Fibrinolytic therapy for arterial or venous thrombotic disorders involves the systemic administration of thrombolytics such as streptokinase, which is associated with serious bleeding complications. With this study, we provide a proof-of-concept of photothermal thrombus ablation with gold nanorods exposed to near-infrared irradiation, both in vitro using materials generated from purified fibrinogen or plasma and in vivo in murine blood vessels. This is the first report of the application of photothermal therapy as an anti-thrombotic measure. Remarkably, the addition of streptokinase had a multimodal additive effect with regard to acceleration of photothermal lysis of thrombi even at a dose significantly below the therapeutic concentration, thus minimizing the life- threatening side effects and adverse complications. This combinatorial approach exhibits great promise for lysing pathological clots while effectively overcoming the drawbacks of existing therapies.

The analysis of data and information available on developing countries clearly suggests that the current systems and trends of urban transportation, with respect to both mobility and safety, are not sustainable. The cities of developing countries are typically characterized by high-density urban areas, absence of proper control on land use, lack of proper roads and parking facilities, poor public transport, lack of road user discipline etc. This results in the transportation problems, namely accident, congestion, and pollution, taking a very different and much severer shape in developing countries than those in cities of developed countries. Sustainable transport is also important for developing countries from the perspective of climate change, i.e., to improve carbon foot print/ecological foot print （E.F） etc. of transportation systems. From various researches, it has been established that worldwide the road transport has got a substantial share of carbon emissions （20-25% on an average）. This makes transportation an important sector for reducing carbon emissions and thus for addressing the climate change issue. These considerations therefore thrust the need for achieving sustainability in urban transport, particularly for developing countries, not just from mobility and safety perspective but also from the perspective of local and global environmental issues. This paper will critically analyze the above issues for developing countries and would suggest possible approaches to address both the transportation needs as well as climate change.

Uttarakhand state comes under special category state where approximately 69.45% population lived in rural area under the population density with varied range of 37 to 607 persons per ***. Although Uttarakhand is having per capita consumption of 1112.29 kWh which is higher than national average per capita consumption of 779 kWh as till date, but remote communities, villages are not able to access clean, cheep and good quality of energy due to uneven terrain, lack of proper transmission & distribution lines [1]. 100% villages are electrified under the RGGVY scheme as per the Ministry of Power Government of India, but due to poor loading of transformer, lack of grid infrastructure and natural calamities, remote house owners are not able to get good quality of power thus affect the livelihood and source of income generation in various means [2]. As Uttarakhand state having future plans to be make state energy sufficient and energy access to all by year 2016-2017, so major ground level initiative have been taken by Government of Uttarakhand. The government of Uttarakhand has incorporated innovative business model to provide good quality of power with non-conventional energy source. Under the initiative invlovement of local people and village level, panchayats have ownership and responsibility to operate these clean energy business model to improve livelihood in remote hilly places of Uttarakhand. Under this analysis, five different type of community models are categorized as Community 1, Community 2, Community 3, Standalone 1 & Standalone 2 for rural &remote communities based on number of unclustered households with the distance covered between 200 m to 20 km, and electrical loads i.e. lighting, fan, mobile chargers, television along with time of day energy consumption patterns. A rare presentation of spontaneous atheroembolic renal disease: A case report

BACKGROUND Atheroembolic renal disease (AERD) is caused by occlusion of the small renal arteries from embolized cholesterol crystals arising from ulcerated atherosclerotic plaques. This usually manifests as isolated renal disease or involvement from systemic atheroembolic disease. Here we report a case of AERD that responded well to steroid therapy. CASE SUMMARY A 62-year-old woman with a history of hypertension and stage IIIa chronic kidney disease was referred for rapidly worsening renal function over a 4-mo period. She complained of swollen legs, dyspnea on exertion, and two episodes of epistaxis about a month prior to admission. She reported no history of invasive vascular procedures, use of radio contrast agents, or treatment with anticoagulants or thrombolytic agents. Urinalysis showed a few red blood cells and granular casts. Serology was positive for cytoplasmic antineutrophil cytoplasmic antibodies (c-ANCA). Non-contrast-enhanced computed tomography of the chest, abdomen, and pelvis showed diffuse atherosclerotic changes in the aortic arch. Thus, c-ANCA-associated vasculitis was suspected, and the patient was started on pulse intravenous methylprednisolone. Her renal biopsy showed evidence of AERD. She was discharged with oral prednisone, and her renal function continued to improve during the initial follow-up. CONCLUSION In cases of non-vasculitis-associated ANCA, a high degree of clinical suspicion is required to pursue the diagnosis of spontaneous AERD in patients with clinical or radiological evidence of atherosclerotic burden. Although no specific treatment is available, the potential role of statins and steroids requires exploration.

＜正＞ In a continuous speech recognition system it is important to model the context dependent variations in the pronunciations of phones. In this work we have attempted to build decision trees for modeling phonetic context-dependency in Hindi. The approach followed is to modify a decision tree built to model context-dependency in American English. The reason the decision trees turn out to be different are that the English and Hindi phonenme sets are not identical. Then even for identical phonemes, the context-dependency is different for the two languages. Linguistic-Phonetic knowledge of Hindi is used to modify the English phone set. Since the Hindi phone set being used is derived from the English phone set. the adaptation of the English tree to Hindi follows naturally. Though here the adaptation is from English to Hindi, the method may be applicable for adapting between any two languages. The decision tree is built using either Hindi data or English data labeled with the correct Hindi contexts. This procedure is discussed and the limitations of both the methods are described.

Aims and Objectives: The aim of this diagnostic observational study was to find an association of final diagnosis of adnexal masses suggested by MRI and compare it as an imaging modality in determining the origin, nature (benign/malignant) & characteristics of adnexal masses by calculating sensitivity, specificity, and diagnostic accuracy. Materials and Methods: The present study was carried out in 90 patients in the department of radio diagnosis and imaging, institute of medical sciences, Banaras Hindu University (BHU). The patients were referred from department of obstetrics & gynecology, institute of medical sciences. Majority of the referred cases were those who had clinical features of abdominal pain, abdominal lump, menstrual irregularity, ascites, and anorexia or weight loss & in whom adnexal mass was suspected clinically. Magnetic resonance imaging was performed using 1.5 Tesla MR Scanner, Magnetom Avanto (Siemens Healthcare). Results: Out of 114 masses, 17 (14.9%) were malignant. The benign adnexal masses were maximum in the age group 20 - 39 years (56/97, 57.7%), while malignant masses were mainly found in women ≥60 years of age (11/17, 64.7%). CA-125 level was grossly elevated in association with 35.3% of the malignant masses. On MRI, the sensitivity for the mass of ovarian origin was (97.7%) and specificity was (73.1%). The diagnostic accuracy was (92.1%). The mass of uterine origin had a sensitivity of (73.1%) and diagnostic accuracy (99.1%). Conclusion: MRI, because of its accuracy in identifying the origin of adnexal mass and characterizing the solid, hemorrhagic, fatty and fibrous content, may obviate surgery or significantly contribute to preoperative planning for a sonographically indeterminate mass. MRI is the state of the art imaging modality for evaluation of adnexal masses with an overall high diagnostic accuracy.

Ulnar nerve injury in closed fracture of lbrearnl in children is uncommon. Commonly, neurapraxia is the reason for this palsy but other severe injuries or nerve entrapment has been reported in some cases. The impor- tance of diagnosis concerning the types of the nerve injury lies in the fact that they have totally different management. We present a case of ulnar nerve deficit in a child following a closed fracture of the forearm bones. It is imperative to diagnose exact cause of palsy as it forms the basis for treatment. MR1 scan can help diagnosis and accordingly guide the management. Simple nelwe contusion should be treated conservatively, and exploration with fixation of the fracture should be done in lacerations and entrapments of the nerve. Surgery is not the treatment of choice in cases that could be managed conserwltively.

In the hilly region due to scarcity of the plain area, buildings like set back-step back are more often used and also as a big surge in the telecommunication industries, rooftop tower adaptation is very common story nowadays. In the present study an analogy has been drawn to find out the influence of the rooftop telecommunication tower on the setback-step back building resting on ground at 20° and 30° slopes. A dynamic analysis has been performed and compared on the 4 legged angled section telecommunication tower which is located on the roof top of set back-step back building by varying positions of tower with the existing host structure built up on ground slope of 20° and 30° in both directions(X and Y).

Objective:The study was aimed al evaluating the antiulcer activity of ethanolic extract of Achyranthes aspera(EEAA) ***:The anti-ulcer assays were performed on pylorus ligation and chronic ethanol induced ulcer *** effects of the EEAA on gastric content volume,*** acidity,total acidity and ulcer index were ***:The percentage of ulcer protection(59.55%and 35.58%) was significantly(P < 0.001) higher in the groups treated with the high dose of EEAA(600 mg/kg),it also reduced the volume of gastric juice and total acidity whereas,gastric pH was increased ***:The results of this study show significant gastroprotective activity of EEAA may be due to presence of phyto-constituents like flavanoids,saponins and tannins.
