Our understanding of immune responses to SARS-CoV-2 and variants of concern(voCs)has been primarily acquired through analysis of Spike-specific IgG responses.However,a more comprehensive understanding of the breadth and longevity of immune responses after infection and vaccination requires analysis of cellular immunity.Herein,we report on T cell immunity in infected and vaccinated individuals,identifying CD4+/CD8+T cell cytokine responses to SARS-CoV-2 and variant peptides(Alpha,B.1.1.7 and Delta,B.1.617.2).

The presence of human-leukocyte antigen(HLA)-antibodies and blood group incompatibility remain a large barrier to kidney transplantation leading to increased morbidity and mortality on the transplant waiting list.Over the last decade a number of new approaches were developed to overcome these barriers.Intravenous immunoglobulin(IVIG) remains the backbone of HLA desensitization therapy and has been shown in a prospective,randomized,placebo controlled trial to improve transplantation rates.Excellent outcomes with the addition of rituximab(anti-B cell) to IVIG based desensitization have been achieved.There is limited experience with bortezomib(anti-plasma cell) and eculizumab(complement inhibition) for desensitization.However,these agents may be good adjuncts for patients who are broadly sensitized with strong,complement-fixing HLA antibodies.Excellent short and long-term outcomes have been achieved in ABO incompatible transplantation with the combination of antibody removal,B cell depletion,and pre-transplant immunosuppression.Kidney paired donation has emerged as a reasonable alternative for programs who cannot provide desensitization or in conjunction with desensitization.Future therapies directed toward cytokines that alter B cell proliferation are under investigation.

Numerous pathogens can infect the olfactory tract,yet the pandemic caused by SARS-CoV-2 has strongly emphasized the importance of the olfactory mucosa as an immune barrier.Situated in the nasal passages,the olfactory mucosa is directly exposed to the environment to sense airborne odorants;however,this also means it can serve as a direct route of entry from the outside world into the brain.As a result,olfactotropic infections can have serious consequences,including dysfunction of the olfactory system,CNS invasion,dissemination to the lower respiratory tract,and transmission between individuals.Recent research has shown that a distinctive immune response is needed to protect this neuronal and mucosal tissue.A better understanding of innate,adaptive,and structural immune barriers in the olfactory mucosa is needed to develop effective therapeutics and vaccines against olfactotropic microbes such as SARS-CoV-2.Here,we summarize the ramifications of SARS-CoV-2 infection of the olfactory mucosa,review the subsequent immune response,and discuss important areas of future research for olfactory immunity to infectious disease.

Radiative decays D((s))*→D((s))γare revisited in light of new experimental data from the BaBar and BESⅢCollaborations.The radiative couplings gD*Dγencoding nonperturbative QCD effects are calculated in the framework of the covariant confined quark model developed by us.We compare our results with other theoretical studies and experimental data.The couplings(in GeV-1)|g(D*+D+γ)|=0.45(9)and|g(D*0D0γ)|=1.72(34)calculated in our model agree with the corresponding experimental data|g(D*+D+γ)|=0.47(7)and|g(D*0D0γ)|=1.77(16).The most interesting case is the decay Ds*→Dsγ,for which a recent prediction based on light-cone sum rules at next-to-leading order|gDs*Dsγ|=0.60(19)deviates from the first(and only to date)lattice QCD result|gDs*Dsγ|=0.11(2)at nearly3σ.Our calculation yields|gDs*Dsγ|=0.29(6),which falls somehow between the two mentioned results,although it is larger than those predicted in other studies using quark models or QCD sum rules.

The focus of outcome-based education has shifted from the process of training to the end“product”of education.This has necessitated a reframing of the teaching,learning,and assessment.Therefore,in 2013,the Accreditation Council for Graduate Medical Education(ACGME)implemented the educational milestones for all accredited residencies and fellowship programs.[1]The milestones have been described as“developmentally based,specialty-specific achievements that residents are expected to demonstrate at established intervals as they progress through training.”[1]Milestones are based on the six core competencies established by the ACGME and American Board of Medical Specialties(ABMS),which consist of medical knowledge(MK),patient care(PC),interpersonal and communication skills(ICS),practice-based learning and improvement(PBLI),professionalism(PROF),and systems-based practice(SBP).[2,3]ACGME,in conjunction with the American Board of Emergency Medicine(ABEM),drafted 23 detailed subcompetencies relevant to emergency medicine(EM).[4]

Inter-layer interference for multi-layered tight gas reservoir in the absence and presence of movable water

Due to the dissimilarity among different producing layers, the influences of inter-layer interference on the production performance of a multi-layer gas reservoir are possible. However, systematic studies of inter-layer interference for tight gas reservoirs are really limited, especially for those reservoirs in the presence of water. In this work, five types of possible inter-layer interferences, including both absence and presence of water, are identified for commingled production of tight gas reservoirs. Subsequently, a series of reservoir-scale and pore-scale numerical simulations are conducted to quantify the degree of influence of each type of interference. Consistent field evidence from the Yan'an tight gas reservoir(Ordos Basin, China) is found to support the simulation results. Additionally, suggestions are proposed to mitigate the potential inter-layer interferences. The results indicate that, in the absence of water, commingled production is favorable in two situations: when there is a difference in physical properties and when there is a difference in the pressure system of each layer. For reservoirs with a multi-pressure system, the backflow phenomenon, which significantly influences the production performance, only occurs under extreme conditions(such as very low production rates or well shut-in periods). When water is introduced into the multi-layer system, inter-layer interference becomes nearly inevitable.Perforating both the gas-rich layer and water-rich layer for commingled production is not desirable, as it can trigger water invasion from the water-rich layer into the gas-rich layer. The gas-rich layer might also be interfered with by water from the neighboring unperforated water-rich layer, where the water might break the barrier(eg weak joint surface, cement in fractures) between the two layers and migrate into the gas-rich layer. Transition metal carbides and nitrides(MXenes)nanosheets are attractive two-dimensional(2D)materials,but they suffer from oxidation/degradation issues during storage and/or applications due to their sensitivity to water and oxygen.Despite the great research progress,the exact oxidation kinetics of Ti_(3)C_(2)T_(x)(MXene)and their final products after oxidation are not fully understood.Herein,we systematically tracked the oxidation process of few-layer Ti_(3)C_(2)T_(x) nanosheets in an aqueous solution at room temperature over several weeks.We also studied the oxidation effects on the electrocatalytic properties of Ti_(3)C_(2)T_(x) for hydrogen evolution reaction and found that the overpotential to achieve a current density of 10 mA cm^(-2)increases from 0.435 to 0.877 V after three weeks of degradation,followed by improvement to stabilized values of around 0.40 V after eight weeks.These results suggest that severely oxidized MXene could be a promising candidate for designing efficient catalysts.According to our detailed experimental characterization and theoretical calculations,unlike previous studies,black titanium oxide is formed as the final product in addition to white Ti(IV)oxide and disordered carbons after the complete oxidation of Ti_(3)C_(2)T_(x).This work presents significant advancements in better understanding of 2D Ti_(3)C_(2)T_(x)(MXene)oxidation and enhances the prospects of this material for various applications.

BACKGROUND Sessile serrated lesions(SSLs)are considered precancerous colorectal lesions that should be detected and removed to prevent colorectal cancer.Previous studies in Vietnam mainly investigated the adenoma pathway,with limited data on the serrated pathway.AIM To evaluate the prevalence,risk factors,and BRAF mutations of SSLs in the Vietnamese population.METHODS This is a cross-sectional study conducted on patients with lower gastrointestinal symptoms who underwent colonoscopy at a tertiary hospital in Vietnam.SSLs were diagnosed on histopathology according to the 2019 World Health Organi-zation classification.BRAF mutation analysis was performed using the Sanger DNA sequencing method.The multivariate logistic regression model was used to determine SSL-associated factors.RESULTS There were 2489 patients,with a mean age of 52.1±13.1 and a female-to-male ratio of 1:1.1.The prevalence of SSLs was 4.2%[95%confidence interval(CI):3.5-5.1].In the multivariate analysis,factors significantly associated with SSLs were age≥40[odds ratio(OR):3.303;95%CI:1.607-6.790],male sex(OR:2.032;95%CI:1.204-3.429),diabetes mellitus(OR:2.721;95%CI:1.551-4.772),and hypertension(OR:1.650,95%CI:1.045-2.605).The rate of BRAF mutations in SSLs was 35.5%.CONCLUSION The prevalence of SSLs was 4.2%.BRAF mutations were present in one-third of SSLs.Significant risk factors for SSLs included age≥40,male sex,diabetes mellitus,and hypertension.

Background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private sector.Methods Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate regression.Results The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the public or private sector.
