 and march 2015. Data included patient's demographics, initial vitals, associated injuries, Ramsey Sedation Scale, Glasgow Coma Scale, head injury lesions, use of sedatives and analgesics, head interventions, ventilator days, and ICU length of stay. Patients were divided into two groups based on the agitation ***: A total of 102 intubated patients were enrolled; of which 46(45%) experienced agitation. Patients in the agitation group were 7 years younger, had significantly lower GCS and sustained higher frequency of head injuries(Pmore likely to be prescribed propofol alone or in combination with midazolam and to have frequent ICP catheter insertion, longer ventilatory days and higher incidence of pneumonia(Pmultivariate analysis, use of propofol alone(OR=4.97; 95% CI=1.35–18.27), subarachnoid hemorrhage(OR=5.11; 95% CI=1.38–18.91) and ICP catheter insertion for severe head injury(OR=4.23; 95% CI=1.16–15.35) were independent predictors for agitation(Pm in trauma ICU and is mainly related to the type of sedation and poor outcomes in terms of prolonged mechanical ventilation and development of nosocomial pneumonia. Therefore, understanding the main predictors of agitation facilitates early risk-stratification and development of better therapeutic strategies in trauma patients.

multi-segmented nanowires for vortex magnetic domain wall racetrack memory

A vortex domain wall's(VW) magnetic racetrack memory's high performance depends on VW structural stability,high speed, low power consumption and high storage density. In this study, these critical parameters were investigated in magnetic multi-segmented nanowires using micromagnetic simulation. Thus, an offset magnetic nanowire with a junction at the center was proposed for this purpose. This junction was implemented by shifting one portion of the magnetic nanowire horizontally in the x-direction(l) and vertically(d) in the y-direction. The VW structure became stable by manipulating magnetic properties, such as magnetic saturation(m_(4)) and magnetic anisotropy energy(K_(u)). In this case, increasing the values of m_(4) ≥ 800 kA/m keeps the VW structure stable during its dynamics and pinning and depinning in offset nanowires,which contributes to maintenance of the storage memory's lifetime for a longer period. It was also found that the VW moved with a speed of 500 m/s, which is desirable for VW racetrack memory devices. moreover, it was revealed that the VW velocity could be controlled by adjusting the offset area dimensions(l and d), which helps to drive the VW by using low current densities and reducing the thermal-magnetic spin fluctuations. Further, the depinning current density of the VW(J_(d)) over the offset area increases as d increases and l decreases. In addition, magnetic properties, such as the m_(4) and K_(u),can affect the depinning process of the VW through the offset area. For high storage density, magnetic nanowires(multisegmented) with four junctions were designed. In total, six states were found with high VW stability, which means three bits per cell. Herein, we observed that the depinning current density(J_(d)) for moving the VW from one state to another was highly influenced by the offset area geometry(l and d) and the material's magnetic properties, such as the m_(4) and K_(u).

This study deals with the analysis of dynamic mechanical,thermal and flammability properties of treated and untreated pineapple leaf fiber(PalF)and kenaf fiber(KF)phenolic *** indicated that storage modulus was decreased for all composites with increases in temperature and pattern of slopes for all composites,having almost the same values of E′at glass transition temperature(Tg).The peak of the loss modulus of pure phenolic composites was shown to be much *** the addition of kenaf/PalF,peaks were higher and shifted towards a high *** Tan delta peak height was low for pure phenolic composites and maximum for 60%PalF phenolic ***-Cole analysis was carried out to understand the phase behavior of the composite *** analysis(TGA)results indicated that pure phenolic composites have better thermal stability than PalF and kenaf phenolic *** and horizontal UL-94 tests were conducted and showed pure phenolic resin is highly fire *** overall results showed that treated KF composites enhanced the dynamic mechanical and thermal properties among all PalF/KF composites.

Analytical Solution for Time-dependent Flow of a Third Grade Fluid Induced Due to Impulsive motion of a Flat Porous Plate

The aim of the present communication is to discuss the analytical solution for the unsteady flow of a third grade fluid which occupies the space y 〉 0 over an infinite porous plate. The flow is generated due to the motion of the plate in its own plane with an impulsive velocity V（t）. Translational symmetries in variables t and y are utilized to reduce the governing non-linear partial differential equation into an ordinary differential equation. The reduced problem is then solved using homotopy analysis method（HAm）. Graphs representing the solution are plotted and discussed and proper conclusions are drawn.

Binding energy of the donor impurities in GaAs-Ga_(1-x)al_xAs quantum well wires with morse potential in the presence of electric and magnetic fields

The behavior of a donor in the GaAs–GaalAs quantum well wire represented by the morse potential is examined within the framework of the effective-mass approximation. The donor binding energies are numerically calculated for with and without the electric and magnetic fields in order to show their influence on the binding energies. moreover, how the donor binding energies change for the constant potential parameters(De, re, and a) as well as with the different values of the electric and magnetic field strengths is determined. It is found that the donor binding energy is highly dependent on the external electric and magnetic fields as well as parameters of the morse potential.
