Objective:To evaluate the changes occur in the vagina, cervix and endometrium during the pre- ovulatory period in Arabian mares by using endoscope as a diagnostic tool as well as the availability of taking an endoscopic biopsy during this ***:Ten cyclic and healthy arabian mares were subjected to ultrasound scanning of their ovaries to estimate the diameter of the preovulatory follicles, collection of blood samples for estimating the hormonal levels (Estradiol-17β and progestrone) for confirming the estrous phase, endoscopic monitoring their vagina, cervix and endometrium, the availability of taking biopsical samples and histological examination of the obtained ***:The obtained data revealed that, the mean diameter of the preovulatory follicles was (3.95±0.34) cm, the plasma estradiol and progestrone levels were (85.98±5.26) pg/mL and (0.73±0.23) ng/mL, respectively, endoscopic monitoring the vagina, cervix and endometrium revealed hyperemic vaginal mucous membrane, soft and opened cervical ostium with abundant estrous mucous and folding, hyperemic and edematous appearance of the endometrium, respectively and the histological features of the biopsical endometrial samples revealed that, The lamina propria-sub mucosa was packed with simple or branched tubular glands lined with simple columnar epithelium showing oval or rounded dark nuclei with different degrees of secretory activities of the ***: The endoscope proved to be a reliable tool for confirming the estrous phase in mares and monitoring the gross features of the vagina, the cervix and the endometrium, in addition to the availability of taking biopsical smears.

A low-complexity multi-antenna relaying scheme is proposed for Orthogonal Frequency Division Multiplexing (OFDM) in the presence of Class-A Impulsive Noise (IN). One way and two way relaying are considered. The signal is transmitted and received by two terminal nodes, each with a single antenna in two time phases. In the proposed design, the processing at the relay consists of Maximal-Ratio Combining (MRC) or Power-based Selection Combining (PSC) for receive combining, Amplify and Forward (af) for power scaling, and Space Time Block Coding (STBC) for transmit diversity. Channel State Information (CSI), Discrete Fourier Transform (DFT), and Inverse Discrete Fourier Transform (IDFT) are not needed. The Selective Mapping (SLM) technique is used at the transmitter to reduce the Peak-to-Average Power Ratio (PAPR) of the OFDM signal. Then, at the receiver, the clipping technique is used to reduce the impulses that result from the impulsive noise. The proposed system reduces the complexity of the conventional system, which uses multi-relay with a single antenna. Simulation results show that the Bit Error Rate (BER) of the proposed scheme outperforms that of the conventional scheme due to the diversity inherent in the proposed scheme.

Background: Spontaneous bacterial peritonitis (SBP) is a life-threatening infection occurring in 8% - 30% of ascitic cirrhotic patients;different laboratory diagnostics play a pivotal role for rapid and effective management of SBP patients. Polymorphonuclear leucocytic (PMNLs) count in Ascitic fluid (af) is the mainstay for the diagnosis, whereas the diagnostic role of alternative biomarkers is rather controversial. In many studies, serum lipopolysaccharide binding protein (LBP) was elevated and Complement 3 (C3) level was significantly consumed in af of SBP patients. Objectives: To evaluate the diagnostic value of serum LBP and af C3 in HCV-cirrhotics with SBP in relation to other well-established serum and af markers. Patients and Methods: One hundred and twenty patients with HCV-cirrhosis and ascites were enrolled and consented: 50 patients with non-SBP ascites in group A and 70 ascitic patients diagnosed with SBP according to clinical suspicion and PMLs count in af ≥ 250 cells/mm3 in group B in addition to 15 healthy individuals considered as a control group. Serum LBP, CBC, kidney and liver function tests, CRP, fasting and 2 h PP blood glucose and HCV antibodies were measured. af samples were sent for C3 level, culture, PMNLs count, LDH, CRP, total proteins and albumin. Results: In patients with SBP, the level of serum LBP was not significantly high (p > 0.05) with best cut off value at 0.4500 and poor AUC (ely). af C3 was significantly reduced in af (p ellent AUC (0.889) with good sensitivity and specificity (82% & 84%, respectively). af culture showed significant difference between both patients groups (p af CRP and af PMNLs count were of high significance in SBP diagnosis (p el showed low significance while af C3 was significantly reduced in patients

Objectives: The aim of this study was to investigate the effectiveness of dietary intervention with apricots, pomegranate juice or lactic acid fermented sobya on improving the plasma levels of lipid profile parameters and total plasma homocysteine (tHcy) among healthy adults. Design of the study: Thirty-five men and women (28.9 ± 3.0 years) with mean body mass index of 23.5 ± 1.2 kg/m2 were randomly assigned to a dietary intervention study. During the pre-feeding study records of 24-hour food intake, anthropometric measurements and blood sampling were collected for biochemical investigations. Thereafter the volunteers were divided into five groups: a control group (C);the other four groups received daily one of the following supplements: 200 g of deep yellow apricots (Prunus armeniaca) (af);250 g pomegranate (Punica granatum) juice (PJ);mixture of 100 g (PJ) and 150 g of fermented sobya (FS), (PJ-FS) or 165 g (FS). after completing the three-week dietary intervention, blood samples were collected and blood indices were again evaluated. Biochemical analysis of total lipids (TL), total glycerides (TG), total cholesterol (TC) and high density lipoprotein cholesterol (HDL-C) and tHcy were completed. The low density lipoprotein-cholesterol level and three indices of risk factors were calculated using the appropriate equations. Results: Three-week dietary intervention improved the levels of lipid profile parameters TG, TC, HDL-C and LDL-C as compared to the respective baseline levels and the response of the control group that did not receive any supplement. Consuming (PJ-FS) had a synergistic beneficially added value on some of the above mentioned lipid profile parameters. The mean plasma tHcy concentrations;a marker for occlusive vascular disease was reduced significantly following the three week consumption of (PJ) or (FS), compared with the respective pre-feeding levels. Objectives: To assess the impact of atrial fibrillation on stroke severity and short-term (1 month) mortality. Materials and Methods: Totally 200 patients admitted to Ain Shams University Specialized Hospital were recruited and diagnosed clinically to have acute ischemic stroke within 3 days. Patients with hemorrhagic infarctions were excluded. History taking about previous heart disease was taken, full general and neurological examinations were done. Full metabolic profile, full cardiac investigations, carotid duplex, MRI brain stroke protocol with initial clinical evaluation and after 1 month re-evaluation using (NIHSS ) scale. Results: All patients underwent transthoracic echocardiography which revealed absence of “A” wave corresponding to atrial fibrillation in 33 patients (16.5%). Those Patients with atrial fibrillation had a median NIHSS score of 11.00 with IQR of 6.00 - 18.50 at admission and 6.00 with IQR of 2.00 - 14.50 after one month. Patients with atrial fibrillation showed significantly higher NIHSS at admission than patients in sinus rhythm, P el stenosis in 117 (79.1%) patients. 51 (34.4%) patients had lacunar infarction, 65 (43.9%) patients had partial anterior circulation infarction, 25 (16.2%) patients had posterior circulation infarction and 7 (4.7%) patients had total anterior circulation infarction. 111 (75%) patients showed leucoaraiosis. Conclusion: Atrial fibrillation was found not to have significantly statistical effect on stroke severity and short term mortality.

The quality of five commercial naphthalene sulfonated superplasticizers are assessed on the basis of DTA, EDAX, XRD, FTIR and HPLC methods of analyses, as well as their chemical and physical properties. The slump behavior of Portland cements with varying C3 A/C4 af ratio is determined using the five admixtures. For the superplasticized concrete, the slump loss, initial setting time and up to 90-day compressive strength values are reported. It is found that, the polymer characteristics of the superplasticizers affect greatly the workability of the cement pastes and concrete. Also increasing the ferrite content in cement leads to the elimination of the differences of the polymer characteristics and causes a good fluidity of the pastes irrespective of the quality of the superplasticizer.

Spontaneous bacterial peritonitis (SBP) is a serious complication of liver cirrhosis, with a recurrence rate up to 70% at 1 yr. Diagnosis of SBP is based on a positive ascitic fluid (af) culture and/or a polymorph nuclear leukocyte count of >250 cells/μL. Third generation cephalosporins had been viewed as the drug of choice. Aim of the work: to study features, and outcomes of spontaneous bacterial peritonitis in National Liver Institute (Menofia University), a single center study. Methods: This work was conducted on 60 patients had SBP. All patient had af analysis, bedside af culture, Blood culture (BC), Serum albumin, Ascitic fluid gradient (SAAG). Results: The more severe the liver disease, as assessed by Child-Turcotte-Pugh (CTP score), the higher the possibility to have positivity of af culture. Patient who had positive BC had significant rising of Creatinine. The pattern of infection in af culture is close to that of BC, but Streptococci take the upper hand in BC and Staphylococcus in af culture. Gram-positive organisms are more common in both af and BC than gram negative. Mortality rate was higher in culture positive SBP patients 46.2%, and BC positive patients, 62.5%. In all positive cultures patients, Staph Species represents 44.8% with mortality rate of 42% of all mortalities. Most of them were resistant to cephalosporin 62.5%. Strep species represents 31% with mortality rate of 20%. E. coli represents 24.2% with mortality rate of 20%. Conclusion: The microbial pattern of organisms responsible for SBP has changed with predominance of gram-positive organisms. Pattern of microbial infection is similar in both blood and ascetic fluid culture, so BC results can be relied upon to guide therapy, in negative af culture.

AIM To identify predictors of need for repeat procedures after initial atrial fibrillation(af) ablation. METHODS We identified a cohort undergoing first time af ablation at our institution from January 2004 to February 2014 who had cardiac magnetic resonance(CMR) imaging performed prior to ablation. Clinical variables and anatomic characteristics(determined from CMR) were assessed as predictors of need for repeat ablation. The decision regarding need for and timing of repeat ablation was at the discretion of the treating physician. RESULTS From a cohort of 331 patients, 142 patients(43%) underwent repeat ablation at a mean of 13.6 ± 18.4 mo afterthe index procedure. Both male gender(81% vs 71%, P = 0.05) and lower ejection fraction(57.4% ± 10.3% vs 59.8% ± 9.4%, P = 0.04) were associated with need for repeat ablation. On pre-ablation CMR, mean pulmonary vein(PV) diameters were significantly larger in all four PVs among patients requiring repeat procedures. In multivariate analysis, increased right superior PV diameter significantly predicted need for repeat ablation(odds ratio 1.08 per millimeter increase in diameter, 95%CI: 1.00-1.16, P = 0.05). There were also trends toward significance for increased left and right inferior PV sizes among those requiring repeat *** Increased PV size predicts the need for repeat af ablation, with each millimeter increase in PV diameter associated with an approximately 5%-10% increased risk of requiring repeat procedures.
