Modalities of Prescription of Regular Physical Activity by Cardiologists in the Management of Hypertensive Patients at the Institute of Cardiology of Abidjan

Introduction-Objective: The clinical management of hypertension is well codified. It is based on drug and non-drug therapies, including regular physical activity. The aim was to evaluate the knowledge of physicians on the importance of regular physical activity (RPA) in the care of hypertensive patients at the Institute of Cardiology of Abidjan (ICA). Material and Methods: That was a prospective and descriptive study carried out from July 20, 2019 to September 7, 2019 at the Outpatient Department of the ICA. Our study population consisted of 32 physicians from the Outpatient Department of the ICA. To achieve our goal, we developed a questionnaire for the physicians, which included seventeen items related to the socio-occupational characteristics, to the level of knowledge of hypertension, to the attitudes and the practices in the management of hypertension, and to the factors limiting the accessibility to the practice of a physical activity. Results: The majority of the health professionals in our study were men (62.50%). Among 32 physicians, 56.25% were cardiologists and 75% had a seniority in position of 1 to 4 years. 81.25% of the physicians responded that the hypertensive patients could not only benefit from non-drug means. The suggestion of a complementary physical activity (jogging and brisk walking) to the hypertension treatment was found 137.5 times. Jogging and brisk walking were the main types of physical activity which were suggested to patients. All the physicians responded 3 times per week when they were asked about the frequency of activities proposed for the clinical management of patients with hypertension. Regarding the medical prescription on the type, frequency, and duration of RPA to patients, there was some discrepancy, although the majority of healthcare professionals recommended brisk walking and jogging. Aim: Physical activity and other lifestyle changes are an important part of the guidelines for anti-hypertensive therapy. The effect of physical activity on hypertension and heart disease has been well described. No study in our context has been done on this issue. The aim of this study was to evaluate regular physical activity in the management of hypertensive patients at the Cardiology Institute of Abidjan and the National Police Hospital. Material and methods: This was a prospective and descriptive study held in the External Consultations and Rehabilitation Departments of the aforementioned structures. It started on July 20, 2017 and ended on September 7, 2017. The data were analyzed on SPSS version 22. A p-value of less than 0.05 was considered to be statistically significant. Results: Out of the 190 hypertensive patients recruited and educated, only 70 actually participated in the physical activities that took place in the Rehabilitation Department of Heart Institute. The mean age of the patients was 52 years ± 15 years. The most affected age group was between 40 and 49 years of age (47.40%).There was a male predominance with a sex ratio of 1.11. 63.16% of the patients had a superior. Cardio-Renal Syndrome: Frequency and Associated Factors in the Abidjan Heart Institute’s Medical Department

Introduction: Cardio-renal syndrome (CRS) is a pathophysiological disorder of the heart and kidneys in which acute or chronic dysfunction of one organ can lead to acute or chronic dysfunction of the other. In Africa, particularly in C?te d’Ivoire, the incidence of cardio-renal syndrome is not precisely known. The aim of this study was to assess the frequency of CRS and to contribute to a better understanding of the condition in the medical department of the Abidjan Heart Institute. Materials and Methods: We conducted a prospective analytical study including all patients with heart failure hospitalised in the medicine department of the Abidjan Heart Institute from March to October 2020. Data were analysed using SPSS software version 22. Results: We included 111 patients in the study. The incidence of CRS was 64%, with a predominance of males (sex ratio 1.8). The mean age was 53 ± 15 years. Patients’ medical history was dominated by hypertension (56.8%), diabetes (15%), dyslipidaemia (18%), obesity (17.1%) and smoking (14.4%). The main causes of heart failure were dilated cardiomyopathy (22.8%) and ischaemic heart disease (21.4%). Symptomatology was mainly congestive heart failure (42.8%). Mean evaluated clearance (MDRD) was 39.9 ± 17.1 ml/min/m2. Doppler echocardiography showed a decrease in left ventricular ejection fraction in 74.3% of patients. Factors statistically associated with the occurrence of cardio-renal syndrome were: age > 60 years (p = 0.04), diabetes (p = 0.03), arterial hypertension (p = 0.001) and Hb Conclusion: The cardio-renal syndrome is a reality and marks an important point in the evolution of cardiac and renal diseases. It is highly frequent in the medical department of the Abidjan Heart Institute, as well as a high rate of CKD.

Hypertensive Retinopathy in Normotensive Blacks: About 3 Cases

In case of hypertension, hypertensive retinopathy is normally an indicator of target organ damage. This sign can be frequently detected even in normotensive subjects. Among 94 normotensive volunteers chosen with regard of the hypertensive status of their parents, we could report the case of 3 black women who had hypertensive retinopathy. As changes in retinal microvasculature precede other signs of damage, we suggest fundus photography routine to be performed more frequently in assessing cardiovascular risks in hypertensive individual and their offspring, especially in black African population, where hypertension is severe.

Prognostic Role of Pulse Pressure in the Acute Phase of Myocardial Infarction

Objective: Pulse pressure is a cardiovascular risk factor. The aim of our study was to assess the short and medium term prognosis of myocardial infarction in the African black having a high pulse pressure. Methods: We performed a comparative retrospective study of 200 patients hospitalized in the Cardiology Institute of Abidjan. The first group (100 patients) had a high pulse pressure (≥60 mmHg). The second (100 patients) had normal pulse pressure (≤60 mmHg). The primary outcome was mortality. Secondary endpoints were represented by hemodynamic complications, rhythmic and conduction disturbances. Both groups of patients were matched. The matching is performed by socio-demographic criteria, the seat of necrosis and cardiovascular risk factors. The authors explained the choice of 60 mmHg as references because in all their studies they have realized that it is from 60 mmHg pulse pressure morbid events that occur in prospective studies and retrospective studies in stiffness of the great arteries is responsible for all these events. Pulse pressure is in itself, an independent cardiovascular risk factor in systolic blood pressure, diastolic blood pressure when associated with myocardial infarction, it becomes from 60 mmHg poor prognostic factor especially in the acute phase. Results: The mean age was 54.46 ± 10.10 years for patients with normal pulse pressure and 52.32 ± 10.89 years in patients with high pulse pressure. Patients with elevated pulse pressure were twice as left ventricular failure (Chi2 = 3.71;P = 0.048 = 2 OR 95% CI = 0.93 to 4.29). These patients had an ejection fraction and fractional shortening double bass (Chi2 = 31.23 P = 0.00001, OR = 2.2 95% CI 0.8 to 17.4) 10 mmHg for increased pulse pressure we observed an average growth of 18% mortality rate. Mortality was three times higher in patients with elevated pulse pressure (Khi2 = 15, 06 ddl = 1 P = 0.0001 OR = 3.34 95% CI 1.72 to 6.52).
