Calcineurin Inhibitor Use and Myoclonus Association. Is There a Clinical Implication?

Background: Calcineurin Inhibitors (CNIs) play a pivotal role in anti rejection therapy for transplant patients. Neurotoxicity is a known side effect that usually manifests as encephalopathy but myoclonus has also been described. Perioperative myoclonus as a manifestation of neurotoxicity, has not been well studied. Methods: We retrospectively reviewed data from 842,762 patients from the Nationwide Inpatient Sample (NIS) database from January 2011 to December 2014. Of those records we compared 56,423 patients requiring CNIs and undergoing Heart Transplant (HT) with 786,339 patients undergoing Coronary Artery Bypass Graft (CABG) surgery as controls. The objective was to study the rates of myoclonus in patients undergoing cardiac surgery, especially those requiring CNIs, and study the outcome of those patients with myoclonus. The NIS database from January 2011 to December 2014 was the source for the analysis. Patients with underlying epilepsy or hypo-ischemic encephalopathy based on ICD-9-CM codes were excluded from the study. Results: A total of 147 patients (0.26%) were found to have myoclonus in the HT group versus 338 patients (0.04%) in the CABG group, p Conclusion: In this study we have compared the rate of myoclonus found in HT patients versus CABG patients. Forecasting the Municipal Solid Waste Using GSO-XGBoost Model

Waste production rises in tandem with population growth and increased *** indecorous disposal of waste paves the way for huge disaster named as climate *** National Environment Agency(NEA)of Singapore oversees the sustainable management of waste across the *** three main contributors to the solid waste of Singapore are paper and cardboard(P&C),plastic,and food ***,they have a negligible rate of *** this study,Machine Learning techniques were utilized to forecast the amount of garbage also known as waste *** waste audit would aid the authorities to plan their waste *** applied models were k-nearest neighbors,Support Vector Regressor,ExtraTrees,CatBoost,and *** XGBoost model with its default parameters performed better with a lower Mean Absolute Percentage Error(MAPE)of 8.3093(P&C waste),8.3217(plastic waste),and 6.9495(food waste).However,Grid Search Optimization(GSO)was used to enhance the parameters of the XGBoost model,increasing its ***,the optimized XGBoost algorithm performs the best for P&C,plastics,and food waste with MAPE of 4.9349,6.7967,and 5.9626,*** proposed GSO-XGBoost model yields better results than the other employed models in predicting municipal solid waste.

A solar reactor for bio-diesel production from Pongamia oil: Studies on transesterfication process parameters and energy efficiency

Over exploitation of non-renewable energy reserves will lead to increase in price of petroleum *** there is a need for suitable and sustainable substitutes(renewable resource) for conventional *** this study,an efficient and environmental friendly method for production of bio-diesel from Pongamia(Karanja) oil has been developed using a solar *** the experimental study,the maximum temperature attained by the pongamia oil during the transesterification process was 64.1℃.The transesterification reaction was studied by varying different parameters such as reactant flow rate(5-20 L·h^(-1)),stirring speed(150-450 r·min^(-1)),catalyst mass loading(0.5%-2%) and methanol to oil ratio(3:1 to15:1).The maximum biodiesel yield was 83.11% at a flow rate of 5 L·h^(-1),stirring speed of 350 r·min^(-1),a methanol to oil ratio of 15:1,catalyst mass loading of 1% and reaction time of 270 *** physical and chemical properties of biodiesel was analyzed as per American Society for Testing Materials(ASTM) standards and it had density of 938 kg·m^(-3),viscosity(28.7×10^(-6) m~2·s^(-1)),acid value(9.45 mg KOH·(g oil)^(-1))and flash point(215℃).The energy efficiency of solar heating process was determined by comparing the net energy ratio of direct heating process and solar heating *** solar heating the net energy ratio(NER) was found to be 31.85 against 5.73 for direct ***,net energy efficiency index was calculated for 10 kg production scale and was found to be increasing when scaled up which means that the solar heating process is more effective even in scaled up production.
