ObjectiveTuberculous pleurisy is one of the most common pleural disease usually develops soon after infection. Tuberculous pleurisy can result in clinical symptoms, pleural fibrosis and pleural thickening. Compared to thoracentesis and puncture the traditional methods of treating tuberculous pleurisy, continuous drainage method was used and compare with the steroids. Few reports were about thoracentesis to treat tuberculous pleurisy but the purpose of our study is to compare the continuous pleural drainage method with or without steroids.MethodRetrospective study of 45 cases with pleural effusion was treated from May 2000 to July 2006 in the Department of Respiratory Medicine of the First Affiliated Hospital of Zhengzhou University by pleural drainage method in addition to antituberculosis chemotherapy. The 28 Cases were cured by continuous drainage method and 17 cases were treated by drainage plus steroids. All the patients were treated by antituberculosis chemotherapy （ATC）.ResultFrom the experiment, we find that the patients with the tuberculous pleurisy getting treatments with the drainage method with steroids have no beneficial effect on the patients' outcomes, in compared to the patients treated by drainage method without steroids. Continuous drainage method was an effective way to treat tuberculous pleurisy. In hospital, the duration between drainage group （12.68±7.20 days） and in drainage plus steroids （12.68±3.67 days） is no difference （P=0.767）. Drainage per day in drainage group is （302.00±268.80 ml） more than drainage plus steroids group （172.35±149.95 ml） but the difference in statistics was not significant （P=0.069） here. Days of drainage in drainage group （6.5±3.2 days） and in drainage plus steroids （6.59±2.35 days） have no difference （P=0.922）. Effect of Non-Homogeneity on Thermally Induced Vibration of Orthotropic Visco-Elastic Rectangular Plate of Linearly Varying Thickness

The analysis presented here is to study the effect of non-homogeneity on thermally induced vibration of orthotropic visco-elastic rectangular plate of linearly varying thickness. Thermal vibrational behavior of non-homogeneous rectangular plates of variable thickness having clamped boundary conditions on all the four edges is studied. For non–homogeneity of the plate material, density is assumed to vary linearly in one direction. Using the method of separation of variables, the governing differential equation is solved. An approximate but quite convenient frequency equation is derived by using Rayleigh-Ritz technique with a two-term deflection function. Time period and deflection at different points for the first two modes of vibration are calculated for various values of temperature gradients, non- homogeneity constant, taper constant and aspect ratio. Comparison studies have been carried out with non-homogeneous visco-elastic rectangular plate to establish the accuracy and versatility.

A simple model presented here is to study the thermal effect on vibration of non-homogeneous orthotropic visco-elastic rectangular plate of parabolically varying thickness having clamped boundary conditions on all the four edges. For non-homogeneity of the plate material, density is assumed to vary linearly in one direction. Study the Thermal Gradient Effect on Frequencies of a Trapezoidal Plate of Linearly Varying Thickness

In this paper, effect of thermal gradient on vibration of trapezoidal plate of varying thickness is studied. Thermal effect and thickness variation is taken as linearly in x-direction. Rayleigh Ritz technique is used to calculate the fundamental frequencies. The frequencies corresponding to the first two modes of vibrations are obtained for a trapezoidal plate for different values of taper constant, thermal gradient and aspect ratio. Results are presented in graphical form.

Vibration of Visco-Elastic Parallelogram Plate with Parabolic Thickness Variation

The main objective of the present investigation is to study the vibration of visco-elastic parallelogram plate whose thickness varies parabolically. It is assumed that the plate is clamped on all the four edges and that the thickness varies parabolically in one direction i.e. along length of the plate. Rayleigh-Ritz technique has been used to determine the frequency equation. A two terms deflection function has been used as a solution. For visco-elastic, the basic elastic and viscous elements are combined. We have taken Kelvin model for visco-elasticity that is the combination of the elastic and viscous elements in parallel. Here the elastic element means the spring and the viscous element means the dashpot. The assumption of small deflection and linear visco-elastic properties of “Kelvin” type are taken. We have calculated time period and deflection at various points for different values of skew angles, aspect ratio and taper constant, for the first two modes of vibration. Study the Effect of Thermal Gradient on Transverse Vibration of Non-Homogeneous Orthotropic Trapezoidal Plate of Parabolically Varying Thickness

The present paper deals with the effect of linearly temperature on transverse vibration of non-homogeneous orthotropic trapezoidal plate of parabolically varying thickness. The deflection function is defined by the product of the equations of the prescribed continuous piecewise boundary shape. The non homogeneity of the plate is characterized by taking linear variation of the Young's modulus and parabolically variation of the density of the material. The non homogeneity is assumed to arise due to the variation in the density of the plate material and it is taken as parabolically. Rayleigh Ritz method is used to evaluate the fundamental frequencies. The equations of motion, governing the transverse vibrations of orthotropic trapezoidal plates, are derived with boundary condition clamped-simply supported-clamped-simply supported. Frequencies corresponding to first two modes of vibration are calculated for the trapezoidal plate for various combinations of the parameters of the non-homogeneity, thermal gradient, taper constant and for different values of the aspect ratios and shown by figures. All The results presented here are entirely new and are not found elsewhere. Comparison can only be made for homogeneous plates, and in that cases the results have been compared with those found in the existing literatures and are in excellent agreement.

Quantification of 17-desacetyl norgestimate in human plasma by liquid chromatography–tandem mass spectrometry (LC–MS/MS) and its application to bioequivalence study

A rapid and sensitive ultra-performance liquid chromatography–tandem mass spectrometry（UPLC–MS/MS） method was developed and validated for the estimation of 17-desacetyl norgestimate in human plasma using solid-phase extraction technique. 17-desacetyl norgestimate D6 was used as the internal standard. Simple gradient chromatographic conditions and mass spectrometric detection enabled accurate and precise measurement of 17-desacetyl norgestimate at sub-picogram levels. The proposed method was validated for a linear range of 20–5000 pg/m L with a correlation coefficient Z 0.9988. The intra-run and inter-run precision and accuracy were within 10%. The overall recoveries for 17-desacetyl norgestimate and 17-desacetyl norgestimate D6 were 96.30% and 93.90%, respectively. The total run time was 4.5 min. The developed method was applied for the determination of the pharmacokinetic parameters of 17-desacetyl norgestimate following a single oral administration of a norgestimate and ethinyl estradiol0.250 mg/0.035 mg tablets in 35 healthy female volunteers.

Imaging and interventions in hilar cholangiocarcinoma: A review

Hilar cholangiocarcinoma is a common malignant tumor of the biliary tree. It has poor prognosis with very low 5-year survival rates. Various imaging modalities are available for detection and staging of the hilar cholangiocarcinoma. Although ultrasonography is the initial investigation of choice, imaging with contrast enhanced computed tomography scan or magnetic resonance imaging is needed prior to management. Surgery is curative wherever possible. Radiological interventions play a role in operable patients in theform of biliary drainage and/or portal vein embolization. In inoperable cases, palliative interventions include biliary drainage, biliary stenting and intra-biliary palliative treatment techniques. Complete knowledge of application of various imaging modalities available and about the possible radiological interventions is important for a radiologist to play a critical role in appropriate management of such *** review the various imaging techniques and appearances of hilar cholangiocarcinoma and the possible radiological interventions.

A Simulation Software for the Prediction of Thermal and Mechanical Properties of Wood Plastic Composites

Modelling and simulation has become an important tool in research and development. Simulation models are used to develop better understanding of the internal properties and impact of various parameters on the final quality of the product or process. Simulation model reduces the number of experiments and saves the wastage of material, time and money and are widely used in automobile industry, aircrafts manufacturing, process engineering, training for military, health care sector and many more. Wood Plastic Composite （WPC） is a bio-composite made by mixing wood fibers and plastic granules together at high temperature by compression molding or injection molding. A large quantity of WPC is rejected due to poor quality and low mechanical strength. There is a need to improve the understanding of the wood plastic composites, with both theoretical and experimental analysis. The impact of various parameters and processing conditions on the final product is not known to the industry people, due to less simulation models in this field. A new simulation software WPC Soft is developed to predict the mechanical and thermal properties of WPC. The software can predict the mechanical and thermal properties of WPC. The simulation results were validated with the experimental results and it was observed that the predicted values are quite close to the experimental values and with the further refining of the model, prediction can be further improved. The present simulation software can be easily used by the industry people and it requires very little knowledge of computers or modeling for its operation.
