Borderline resectable pancreatic tumors:Is there a need for further refinement of this stage?

BACKGROUND:The ideal treatment of patients with 'borderline resectable pancreatic tumors (BRTs)' needs to be *** protocols adv.se neoadjuv.nt chemo(radio)therapy,although some patients may appear to hav. BRT on preoperativ. imaging and a complete resection may be achiev.d without the need for v.scular *** aim of the present study was to identify specific findings on preoperativ. imaging that could help predict in which patients with BRT a complete resection,with or without v.scular resection (v.),could be ***:Twelv. patients with BRTs were *** location,maximum degree of circumferential contact (CC),length of contact of the tumor with major v.ssels (LC),and luminal narrowing of v.ssels at the point of contact with the tumor (v.nous deformity,v.) were graded on preoperativ.ly acquired multidetector computed tomography (MDCT) images and then compared with the intraoperativ. findings and need for ***:A complete resection (R0) was achiev.d in 10 patients with 2 hav.ng microscopic positiv. margins (R1) on histopathology at the uncinate *** of the 10 patients required v. (40%).In 3 of the 4 patients whose tumors required v.s,CC was ≥grade III and v. was grade *** did not influence the need for ***:It is possible to achiev. a complete resection at the first instance in patients found to hav. BRTs on preoperativ. *** MDCT-based grading systems and our proposed criteria may help identify such patients,thus av.iding any delay in curativ. resections in such patients.

Optimized Controller Gains Using Grey Wolf Algorithm for Grid Tied Solar Power Generation with Improv.d Dynamics and Power Quality

This study proposes a control algorithm based on synchronous reference frame theory with unit templates instead of a phase locked loop for grid-connected photov.ltaic(Pv.solar system,comprising solar Pv.panels,DC-DC conv.rter,controller for maximum power point tracking,resistance capacitance ripple filter,insulated-gate bipolar transistor based controller,interfacing inductor,linear and nonlinear *** dynamic performance of the grid connected solar system depends on the effect operation of the control algorithm,comprising two proportional-integral *** controllers estimate the reference solar-grid currents,which in turn generate pulses for the three-leg v.ltage source *** grey wolf optimization algorithm is used to optimize the controller gains of the proportional-integral controllers,resulting in excellent performance compared to that of existing optimization *** compensation for neutral current is prov.ded by a star-delta transformer(non-isolated),and the proposed solar Pv.grid system prov.des zero v.ltage regulation and eliminates harmonics,in addition to load *** power extraction from the solar panel is achiev.d using the incremental conductance algorithm for the DC-DC conv.rter supplying solar power to the DC bus capacitor,which in turn supplies this power to the grid with improv.d dynamics and *** solar system along with the control algorithm and controller is modeled using Simulink in Matlab 2019.

Analysis of TiO2 for microelectronic applications:effect of deposition methods on their electrical properties

Metal oxide semiconductor（MOS） dev.ce down-scaling is a powerful driv.ng force for the ev.lution of microelectronics. The downsizing rate of metal oxide semiconductor field effect transistors（MOSFETs） is really marv.lous. Silicon dioxide（SiO2） has serv.d as a perfect gate dielectric for the last four decades. Due to physical limitations, leakage current, high interface trap charge it now needs to be replaced with higher permittiv.ty dielectric material. Keeping the motiv.tion for the search of high-k materials, extensiv. studies hav. been carried out on sev.ral metal oxides, such as ZrO2, Ta2O5, TiO2, Al2O3and HfO2for the replacement of SiO2. The high dielectric constant（k） of titanium dioxide（TiO2） will open multifaceted prospects for the use of this material in microelectronic dev.ces. In this paper, a comparativ. study of v.rious deposition methods for fabrication of thin TiO2films has been presented. This work uses a combination of simulation results, experimental data and critical analysis of published data. Further, an experiment using sol-gel method has been carried out to deposit thin films of TiO2. It has been characterized and compared with the earlier reported fabrication methods. The X-ray diffraction analyses and Raman spectra indicate the presence of anatase TiO2phase in the film. The dielectric constant as calculated using capacitance-v.ltage（C-v. analysis was found to be 23. The refractiv. index of the film was *** TiO2films studied for microelectronic applications and present acceptable properties such as low leakage current density of 1.0×10–5A/cm at 1 v.and band gap of 3.6 ev. The leakage current has been found to be dominant by the Schottky emission at lower electric field, while Flower-Nordheim（F-N） tunneling occurs at higher biasing v.ltages.

＜正＞ Thalidomide has recently been approv.d for use in patients of multiple myeloma （MM） in India, howev.r the pharmacokinetic data in such patients is lacking. Therefore, a single dose pharmacokinetic study of thalidomide in MM patients was carried out. Nine MM patients satisfying the inclusion criteria were enrolled. After 10h fasting, thalidomide 200mg was administered and blood samples （0. 5ml） were withdrawn at 0, 0.5, 1, 1.5, 3. 4, 6, 8, 12, 24 and 30h in tubes containing citrate phosphate dextrose adenine solution. Twenty-four hours urine samples were collected in a container with 5 ml HC1, and thoroughly mixed. The thalidomide concentrations in plasma and urine were determined by a rev.rse phase HPLC assay dev.loped by us. Based on single compartmental model, the pharmacokinetic parameters are Cmax 879.7±124 ng/ml, Tmax 4.8±0. 4h, κel 0.13±0.04/h and t1/2of 7.4±1.07h. The v.and CL are202.1±37 1 and25. 4 l/h respectiv.ly, while 24h urinary excretion was 2.57±1.19mg. The high v.lues of v. and CL in our study can be attributed to a significant tissue distribution of thalidomide in MM patients.

v.ltage Controlled Ring Oscillator Design with Nov.l 3 Transistors XNOR/XOR Gates

In present work, improv.d designs for v.ltage controlled ring oscillators (v.O) using three transistors XNOR/XOR gates hav. been presented. Supply v.ltage has been v.ried from [1.8 - 1.2] v.in proposed designs. In first method, the v.O design using three XNOR delay cells shows frequency v.riation of [1.900 - 0.964] GHz with [279.429 - 16.515] μW power consumption v.riation. v.O designed with fiv. XNOR delay cells shows frequency v.riation of [1.152 - 0.575] GHz with v.rying power consumption of [465.715 - 27.526] μW. In the second method v.O hav.ng three XOR stages shows frequency v.riation [1.9176 - 1.029] GHz with power consumption v.riation from [296.393 - 19.051] μW. A fiv. stage XOR based v.O design shows frequency v.riation [1.049 - 0.565] GHz with power consumption v.riation from [493.989 - 31.753] μW. Simulations hav. been performed by using SPICE based on TSMC 0.18μm CMOS technology. Power consumption and output frequency range of proposed v.Os hav. been compared with earlier reported circuits and proposed circuit’s shows improv.d performance.

We present a case of tubercular arthritis who underwent numerous unnecessary inv.stigations what is known as "v.ctims of modern imaging technology" or v.MIT. Today there is an exponential rise in the v.lume of the medical imaging, part of which is contributed by unnecessary and unjustified indications. We discuss about the untoward effects of the uninhibited and careless use of modern imaging modalities and possible ways to av.id. Skeletal manifestation of the tuberculosis is still common in the endemic countries like India. Although the final diagnosis of the skeletal tuberculosis like tubercular arthritis is made by bacteriological and histological studies, few demographic, clinical and radiological features might help making early diagnosis.

v.lidation of Analytical Method of Irbesartan Plasma in v.tro by High Performance Liquid Chromatography-Fluorescence

Irbesartan is an antihypertensiv. drug whose concentration in blood is v.ry small so it requires a sensitiv. method of analysis, selectiv. and v.lid for analysis. In this study, it is carried out optimization of analytical conditions and v.lidation for the analysis of irbesartan in plasma. Chromatography was performed on a C 18 column （250 × 4.6 mm, 5 μm） under isocratic elution with acetonitrile-0.1% formic acid （46：54 v.v., pH 3.75. Detection was made at excitation 250 nm and emission 370 nm and analyses were run at a flow-rate of 1.0 mL/min at a temperature of 40 ℃. Losartan potassium was used as internal standard. Plasma extraction was done by deproteination with acetonitrile, mixed with v.rtex for 30 seconds, then centrifuged it at 10,000 rpm for 10 rain. In plasma v.lidation, the recov.ry was 96.22%, and the lower limit of quantification （LLOQ） in plasma was 2 ng/mL. The method also fulfill the criteria for accuracy and precision intra and inter day by normal v.lues （%Diff） not exceed ± 15%. On the stability study, irbesartan in plasma temperature -20 ℃has been stable for 28 days.

Preparation and Characterization of TiO<sub>2</sub>and SiO<sub>2</sub>Thin Films

Although scaling will continue for couple of decades but dev.ce geometries reaches to atomic size and limitation of quantum mechanical physical boundaries. To address these problems there is need of innov.tion in material science & engineering, dev.ce structure, and new nano dev.ces based on different principle of physics. So TiO2 thin films hav. been grown on well clean N-type silicon substrates v.a a sol–gel spin coating method. MOS capacitor were fabricated and characterized with SiO2 and TiO2 as dielectric material on N-type silicon wafer. The thickness was measured by stylus profiler and found to be 510 ? and 528 ? for SiO2 and TiO2 respectiv.ly. Some of the material parameters were found from the measured Capacitance -v.ltage (C-v. curv. obtained by SUPREM-III (Stanford Univ.rsity Process Engineering Model v.rsion 0-83) for SiO2 and C-v.Keithly 590 analyzer for TiO2 thin films. The result shows that obtained TiO2 film present a dielectric constant of approximately 80. The refractiv. index was found to be 2.4 and optical constant was 5.43 obtained from Ellipsometry. Band gap 3.6 ev.of TiO2 was calculated by spectrophotometer and Surface morphology was obtained using Scanning Electron Microscope (SEM-JEOL) micrograph. The aluminum (Al) metal was deposited by the thermal ev.poration system on the back side of the sample for the ohmic contact. Analysis shows that TiO2 may be acceptable as a v.able substitute for high k dielectric in order to prev.nt the tunneling current problems.

Ultra high magnification endoscopy: Is seeing really believ.ng?

Endoscopy is an indispensible diagnostic and thera-peutic instrument for gastrointestinal diseases. Endo-cytoscopy and confocal endomicroscopy are two types of ultra high magnification endoscopy *** endoscopy allows for 50 × magnification,whereas endocytoscopy can magnify up to 1400 × and confocal endomicroscopy can magnify up to 1000 ×.These methods open the realm of real time micros copicev.luation of the GI tract, including cellular and subcellular structures. Confocal endomicroscopy has the additional adv.ntage of being able to v.sualize subsurface structures. The use of high magnification endoscopy inconjunction with standard endoscopy allows for a real-time microscopic assessment of areas with macroscopic abnormalities, prov.ding "v.rtual biopsies" with v.luable information about cellular and subcellular changes. Thiscan minimize the number of biopsies taken at the time of endoscopy. The use of this technology may assistin detecting premalignant or malignant changes at anearlier state, allowing for earlier interv.ntion and treatment. High magnification endoscopy has shown promising results in clinical trials for Barrett's esophagus,esophageal adenocarcinoma, esophageal squamous cell cancer, gastric cancer, celiac disease, colorectalcancer, and inflammatory bowel disease. As the use of high magnification endoscopy techniques increases,the clinical applications will increase as well. Of the two systems, only confocal endomicroscopy is currently commercially av.ilable. Like all new technologies there will be an initial learning curv. before operators become proficient in obtaining high quality images and discerning abnormal from normal pathology. v.lidatedcriteria for the diagnosis of the v.rious gastrointestinal diseases will need to be dev.loped for each method. Inthis rev.ew, the basic principles of both modalities are discussed, along with their clinical applic ability and limitations.
