The coronavirus disease 2019(COVID-19)pandemic has caused changes in the global health system,causing significant setbacks in healthcare systems *** pandemic has also shown resilience,flexibility,and creativity in reacting to the *** severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection targets most of the respiratory tract,resulting in a severe sickness called acute respiratory distress syndrome that may be fatal in some *** the lung is the primary organ targeted by COVID-19 viruses,the clinical aspect of the disease is varied and ranges from asymptomatic to respiratory ***,due to an unorganized immune response and several affected mechanisms,the liver may also experience liver cell injury,ischemic liver dysfunction,and drug-induced liver injury,which can result in respiratory failure because of the immune system’s disordered response and other compromised processes that can end in multisystem organ *** with liver cirrhosis or those who have impaired immune systems may be more likely than other groups to experience worse results from the SARS-CoV-2 *** thus intend to examine the pathogenesis,current therapy,and consequences of liver damage concerning COVID-19.

The exponential growth in communication networks,data technology,advanced libraries,and mainly World Wide Web services has played a pivotal role in facilitating the retrieval of various types of information as ***,this progress has also led to security concerns related to the transmission of confidential ***,safeguarding these data during communication through insecure channels is crucial for obvious *** emergence of steganography offers a robust approach to concealing confidential information,such as images,audio tracks,text files,and video files,in suitable media carriers.A novel technique is envisioned based on back-propagation *** to the proposed method,a hybrid fuzzy neural network(HFNN)is applied to the output obtained from the least significant bit substitution of secret data using pixel value dif-ferences and exploiting the modification *** simulation and test results,it has been observed that the proposed methodology achieves secure steganography and superior visual *** the experiments,we observed that for the secret image of the cameraman,the PSNR&MSE values of the proposed technique are 61.963895 and 0.041361,respectively.

Incidence of exclusive extrapelvic skeletal metastasis in prostate carcinoma on bone scintigraphy

BACKGROUND Bone is one of the common sites of metastasis from prostate *** scintigraphy(BS)is one of the most sensitive imaging modalities currently used for bone metastatic *** metastasis in prostate carcinoma commonly involves pelvic bones but rarely involves extrapelvic-extraspinal *** To retrospectively analyze the BS data to determine the pattern of skeletal metastases in the prostate *** This retrospective observational study involves patients with biopsy-proven prostate carcinoma referred for BS for staging *** with abnormal BS were evaluated for the pattern of skeletal involvement and data were pre-sented in descriptive format in the form of *** A total of 150 patients with biopsy-proven prostate cancer who were referred for staging were included in the *** of 150 patients(8.67%)had no abnormal uptake on planar images,ruling out metastatic ***-four patients(16%)had heterogeneous uptake in the spine with distribution charac-teristic of degenerative disease and no scan pattern of metastatic *** patients(20%)had multifocal uptake involving both pelvic and extra pelvic bones on planar images typical for skeletal metastasis and were considered ***-three out of 150 patients(55.3%)had increased tracer uptake,which was indeterminate,thus,single photon emission computed tomography-computed tomography(SPECT-CT)was acquired,which showed 51 with metastatic disease,31 benign lesions,and one indeterminate *** of 150 patients had exclusive pelvic bone uptake,which was found to be metastatic in 4/7 patients in *** six out of 150 patients showed exclusive extrapelvic tracer uptake,of which only 3 had vertebral metastatic *** of the patients with increased uptake exclusively in the extrapelvic-extraspinal location was *** The incidence of exclusive extrapelvic skeletal metastatic disease in prostate carcinom

Inflammatory bowel disease(IBD)is a chronic condition characterized by immune-mediated inflammation in the gastrointestinal tract,which follows a relapsing and remitting *** from affecting the gastrointestinal tract,IBD also has extra-intestinal manifestations(EIMs).While the etiology of extraintestinal manifestation remains unclear,it is theorized to be based on immunological responses influenced by genetic *** involvement is one of the EIMs observed in ulcerative colitis and Crohn’s *** renal manifes-tations in IBD patients encompass a range of conditions including nephrolithiasis,amyloidosis,tubulointerstitial nephritis,glomerulonephritis(GN),obstructive pathologies,and chronic kidney disease(CKD).The incidence of CKD in IBD patients varies from 5%-15%.The decline in renal function can stem from various factors such as direct inflammatory damage to the kidneys leading to glomerular or tubular injury,or from complications like recurrent stones,amyloidosis,or ***,nephrotoxic medications used in treating IBD,such as TNF-αinhibitors,calcineurin inhibitors,and aminosalicylates,can exacerbate the decline in renal ***,there is a lack of consensus regarding these patients'screening and renal function *** review aims to assess the existing literature on the different renal complications among individuals with IBD,shedding light on their pathophysiology and management.

Paradigm shift in transfusion practices during early COVID-19 pandemic:A single center retrospective study

BACKGROUND The advent of coronavirus disease 2019(COVID-19)unveiled the worst national blood crisis that the United States had witnessed in over a *** the pandemic influencing the different stages of the acquisition of blood products outside the hospital setting,we aimed to explore the possible barriers contributing to the shortage of blood products within the medical *** COVID era and pre-COVID *** We conducted a retrospective cross-sectional study on hospitalized patients distinguishing the pattern of blood transfusion during the COVID and pre-COVID era in a community *** was tabulated to include the number of red blood cell(RBC)transfusions and if transfusions met restrictive blood transfusion criteria as per institutional ***-square was applied to test the statistical association between qualitative *** t test and Mann Whitney U test were applied respectively to test the mean difference of quantitative *** A total of 208 patients were included in the study,of which 108 were during COVID era and 100 were during pre-COVID *** leading reason for admission in both the COVID era and pre-COVID era transfused patients was shortness of breath(53.7%and 36%P=0.001),followed by gastrointestinal bleeding(25.9%and 21%P=0.001).There was a higher percentage of RBC transfusions in the intensive care unit in the COVID-era group than in the pre-COVID era group(38.9%vs 22%,P=0.008).The restrictive transfusion criteria were met in 62%vs 79%in the COVID and pre-COVID eras,respectively(P=0.008).CONCLUSION The COVID-era group received RBC transfusions with less stringent adherence to restrictive blood transfusion practices in comparison to pre-COVID era group.

Novel Lyapunov-based rapid and ripple-free MPPT using a robust model reference adaptive controller for solar PV system

The technological,economic,and environmental benefits of photovoltaic(PV)systems have led to their wide-spread adoption in recent years as a source of electricity ***,precisely identifying a PV system’s maximum power point(MPP)under normal and shaded weather conditions is crucial to conserving the maximum generated *** of the biggest concerns with a PV system is the existence of partial shading,which produces multiple peaks in the P–V characteristic *** these circumstances,classical maximum power point tracking(MPPT)approaches are prone to getting stuck on local peaks and failing to follow the global maximum power point(GMPP).To overcome such obstacles,a new Lyapunov-based Robust Model Reference Adaptive Controller(LRMRAC)is designed and implemented to reach GMPP rapidly and *** proposed controller also achieves MPP accurately under slow,abrupt and rapid changes in radiation,temperature and load *** and OPAL-RT real-time simulators in various scenarios are performed to verify the superiority of the proposed approach over the other state-of-the-art methods,i.e.,ANFIS,INC,VSPO,and P&*** and GMPP are accomplished in less than 3.8 ms and 10 ms,*** on the results presented,the LRMRAC controller appears to be a promising technique for MPPT in a PV system.

RNA editing changes the nucleotides at the transcript level of mitochondrial genes which results in synthesis of functional *** study was designed to find the editing sites which could be implicated in male fertility restoration and to develop editing based markers for differentiation of cytoplasmic male sterility and maintainer lines from each *** and RNA from young panicles were isolated from three-line system of hybrid rice PRH10,wild abortive(WA)cytoplasm based male sterile(A line Pusa 6A),maintainer(B line Pusa 6B)and restorer(R line PRR78)*** 6A and PRR78 having the same WA cytoplasm are allo-nuclear and iso-cytpolasmic *** genomic and cDNA amplicons for eight mitochondrial genes(18SrRNA,atp6,atp9,cobII,coxI,coxIII,nadI and rps3)were sequenced and *** in genomic and cDNA sequences were considered as *** hundred and thirty editing sites having base substitution or insertion/deletion were identified with the highest in 18SrRNA(5.74%)and the lowest in coxI(0.60%).The highest editing sites were observed in fertile maintainer Pusa 6B followed by PRR78 and Pusa 6A,of which random five editing sites in five different rice mitochondrial transcripts namely atp9,cobII,coxIII,rps3 and 18SrRNA were chosen and validated through cleaved amplified polymorphism sequence(CAPS)analysis and found to be partially edited in four *** identical editing sites of different mitochondrial genes from maintainer and restorer lines might reflect their possible contribution to fertility restoration of sterile WA cytoplasm.

Preliminary slope stability analysis and discontinuities driven susceptibility zonation along a crucial highway corridor in higher Himalaya, India

Identification of failure susceptible slopes through different rock engineering approach is highly valuable in landslide risk management along crucial highway corridors in the high mountainous region. In this study, a critical highway(NH-5) segment in higher Himalaya has been investigated using the various rock mass characterization schemes based on detailed field observations. Since the highway corridor is highly susceptible to discontinuities-driven failures, consisting of jointed rock masses;Mean and Combined kinematic feasibility analysis has been performed for 20 highway slopes. Observed slope mass classes have been compared to the feasibility percentage of discontinuities driven failures(wedge, toppling, and planar) and accordingly the kinematic feasibility zonation along highway segment has been done for each as well as overall failure types. Based on the slope mass conditions and discontinuities driven failures probability(%), responsive remedial measures have been proposed for individual highway slopes to ensure safe and uninterrupted transportation.

Worldwide,a majority of routine endoscopic procedures are performed under some form of sedation to maximize patient ***,benzodiazepines and opioids continue to be widely ***,in recent years,Remimazolam is gaining immense popularity for procedural sedation in gastrointestinal(GI)*** is an ultra-short-acting benzodiazepine sedative which was approved by the Food and Drug Administration in July 2020 for use in procedural *** has shown a favorable pharmacokinetic and pharmacodynamic profile in terms of its non-specific metabolism by tissue esterase,volume of distribution,total body clearance,and negligible drug-drug *** also has satisfactory efficacy and has achieved high rates of successful sedation in GI ***,studies have demonstrated that the efficacy of Remimazolam is non-inferior to Propofol,which is currently a gold standard for procedural sedation in most parts of the ***,the use of Propofol is associated with hemodynamic instability and respiratory *** contrast,Remimazolam has lower incidence of these adverse effects intra-procedurally and hence,may provide a safer alternative to Propofol in procedural *** this comprehensive narrative review,highlight the pharmacologic characteristics,efficacy,and safety of Remimazolam for procedural *** also discuss the potential of Remimazolam as a suitable alternative and how it can shape the future of procedural sedation in gastroenterology.
