Ventricular Repolarization: Epidemiology and Clinical Correlates among Type-2 Diabetics with Uncontrolled Arterial Hypertension in Western Region of the Republic of Macedonia

This study aimed to describe the epidemiology of repolarization dispersion (QT dispersion and Tpeak-Tend dispersion) and further describe their associated clinical correlates among uncontrolled arterial hypertension in type-2 Diabetics patient, in western region of the Republic of Macedonia. Abnormal ventricular repolarization is associated with increased cardiovascular risk. Data relating to the frequency of increased repolarization dispersion, among uncontrolled arterial hypertension in type-2 Diabetics patient in western region of the Republic of Macedonia, are scarce. A total of 600 patients were enrolled into this observation study. Study participans were selected among primary care patient, who were receiving ongoing care for diabetes mellitus and hypertension during 1 calendar year. Twelve lead resting electrocardiography, QT, QTc, Tpeak-Tend-intervals and dispersions, were determined manually, and were compared between groups. Patients with uncontrolled BP have greater frequency of: prolonged ***, (61.3% vs.33.6%;p = 0.0005), prolonged Tpeak-Tend interval (65.3% vs. 34.7%;p = 0.005), increased dispersion of QTc. interval (65.9% vs. 34.1%;p = 0.00), increased disperion of Tpeak-Tend interval (65.5% vs. 34.5%;p = 0.002). Females with uncontrolled BP have greater frequency of: increased dispersion of QTc. interval (61.2% vs. 38%;p = 0.02), increased dispersion of Tpeak-Tend interval (63.1% vs. 31.5%;p = 0.008). Hypertensive diabetic patients with uncontrolled BP and abnormal ventricular repolarization have greater BMI (p = 0.000;95%CI 3.849 - 7.871), longer duration of D.M (p = 0.000;95%CI 1.600 - 1.981), longer duration of arterial hypertension (p = 0.000;95%CI 1.468 - 1.850) and less controlled glycemia (p = 0.000;95%CI 1.556 - 3.004). Essential role of small bowel capsule endoscopy in reclassification of colonic inflammatory bowel disease type unclassified

AIM To evaluate the role of small bowel capsule endoscopy(SBCE) on the reclassification of colonic inflammatory bowel disease type unclassified(IBDU).METHODS We performed a multicenter, retrospective study including patients with IBDU undergoing SBCE, between 2002 and 2014. SBCE studies were reviewed and the inflammatory activity was evaluated by determining the Lewis score(LS). Inflammatory activity was considered significant and consistent with Crohn's disease(CD) when the LS ≥ 135. The definitive diagnosis during follow-up(minimum 12 mo following SBCE) was based on the combination of clinical, analytical, imaging, endoscopic and histological *** Thirty-six patients were included, 21 females(58%) with mean age at diagnosis of 33 ± 13(15-64) years. The mean follow-up time after the SBCE was 52 ± 41(12-156) mo. The SBCE revealed findings consistent with significant inflammatory activity in the small bowel(LS ≥ 135) in 9 patients(25%); in all of them the diagnosis of CD was confirmed during follow-up. In 27 patients(75%), the SBCE revealed no significant inflammatory activity(LS < 135); among these patients, the diagnosis of Ulcerative Colitis(UC) was established in 16 cases(59.3%), CD in 1 case(3.7%) and 10 patients(37%) maintained a diagnosis of IBDU during follow-up. A LS ≥ 135 at SBCE had a sensitivity = 90%, specificity = 100%, positive predictive value = 100% and negative predictive value = 94% for the diagnosis of *** SBCE proved to be fundamental in the reclassification of patients with IBDU. Absence of significant inflammatory activity in the small intestine allowed exclusion of CD in 94% of cases.

Physiological and Biochemical Characterization of JilóSeeds (Solanum gilo) in Different Harvest Times

The evaluation of seed quality during the development process is important for the definition of the moment of harvest. In this context, the objective in this study was to evaluate the quality of Jiló seeds during development through physiological, biochemical and electrophoretic tests of enzymes. Six genetic materials, JIL white, JIL 001, JIL 005, JIL 006, H1 and H2 were used. Fruits were harvested at 35, 40, 45 days after anthesis (DAA). A total of 50% of all fruits harvested at 45 DAA remained at rest for seven days which were labeled as 45 DAAr. The seeds at each maturation stage were extracted from the fruits and evaluated by water content, weight of one thousand seeds, electric conductivity, germination, germination speed index, seedling emergence, and seedling emergence rate index. The chemical composition of the seeds was also evaluated, as well as the expression of the enzymes esterase, isocitrate lyase, malate dehydrogenase, alcohol dehydrogenase, superoxide dismutase, catalase, peroxidase and endo-β-mannanase. The highest values of germination and vigor were found in seeds harvested at 45 DAA and 45 DAAr, mainly for hybrids H1 and H2. The highest values of lipids and proteins and lower carbohydrates were found in the most immature seeds of Jiló. The enzymes analyzed showed higher expression in the initial stages of development, however reduced with the advancement of the maturation stage. Endo-β-mannanase expression increased with seed development. Jiló seeds harvested at 45 DAAr presented the highest of germination and vigor values. This “being” considered as the ideal harvest point.

Recent Extremes of Drought and Flooding in Amazonia: Vulnerabilities and Human Adaptation

The present study focuses on the impacts of extreme drought and flooding situations in Amazonia, using level/discharge data from some rivers in the Amazon region as indicators of impacts. The last 10 years have featured various “once in a century” droughts and floods in the Amazon basin, which have affected human and natural systems in the region. We assess a history of such hazards based on river data, and discuss some of the observed impacts in terms of vulnerability of human and natural systems, as well as some of adaptation strategies implemented by regional and local governments to cope with them. A critical perspective of mitigation of drought and flood policies in Amazonia suggests that they have been mostly ineffective in reducing vulnerability for the majority of the population, constituting, perhaps, examples of maladaptation via the undermining of resilience.

Impact of the timing of capsule endoscopy in overt obscure gastrointestinal bleeding on yield and rebleeding rate-is sooner than 14 d advisable?

AIM To evaluate the impact of the timing of capsule endoscopy(CE) in overt-obscure gastrointestinal bleeding(OGIB). METHODS Retrospective, single-center study, including patients submitted to CE in the setting of overt-OGIB between January 2005 and August 2017. Patients were divided into 3 groups according to the timing of CE(≤ 48 h; 48 h-14 d; ≥ 14 d). The diagnostic and therapeutic yield(DY and TY), the rebleeding rate and the time to rebleed were calculated and compared between groups. The outcomes of patients in whom CE was performed before(≤ 48 h) and after 48 h(> 48 h), and before( 48 h groups(P = 0.03).CONCLUSION Performing CE within 48 h from overt-OGIB is associated to a higher TY and a lower rebleeding rate and longer time to rebleed.

The prevalence of pediatric obesity continues to increase worldwide,bringing with it various metabolic,functional,social,and psychological complications.1 Both the prevention and treatment of childhood obesity must be based on multidisciplinary approaches combining nutrition,physical activity(PA),and psychological support.2 As an essential element of these multicomponent strategies,regular physical activity has been acknowledged as having beneficial effects on children's and adolescents,body composition,physical fitness,and metabolic profile,as well as on their health-related quality of life,social and psychological health,and academic achievement.3,4 Given that only a relatively small proportion of children and adolescents meet the public health recommendations for PA,5 and given that weight loss exercise-based interventions suffer from a high attrition rate,there is a clear need for appropriate PAs.

Many studies have been dedicated to the development of scaffolds for improving post-traumatic nerve regeneration. The goal of this study was to assess the effect on nerve regeneration, associating a hybrid chitosan membrane with non-differentiated human mesenchymal stem cells isolated from Wharton＇s jelly of umbilical cord, in peripheral nerve reconstruction after crush injury. Chromosome analysis on human mesenchymal stem cell line from Wharton＇s jelly was carried out and no structural alterations were found in metaphase. Chitosan membranes were previously tested in vitro, to assess their ability in supporting human mesenchymal stem cell survival, expansion, and differentiation. For the in vivo testing, Sasco Sprague adult rats were divided in 4 groups of 6 or 7 animals each： Group 1, sciatic axonotmesis injury without any other intervention （Group 1-Crush）; Group 2, the axonotmesis lesion of 3 mm was infiltrated with a suspension of 1 250 -1 500 human mesenchymal stem cells （total volume of 50 pL）（Group 2-CrushCell）; Group 3, axonotmesis lesion of 3 mm was enwrapped with a chitosan type Ill membrane covered with a monolayer of non-differentiated human mesenchymal stem cells （Group 3-CrushChitlllCell） and Group 4, axonotmesis lesion of 3 mm was enwrapped with a chitosan type III membrane （Group 4-CrushChiUll）. Motor and sensory functional recovery was evaluated throughout a healing period of 12 weeks using sciatic functional index, static sciatic index, extensor postural thrust, and withdrawal reflex latency. Stereological analysis was carded out on regenerated nerve fibers. Results showed that infiltration of human mesenchymal stem cells, or the combination of chitosan membrane enwrapment and human mesenchymal stem cell enrichment after nerve crush injury provide a slight advantage to post-traumatic nerve regeneration. 自从里根于1983年3月23日向全国发表电视演说以来,他的战略防御倡义(SDI)已在大西洋两岸赞同派和反对派之间引起了热烈的辩论,辩论的中心问题是关于这种防御系统的价值和技术可行性。作为外行,我们认为,无论是直接同SDI机构搞合作,还是搞独立的欧洲防御倡议(EDI),都能产生实际的效果。这些计划不但能防御近程和中程的弹道导弹,而且能加强欧洲的常规防御力量。本文首先回顾一下弹道导弹防御(BMD)的基本目的,然后研究苏联对欧洲的特殊威胁,欧洲战略防御系统可能是个什么样以及可能采用的武器技术。最后讨论与核威胁无直接关系的在防御策略方面可能带来的好处。
