Surgical Treatment of Osteonecrosis of the Femoral Head Using Minimally Invasive Surgical Drilling and Cancellous Grafting at Brazzaville University Hospital

Introduction: Osteonecrosis of the femoral head (ONTF) is a debilitating condition. Several treatments have been proposed with controversial results. The aim of our study was to evaluate treatment by surgical drilling coupled with in situ cancellous grafting. Materials and methods: Our study was a case-control study conducted at Brazzaville University Hospital from 1st January 2018 to 31 December 2023. It compared two groups of patients with ONTF: non-operated (13 patients, 20 hips) and operated (22 patients, 35 hips). We used the visual digital scale (VDS) for pain assessment, the Merle D’Aubigne-Postel (MDP) scoring system for clinical and functional assessment, and the evolution of necrosis. Results: The group of non-operated patients had a mean age of 35.69 ± 3.4 years, no improvement in pain with an EVN above seven at the last recoil and a mean global MDP score falling from 12.7 before offloading to 10.13 at one year. The group of patients operated on had a mean age of 37.86 ± 7.02 years, a significant reduction in pain (p = 0.00004) and a significantly increased MDP score (p = 0.0034). A comparison of the two groups of patients showed significant stabilization of the necrotic lesions in the operated patients (p = 0.00067), with better satisfaction in the same group. Conclusion: Surgical drilling combined with grafting in the treatment of early-stage ONTF has improved progress in our series. The technique is reproducible and less invasive. It has made it possible to delay unfavorable progression and, consequently, hip replacement surgery.

Electric vehicles(EVs) and grid storage for renewable energies will require batteries in tens of millions of tons, and thus they need to be sustainable. EVs call also for batteries with higher energy density than currently available, and this value is intrinsically limited by the properties of the transition metal oxide(TMO) cathodes [1]. There are two possible strategies to overcome these limitations: i) increase the operating voltage in the >4.4 V range; and ii) attempt to use more than one electron per transition metal [2]. The first strategy faces problems of electrolyte stability limit and oxygen evolution, and thus safety hazards. This article presents the results of comparative study of two PV solar modules technologies,namely monofacial and *** study main objective is to identify conditions and parameters that make it possible to obtain better energy and economic efficiency from one or other of two *** study reason lies in revival observed on bifacial module in recent years where all the major manufacturers of PV solar panels are developing them where in a few years,this technology risks being at the same price as the monofacial solar panel with better *** indicator used is energy levelized cost(LCOE)which is function technology type,energy productivity,annual investment and operation *** achieve this,a 3.685 MWc solar PV power plant was dimensioned and simulated under Matlab for a 3.5 ha site with a 2,320,740,602 FCFA budget for monofacial installation,against 1,925,188,640 FCFA for 2.73 MWc bifacial *** LCOE comparative analysis of two technologies calculated over a period of 25 years,showed that plant with bifacial panels is more beneficial if bifacial gain is greater than 9%.It has further been found that it is possible to gain up to 40%of invested cost if bifacial gain reaches 45%.Finally,a loss of about 10%of invested cost could be recorded if bifacial gain is less than 9%.

Introduction: Diabetes mellitus, a metabolic disease, is now an important public health problem across the world. Our aim was to study the epidemiological, clinical, paraclinical, and evolutionary aspects of acute metabolic complications (AMC) of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a descriptive cross-sectional study with data collection based on the medical records of patients presenting with acute metabolic complications of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital during the 3 years study period (from January 2020 to December 2022). Results: Over 788 patients hospitalized during the study period, 157 had an acute metabolic complication of diabetes, which is a hospital prevalence of 19.9%. Among these 157 cases, 140 were suitable for analysis and therefore constituted our study sample. The mean age of the patients was 49.9 ± 14.7 years, with 19 as minimum and 90 years as maximum. Most of them were women (52.4%). Hypertensive diabetics accounted for 54.9% of cases. Ketoacidosis was the most common complication (62.1%), followed by hyperosmolar hyperglycemic syndrome (23.6%) and hypoglycemia (14.3%). Infection (69.3%) and therapeutic noncompliance (22.9%) were the most common triggers. The average length period of their hospital stay was 7.1 ± 5.1 days, and the outcome was satisfactory in 87.9% of the cases. Unfortunately, there were 3.6% deaths during hospitalization. Conclusion: Acute metabolic complications of diabetes were dominated by ketoacidosis. Infection was the dominant factor in decompensation. Therapeutic education of diabetic patients as well as clinical and biological monitoring must be more stringent and rigorous.

Evaluation of Soil Quality under Cocoa Trees in the Marahoué Region (Central-Western Côte d’Ivoire)

This study, which assesses the quality of soils under cocoa trees in the Marahoué region of Côte d’Ivoire, is being carried out against a backdrop of declining crop yields due to a number of factors, including the loss of soil fertility. The aim was to identify homogeneous groups of soils in terms of fertility and propose sustainable management methods for each group. The study was carried out on twenty-four cocoa orchards belonging to the SCOOP KAPATCHIVA cooperative, spread over eight sectors. A morphological characterisation of the soils was carried out in the field, followed by physico-chemical laboratory analyses of soil samples taken in the field. Soil fertility was analyzed on the basis of these two groups of data. The results showed that soil depths were generally high, except in some high altitude areas, where soils were often enriched in ferromagnesian elements and compacted. The texture of the soils varied between loamy and sandy-loamy, influencing cocoa production. Indeed, the soils of the plots with high production were looser and without ferromagnesian concretions, while those of the plots with low production were more gravelly and compact. Biological activity in the upper soil layer (0 - 30 cm) was notable with an abundance of termites and earthworms. The organic matter and nitrogen content varies considerably between sectors, directly impacting soil fertility. The soils were slightly acidic and showed significant differences in the levels of exchangeable magnesium and potassium, and not at all for phosphorus which appeared at very low doses. Chemical analysis also revealed that nutrient balance was a key factor in orchard productivity, with varying ratios of calcium, magnesium and potassium. Knowledge of these results made it possible to make recommendations for soil management with a view to improving cocoa yields in the study area.

This study focuses on air quality in southern Benin in order to show public authorities what the Beninese populations are exposed to for efficient decision-making. Two sampling campaigns were carried out, one in the wet period and the other in the dry season. The measurements were taken using a monitor called an “Air Quality Monitor”. For data processing, the multiple comparison methods of Dun (1961) and the Wilcoxon test were used. To maintain legitimacy, all spatial data were included in the official cartographic repository of Benin: WGS 1984, Transverse Mercator Universe Projection (UTM), Zone 31 North. The Moran statistic was used to measure the levels of spatial autocorrelation of the variables studied and to test the significance. In order to locate the spatial subsets, the local spatial association indices of Anselin Local Moran and Getis-Ord, Gi* were used. In terms of results, on the 13 monitoring sites and the 8 parameters chosen to determine air quality, we do not note any significant inter-seasonal difference. Of the eight parameters, only three parameters present spatial autocorrelation leading to predictions of ambient air quality over the entire study area based on the distance separating the points, namely, PM2.5, PM10 and ambient air quality index (AQI). The localities affected by atmospheric pollution in South Benin are located in the south-western part of Benin, headed by Cotonou, which is heavily polluted by CO2, TCOV, PM10 and PM2.5.

Rabies by Dog Bites in Benin: Determinants of Good Knowledge of the Rabies Risk among Subjects Exposed in the Plateau Department in 2023

Rabies, a viral zoonosis, is one of the neglected tropical diseases (NTDs) that mainly affects poor and vulnerable populations. Human rabies is endemic in Benin. Although listed among the notifiable diseases, it nevertheless remains a neglected and under-reported disease. The objective of this study was to investigate the determinants of knowledge of rabies risk among subjects exposed to human rabies by dog bites in the Plateau department of Benin. Methods: This was a cross-sectional study with an analytical aim which involved a sample of 900 people aged at least 18 years old chosen randomly and residing in the Plateau department for at least six months. A pre-tested and validated digital questionnaire served as a data collection tool. Data analysis was done with R 3.4.1 software. The prevalence ratio was used as a measure of association. The confidence threshold was set at 5%. We carried out a multiple logistic regression using the likelihood ratio (top-down approach) in order to identify the main factors which influenced the good knowledge of human rabies in the population of Plateau. Results: In the study population, 80.89% of the subjects surveyed lived in a rural area. The sex ratio (Male/Female) was 1.11. The average age of the subjects was 40.24 ± 14.52 years. The prevalence of dog bites was 4.11% (95% CI = [3.00;5.63]) or 37 subjects bitten. After evaluation of knowledge of rabies risk in the study population, the main factors which positively influenced good knowledge of human rabies were age (p linear trend 0.01), residence of the subject in the municipality of Pobè (ORa = 8.34;95% CI = 0.83 - 1.23;p < 0.01), individual dwelling (ORa = 17.95;95% CI = 13.12 - 49.58;p < 0. 01), educational level of at least secondary (ORa = 5.53;CI 95 = 3.06 - 10.15;p = 0.01), the existence of a veterinary center (ORa = 3.66;CI 95 = 1.07 - 12.43 p = 0.01) and knowledge of a dog bites victim (ORa = 6.55;95% CI = 2.73 - 8.25;p < 0.01).
