HIV Infected Elderly Women: From Rhetoric to Reality—Experience from Eastern India

Background: Detection of HIV among elderly is increasing globally and this is going to be a major public health issue in recent future. In India, data regarding demographical and clinical picture of HIV among elderly are sparse more so for elderly women. Elderly individuals especially women are still largely outside the light of HIV awareness programs and represent a major portion of undiagnosed HIV infected population with a poor quality of life. Methods and Materials: This cross-sectional study was conducted at HIV clinic of a tertiary care hospital in eastern India. 567 cases (men 457, women 110) that were detected to be HIV-positive at or after age of 50 years from 2008 to 2012 were analyzed on socio-demographical and clinical basis. Results: Detection of HIV was found to be increasing (5.7% in 2008 to 12.8% in 2012) among the elderly over last five years. Illiteracy (p < 0.01), unemployment (p < 0.001), seroconcordance of spouse (p = 0.02) and widowhood (p = 0.017) were seen more among elderly women. Frailty was the most common clinical presentation among elderly men (31%) whereas most of the elderly women were asymptomatic (27%) at the time of diagnosis. Mean CD4 count among HIV infected elderly men and women were 112 cells/dl and 137 cells/dl respectively at the time of diagnosis. Conclusions: HIV in elderly is an emerging entity in India and posing elderly women a worse socioeconomic burden despite a better clinical profile at the time of presentation. Clinicians, social workers and policymakers should come forward with wider awareness program targeted for elderly HIV patients especially women.

Fine needle aspiration diagnosis of isolated pancreatic tuberculosis: A case report

Tuberculosis(TB) involving the pancreas are uncommon, especially when present in immunocompetent hosts. Pancreatic TB is more frequently associated with miliary TB or widely disseminated disease. Pancreatic TB may present as cystic or solid pancreatic masses, pancreatic abscess or acute or chronic pancreatitis. Majority of the cases are diagnosed after surgical exploration for presumed pancreatic malignancy and preoperative diagnosis is quite difficult. However, improvement in imaging techniques and the resulting imageguided interventions gradually can obviate the need for more invasive diagnostic surgical procedures and expedite the planning of therapy. Herein, we report a rare case of isolated pancreatic TB which presented with pancreatic mass lesion in an immunocompetent host. Diagnosis was made by contrast enhanced computed tomography and guided fine needle aspiration of the pancreatic mass which revealed acid-fast bacillion Ziehl-Neelsen stain. The case was treated successfully with antituberculous drugs. Pancreatic tuberculosis should be considered in the differential diagnosis of a pancreatic mass when the patient is young, residing in the endemic zone of tuberculosis. Every attempt should be made to diagnose the cases to prevent unnecessary operation.

Transcriptional co-activators: emerging roles in signaling pathways and potential therapeutic targets for diseases

Specific cell states in metazoans are established by the symphony of gene expression programs that necessitate intricate synergic interactions between transcription factors and the *** of these regulatory molecules is associated with cell state transitions,which in turn is accountable for diverse maladies,including developmental disorders,metabolic disorders,and most significantly,cancer.A decade back most transcription factors,the key enablers of disease development,were historically viewed as‘undruggable’;however,in the intervening years,a wealth of literature validated that they can be targeted indirectly through transcriptional co-activators,their confederates in various physiological and molecular *** co-activators,along with transcription factors,have the ability to initiate and modulate transcription of diverse genes necessary for normal physiological functions,whereby,deregulation of such interactions may foster tissue-specific disease ***,it is essential to analyze how these co-activators modulate specific multilateral processes in coordination with other *** proposed review attempts to elaborate an in-depth account of the transcription co-activators,their involvement in transcription regulation,and context-specific contributions to pathophysiological *** review also addresses an issue that has not been dealt with in a comprehensive manner and hopes to direct attention towards future research that will encompass patient-friendly therapeutic strategies,where drugs targeting co-activators will have enhanced benefits and reduced side *** insights into currently available therapeutic interventions and the associated constraints will eventually reveal multitudes of advanced therapeutic targets aiming for disease amelioration and good patient prognosis.

Impact of dam on inundation regime of flood plain wetland of punarbhaba river basin of barind tract of Indo-Bangladesh

Present study raises a serious issue of wetland loss and transformation due to damming and water *** present study,it is noticed that overall rainfall trend(-0.006)of the study period(1978-2015)remains unchanged but riparian wetland area is attenuated after damming both pre monsoon(March to May)and post monsoon season(October to December).Total wetland area in pre-and post-monsoon seasons is respectively reduced from 42.2 km^(2) to 27.87 km^(2),and from 277.85 km^(2) to 220.90 km^(2) in post dam *** of frequently inundated wetland area into sparsely inundated wetland is mainly triggered by flow modification due to installation of Komardanga dam and Barrage over Punarbhaba and its major tributary Tangon *** inundated seasonal wetland area is rapidly reclaimed for agricultural *** extreme issue will invite instability in socio-ecological setup of the neighbouring region.

Pain in chronic pancreatitis: Managing beyond the pancreatic duct

Chronic pancreatitis(CP)continues to be a clinical *** or recurrent abdominal pain is the most compelling symptom that drives patients to seek medical ***,in spite of using several treatment approaches in the clinical setting,there is no single specific treatment modality that can be earmarked as a cure for this ***,ductal hypertension has been associated with causation of pain in CP;and patients are often subjected to endotherapy and surgery with a goal to decompress the pancreatic *** studies on humans(clinical and laboratory based)and experimental models have put forward several mechanisms,including neuroimmune alterations,which could be responsible for *** might explain the partial or no response to single modality treatment in a significant proportion of *** current review discusses the recent concepts of pain generation in CP and evidence based therapeutic approaches(other than ductal decompression)to handle persistent or recurrent *** focus primarily on parenchymal and neural components;and discuss the role of antioxidants and the existing controversies,drugs that interfere with neural transmission,pancreatic enzyme supplementation,celiac neurolysis,and pancreatic resection *** review concludes with the treatment approach that we follow at our institute.

Correlating mode-I fracture toughness and mechanical properties of heat-treated crystalline rocks

For the effect of thermal treatment on the mode-I fracture toughness(FT), three crystalline rocks(two basalts and one tonalite) were experimentally investigated. Semi-circular bend specimens of the rocks were prepared following the method suggested by the International Society for Rock Mechanics(ISRM)and were treated at various temperatures ranging from room temperature(25 ℃) to 600 ℃. Mode-I FT was correlated with tensile strength(TS), ultrasonic velocities, and Young’s modulus(YM). Additionally,petrographic and X-ray diffraction analyses were carried out to find the chemical changes resulting from the heat treatment. Further, scanning electron microscopy(SEM) was conducted to observe the micro structural changes when subjected to high temperatures. These experiments demonstrate that heat treatment has a strong negative impact on the FT and mechanical properties of the rocks. From room temperature to 600 ℃, mode-I FT values of massive basalt, giant plagioclase basalt, and tonalite were reduced by nearly 52%, 68%, and 64%, respectively. Also, at all temperature levels, FT and mechanical properties are found to be exponentially correlated. However, the exact nature of the relationship mainly depends on rock type. Besides, TS was found to be a better indicator of degradation degree than the mode-I FT. SEM images show that micro crack density and structural disintegration of the mineral grains increase with temperature. These physical changes confirm the observed reduction in the stiffness of heat-treated crystalline rocks.

Field based index of flood vulnerability (IFV): A new validation technique for flood susceptible models

The flood hazard management is one of the major challenges in the floodplain regions *** the rise in population growth and the spread of infrastructural development,the level of risk has increased over ***,the prediction of flood susceptible area is a key challenge for the adoption of management *** susceptibility modeling is technically a common work,but it is still a very tough job to validate flood susceptible models in a very rigorous and scientific ***,the present work in the Atreyee River Basin of India and Bangladesh was planned to establish artificial neural network(ANN),radial basis function(RBF),random forest(RF)and their ensemble-based flood susceptibility *** flood susceptible models were constructed based on nine flood conditioning *** flood susceptibility models were validated in a conventional way using the receiver operating curve(ROC).To validate the flood-susceptible models,a two dimensional(2D)hydraulic flood simulation model was ***,the index of flood vulnerability model was developed and applied for validating the flood susceptible models,which was a very unique way to validate the predictive *** test and Wilcoxon Signed rank test were employed to compare the generated flood susceptible *** showed that 11.95%-12.99%of the entire basin area(10188.4 km^(2))comes under very high flood-susceptible *** evaluation results have shown that the performance of ensemble flood susceptible models outperforms other standalone machine learning *** flood simulation model and IFV model were also spatially adjusted with the flood susceptibility ***,the present study recommended for the ensemble flood susceptibility prediction and IFV based validation along with conventional ways.

Peripancreatic collections in acute pancreatitis: Correlation between computerized tomography and operative fi ndings

AIM: To evaluate the ability of contrast-enhanced computerized tomography (CECT) to characterize the nature of peripancreatic ***: Twenty five patients with peripancreatic collections on CECT and who underwent operative intervention for severe acute pancreatitis were retrospectively studied. The collections were classified into (1) necrosis without frank pus; (2) necrosis with pus; and (3) fluid without necrosis. A blinded radiologist assessed the preoperative CTs of each patient for necrosis and peripancreatic fluid collections. Peripancreatic collections were described in terms of volume, location, number, heterogeneity, fluid attenuation, wall perceptibility, wall enhancement, presence of extraluminal gas, and vascular ***: Fifty-four collections were identif ied at operation, of which 45 (83%) were identif ied on CECT. Of these, 25/26 (96%) had necrosis without pus, 16/19 (84%) had necrosis with pus, and 4/9 (44%) had fluid without necrosis. Among the study characteristics, fluid heterogeneity was seen in a greater proportion of collections in the group with necrosis and pus, compared to the other two groups (94% vs 48% and 25%, P = 0.002 and 0.003, respectively). Among the wall characteristics, irregularity was seen in a greater proportion of collections in the groups with necrosis with and without pus, when compared to the group with fluid without necrosis (88% and 71% vs 25%, P = 0.06 and P < 0.01, respectively). The combination of heterogeneity and presence of extraluminal gas had a specif icity and positive likelihood ratio of 92% and 5.9, respectively, in detecting pus. CONCLUSION: Most of the peripancreatic collections seen on CECT in patients with severe acute pancreatitis who require operative intervention contain necrotic tissue. CECT has a somewhat limited role in differentiating the different types of collections.

Obtaining vehicle parameters from bridge dynamic response:a combined semi-analytical and particle filtering approach

Dynamic load imposed on the bridge by mov- ing vehicle depends on several bridge-vehicle parameters with various uncertainties. In the present paper, particle filter technique based on conditional probability has been used to identify vehicle mass, suspension stiffness, and damping including tyre parameters from simulated bridge accelerations at different locations. A closed-form expres- sion is derived to generate independent response samples for the idealized bridge-vehicle coupled system consider- ing spatially non-homogeneous pavement unevenness. Thereafter, it is interfaced with the iterative process of particle filtering algorithm. The generated response sam- ples are contaminated by adding artificial noise in order to reflect field condition. The mean acceleration time history is utilized in particle filtering technique. The vehicle- imposed dynamic load is reconstructed with the identified parameters and compared with the simulated results. The present identification technique is examined in the presence of different levels of artificial noise with bridge response simulated at different locations. The effect of vehicle velocity, bridge surface roughness, and choice of prior probability density parameters on the efficiency of the method is discussed.
