Nonalcoholic fatty liver disease in patients with inflammatory bowel disease: Beyond the natural history

BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is a frequently reported condition in patients with inflammatory bowel disease(IBD).Both intestinal inflammation and metabolic factors are believed to contribute to the pathogenesis of IBDassociated NAFLD.AIM To evaluate the prevalence of steatosis and liver fibrosis(LF)in a cohort of IBD patients and the identification of metabolic-and IBD-related risk factors for NAFLD and LF.METHODS IBD patients were consecutively enrolled from December 2016 to January 2018.Demographic,anthropometric and biochemical data were collected so as eating habits.Abdominal ultrasound and transient elastography were performed to evaluate the presence of NAFLD and LF respectively.RESULTS A total of 178 consecutive patients were enrolled and included in the analysis(95 Ulcerative colitis,83 Crohn’s disease).NAFLD was detected by imaging in 72(40.4%)patients.Comparison between patients with and without NAFLD showed no significant differences in terms of IBD severity,disease duration,location/extension,use of IBD-related medications(i.e.,steroids,anti-TNFs,and immunomodulators)and surgery.NAFLD was significantly associated with the presence of metabolic syndrome[MetS;odds ratio(OR):4.13,P=0.001]and obesity defined by body mass index(OR:9.21,P=0.0002).IBD patients with NAFLD showed higher caloric intake and lipid consumption than those without NAFLD,regardless disease activity.At the multivariate analysis,male sex,advanced age and high lipid consumption were independent risk factors for the development of NAFLD.An increased liver stiffness was detected in 21 patients(16%)and the presence of MetS was the only relevant factor associated to LF(OR:3.40,P=0.01).CONCLUSION In this study,we demonstrate that risk factors for NAFLD and LF in the IBD population do not differ from those in the general population.

Application of ultrasonography-elastography score to suspect porto-sinusoidal vascular disease in patients with portal vein thrombosis

Background:Porto-sinusoidal vascular disease(PSVD)and portal vein thrombosis(PVT)are causes of portal hypertension characterized respectively by an intrahepatic and a pre-hepatic obstacle to the flow in the portal system.As PVT may be a consequence of PSVD,in PVT patients at presentation,a pre-existing PSVD should be suspected.In these patients the identification of an underlying PSVD would have relevant implication regarding follow-up and therapeutic management,but it could be challenging.In this setting ultrasonography may be valuable in differential diagnosis.The aim of the study was to use ultrasonography to identify parameters to discriminate between PSVD and“pure”PVT and then to suspect PVT secondary to a pre-existing PSVD.Methods:Fifty-three patients with histologically proven PSVD and forty-eight patients affected by chronic PVT were enrolled and submitted to abdominal ultrasonography with elastography by acoustic radiation force impulse(ARFI).Results:ARFI was higher and superior mesenteric vein(SMV)diameter was wider in PSVD patients than in PVT patients.Thus,a prognostic score was obtained as linear combinations of the two parameters with a good discrimination capacity between PSVD and PVT(the area under the curve=0.780;95%confidence interval:0.690-0.869).Conclusions:A score based on ARFI and SMV diameter may be useful to suspect an underlying PSVD in patients with PVT and to identify a subgroup of patients to be submitted to liver biopsy.

Development of an urban health and wellbeing index for work precincts:A comparative study in Sydney,Australia

Work precincts are recognized for their significant role as generators of employment and associated commerce within urban areas.This study describes a method for analyzing the physical characteristics of urban work precincts in promoting the health and wellbeing of their occupants.The following physical parameters are analyzed:public transport accessibility,green and blue spaces,food environments,fitness facilities,supermarkets,and grocery stores.The parameters are assessed using quantitative spatial analysis based on street network data,as well as point of interest data acquired from OpenStreetMap(OSM).The streets and their intersections are stored in the OSM database as links and nodes,respectively.The evaluation of the performance metrics involves measuring the street network distance from each node to the closest node of interest for each parameter.The metrics are then combined,forming an urban health and wellbeing index(UHWI),which can be used to compare the performance of different precincts.The method was tested by investigating four work precincts in Sydney,Australia,all hosting a large office building belonging to the same business institution.Our results identified two of the four precincts with a high UHWI and resulted in the Identification of one underperforming precinct.

Influenza is an acute respiratory infectious disease caused by the influenza virus,affecting people globally and causing significant social and economic losses.Due to the inevitable limitations of vaccines and approved drugs,there is an urgent need to discover new anti-influenza drugs with different mechanisms.The viral ribonucleoprotein complex(vRNP)plays an essential role in the life cycle of influenza viruses,representing an attractive target for drug design.In recent years,the functional area of constituent proteins in vRNP are widely used as targets for drug discovery,especially the PA endonuclease active site,the RNA-binding site of PB1,the cap-binding site of PB2 and the nuclear export signal of NP protein.Encouragingly,the PA inhibitor baloxavir has been marketed in Japan and the United States,and several drug candidates have also entered clinical trials,such as favipiravir.This article reviews the compositions and functions of the influenza virus vRNP and the research progress on vRNP inhibitors,and discusses the representative drug discovery and optimization strategies pursued.

Structure and functions of the translation initiation factor eIF4E and its role in cancer development and treatment

In eukaryotic cells, protein synthesis is a complex and multi-step process that has several mechanisms to start the translation including cap-dependent and cap-independent initiation. The translation control of eukaryotic gene expression occurs principally at the initiation step. In this context, it is critical that the eukaryotic translation initiation factor eIF4E bind to the 7-methylguanosine （m7G） cap present at the 5＇- UTRs of most eukaryotic mRNAs. Combined with other initiation factors, elF4E mediates the mRNA recruitment on ribosomes to start the translation. Moreover, the eIF4E nuclear bodies are involved in the export of specific mRNAs from the nucleus to the cytoplasm. In this review, we focus on the elF4E structure and its physiological functions, and describe the role of eIF4E in cancer development and progression and the current therapeutic strategies to target eIF4E.

Do environmental cues,and commensal bacteria in particular,have an impact on autoimmunity,and can we modulate the production of gut microbial metabolites to re-equilibrate immune responses as well as restrain autoimmune diseases?Mari?o et al.1 elegantly addressed these issues in a very recent Nature Immunology paper.They report that diets designed to release large amounts of the microbial metabolites acetate and butyrate finely tune autoreactive immune responses,which lends weight to the fascinating hypothesis that medicinal foods and metabolites might contribute to treating autoimmune diseases.

How the COVID-19 pandemic has affected the colorectal cancer screening in Italy:A minireview

The coronavirus disease 2019(COVID-19)pandemic has caused detrimental effects on many aspects of healthcare practice.Screening programs for the commonest malignancies,namely colorectal cancer(CRC),breast cancer and cervical cancer have been discontinued or interrupted since the beginning of restriction measures aimed to limit transmission of the new coronavirus infection.Robust evidence exists in favour of the role of screening campaigns in reducing mortality from CRC.In fact,the majority of pre-malignant lesions of the colon and rectum can be diagnosed with colonoscopy and treated by endoscopic or surgical resection.Besides,colonoscopy screening allows the diagnosis of CRCs in their pre-clinical stage.Italy was one of the first European countries where a high level of COVID-19 infections and deaths was observed,and one of the first where lockdowns and strict measures were adopted to reduce the risk of COVID-19 diffusion among the population.A systematic review of the literature was performed,including the PubMed,Scopus,Web of Sciences,and Reference Citation Analysis databases,with the aim of critically evaluating the impact of the COVID-19 pandemic on CRC screening in Italy.We found that reduction of CRC screening activity surpassed 50%in most endoscopic units,with almost 600000 fewer CRC screening exams conducted in the first 5 mo of 2020 vs the same period of 2019.While the consequences of the discontinuation of endoscopy screening for the prognosis and mortality of CRC will be evident in the next few years,recent data confirm that CRC is currently treated at a more advanced stage than in the pre-COVID-19 era.Since delays in CRC prevention and early diagnosis may translate to increased CRC-specific mortality,world healthcare systems should adopt strategies to maintain the regularity of CRC screening during subsequent peaks of the COVID-19 pandemic,or future events that might hamper screening programs.
