Single nucleotide polymorphism within chromosome 8q24 is associated with prostate cancer development in Saudi Arabia

Objective: Genome-wide association studies have demonstrated that single nucleotide polymorphisms (SNPs) are important risk factors for the development of prostate cancer (PCa). Preliminary studies have suggested that the incidence of PCa in Saudi males is low but is probably familial or genetically ***: To identify any possible association of SNP with PCa development in Saudi patients, we investigated a group of SNPs in Saudi PCa patients (n=85) and compared the outcomes to healthy normal controls (n=115) and nodular hyperplasia patients (n=120). DNA was extracted from paraffin-embedded formalin fixed tissue or whole blood from both patients’ groups and healthy control group. A total of thirteen SNPs were genotyped using TaqMan® minor groove binder polymerase chain reaction ***: The rs16901979A, s629242T and rs1447295A alleles were found at significantly higher frequency in PCa patients than controls (p< 0.05). The rs16901979 CA genotype was found at significantly greater frequency in PCa patients than in healthy controls (43% vs. 14%, odds ratio=4.6, p=0.0001) and benign hyperplasia group (43% vs. 25%, odds ratio=2.2, p=0.009).Conclusion: Our study has highlighted the association of rs16901979 SNP with PCa in Saudi males. Such findings have important implications in the PCa diagnosis and in screening unaffected family members of Saudi patients.

Electroconvulsive Therapy (ECT) in Sudan, Probing Differences between Africa and the West

It is a retrospective qualitative study of all patients that had received ECT treatment in Khartoum National Psychiatric Hospitals in the period between January 2010 and December 2010 with special reference to indications, side effects, and outcome using the same outcome criteria used by the royal college of Psychiatrists (UK) for modified audit format. 85% from 269 of overall Sudanese patients who had received ECT showed remarkable clinical response, with no major adverse side effects. Interestingly, the response rate was over 90% for patients with mania spectrum diagnosis. 85% of our patients were under the age of 40. Two thirds of the patients, who received ECT, were male. The main indication for ECT (45%) was mania, while depression and mixed affective states constituted less than 25% of the sample. Conclusion: In comparison to the West, ECT plays a major role in the treatment of mania and manic spectrum disorders in Africa.

Surviving a Catastrophic Upper Gastrointestinal Bleeding Caused by Esophageal-Subclavian Fistula: Case Report

Arterio-esophageal fistula (AEF), whether congenital or acquired, is a rare condition which can lead to fatal upper gastrointestinal bleeding. We report here a young man who developed a subclavian-esophageal fistula (SEF) secondary to chicken bone impaction in the upper esophagus. The diagnosis was reached by urgent upper endoscopy and Computed Tomography of the chest which showed pseudo-aneurysmal changes at left subclavian artery with leaked contrast through the fistula towered the esophagus. Urgent endo-vascular angiography confirmed the subclavian arterio-esophageal fistula that was managed uneventfully using covered 6mm Viban stent-graft. The patient survived this serious condition and was discharged home in good condition.
