Chronic opioids in gastroparesis: relationship with gastrointestinal symptoms, healthcare utilization and employment

AIM To examine the relationship of chronic scheduled opioid use on symptoms, healthcare utilization and employment in gastroparesis(Gp) patients. Methods Patients referred to our tertiary care academic center from May 2016 to July 2017, with established diagnosis or symptoms suggestive of Gp filled out the Patient Assessment of Upper GI Symptoms, abdominal pain and demographics questionnaires, and underwent gastric emptying and blood tests. They were asked about taking pain medicines and the types, doses, and duration. We used Mann Whitney U test, Analysis of Variance, Student's t test and χ2 tests where appropriate for data analyses.RESULTS Of 223 patients with delayed gastric emptying, 158(70.9%) patients were not taking opioids(Gp NO), 22(9.9%) were taking opioids only as needed, while 43(19.3%) were on chronic(> 1 mo) scheduled opioids(Gp CO), of which 18 were taking opioids forreasons that included gastroparesis and/or stomach pain. Median morphine equivalent use was 60 mg per day. Gp CO reported higher severities of many gastrointestinal symptoms compared to Gp NO including nausea(mean ± SE of mean of 4.09 ± 0.12 vs 3.41 ± 0.12, P = 0.011), retching(2.86 ± 0.25 vs 1.98 ± 0.14, P = 0.003), vomiting(2.93 ± 0.24 vs 2.07 ± 0.15, P = 0.011), early satiety(4.17 ± 0.19 vs 3.57 ± 0.12, P = 0.004), post-prandial fullness(4.14 ± 0.18 vs 3.63 ± 0.11, P = 0.022), loss of appetite(3.64 ± 0.21 vs 3.04 ± 0.13, P = 0.039), upper abdominal pain(3.86 ± 0.20 vs 2.93 ± 0.13, P = 0.001), upper abdominal discomfort(3.74 ± 0.19 vs 3.09 ± 0.13, P = 0.031), heartburn during day(2.55 ± 0.27 vs 1.89 ± 0.13, P = 0.032), heartburn on lying down(2.76 ± 0.28 vs 1.94 ± 0.14, P = 0.008), chest discomfort during day(2.42 ± 0.20 vs 1.83 ± 0.12, P = 0.018), chest discomfort at night(2.40 ± 0.23 vs 1.61 ± 0.13, P = 0.003), regurgitation/reflux during day(2.77 ± 0.25 vs 2.18 ± 0.13, P = 0.040) and bitter/acid/sour taste in the mouth(2.79 ± 0.27 vs 2.11 ± 0.14, P = 0.028). Sugarcane bagasse amendment mitigates nutrient leaching from a mineral soil under tropical conditions

Large quantities of organic by-products are generated by the sugarcane industry during sugar extraction process.These by-products may be used as soil amendments to improve soil quality,as nutrient leaching is common in mineral soils of Florida in USA due to their sandy texture and frequent rain events.A soil column study was designed to evaluate the effects of bagasse application at 85 t ha^(-1) of fresh bagasse,170 t ha^(-1) of fresh bagasse,and 170 t ha^(-1) of fresh bagasse+nitrogen(N)on the leaching potential of carbon(C),N,phosphorus(P),and potassium(K).Bagasse was incorporated within the topsoil(0-15 cm)in outdoor soil columns exposed to natural conditions,with periodical irrigation during the experiment.After approximately 57 weeks,the distributions of C,N,P,and K were evaluated at three soil depths(i.e.,0-15,15-30,and 30-53 cm)within the soil columns.Total organic C(TOC),N,and P in leachates decreased significantly from the soils amended with bagasse compared with the control with no bagasse and no N.Based on K content changes in the columns,bagasse could also be utilized as a potential source of K for plants.Overall,application of bagasse as a soil amendment might be an effective way to sustainably reutilize organic by-products while reducing concerns regarding major nutrients entering groundwater resources.The results of this study could assist in developing nutrient management plans of using sugarcane bagasse as a potential soil amendment in mineral soils.

An excessive use of non-linear devices in industry results in current harmonics that degrades the power quality with an unfavorable effect on power system performance.In this research,a novel control techniquebased Hybrid-Active Power-Filter(HAPF)is implemented for reactive power compensation and harmonic current component for balanced load by improving the Power-Factor(PF)and Total–Hormonic Distortion(THD)and the performance of a system.This work proposed a soft-computing technique based on Particle Swarm-Optimization(PSO)and Adaptive Fuzzy technique to avoid the phase delays caused by conventional control methods.Moreover,the control algorithms are implemented for an instantaneous reactive and active current(Id-Iq)and power theory(Pq0)in SIMULINK.To prevent the degradation effect of disturbances on the system’s performance,PS0-PI is applied in the inner loop which generate a required dc link-voltage.Additionally,a comparative analysis of both techniques has been presented to evaluate and validate the performance under balanced load conditions.The presented result concludes that the Adaptive Fuzzy PI controller performs better due to the non-linearity and robustness of the system.Therefore,the gains taken from a tuning of the PSO based PI controller optimized with Fuzzy Logic Controller(FLC)are optimal that will detect reactive power and harmonics much faster and accurately.The proposed hybrid technique minimizes distortion by selecting appropriate switching pulses for VSI(Voltage Source Inverter),and thus the simulation has been taken in SIMULINK/MATLAB.The proposed technique gives better tracking performance and robustness for reactive power compensation and harmonics mitigation.As a result of the comparison,it can be concluded that the PSO-basedAdaptive Fuzzy PI system produces accurate results with the lower THD and a power factor closer to unity than other techniques.

Force Sensitive Resistors-Based Real-Time Posture Detection System Using Machine Learning Algorithms

To detect the improper sitting posture of a person sitting on a chair,a posture detection system using machine learning classification has been proposed in this work.The addressed problem correlates to the third Sustainable Development Goal(SDG),ensuring healthy lives and promoting well-being for all ages,as specified by the World Health Organization(WHO).An improper sitting position can be fatal if one sits for a long time in the wrong position,and it can be dangerous for ulcers and lower spine discomfort.This novel study includes a practical implementation of a cushion consisting of a grid of 3×3 force-sensitive resistors(FSR)embedded to read the pressure of the person sitting on it.Additionally,the Body Mass Index(BMI)has been included to increase the resilience of the system across individual physical variances and to identify the incorrect postures(backward,front,left,and right-leaning)based on the five machine learning algorithms:ensemble boosted trees,ensemble bagged trees,ensemble subspace K-Nearest Neighbors(KNN),ensemble subspace discriminant,and ensemble RUSBoosted trees.The proposed arrangement is novel as existing works have only provided simulations without practical implementation,whereas we have implemented the proposed design in Simulink.The results validate the proposed sensor placements,and the machine learning(ML)model reaches a maximum accuracy of 99.99%,which considerably outperforms the existing works.The proposed concept is valuable as it makes it easier for people in workplaces or even at individual household levels to work for long periods without suffering from severe harmful effects from poor posture.

Controlling feedback control systems in continuous action spaces has always been a challenging problem.Nevertheless,reinforcement learning is mainly an area of artificial intelligence(AI)because it has been used in process control for more than a decade.However,the existing algorithms are unable to provide satisfactory results.Therefore,this research uses a reinforcement learning(RL)algorithm to manage the control system.We propose an adaptive speed control of the motor system based on depth deterministic strategy gradient(DDPG).The actor-critic scenario using DDPG is implemented to build the RL agent.In addition,a framework has been created for traditional feedback control systems to make RL implementation easier for control systems.The RL algorithms are robust and proficient in using trial and error to search for the best strategy.Our proposed algorithm is a deep deterministic policy gradient,in which a large amount of training data trains the agent.Once the system is trained,the agent can automatically adjust the control parameters.The algorithm has been developed using Python 3.6 and the simulation results are evaluated in the MATLAB/Simulink environment.The performance of the proposed RL algorithm is compared with a proportional integral derivative(PID)controller and a linear quadratic regulator(LQR)controller.The simulation results of the proposed scheme are promising for the feedback control problems.

Background Schistosoma mekongi is a human blood fluke causing schistosomiasis that threatens approximately 1.5 million humans in the world. Nonetheless, the limited availableS. mekongi genomic resources have hindered understanding of its biology and parasite-host interactions for disease management and pathogen control. The aim of our study was to integrate multiple technologies to construct a high-quality chromosome-level assembly of theS. mekongi genome.Methods The reference genome forS. mekongi was generated through integrating Illumina, PacBio sequencing, 10 × Genomics linked-read sequencing, and high-throughput chromosome conformation capture (Hi-C) methods. In this study, we conducted de novo assembly, alignment, and gene prediction to assemble and annotate the genome. Comparative genomics allowed us to compare genomes across different species, shedding light on conserved regions and evolutionary relationships. Additionally, our transcriptomic analysis focused on genes associated with parasite-snail interactions inS. mekongi infection. We employed gene ontology (GO) enrichment analysis for functional annotation of these genes.Results In the present study, theS. mekongi genome was both assembled into 8 pseudochromosomes with a length of 404 Mb, with contig N50 and scaffold N50 lengths of 1168 kb and 46,759 kb, respectively. We detected that 43% of the genome consists of repeat sequences and predicted 9103 protein-coding genes. We also focused on proteases, particularly leishmanolysin-like metalloproteases (M8), which are crucial in the invasion of hosts by 12 flatworm species. Through phylogenetic analysis, it was discovered that the M8 gene exhibits lineage-specific amplification among the genusSchistosoma. Lineage-specific expansion of M8 was observed in blood flukes. Additionally, the results of the RNA-seq revealed that a mass of genes related to metabolic and biosynthetic processes were upregulated, which might be beneficial for cercaria production.Conclusions This study delivers a high-quality, chromos

Immediate in-hospital outcomes after percutaneous revascularization of acute myocardial infarction complicated by cardiogenic shock

BACKGROUND Cardiogenic shock(CS)is a life-threatening complication of acute myocardial infarction with high morbidity and mortality rates.Primary percutaneous coro-nary intervention(PCI)has been shown to improve outcomes in patients with CS.AIM To investigate the immediate mortality rates in patients with CS undergoing primary PCI and identify mortality predictors.METHODS We conducted a retrospective analysis of 305 patients with CS who underwent primary PCI at the National Institute of Cardiovascular Diseases,Karachi,Pak-istan,between January 2018 and December 2022.The primary outcome was immediate mortality,defined as mortality within index hospitalization.Uni-variate and multivariate logistic regression analyses were performed to identify predictors of immediate mortality.RESULTS In a sample of 305 patients with 72.8%male patients and a mean age of 58.1±11.8 years,the immediate mortality rate was found to be 54.8%(167).Multivariable analysis identified Killip class IV at presentation[odds ratio(OR):2.0;95%co-nfidence interval(CI):1.2-3.4;P=0.008],Multivessel disease(OR:3.5;95%CI:1.8-6.9;P<0.001),and high thrombus burden(OR:2.6;95%CI:1.4-4.9;P=0.003)as independent predictors of immediate mortality.CONCLUSION Immediate mortality rate in patients with CS undergoing primary PCI remains high despite advances in treatment strategies.Killip class IV at presentation,multivessel disease,and high thrombus burden(grade≥4)were identified as independent predictors of immediate mortality.These findings underscore the need for aggressive management and close monitoring of patients with CS undergoing primary PCI,particularly in those with these high-risk characteristics.

Mori Cortex extract ameliorates nonalcoholic fatty liver disease( NAFLD) and insulin resistance in high-fat-diet/streptozotocininduced type 2 diabetes in rats

Nonalcoholic fatty liver disease(NAFLD) and type 2 Diabetes Mellitus(T2 DM) are highly prevalent diseases and are closely associated, with NAFLD being present in the majority of T2 DM patients. In Asian traditional medicine, Mori Cortex is widely used for the treatment of diabetes and hyperlipidemia. However, whether it has a therapeutic effect on T2 DM associated with NAFLD is still unknown. The present study showed that the oral treatment with Mori Cortex extract(MCE; 10 g·kg-1·d-1) lowered the blood lipid levels and reversed insulin resistance(IR) in high fat-diet/streptozotocin-induced type 2 diabetes in rats. The expression levels of sterol receptor element-binding protein-1 c(SREBP-1 c) and carbohydrate-responsive element binding protein(Ch REBP), which are involved in steatosis in NAFLD rats, were measured in the liver samples. MCE decreased the protein and m RNA expression levels of SREBP-1 c and Ch REBP. In conclusion, down-regulation of SREBP-1 c and Ch REBP might contribute to the protective effect of MCE on hepatic injury and IR in the rats with T2 DM associated with NAFLD.

Mass Limit of Neutron Star

The mass limit of neutron star has still remained a mystery. The existing Tolman-Oppenheimer-Volkoff (TOV) equation for calculating the limit always gives different values, by introducing different assumptions and having been predicted like 0.7 Mo, 3.2 Mo, 3.6 Mo, where Mo = 1.98 × 1030 Kg. There is a need of some better technique to adopt other than TOV relation to seek out the value. In this paper, a new relation between the mass of the collapsing star and its average density ρ′ has been derived and used to calculate the limit of neutron star. The conditions in radii between Schwarz Child’s radius and the actual radius of the collapsing star have been introduced to calculate the mass of star above which it will transform into a black hole and below it to a neutron star. A new constant, JN = 8.53707554 × 1039 N-3/2s-3Kg3 has been proposed with which if we introduce the average density of the collapsing neutron star, its mass limit can be calculated very easily. By putting the most possible mass density, which is the minimum required density for a collapsing star to transform into the black hole, it has been found that the mass limit of neutron star is quite higher than it has been assumed. The definition for black hole has also been re-defined on the basis of said radii conditions.

Constraints and Opportunities for Sustainable Livelihoods and Cash Income Generation from NTFPs in the Mountains of Northern Parts of Pakistan

Pakistan′s forest resource base is mostly found in the mountains of the North-West Frontier Province (NWFP) supporting the livelihood of the rural poor and providing different ecological services.The present study was,therefore,initiated with the aim to evaluate different constraints and opportunities for sustainable livelihoods and cash income generation from NTFPs in the mountainous area of NWFP,Pakistan during 2008.Information was collected through questionnaire and interviews during field trips.A total of 117 NTFPs species have been recorded which are being used locally for various purposes such as fuel wood,fodder,medicinal plants,vegetables,mushroom,agricultural tools making,furniture,thatching,shade,fencing/poles,ornamental purpose and animal products.The majority of plants are multi-functional like Pinus wallichiana provides timber,firewood,torch wood,the leaves/small branches are used as thatch for roofing,split logs are used for fencing and the decomposed needles are collected as humus for agricultural fields.These products were widely used by the indigenous community supporting their livelihood.The study proposes protection and sustainable management of these valuable resources for rural livelihoods,which might be useful for developing regional strategies of sustainable management of forest resources.
