Epidemiology and Sequelae of Workplace Hand Injuries at a Tertiary Trauma Care Centre

Aims & Objectives: To study the epidemiology of workplace hand injuries, to understand relation between mode of injury and prognosis, to investigate into the possible contributing factors and to study the Socio-economic impact. Design of Study: For the prospective study there were 2 sources of data: 1) Proforma which was filled by the hand surgeon and 2) A questionnaire that was filled up by safety officer. For the retrospective study case, sheets were retrieved from the Medical Records section and a telephonic or personal interview was carried out by a single interviewer. Setting: Department of Plastic and Reconstructive Surgery, Parvathy Hospital, Chennai, India. Patients: A total of 419 industrial injury patients treated from January 2008 to January 2013 were included in the study. Further follow-up regarding return to work and residual disability continues. Main Outcome Measures: Personal details (Age, Gender, Handedness, Work experience, Income). Type of injury, mode of injury, day and time of injury, treatment given, disability and number of working days lost were measured. Results: Those workers who were experienced less than 6 months had more workplace injuries and the commonest contributing factor was co-workers carelessness followed by long working hours and faulty machinery. Maximum injuries occurred on Monday and morning shift. Conclusions: Constant supervision of trainees for 2 years, shorter working hours with regular breaks, regular health checkups for personnel, regular servicing of machines can prevent occurrence of hand injuries. Corrective surgery within 6 hours, systematic and protocol based physiotherapy and rehabilitation can reduce the economic loss and disability arising out of work place injuries.

This article studies a design and implementation of state-feedback control problem for dc-dc Positive Output Super Lift Luo (POSLL) converter by considering the line and load disturbances for needing desired power source for various portable electronic equipments like battery charger, hard disk drives, medical device, LED TV etc. The POSLL’s dynamic performance becomes non-linear in nature;the designed controller able to get superior dynamic performance given by load estimation is done by using an observer and by combining the state-feedback control with the load estimator, a controller which is explicitly developed with strong robustness using separation principle. An effectual stability analysis is exemplified to prove that by carefully selecting the state feedback control and observer gain matrix, the output voltage of the dc-dc POSLL converter tracks the desired value irrespective of the uncertainties. Extensive simulation is carried out using MATLAB/Simulink model. The result based on time domain analysis is done by using the controllers for various disturbances given to the converter.

Standardization of DNA Extraction Method from Mature Dried Leaves and ISSR-PCR Conditions for Melia dubia Cav. —A Fast Growing Multipurpose Tree Species

Melia dubia Cav. of family Meliaceae is a fast growing, high value tree species native to India. Isolating DNA from matured dried leaves of M. dubia was difficult due to accumulation of secondary metabolites, majorly polyphenolics, which resulted in dark brown to black colour of the pellet. In this study, a modified STE-(Sucrose, Tris-HCl and Ethylene Diamine Tetra Acetic Acid) CTAB (hexadecyltrimethylammonium bromide) method was standardized for removal of polyphenolics. The protocol developed yielded 200 - 1000 ng/μl of quality DNA without any impurities as evident by A260/280 ratio ranging from 1.75 - 2.0. It was also suitable for extracting quality DNA from other members of Meliaceae like Azadirachta indica and Melia azedarach. In downstream applications, the extracted DNA was used for PCR amplification by using ISSR and SSR markers. ISSR PCR conditions were optimized in a reaction volume of 25 μl, consisting of 30 ng of template DNA, 1.5 mM MgCl2, 200 μM of each of dNTPs and 2 U of Taq polymerase. The best amplification was observed and the same was applicable for SSR markers.

The ever-increasing number of materials science articles makes it hard to infer chemistry-structure-property relations from *** used natural language processing methods to automatically extract material property data from the abstracts of polymer *** a component of our pipeline,we trained MaterialsBERT,a language model,using 2.4 million materials science abstracts,which outperforms other baseline models in three out of five named entity recognition *** this pipeline,we obtained~300,000 material property records from~130,000 abstracts in 60 *** extracted data was analyzed for a diverse range of applications such as fuel cells,supercapacitors,and polymer solar cells to recover non-trivial *** data extracted through our pipeline is made available at *** which can be used to locate material property data recorded in *** work demonstrates the feasibility of an automatic pipeline that starts from published literature and ends with extracted material property information.

Myeloid cells,such as neutrophils,are produced in the bone marrow in high quantities and are important in the pathogenesis of vascular diseases such as pulmonary hypertension(PH).Although neutrophil recruitment into sites of inflammation has been well studied,the mechanisms of neutrophil egress from the bone marrow are not well *** computational flow cytometry,we observed increased neutrophils in the lungs of patients and mice with ***,we found elevated levels of IL-6 in the blood and lungs of patients and mice with *** observed that transgenic mice overexpressing Il-6 in the lungs displayed elevated neutrophil egress from the bone marrow and exaggerated neutrophil recruitment to the lungs,resulting in exacerbated pulmonary vascular remodeling,and dysfunctional ***,we found that IL-6-induced neutrophil egress from the bone marrow was dependent on interferon regulatory factor 4(IRF-4)-mediated CX3CR1 expression in ***,Cx3cr1 genetic deficiency in hematopoietic cells in Il-6-transgenic mice significantly reduced neutrophil egress from bone marrow and decreased neutrophil counts in the lungs,thus ameliorating pulmonary remodeling and *** summary,these findings define a novel mechanism of IL-6-induced neutrophil egress from the bone marrow and reveal a new therapeutic target to curtail neutrophil-mediated inflammation in pulmonary vascular disease.

Malignant peripheral nerve sheath tumor of proximal third tibia

A 16-year-old man had aswelling over the anterior aspect of the proximal third of the tibia for 1 year, which was peanut size initially and progressively increased to its present size of 10 cm × 8 cm. He underwent fine needle aspiration cytology(FNAC) twice during this period and reported aspindle cell sarcoma. Malignant peripheral nerve sheath tumor(MPNST) is a malignancy of the connective tissue surrounding the nerves. Previously, MPNST was also known as neurofibrosarcoma, malignant schwannoma, andneurogenic sarcoma. We are reporting this case for its rarity and peculiar mode of presentation. FNAC/core biopsy can be used as an effective tool to achievethe correct pathological diagnosis.

Microwave-Assisted Rapid Extracellular Biosynthesis of Silver Nanoparticles Using Carom Seed (Trachyspermum copticum) Extract and in Vitro Studies

Microwave-assisted rapid extracellular biosynthesis of silver nanoparticles was carried out by using carom seed (Trachyspermum copticum) extract as the reducing agent. The reaction mixture containing AgNO3 and carom seed extract when exposed to microwave irradiation resulted in reducing silver ions to bio-functionalized silver nanoparticles of size 6- 50 nm. The AgNP were characterized by UV-vis spectroscopy (UV-vis), X-ray diffraction (XRD), energy dispersive X-ray analysis (EDAX), field emission scanning electron microscopy (FESEM), transmission electron microscopy (TEM) and atomic force microscopy (AFM). Themogravimetric analysis (TGA) and fourier transform infrared spectroscopy (FTIR) are used to understand the possible mechanism of biosynthesis. In this study, we have also investigated the antimicrobial and antioxidant activities of bio-functionalized AgNP. The antibacterial activity is investigated by measuring the zone of inhibition and antioxidant study is done using 1,1-diphenyl-2-picryl hydrazyl (DPPH) method.

Endoscopic submucosal dissection of gastric tumors: A systematic review and meta-analysis

AIM: To systematically review the medical literature in order to evaluate the safety and efficacy of gastric endoscopic submucosal dissection(ESD).METHODS: We performed a comprehensive literature search of MEDLINE, Ovid, CINAHL, and Cochrane for studies reporting on the clinical efficacy and safety profile of gastric ***: Twenty-nine thousand five hundred and six tumors in 27155 patients(31% female) who underwent gastric ESD between 1999 and 2014 were included in this study. R0 resection rate was 90%(95%CI: 87%-92%) with significant between-study heterogeneity(P < 0.001) which was partly explained by difference in region(P = 0.02) and sample size(P = 0.04). Endoscopic en bloc and curative resection rates were 94%(95%CI: 93%-96%) and 86%(95%CI: 83%-89%) respectively. The rate of immediate and delayed perforation rates were 2.7%(95%CI: 2.1%-3.3%) and 0.39%(95%CI: 0.06%-2.4%) respectively while rates of immediate and delayed major bleeding were 2.9%(95%CI: 1.3-6.6) and 3.6%(95%CI: 3.1%-4.3%). After an average follow-up of about 30 mo post-operative, the rate of tumor recurrence was 0.02%(95%CI: 0.001-1.4) among those with R0 resection and 7.7%(95%CI: 3.6%-16%) among those without R0 resection. Overall, irrespective of the resection status, recurrence rate was 0.75%(95%CI: 0.42%-1.3%).CONCLUSION: Our meta-analysis, the largest and most comprehensive assessment of gastric ESD till date, showed that gastric ESD is safe and effective for gastrictumors and warrants consideration as first line therapy when an expert operator is available.
