AIm To analyze the effect of intralesional steroid injections in addition to endoscopic dilation of benign refractory esophageal *** A comprehensive search was performed in three databases from inception to 10 April 2017 to identify trials, comparing the efficacy of endoscopic dilation to dilation combined with intralesional steroid injections. Following the data extraction, meta-analytical calculations were performed on measures of outcome by the randomeffects method of Der Simonian and Laird. Heterogeneity of the studies was tested by Cochrane's Q and I^2 statistics. Risk of quality and bias was assessed by the Newcastle Ottawa Scale and JADAD assessment *** Eleven articles were identified suitable for analyses, involving 343 patients, 235 cases and 229 controls in total. Four studies used crossover design with 121 subjects enrolled. The periodic dilation index(PDI) was comparable in 4 studies, where the pooled result showed a significant improvement of PDI in the steroid group(mD:-1.12 dilation/month, 95% CI:-1.99 to -0.25 P = 0.012; I^2 = 74.4%). The total number of repeat dilations(TNRD) was comparable in 5 studies and showed a non-significant decrease(mD:-1.17, 95%CI:-0.24-0.05, P = 0.057; I^2 = 0), while the dysphagia score(DS) was comparable in 5 studies and did not improve(SmD: 0.35, 95%CI:-0.38, 1.08, P = 0.351; I^2 = 83.98%) after intralesional steroid *** Intralesional steroid injection increases the time between endoscopic dilations of benign refractory esophageal strictures. However, its potential role needs further research.

Shortwave radiometers such as pyranometers, pyrheliometers, and photovoltaic cells are calibrated with traceability to consensus reference, maintained by Absolute Cavity Radiometers (ACRs). The ACR is an open cavity with no window that measures the extended broadband spectrum of the terrestrial direct solar beam irradiance, unlike shortwave radiometers that cover a limited range of the spectrum. The difference between the two spectral ranges may lead to calibration bias that can exceed 1%. This article describes a method to reduce the calibration bias resulting from using broadband ACRs to calibrate shortwave radiometers by using an ACR with Schott glass window to measure the reference broadband shortwave irradiance in the terrestrial direct solar beam from 0.3 μm to 3 μm. Reducing the calibration bias will result in lowering the historical solar irradiance by at least 0.9%. The published results in this article might raise the awareness of the calibration discrepancy to the users of such radiometers, and open a discussion within the solar and atmospheric science community to define their expectation from such radiometers to the radiometers’ manufacturers and calibration providers.

氢气是炼油厂加工烃类和提高油品质量的重要原料气,但是在工业上氢气的制备成本高,其主要来源于化石资源。在石油炼制过程中,会产生大量富含氢气和烃类等高价值组分的炼厂气,常被作为燃料气直接燃烧,造成了严重的资源浪费。因此,利用高效的分离方法和工艺设计,从炼厂气中回收氢气是近年来重要的提高氢气利用效率的方法。但是,炼厂尾气存在来源复杂、多组分、分离方法多等特点,确定合适的分离方法和优化的分离序列十分困难,因此存在“组合爆炸”的问题。基于以上问题,本论文基于三角坐标-矢量分析的图形设计法,得到了10股炼厂气综合回收的优化分离序列流程和设计方案,基于流程模拟,结合灵敏度分析和响应面法,对其过程参数进行优化,以及对整个过程系统进行技术经济评价,实现了氢气、轻烃等资源组分的高效回收。首先,在三角坐标系下,以组分的产品归属及分离性质相似程度为合并法则,将原料简化为燃料气、氢气和轻烃构成的三元体系,将来自中国某石化公司的10种股不同成分的炼厂尾气标注在三角坐标体系中。根据炼厂气组成的特点,将位于三角坐标系同一分离单元优势区域的原料气进行合并,得到两个混合物流,分别落在变压吸附(PSA)分离区的F1和浅冷(SC)分离区的F2。基于该两个混合原料物流,通过绘制对应的分离单元矢量对,依次确定对应的分离方法,得到最终的包含浅冷(SC)、氢气膜分离(GPm)、轻烃膜分离(RPm)和变压吸附(PSA)四种技术的梯级耦合回收工艺。最后采用多塔精馏工艺对冷凝轻烃进行分离,得到液化石油气(LPG)、轻石脑油(C+)和乙烷(CH)等产品。然后,利用Uni Sim Design对整个过程进行模拟,通过灵敏度分析初步确定了膜面积(GPm和RPm)切割比、膜渗透侧压力、精馏塔回流比和塔板数等过程参数适宜值,充分利用分离过程的“双向富集”效应。结果表明,当GPm和RPm的面积分别为3772.6m和600 m时,GPm和RPm的级切割分别为0.51和0.80。最后,利用响应面方法(RSm)对过程参数进行优化。根据所建立的流程模型,首先通过基于(Central Composite Design,CCD)的实验设计方法筛选出对年度总收益(TAP)具有显著性影响的因素:GPm膜面积、RPm膜面积和压缩机K-2的出口压力,然后通过最陡爬坡法确定响应面分析的中心点,最后根据Box-Behnken中心组合实验设计原理,拟合了TAP与三个关键因素的关联方程并进行响应面分析,得出冷凝-膜-PSA-精馏耦合系统的最优操作参数使TAP最大。通过对工艺方案进行经济评估,确定了年度运营费用和总投资,结果表明优化方案的年生产总利润为38.62×10美元,投资回收期小于4个月,氢气和轻烃回收率分别达到96%和99%,氢气生产单耗仅0.227 k Wh·Nm,与国外专利技术相比,能耗减少幅度达27%。

BACKGROUND The debate regarding diagnostic classification systems in psychiatry(categorial vs dimensional systems)has essential implications for the diagnosis,prevention and treatment of stress *** previously found a unique pattern of stress reaction in a study executed during the coronavirus disease 2019 pandemic using large representative samples in two countries,and termed it the Complex Stress Reaction Syndrome(CSRS).AIm To investigate CSRS,Type A(psychiatric symptoms,spanning anxiety,depression,stress symptoms,and posttraumatic stress disorder(PTSD)),with or without long-coronavirus disease(COVID)residuals(CSRS,Type B,neuropsychiatric symptoms spanning cognitive deficits and fatigue,excluding systemic symptoms).Our two-tailed hypothesis was that CSRS is a condition related to an unrecognized type of stress reaction in daily life in the general population(Type A)or that it is related to the severe acute respiratory syndrome coronavirus 2 infection and its long-COVID residuals(Type B).mETHODS 977 individuals in four continents(North America,Europe,Australia and the middle East)completed the online study questionnaire in six languages using the Qualtrics *** study was managed by six teams in six countries that promoted the study on social *** questionnaire assessed anxiety,depression,stress symptoms and PTSD(CSRS,Type A),cognitive deficits and fatigue(CSRS,Type B).The data were analyzed using Proportion Analyses,multivariate Analysis of Co-Variance(mANCOVA),linear regression analyses and validated clinical cutoff *** The results of the Proportion Analyses showed that the prevalence of 4 symptoms spanning anxiety,depression,stress symptoms,and PTSD was significantly higher than the most prevalent combinations of fewer symptoms across 4 continents,age groups,and *** supports the transdiagnostic argument embedded in the CSRS(Type A).The same pattern of results was found in infected/recovered *** prevalence of the

OBJECTIVE: To describe the disease course and natural history of Type A Niemann-Pick disease (NPD). mETHODS: Ten patients with NPD-A (six male, four female; age range at entry: 3 to 6 months) were serially evaluated including clinical neurologic, ophthalmologic, and physical examinations, and assessment of development. Laboratory analyses, abdominal and brain ultrasounds, and chest radiographs also were obtained and information on intercurrent illnesses and cause of mortality was collected. RESULTS: All affected infants had a normal neonatal course and early development. The first symptom detected in all patients was hepatosplenomegaly. Developmental age did not progress beyond 10 months for adaptive behavior, 12 months for expressive language, 9 months for gross motor skills, and 10 months for fine motor skills. Non-neurologic symptoms included frequent vomiting, failure to thrive, respiratory infections, irritability, and sleep disturbance. Neurologic examination at the time of presentation was normal in most patients. Later neurologic examinations revealed progressive hypotonia with loss of the deep tendon reflexes. All patients had cherry red spots by 12 months. The median time from diagnosis to death was 21 months. The cause of death was respiratory failure in nine patients and complications from bleeding in the tenth. CONCLUSIONS: The clinical course in Type A Niemann-Pick disease is similar among affected patients and is characterized by a relentless neurodegenerative course that leads to death, usually within 3 years.

Background: Burns is one of the most common and severe forms of trauma, as they cause an alteration in the protective barrier exerted by the skin. The objective of this work is to describe the bacterial pathogens that cause infections in the different body segments as well as to know the susceptibility in patients with burns during a period of two years. methods: This study was conducted over a period of 2 years, January 2017 to January 2019. The analysis of the isolated microbiological agents was obtained from culture samples in different anatomic areas. The variables were measured in the general population and also by groups, considering: age, sex, TBSA with Wallace's rule of nine, anatomic area affected, type of burn, depth of burn, microorganism reported in cultures, antibiotics empirically administered and mortality rate. Results: Regardless of the anatomical segment studied, the most frequently isolated microorganism was m>Pseudomonas aeruginosam>. Conclusions: Identification of the microorganisms that cause infection reduces the hospital stay, as well as the admission to the ICU. A fundamental research guideline is to pay more attention to the post-burn consequences, and the sequelae and functional limitations that it entails, since few since few studies focus on this aspect.
