Background: measles is a highly contagious infection caused by the measles virus with a worldwide distribution. measles is one of the diseases that have been reported in our country since 1945. It is known that international travelers are an important source of infectious pathologies. Our goal is to document a case of imported measles and the difficulty of diagnosing it, especially in non-epidemic times. Cases Presentation: We presently report a 20-year-old woman who was hospitalized at the Infectious disease Service for fever and maculopapular rash. She had traveled outside of Albania. measles ELISA IgM (blood) resulted positive while other serological examinations resulted negative. Our case was treated with antibiotics, multivitamins and intravenous fluids. She was subsequently discharged home in good clinical condition. Conclusions: measles should be included in the differential diagnosis of patients with symptoms of fever and rash, in particular when they have traveled abroad. Patients who have received the measles vaccine should not be excluded from clinical suspicion and further diagnostic tests for this disease as it can affect this group of patients as well.

The period in Kosovo from the 1970s to the early 1980s is important yet insufficiently explored. The period is characterized by a liberal approach toward the development of the region. Such development of Kosovo was evident in all spheres of life, including architecture, which functions as an integral part of a society and its progress. This progress was ＂supported＂ with an intention to overpower the existing architectural environment and the overall identity of the region in some way. However, when this progress was realized wherein numerous buildings considered important were materialized, an intention to have the earlier and ＂original＂ spatial structures to be replaced became evident. In the case of Kosovo, the term ＂liberal＂ can be understood negatively and positively. From the architectural viewpoint, desi^Ins tend to follow technological advances and ＂trends＂ of the time. Consequently, the materialization of such buildings, which are surrounded by symbolisms related to different types of identity and leading toward regional modernism, is an important part to emphasize. The current study aims to consider important thoughts about architectural identity and its categories in general but specific to the context of Kosovo. The history of the region has a critical story of influences toward its formation. Although several philosophies were accepted by the mass, others produced the antidote of the actual input. This study considers theimportance of finding the best possible interpretation of Kosovo＇s architectural identity, such that Kosovo can be well known in the future as it was in the past.

Sepsis: A Systematic Review of Antibiotic Resistance and Antimicrobial Therapies

Sepsis is a medical emergency that depicts the body’s systemic immune reaction to an infectious process that could result in organ dysfunction and death. Early sepsis pathogen identification and early delivery of antimicrobial therapy result in better clinical outcomes for patients diagnosed with sepsis and septic shock. method: Systematic reviews and meta-analyses that discussed sepsis pathogen identification, antibiotic resistance and timing in the treatment of sepsis were taken into account irrespective of the approach of the included studies, quantitative or qualitative. This literature review gathers data from twelve primary studies to perform a systematic review and evaluate existing empirical antibiotic therapies in order to treat the various degrees of sepsis and minimize antibiotic resistance. The remaining reviews, case studies, etc., were used as supplementary references. The studies compiled data from laboratory tests including lactate levels, fluid resuscitation, and diagnostic diagnostics like the SOFA score. Results: This paper identifies multiple factors that must be taken into account when treating individuals with sepsis. Prior to the administration of broad-spectrum empiric antibiotics, the severity and morbidity of the disease must be addressed. Excessive antibiotic usage has been associated with increased sepsis hospitalized mortality rates, and thus it is crucial to minimize antibiotic misuse in non-septic patients. This can be accomplished by differentiating between Gram-negative and Gram-positive bacteria, the purpose of biomarkers and identifying the antibiotic resistance pathway. Many patients from low economic statuses may experience difficulties accessing healthcare services and resources for sepsis treatment when diagnosed with sepsis. We describe a case of mediterranean spotted fever complicated with thrombosis of the left central retinal vein. A 41-year-old woman patient living in the city Scodra was referred to our hospital in October 2017 for high fever 40°C, muscular and articular pains, severe headache, maculopapular rash, chills, photophobia and visual loss in his left eye. Ocular examination showed vision acuity: OD 8/10, OS 2/10. Funduscopic examination complemented with fluorescein angiography showed: optic nerve papilla with clear contours, diffuse hemorrhages in the contest off retinal vein thrombosis, white retinal lesions, vascular sheathing and macular cystoids edema. R. conorii antibodies were identified by ELISA anti Rickettsia conorii IgM, IgG and Weil-Felix Ox-19 test. Investigation of other infective agents and the immunological panel were negative. After treatment with doxycycline 200 mg/day for 10 days vision acuity was OD: 8/10 OS: 6/10;FO: There are less hemorrhages in fluorescein angiograph and OCT showed a reduced macular cystoids edema. mediterranean spotted fever should be considered in the differential diagnosis of a patient who presents with an acute febrile disease accompanied by maculopapular rash especially in the seasons of spring, summer or autumn.

Association of human leukocyte antigen DQB1 and DRB1 alleles with chronic hepatitis B

AIM: To investigate the effect of human leukocyte antigen(HLA) DRB1 and DQB1 alleles on the inactive and advanced stages of chronic hepatitis ***: Patient records at a single institution's hepatology clinic were reviewed. Demographic data, laboratory results, endoscopy results, virological parameters, biopsy scores and treatment statuses were recorded. In total, 355 patients were eligible for thestudy, of whom 226(63.7%) were male. Overall, 82(23.1%) were hepatitis B early antigen(HBeAg) positive, 87(24.5%) had cirrhosis, and 66(18.6%) had inactive disease. The presence of DQB1 and DRB1 alleles was determined by polymerase chain reaction with sequence-specific primers. The distribution of the genotyped alleles among patients with cirrhosis and patients with chronic active hepatitis was ***: The most frequent HLA DQB1 allele was DQB1*03:01(48.2%), and the most frequent HLA DRB1 allele was DRB1*13/14(51.8%). DQB1*05:01 was more frequent in patients with active disease than in inactive patients(27% vs 9.1%; P = 0.002, Pc = 0.026). DRB1*07 was rare in patients with cirrhosis compared with non-cirrhotics(3.4% vs 16%; P = 0.002, Pc = 0.022). Older age(P dictor of cirrhosis(P = 0.015). A bioinformatics analysis revealed that a polymorphic amino acid sequence in DRB1*07 may alter interaction with the T-cell recognition ***: This study demonstrates that HLA alleles may influence cirrhosis development and disease activity in Turkish chronic hepatitis B patients.

The Use of Mineral Nutrients for Biomass Production by Young Birch Stands and Stands Vitality in different Forest Growing Conditions

The data of field studies about mineral nutrient content in the biomass components of young birch stands (9-15 years old) in different forest growing conditions and soil types have been analyzed. In forest growing conditions on fertile soils (ASG, TSC and SP) the total amount of biomass produced by young birch stands divides into fractions as follows: stem wood 51.8%-59.5%; branch wood 9.8%-12.4%; foliage 5.7%-6.8%; stump wood and roots 25%-30%. In forest types on lean soils (TP) the same indices are 32.3%-41.8%, 18.2%-24.2%, 13.2%-16.1% and 26.8%-27.4%, respectively. The stand performance closely correlates with the basic nutrient (P, N, K, C and Mg) availability in forest soils. In lean typicpodzol soils (TP) the content of mineral nutrients is no higher than 20%-48% of that in more fertile soils (ASG, TSC and SP). In young birch stands the take-up of mineral nutrients from 0-40 cm soil layer for developing the above-ground biomass makes a fairly small proportion of the total: up to 4.8%-6.2% for P; 4.9%-12.2% for N; 1.1%-4.1% for K; 11.6% for Ca; 0.8%-7.7% for Mg; in leaner soils the same indices are P 0.1%-0.4%, N 1%-1.5%, K 0.2%-0.6%, Ca 0.1%-0.9%, Mg 0.1%-0.8%, respectively. The analyses of pest damages in young birch stands confirm a hypothesis that the degree of pest damage depends on the stand vitality as described by the site index.

背景与目的脂肪移植是一项很有前景的软组织填充增大技术，但移植疗效持久性存在高度不可预知性，而且影响移植成活率的因素尚未明确。因为具有分化的潜能和释放生长因子的能力，脂肪来源干细胞（adipose-derivedstemcells，ADSCs）可在移植修复过程中起关键作用。我们旨在确定ADSCs在人体脂肪中呈现的生物学特性是否与移植体积的体内效果保持存在关联。方法利用标准的离心分离技术对从8例人体受术者获取的脂肪抽吸物进行处理，然后将其注射至6周龄无胸腺裸鼠躯体两侧。测量移植物的质量和体积，于移植后8周进行包含针对血管的CD31^＋染色的组织学评估。从各供体脂肪抽吸物中分离获取基质血管细胞群（stromalvascularfraction，SVF），利用多参数流式细胞术对其表面标记物进行分析，并对其增值、分化能力和正常含氧量／低含氧量血管内皮细胞生长因子（vas—cularendothelialgrcwlhfactor，VEGF）的分泌情况进行实验研究。结果从不同供体中获取的SVF，其CD34^＋祖细胞含量百分率存在较大差异，其平均值为21．3％±15．O％（X±s）。SVF细胞的增殖速率和成脂潜能在不同供体间存在较为中等差异。在小鼠异种移植研究中，移植后8周体积保存的量约为36％～68％，总体平均值为52％±11％。SVF中CD34^＋祖细胞的含量与脂肪移植效果持久性方面存在较强的关联（P〈0．05）。结论在不同患者之间，其脂肪组织的内在生物学特性存在差异。特别是SVF中CD34^＋祖细胞的浓度可能是影响脂肪移植持久性的因素之一。

目的在围介入治疗间期,探讨一种疗效好、副作用少的辅助治疗方法 .方法建立肝癌细胞CBRH7919的大鼠动物模型,选22只接种瘤块组织的Wistar大鼠,3周瘤块形成后全部给予肝动脉结扎.分为2组：（1）对照组（12只）：给予腹腔注射等量生理盐水,（2）干扰素-α（IFN-α）和全反式维甲酸（ATRA）联合高剂量组（10只）：腹腔给予IFN-α（1000 000 U·kg~^-1·qod~^-1）隔天1次,腹腔给予ATRA（10 mg·kg-1·d-1）1次/d,于肝动脉结扎后第1天开始用药,治疗10 d后处死动物,测肿瘤组织中转移相关基因表达水平、蛋白表达水平、肿瘤微血管密度及肿瘤细胞凋亡.结果 RT-PCR法检测肿瘤组织中转移相关基因VEGF、bFGF的mRNA表达,两组相比,IFN-α联合ATRA处理组的肿瘤组织中bFGF和VEGF表达显著下降（P＜0.05）.免疫组化法检测肿瘤组织中VEGF、bFGF的蛋白表达水平与RT-PCR结果一致,IFN-α和ATRA联合处理组的肿瘤组织中bFGF和VEGF下降明显,bFGF表达很弱,而VEGF几乎不表达.ELISA法检测血清中VEGF水平,对照组血清VEGF浓度为（92.5±13.9）pg/ml,IFN-α和ATRA联合处理组血清VEGF浓度为（69.85±20.4）pg/ml,两者相比有统计学差异（P＜0.05）.免疫组化法检测肿瘤微血管密度（MVD）,对照组的肿瘤组织中可见到较多的新生血管,而在IFN-α和ATRA联合处理组的肿瘤组织中新生血管明显减少.MVD在对照组为（114±18）/HP,IFN-α和ATRA联合处理组为（66±5）/HP（P＜0.01）.肿瘤细胞凋亡的检测,两组相比,IFN-α和ATRA联合处理组,肿瘤组织出现较大面积的坏死,流式细胞仪检测发现坏死细胞（Annexin V-PI＋）显著增多,而早期凋亡细胞比例并明显增高.TUNEL法镜下观察到凋亡细胞的细胞核被染成蓝紫色,染色质分布不均.对照组和治疗组的凋亡指数分别为（3.74±1.57）%、（12.34±4.78）%,差异具有显著性（P＜0.05）.结论联合应用IFN-α及ATRA可以抑制肿瘤新生血管形成,进而抑制肝癌生长和转移,提示两种作为抗肿瘤血管形成的药物在介入治疗间期的早期联合应用,在临床防治肝癌术后的复发转移中有一定的作用.
