椎弓峡部螺钉内固定及骨块移植术治疗脊柱分离与滑脱

脊柱分离或滑脱是由峡部缺损所致。Wilfse等认为峡部缺损与反复劳损致疲劳骨折有关,但也有一定的遗传因素。大多数脊柱分离和轻度滑脱的病人,多采用非手术治疗,但仍有持续性疼痛的患者可手术治疗,传统的方法是脊柱融合术。

OBJECTIVES To investigate complications within 30-days following first-time ablation for atrial fibrillation(AF),including a composite of cardiac tamponade,hematoma requiring intervention,stroke or death,in patients≥75 years of age,compared to pa-tients aged 65−74 *** addition,one-year all-cause mortality and AF relapse were ***&RESULTS All patients receiving their first catheter ablation for AF between 2012 and 2016 were identified us-ing Danish nationwide *** aged 65−74 years served as the reference group for patients≥75 *** of AF within one year was defined as cardioversion following a three-month blanking period,re-ablation or confirmed relapse within *** composite complication outcome did not differ between the two age groups,with 39/1554(2.8%)in patients 65−74 years of age,versus 5/199(2.5%)in older patients(adjusted HR=0.94),95%CI:0.37−2.39,P=0.896).Patients≥75 years or older had no increased hazard of death within 30 days after the procedure,with an incidence of 3/1554(0.2%)in younger pa-tients and 2/199(1.0%)in patients≥75 years of age(adjusted HR=4.71,95%CI:0.78−28.40,P=0.091).There was no difference in relapse of AF after one year between age groups(≥75 years adjusted HR=1.00,95%CI:0.78-1.26,P=0.969).CONCLUSION In patients≥75 years of age selected for catheter ablation for AF,the incidence of periprocedural complica-tions,as well as one-year freedom from AF showed no statistical difference,when compared to patients 65−74 years of age.

＜正＞ AAR was recognised as a durability challenge in Norway some 15 years ago. Since then, several comprehensive national research projects on this subject have been carried out. These projects have strongly focused on test methods and corresponding criteria for the prediction of AAR as observed on Norwegian concrete structures. In 1996 the Norwegian Concrete Association produced a publication giving guidelines for handling AAR in Norway. Based on technological knowledge obtained after 1996, and the fact that the publication now shall be a harmonised normative reference document to the new concrete materials standard, NS-EN 206, a revised version of the publication will be provided in September *** 2004 edition is divided into two major parts. Part 1 is in formal terms describing the mandate and the use of the publication, and how concrete constituents and concrete recipes shall be tested and evaluated with respect to potential AAR. Individual aggregates and blends of aggregates shall be evaluated by petrographical analyses as a first step. The evaluation based on result from these analyses can be reassessed by a mortar bar expansion test, while a concrete prism expansion test can be used to reassess the evaluation from any of these tests. For the evaluation of binders and concrete compositions, only the concrete prism expansion test can be used. Part 2 gives advisory guidelines for how the concrete industry can fulfil the requirements given in the Part 1 specifications. More complementary descriptions of the test methods and corresponding evaluation criteria are presented and illustrated by examples. Part 2 also provides a survey of binders and corresponding alkali contents documented to be suited for production of AAR resistant concrete containing all types and amounts of Norwegian reactive aggregates. This survey will be updated whenever new binders obtain satisfactory documentation. The revised publication will be a valuable and updated tool for handling AAR

*** strongest locus which associated with type 2 diabetes(T2D)by the common variant rs7903146 is the transcription factor 7-like 2 gene(TCF7L2).We aimed to quantify the interaction of diet/lifestyle interventions and the genetic effect of TCF7L2 rs7903146 on glycemic traits,body weight,or waist circumference in overweight or obese adults in several randomized controlled trials(RCTs).*** October 2016 to May 2018,a large collaborative analysis was performed by pooling individualparticipant data from 7 *** RCTs reported changes in glycemic control and adiposity of the variant rs7903146 after dietary/lifestyle-related interventions in overweight or obese *** treatment interaction models which used the genetic effect encoded by the allele dose and common covariates were applicable to individual participant data in all *** the joint analysis,a total of 7 eligible RCTs were included(n=4,114).Importantly,we observed a significant effect modification of diet/lifestyle-related interventions on the TCF7L2 variant rs7903146 and changes in fasting *** with the control group,diet/lifestyle interventions were related to lower fasting glucose by-3.06(95%CI,-5.77 to-0.36)mg/dL(test for heterogeneity and overall effect:I^(2)=45:1%,pk ***,regardless of genetic risk,diet/lifestyle interventions were associated with lower waist ***,there was no significant change for diet/lifestyle interventions in other glycemic control and adiposity traits per one copy of TCF7L2 risk *** findings suggest that carrying the TCF7L2 T risk allele may have a modestly greater benefit for specific diet/lifestyle interventions to improve the control of fasting glucose in overweight or obese adults.
