The endoscopic appearance of portal hypertensive intestinal vasculopathy is well described in the stomach, the jejunum, and the colon, but there is no descrip tion of changes that occur in the ileum. The terminal ileum was intubated at colonoscopy in 44 patients with cirrhosis and portal hypertension and 46 control patients. Changes in the terminal ileum were noted. In addition, anorectal varices and colopathy were carefully noted and recorded. Ileal varices and/or portal hy pertensive ileopathy were present in 16 of 44 (36%) patients with cirrhosis and portal hypertension but not in any control patient (p < 0.01). Ileal varices were present in 8 patients (18%) with cirrhosis/portal hypertension and none of the control patients (p < 0.05). Portal hypertensive ileopathy was noted in 11 patients (25%) with cirrhosis and portal hypertension and in none of the control patients (p < 0.05). Ileal varices had no association with any other parameter studied. However, ileopathy was observed in 9 of the 23 patients with colopathy and in only two of the 21 patients without colopathy (p < 0.05). Ileal varices and mucosal changes of portal hypertensive ileopathy occur in patients with cirrhosis and portal hypertension. In the population studied, ileopathy was significantly more common in patients who had portal hypertensive colopathy.

Importance: This post-marketing surveillance study was conducted to evaluate real-world information about the efficacy and safety of oral Tinefcon? tablets (Sphaeranthus indicus based) in plaque psoriasis patients. Materials and Methods: Patients aged at least 18 years and older with clinical diagnosis of plaque psoriasis, were enrolled in this open label, non-comparative, multicenter trial. All eligible subjects received four 700 mg Tinefcon? tablets/day for 12 weeks. The primary outcome measure was percent change in Psoriasis Area Severity Index (PASI) score from baseline to week 12. The secondary outcome measures were Physician Global Assessment (PGA), Nail Psoriasis Severity Index (NAPSI), Psoriatic Arthritis Evaluation and Gene Expression Profiling and Immunohistochemistry. Results: After completion of Tinefcon? treatment at 12 weeks, more than half of subjects (52%) achieved PASI 50 response;PASI 75 response was attained in 68 (23%) subjects and PASI 90 response in 22 (7%) subjects. Five subjects with severe psoriasis achieved PASI 90 without receiving any concomitant medication. Reduction in severity as assessed by PGA was observed in more than half of patients with moderate disease. Histopathological evaluation revealed that epidermal thickness was considerably reduced in 66% of subjects. The expression of inflammatory marker S100A9 protein was(meaningfully reduced in 60% patients with non-significant reduction of Keratin 10 protein expression. Gene expression analysis showed increase down regulation of SERPINB4;PI3 and KRT16 genes after a 12-week treatment period in subjects with higher PASI scores. Conclusion: Oral Tinefcon? tablets showed good efficacy and had a favorable safety profile in plaque psoriasis patients.

目的应用64层螺旋CT灌注扫描成像技术描述正常胰腺和胰腺肿瘤的血流动力学特征，探讨灌注CT在胰腺肿瘤诊断中的应用价值。方法对36例非胰腺疾病（对照）、105例胰腺肿瘤和8例十二指肠乳头癌进行胰腺CT灌注扫描，分别测量血流量（BF）、血容量（BV）和表面通透性（PS）值。结果正常胰腺组织BF、BV和PS的平均值分别为（135．24±48．36）ml·min^-1·kg^-1、（200．55±54．96）ml／kg和（49．75±24．27）ml·min^-1·kg^-1；胰腺癌分别为（31．77±19．36）ml·min^-1·kg^-1、（66．84±39．49）ml／kg和（37．64±27．14）ml·min^-1·kg^-1；十二指肠乳头癌分别为（93．04±48．13）ml·min^-1·kg^-1（97．12±89．80）ml／kg和（41．08±18．85）ml·min^-1·kg^-1；胰腺囊肿的3项参数值接近零。胰腺癌的3项灌注参数均显著低于正常胰腺（P〈0．05）；十二指肠乳头癌的BF和BV值显著低于正常胰腺（P〈0．05），而PS值与正常胰腺无显著差异；胰腺囊肿与正常胰腺差异非常显著（P〈0．01）。结论正常胰腺CT灌注表现为组织血流量、血容量均匀，毛细血管表面通透性一致的实质性器官；胰腺癌表现为低灌注和毛细血管表面通透性降低的肿瘤；十二指肠乳头癌仅表现为低灌注，表面通透性不变；胰腺囊肿内无血流灌注。64层灌注CT有助于胰腺肿瘤的鉴别诊断。

虽然不能一概否认全球化(可能)带来的一些环境惠益,但并没有充分证据表明,全球化带来的好处与其所造成的损失成正比。无论使用定量或定性的方式,现行可用的指标都表明,印度从整体上日益走向一种生态不可持续的发展,无数的民众面临着日趋严重的环境危机。对此,根本性的应对方案或出路在于向一种"激进的生态民主"的转型:在这个社会中,每个公民都有权利和充分的机会参与到关系社会、政治和经济制度的决策中,而所有决策建立在生态永续和人人平等这两大原则的基础之上。无疑,这种激进生态民主需要在治理结构与方式上发生巨大的转变,必将会遭遇到既存政治和企业权力的抵制。但许多迹象显示,这一转变在印度的未来几十年里是有可能发生的。

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（P〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，P〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少PONV，主要是通过减少不良反应而获得。

用光化学方法将能够100%固化的低分子量活性液态前驱体固化成拥有优良质子电导率的固体膜,从而通过这种简单的过程制备了用于燃料电池的聚合物电解质膜(PEM)。这种方法不需要熔融挤出或溶剂重塑等进一步处理过程,而是直接将液态前驱体固化为薄膜,并使薄膜保持在所需要的尺寸。通过化学交联,可以在薄膜中增加磺酸基团数目以此达到高的质子传导率而又不增加薄膜的水溶性,水溶性会破坏非交联的聚合物。用这种薄膜制备了膜电极组件(MEA),它的电池性能优于基于商业化材料的电池。而且,由于液态前驱体的存在,通过柔性平面印刷或微模塑技术可以在薄膜表面上形成具有高精确性,微型的三维结构。相比于普通平面膜,这种有表面微结构的膜在膜与催化剂层之间提供了一个更大的接触面积,进而使其组成的MEA具有更高的功率密度。因此它能使燃料电池微型化,促进燃料电池在移动设备中的应用。
