背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊... 详细信息

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（P〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，P〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少PONV，主要是通过减少不良反应而获得。

A theoretic.l investigation c.nc.rning the influenc. of slip veloc.ty on the flow of blood through an artery having its wall permeable has been c.rried out. Here blood is treated as a homogeneous Newtonian fluid. The flow is c.arac.erized by three parameters： /3 the ratio of radius to length of the arterial segment, Re the c.arac.eristic.Reynolds number assoc.ated with the pressure outside the arterial segment and c.the filtration c.e^c.ent. The problem has been solved by the use of a perturbation tec.nique, e is c.nsidered to be very small, ensuring the validity of the perturbation method. The c.mputed numeric.l results are presented graphic.lly to depic. the variations in veloc.ty, volumetric.flow rate, wall shear stress and flow resistanc..

This paper deals with the theoretic.l investigation of a fundamental problem of magne- tohydrodynamic.（MHD） flow of blood in a c.pillary in the presenc. of thermal radiation and c.emic.l reac.ion. The unsteadiness in the flow and temperature fields is c.used by the time-dependenc. of the stretc.ing veloc.ty and the surfac. temperature. The fluid is c.nsidered to be non-Newtonian, whose flow is governed by the equation of a third-order fluid. The problem is first reduc.d to solving a system of c.upled nonlinear differential equations involving several parameters. c.nsidering blood as an elec.ric.lly c.nduc.ing fluid and using the present analysis, an attempt is made to c.mpute some parameters of the blood flow by developing a suitable numeric.l method and by devising an appropri- ate finite differenc. sc.eme. The c.mputational results are presented in graphic.l form, and thereby some theoretic.l predic.ions are made with respec. to the hemodynamic.l flow of the blood in a hyperthermal state under the ac.ion of a magnetic.field. c.m- putational results for the variation in veloc.ty, temperature, c.nc.ntration, skin-fric.ion c.efi^ic.ent, Nusselt number and Sherwood number are presented in graphic.l/tabular form. Sinc. the study takes c.re of thermal radiation in blood flow, the results reported here are likely to have an important bearing on the therapeutic.proc.dure of hyperthermia, partic.larly in understanding blood flow and heat transfer in c.pillaries.

In this paper, we investigate the implic.tions of elec.ro-osmosis on elec.rohydrodynamic.transport of a non-Newtonian fluid on a hydrophobic.mic.o-c.annel by developing a suitable analytic.l method. Veloc.ty-slip and temperature-jump c.nditions are paid due attention. An attempt has been made to examine the effec.s of rheologic.l and elec.ro-osmotic.parameters on the kinematic. of the fluid. The nonlinear Poisson-Boltzmann equation governing the formation of the elec.ric.l double layer and the body forc. that is generated by the applied potential are ac.ounted for in the study. Perturbation solutions are presented. In order to exhibit the applic.bility of the analysis, the problem of elec.ro-osmotic.flow and heat transfer of blood in an arteriole has been taken up as an illustrative example of a real-life problem. An intensive quantitative study has been made through numeric.l c.mputation of the physic.l variables involved in the analysis, whic. are of spec.al interest in the study. The c.mputational results are presented graphic.lly. The study reveals that the temperature of blood c.n be c.ntrolled by inc.easing/dec.easing the Joule heating parameter.

Theoretic.l investigation on the struc.ures of the oc.ac.ordinated [Nd(c.3)4]5-and the nonac.ordinated [Nd(c.3)***2]5-c.mplexes, using the SPARKLE parameters of the lanthanides within MOPAc. revealed that they possessed dodec.hedral and square antiprismatic.struc.ures respec.ively with an average Nd-O distanc. of 0.249 nm. These struc.ures and the Nd-O distanc.s agreed well with those experimentally found in the c.ystal struc.ures. Replac.ng the water molec.le with a fluoride ion or a mondentatec.rbonato ligand resulted in a nonac.ordinated distorted square antiprismatic.struc.ures where the trans-c.rbonato groups were twisted. The c.rresponding dec.c.ordinated struc.ures with two fluoride ions or a bidentatec.rbonato group, [Nd(c.3)4·F2]7-and [Nd(c.3)5]7-, were also investigated. In both c.ses c.nsiderable twisting of the transc.rbonato groups was observed.

A problem motivated by the investigation of the heat and mass transfer in the unsteady magnetohydrodynamic.（MHD） flow of blood through a vessel is solved numeric.lly when the lumen of the vessel has turned into the porous struc.ure. The time-dependent permeability and the osc.llatory suc.ion veloc.ty are c.nsidered. The c.mputational results are presented graphic.lly for the veloc.ty, the temperature, and the c.nc.ntration fields for various values of skin fric.ion c.effic.ents, Nusselt numbers, and Sherwood numbers. The study reveals that the flow is apprec.ably influenc.d by the presenc. of a magnetic.field and also by the value of the Grashof number.
