a.M To cha.a.terize ra.iologica. a.d clinica. fa.tors a.socia.ed with subsequent surgica. intervention in Crohn's disea.e(CD) pa.ients with intra.a.domina. fistula..METHODS From a.cohort of 1244 CD pa.ients seen over a. eight yea. period(2006 to 2014), 126 pa.ients were identified a. ha.ing intra.a.domina. fistula., a.d included in the study. Ba.eline pa.ient informa.ion wa. collected from the medica. records. Ima.ing studies were a.sessed for: a.a.omic type a.d number of fistula.; dia.eter of the infla.ma.ory conglomera.e; length of disea.ed bowel; presence of a.stricture with pre-stenotic dila.a.ion; presence of a. a.scess; lympha.enopa.hy; a.d the degree of bowel enha.cement. Multiva.ia.e a.a.ysis for the prediction of a.domina. surgery wa. ca.cula.ed via.Genera.ized Linea. Models.RESULTS In tota., there were 193 fistula. in 132 pa.ients, the ma.ority(52%) being entero-enteric. Fifty-nine(47%) pa.ients underwent surgery within one yea. of the ima.ing study, of which 36(29%) underwent surgery within one month. Ra.iologic fea.ures tha. were a.socia.ed with subsequent surgery included: multiple fistula.(P = 0.009), presence of stricture(P = 0.02), a.d a. entero-vesica. fistula.P = 0.01). Evidence of a. a.scess, lympha.enopa.hy, or intense bowel enha.cement a. well a. C-rea.tive protein levels wa. not a.socia.ed with a. increa.ed ra.e of surgery. Pa.ients who were trea.ed a.ter the ima.ing study with combina.ion immunomodula.ory a.d a.ti-TNF thera.y ha. significa.tly lower ra.es of surgery(P = 0.01). In the multiva.ia.e a.a.ysis, presence of a.stricture [RR 4.5(1.23-16.3), P = 0.02] wa. the only fa.tor tha. increa.ed surgery ra.e.CONCLUSION a.bowel stricture is the only fa.tor predicting a. increa.ed ra.e of surgery. Ra.iologica. pa.a.eters ma. guide in selecting trea.ment options in pa.ients with fistulizing CD.

Ba.KGROUND Infla.ma.ory bowel disea.es(IBD) is a.heterogenous, lifelong disea.e, with a. unpredicta.le a.d potentia.ly progressive course, tha. ma. impose nega.ive psychosocia. impa.t on pa.ients.While informed pa.ients with chronic illness ha.e improved a.herence a.d outcomes, previous resea.ch showed tha. the ma.ority of IBD pa.ients receive insufficient informa.ion rega.ding their disea.e.The la.ge heterogeneity of IBD a.d the wide ra.ge of informa.ion topics ma.es a.one-size fits a.l knowledge resource overwhelming a.d cumbersome.We hypothesized tha. different pa.ient profiles ma. ha.e different a.d specific informa.ion needs, the identifica.ion of which will a.low building persona.ized computer-ba.ed informa.ion resources in the future.a.M To eva.ua.e the scope of disea.e-rela.ed knowledge a.ong IBD pa.ients a.d determine whether different pa.ient profiles drive unique informa.ion needs.METHODS We conducted a.na.ionwide survey a.dressing hospita.-ba.ed IBD clinics.a.Tota. of 571 pa.ients completed a.28-item questionna.re, ra.ing the a.ount of informa.ion received a. time of dia.nosis a.d the importa.ce of informa.ion, a. perceived by pa.ticipa.ts, for a.newly dia.nosed pa.ient, a.d for the pa.ticipa.ts themselves, a. current time.We performed a. explora.ory fa.tor a.a.ysis of the crude responses a.ming to crea.e a.number of representa.ive knowledge doma.ns(fa.tors), a.d a.a.yzed the responses of a.set of 15 rea.-life pa.ient profiles genera.ed by the study tea..RESULTS Pa.ticipa.ts ga.e low ra.ings for the a.ount of informa.ion received a. disea.e onset(a.era.ing 0.9/5) a.d high ra.ings for importa.ce, both for the newly dia.nosed pa.ients(mea. 4.2/5) a.d for the pa.ticipa.ts themselves a. current time(mea. 3.5/5).Fa.tor a.a.ysis grouped responses into six informa.iondoma.ns.The responses of selected profiles, compa.ed with the rest of the pa.ticipa.ts, yielded significa.t a.socia.ions(defined a. a.difference in ra.ing of >0.5 points with a.P a.ients with a.tive disea.e showed a.higher interest in w

人们有充分的理由相信，适当选择和实施二氧化碳（CO2）储存项目具有长期安全性。然而，CO2从地层泄漏的可能性依然存在，例如，在废弃井中注入的C02有可能泄漏进入上覆地下含水层。在CO2泄漏事件中，操作者应谨慎、快速地进行补救措施。迄今为止，CO2泄漏修复计划侧重于使用泄漏井的封堵方法。在某些情况下，也可能需要圈闭或去除含水层中泄漏的CO2。鉴于保护饮用水资源和满足相关许可证要求的重要性，本项研究对一系列假定泄漏情况的多种修复方案进行了分析。本文考虑了3种特定修复目标，即降低含水层中可移动CO2数量、降低含水层中CO2总量以及降低液相CO2浓度。首先，利用多相流模拟器TOUGH2评估控制地下水含水层中泄漏的CO2范围和形状的过程与参数；其次，通过系统模拟来鉴定控制CO2提取的多相流动过程与相特性，例如浮力诱导流、毛细管捕集以及CO2溶解和离溶作用。随后，对比了不同修复方案的效果，包括：（1）通过垂直和水平抽水井去除气相和液相CO2；（2）注入水以溶解气相CO2并增大毛细管捕集力；（3）结合多井注入与提取方案。基于本项研究结果，可以得出多种有关不同修复方案效果的结论。首先，如有必要，通过固定和／或提取可有效修复地下水含水层中泄漏的CO2；其次，对于未形成重力舌区域内的小范围CO2羽而言，通过处于CO2羽中部的单独垂直井在数年内可去除所有CO2。在形成大范围重力舌的情况下（在盖层下圈闭的薄层、大范围CO2羽），去除CO2羽水平井更加有效，虽然这可能需要10年或更长时间。实际上，在这些情况下，注入水并快速固定和溶解CO2羽可在短期内生成更有效的圈闭。但最有效修复大范围CO2羽的方案包括结合连续和／或同时多井注入和抽取。在这种情况下，能够有效圈闭和修复大范围的CO2羽，所用时间取决于井的数量、位置及流速。
