Augmentation of nasal tip and/or dorsum forms the keystone of any Septorhinoplasty surgery. The grafts available for augmentation are numerous and varied. Choice of the graft depends upon the type of augmentation required, patient characteristics and, most importantly, the surgeon. In this article, we would like to present our experience with various augmentation grafts. In our experience, autografts are best grafts for augmentation. But in cases of revision surgeries or deficiency of autografts, allografts can be used. Our choice of allograft is Poly Diaxone Sheath or PDS.

Intratympanic Injections: An Unsolved Mystery

Aims: The aim of this study was to test the effectiveness of intratympanic dexamethasone injections as a treatment for severe disabling tinnitus and also observe its effect on hearing loss if any. Materials and Methods: Thirty patients with severe disabling tinnitus in the age group 20 to 60 years were selected and randomly assigned to receive intratympanic injections of a dexamethasone solution 4 mg/ml (0.5 ml) or isotonic saline (0.5 ml) solution under topical anaesthesia, once per week for 4 weeks using a zero degree endoscope. Improvement in tinnitus was assessed using a visual analog scale, considering 2-point improvement as significant and alteration in hearing if any was noted by pure tone audiometery before and after the therapy. Results: The improvement in tinnitus was not significant, with no alteration in audiometery reports. Conclusions: Intratympanic therapy is an attractive mode of treatment because of its highly targeted delivery, low concentration of the drugs required and a very good patient tolerance. Although there has been no breakthrough in intratympanic therapy for tinnitus or other otological conditions, accessibility to the inner ear through the semipermeable round window membrane holds many promises in the near future.

Primary Laryngeal Amyloidosis: A Case Report

Amyloidosis is a benign, indolent condition which presents with an abnormal deposition of extracellular protein in various organs of the body. It usually presents with hoarseness of voice to an otolaryngologist. A sits clinical appearance mimics an early laryngeal cancer. It remains a diagnostic challenge to the clinician. It is essential to differentiate a primary laryngeal amyloidosis from malignancy to have a holistic approach towards the management of the disease. Here in this article, a case is reported in order to have a better outlook about this rare condition and to differentiate it from laryngeal malignancy for an effective management and cure.

An organization’s costing system is a system that helps the management with the strategy planning while the system plays an important role in providing accurate cost information about the products and *** other words,the costing management system is important to provide timely and quality information to help managers in their decision-making *** quality graduates is the main objective of any university and the cost of quality or the cost of poor quality is one that is often difficult to measure in higher *** can be partly attributed to the fact that most accounting systems are not structured to capture important cost-of-quality *** order to succeed,many organizations tend to shift from conventional or traditional costing system to Activity-based costing(ABC).ABC provides both higher education administrators and policymakers with better information on which to base *** use of multiple cost pools and drivers under ABC leads to more detailed and accurate product costing than that provided by traditional cost *** paper attempts to investigate the possibilities and limitations of implementing ABC in *** specifically the study makes use of the case study method to illustrate the implementation of activity-based costing of library services in a private university in the Kingdom of *** research methodology of the case study is a combination of both descriptive and quantitative *** study identified six categories of cost pools which are related to the activities of the library as well as 17 relevant cost ***,ABC should lead to a better understanding of what drives costs and,by extension,what changes are necessary to reduce *** activities that do not add value to services can be identified and *** ABC is feasible in an academic library and that much can be gained from the process of analyzing ***,more work needs to be done

Thornwaldt Cyst: Case Report

Thornwaldt cyst is benign, mucosal congenital cyst which is located in upper nasopharynx. It developed in nasopharyngeal bursa if opening of bursa is occluded due to infection or inflammation. It is rare congenital cyst present in nasopharyngeal bursa. Its incidence is 3% in adults [1]. The usual age of presentation is 2nd and 3rd decade [1]. It is usually asymptomatic and incidental finding on MRI but it can present as nasal obstruction, post nasal drip, halitosis, occipital headache, foreign body sensation in throat [1] [2]. Here we are presenting a case report of 23 years old male presented with foreign body sensation in throat since 5 months. On oral and nasal endoscopic examination cystic mass in nasopharynx seen, bulging in oropharynx. MRI shows cystic mass arising from left side of nasopharynx popping up in oropharynx. Cyst then excised with diode laser with both endonasal and transoral approach with zero degree rigid endoscope. Histopathology confirmed the cyst as thornwaldt cyst.

Burn-Up Measurements on Dissolver Solution of Mixed Oxide Fuel Using HPLC-Mass Spectrometric Method

Burn-up measurement on an irradiated mixed oxide (MOX) test fuel pellet was carried out through measurements on the dissolver solution by HPLC-Thermal Ionization Mass Spectrometric (TIMS) technique. The studies carried out using HPLC as well as TIMS for quantification of burn-up value are described. While in one case, both the separation and determination of elements of interest (U, Pu and Nd) were carried out by HPLC;in another case, TIMS technique was used to quantify them from the HPLC separated *** rapid separation procedures developed in our laboratory earlier were employed to isolate pure fractions of the desired elements. The individual lanthanide fission products (La to Eu) were separated from each other using dynamic ion-exchange chromatographic technique whereas uranium and plutonium were separated from each other using reversed phase chromatographic technique. The pure fractions of U, Pu and Nd obtained after HPLC separation procedure for “spiked” and “unspiked” dissolver solutions were used in TIMS measurements for the first time in our laboratory. In TIMS analysis, isotopic abundances of uranium, plutonium and neodymium fractions obtained from HPLC separation procedure on an “unspiked” fuel sample were measured. For the determination of U, Pu and Nd by isotopic dilution mass spectrometric technique (IDMS), known quantities of tracers enriched in 238U, 240Pu and 142Nd were added to the pre-weighed dissolver solution and subjected to HPLC separation procedures. The isotope ratios viz. 142Nd/(145Nd +146Nd), 238U/233U and 240Pu/239Pu in the pertinent “spiked” fractions were subsequently measured by TIMS. The spikes were pre-standardized in our laboratory employing reverse isotopic dilution technique against the standard solutions available in our laboratory (for 238U, 239Pu and 142Nd, standard solutions of 233U, 239Pu (of higher abundance than in the sample) and 150Nd were employed as spikes). Ovarian cancer is the second most common gyneco-logical cancer and the leading cause of death in the United States. In this article we review the diagnosis and current management of epithelial ovarian cancer which accounts for over 95 percent of the ovarian malignancies. We will present various theories about the potential origin of ovarian malignancies. We will discuss the genetic anomalies and syndromes that may cause ovarian cancers with emphasis on Breast cancer type 1/2 mutations. The pathology and pathogenesis of ovarian carcinoma will also be presented. Lastly, we provide a comprehensive overview of treatment strategies and staging of ovarian cancer, conclusions and future directions.
