Circulatory and hepatic failure at admission predicts mortality of severe scrub typhus patients: A prospective cohort study

Objective:To study the spectrum and outcome of severe scrub typhus in adult patients and to predict the hospital mortality by organ failure on admission.Methods:This was a prospective observational cohort study conducted between July 2017 and October 2020 at the medical emergency centre of PGIMER,Chandigarh,India.One hundred and twenty-six patients aged≥13 years were diagnosed with scrub typhus.Severe disease was defined as the presence of organ failure based on the Sequential Organ Failure Assessment(SOFA)score on admission.Results:About two-thirds of the patients were from geographic regions outside the endemic sub-Himalayan belt.Fever(99.21%)and dyspnea(79.36%)were the most frequent complaints.Respiratory failure(81.75%)was the most common organ failure,followed by hepatic(52.38%),coagulative(47.62%),circulatory(33.33%),renal(21.43%),and cerebral dysfunction(13.49%).The median(Q1,Q3)SOFA score was 8(6,9),and 48.41%of the patients had a quick-SOFA score≥2.Organ supports with invasive ventilation(40.48%),vasopressors(36.51%),and renal replacement therapy(7.14%)were frequently required.The in-hospital mortality was 11.90% and was independently predicted by circulatory and hepatic failures on multivariate logistic regression(OR 11.12,95%CI 1.73-71.31 and OR 8.49,95%CI 1.18-61.41,respectively).Conclusions:Most patients had pulmonary dysfunction;circulatory or hepatic failure on admission strongly predicts death.

To the editor:We read with interest the article by Huynh G et al[1].We congratulate them for performing a cross-sectional survey among healthcare personnel(HCP)to understand their knowledge and attitudes towards the coronavirus disease 2019(COVID-19)vaccine immediately before the vaccination.It is essential to consider HCP’s attitudes about the vaccination because previous experiences with the influenza epidemic have shown a low acceptance and uptake of influenza vaccine among the general population and,more strikingly,among HCP,despite being a highly vulnerable subgroup[2].

The mineralogical and geochemical characteristics of the K-rich granites from the Armoor granitic rocks in the northeastern portion of the Eastern Dharwar Craton(EDC) are presented.In order to understand its physicochemical conditions,the petrogenesis of the granitoid was explained from biotite chemistry and geochemical systematics.Studies of mineral chemistry expose that compositionally,K-feldspar and plagioclase in Armoor granite rocks range from An0,Ab_(3-5.9),Or_(94-96.9) and An_(5-29,-Ab71.9-94.9),Or_(0-1.5),respectively.The mineral chemistry of biotite crystals exhibits composition that varies from primary to re-equilibrated primary biotites.Although biotites from the Armoor granites generally exhibit an I-type trend,with calc-alkaline parental magma in a subduction setting.Biotite chemistry of granites displays magnetite(oxidized)series nature,which has oxygen fugacity(fO_(2))=-15.1 to-16.7(log_(10) bar),under high oxidizing conditions.Temperature and pressure estimates for the crystallization of Armoor granites based on biotite composition are T=612-716 ℃ and 1.0-0.4 kbar,respectively.Geochemically,these rocks are metaluminous to slightly peraluminous and magnesian,with calc-alkaline potassiumrich granite.On the chondrite normalized REE diagram,the granites have positive europium anomalies;rich Sr/Y,(Dy/Yb)_(N) ratios and reduced Mg#,Rb/Sr,Rb,Sr indicate that the melting of earlier rocks,crystal accumulation and residual garnet source formed at high pressures.The examined granites show that they are produced from the melting of crustal sources.Thus,the extensive analyses of the described Armoor granite suggest that they were produced by crust sources and developed under oxidizing conditions in subduction setting.

The 21^(st) century has started with several innovations in the medical sciences,with wide applications in health care management.This development has taken in the field of medicines(newer drugs/molecules),various tools and technology which has completely changed the patient management including abdominal surgery.Surgery for abdominal diseases has moved from maximally invasive to minimally invasive(laparoscopic and robotic)surgery.Some of the newer medicines have its impact on need for surgical intervention.This article focuses on the development of these emerging molecules,tools,and technology and their impact on present surgical form and its future effects on the surgical intervention in gastroenterological diseases.

MOALG: A Metaheuristic Hybrid of Multi-Objective Ant Lion Optimizer and Genetic Algorithm for Solving Design Problems

This study proposes a hybridization of two efficient algorithm’s Multi-objective Ant Lion Optimizer Algorithm(MOALO)which is a multi-objective enhanced version of the Ant Lion Optimizer Algorithm(ALO)and the Genetic Algorithm(GA).MOALO version has been employed to address those problems containing many objectives and an archive has been employed for retaining the non-dominated solutions.The uniqueness of the hybrid is that the operators like mutation and crossover of GA are employed in the archive to update the solutions and later those solutions go through the process of MOALO.A first-time hybrid of these algorithms is employed to solve multi-objective problems.The hybrid algorithm overcomes the limitation of ALO of getting caught in the local optimum and the requirement of more computational effort to converge GA.To evaluate the hybridized algorithm’s performance,a set of constrained,unconstrained test problems and engineering design problems were employed and compared with five well-known computational algorithms-MOALO,Multi-objective Crystal Structure Algorithm(MOCryStAl),Multi-objective Particle Swarm Optimization(MOPSO),Multi-objective Multiverse Optimization Algorithm(MOMVO),Multi-objective Salp Swarm Algorithm(MSSA).The outcomes of five performance metrics are statistically analyzed and the most efficient Pareto fronts comparison has been obtained.The proposed hybrid surpasses MOALO based on the results of hypervolume(HV),Spread,and Spacing.So primary objective of developing this hybrid approach has been achieved successfully.The proposed approach demonstrates superior performance on the test functions,showcasing robust convergence and comprehensive coverage that surpasses other existing algorithms.

Taurine is a sulfur-containing,semi-essential amino acid that occurs naturally in the body.It alternates between inflammation and oxidative stress-mediated injury in various disease models.As part of its limiting functions,taurine also modulates endoplasmic reticulum stress,Ca^(2+)homeostasis,and neuronal activity at the molecular level.Taurine effectively protects against a number of neurological disorders,including stro ke,epilepsy,cerebral ischemia,memory dysfunction,and spinal cord injury.Although various therapies are available,effective management of these disorders remains a global challenge.Approximately 30 million people are affected worldwide.The design of taurine fo rmation co uld lead to potential drugs/supplements for the health maintenance and treatment of central nervous system disorders.The general neuroprotective effects of taurine and the various possible underlying mechanisms are discussed in this review.This article is a good resource for understanding the general effects of taurine on various diseases.Given the strong evidence for the neuropharmacological efficacy of taurine in various experimental paradigms,it is concluded that this molecule should be considered and further investigated as a potential candidate for neurotherapeutics,with emphasis on mechanism and clinical studies to determine efficacy.

Colorectal diseases are increasing due to altered lifestyle,genetic,and environmental factors.Colonoscopy plays an important role in diagnosis.Advances in colonoscope(ultrathin scope,magnetic scope,capsule)and technological gadgets(Balloon assisted scope,third eye retroscope,NaviAid G-EYE,dye-based chromoendoscopy,virtual chromoendoscopy,narrow band imaging,i-SCAN,etc.)have made colonoscopy more comfortable and efficient.Now in-vivo microscopy can be performed using confocal laser endomicroscopy,optical coherence tomography,spectroscopy,etc.Besides developments in diagnostic colonoscopy,therapeutic colonoscopy has improved to manage lower gastrointestinal tract bleeding,obstruction,perforations,resection polyps,and early colorectal cancers.The introduction of combined endo-laparoscopic surgery and robotic endoscopic surgery has made these interventions feasible.The role of artificial intelligence in the diagnosis and management of colorectal diseases is also increasing day by day.Hence,this article is to review cutting-edge developments in endoscopic principles for the management of colorectal diseases.

Determination of the time of refractive stability after uneventful phacoemulsification in Indian eyes

BACKGROUND Knowledge about refractive stabilization and the accuracy of postoperative refractive error measurements are crucial for improved patient outcomes after phacoemulsification.Existing guidelines typically recommend waiting 4-6 wk before prescribing corrective lenses.Our research focused on identifying factors that influence refractive errors in the early stages of post-cataract surgery,thus contributing to the existing literature on this topic.AIM To investigate the time required for refraction stability after uneventful phacoemulsification surgery.METHODS We compared the variation and statistical significance of the difference in spherical,cylindrical components,and the spherical equivalent between the 1-and 6-wk follow-up period in a group of 257 eyes that underwent uneventful phacoemulsification with foldable intraocular lens implantation,all performed by a single experienced surgeon.The Wilcoxon-Signed Rank Test was utilized to assess the magnitude of the change and determine its statistical significance.The refractive stability was defined as the point at which the change in spherical equivalent was within±0.5 dioptres for two consecutive visits.RESULTS The average age of the patients was 64.9±8.9 yr.The differences observed in both the visits in spherical power(0.1±0.2),cylinder power(0.3±0.4),and spherical equivalent(0.2±0.2)were minimal and not statistically significant.The majority of eyes(93.4%)achieved refractive stability within 6 wk after the surgery.The cylindrical power differed between age groups at the 6th wk post-operative and the difference was statistically significant(P value 0.013).There were no significant differences in refractive stability when considering sex and axial length.CONCLUSION Phacoemulsification with foldable intraocular lens implantation results in no significant changes in refraction for the majority of cases during the 6-wk follow-up period.Therefore,a spectacle prescription can be given at the completion of 1 wk.

This work aims to present and explore thermal management techniques for the wire arc additive manufacturing(WAAM)of IN718 components.Excessive heat can be mitigated via air or water cooling.In this study,the material was deposited under four different heat-input conditions with air or water cooling.In air cooling,the layer is deposited in a normal atmospheric air environment,whereas with water cooling,the material is deposited inside a water tank by varying the water level.To validate the air and water cooling thermal management techniques,IN718 single-pass and multilayer linear walls were deposited using the bidirectional gas metal arc welding based WAAM setup under four different heat input conditions.During the deposition of single layers,the temperature profiles were recorded,and the geometric and microstructural features were explored.For multilayer wall structures,the mechanical properties(hardness,tensile strength,and elongation)were determined and assessed using the corresponding microstructural features explored through scanning electron microscopy(SEM),energy dispersive spectroscopy(EDS),and electron backscatter diffraction(EBSD)analyses.The microstructure observed through SEM analysis in the building direction was found to be nonhomogenous compared with that in the deposition direction.Moreover,water cooling was found to govern bead characteristics,such as wall width and height.The grain size and anisotropy of the mechanical properties also decreased in the water-cooled case.Hence,water cooling is an economical and efficient method to mitigate excessive heat accumulation in WAAM-deposited IN718.
