Trisubstituted Aryl Cyclohexanecarboxylates (TACC): A Simple, New Molecular Scaffold for Antibiotics Design

A new class of potential antibacterial agents has been synthesized on a new molecular scaffold of cyclohexane carboxylate. We have tagged this new class of compounds TACCs (Trisubstituted Aryl Cyclohexanecarboxylate). These new molecules are structural analogues of an Activators of Self-Compartmentalizing Proteases 4 and 5 (ACP 4 and 5), and were synthesized to circumvent the drug-like property (drug-ability) challenges and liability noted in ACP 4 and 5. A pseudo-Robinson annulation protocol was used to furnish this new class of potential antibiotics. Structure-activity relationship (SAR) study was done to identify the pharmacophore(s) in this molecular scaffold. A selection of these compounds was used in our preliminary antibacterial inhibitory activities’ studies on Bacillus mycoides and Bacillus subtilis. These preliminary studies show that the TACCs exhibited equal, and in some cases better, antibacterial activity than ACP 4 and 5.

Bcl-2 down modulation in WEHI-3B/CTRES cells resistant to Cholera Toxin(CT)-induced apoptosis

The very different effects of Cholera Toxin (CT) on cell growth and proliferation may depend on the type of ganglio-side receptors in cell membranes and different signal transduction mechanisms triggered,but other functions relatedto the drug resistance mechanisms can not be *** effect of CT treatment on the "in vitro" clonogenicity,thePopulation Doubling Time (PDT),apoptosis,PKA activation and Bax and Bcl-2 expression was evaluated in WEHI-3Bcell line and its CT-resistant subclone (WEHI-3B/CTRES).In WEHI-3B parental cells the dramatic accumulation ofcAMP induced by CT correlated well with PKA activation,increased PDT value,inhibition of clonogenicity and apop-tosis.H-89 treatment inhibited PKA activation by CT but did not protect the cells from apoptosis and growth *** WEHI-3B/CTRES no significant CT-dependent accumulation of cAMP occurred with any increase of PKA activityand *** CT resistant cells (WEHI-3B/CTRES),Bcl-2 expression was down regulated by both CT or drug treatment(eg.,ciprofloxacin,CPX) although these cells were protected from CT-dependent apoptosis but not from *** from other cell models described,down regulation of Bcl-2 is proved to be independent on cAMPaccumulation and PKA *** observations support the implication of cAMP dependent kinase (PKA) in theinhibition of WEHI-3B cells growth and suggest that,in WEHI-3B/CTRES,Bcl-2 expression could be modulated byCT in the absence of cAMP *** in consideration of many contradictory data reported in literature,ourcell models(of one sensitive parental cell strain and two clones with different uncrossed specific resistance to CT andCPX)provides a new and interesting tool for better investigating the relationship between the CT signal transductionmechanisms and Bcl-2 expression and function.

Gastric cancer is a common neoplastic disease and, more precisely, is the third leading cause of cancer death in the world, with differences amongst geographic areas. The definition of advanced gastric cancer is still debated. Different stadiating systems lead to slightly different stadiation of the disease, thus leading to variations between the single countries in the treatment and outcomes. In the present review all the possibilities of treatment for advanced gastric cancer have been analyzed. Surgery, the cornerstone of treatment for advanced gastric cancer, is analyzed first, followed by an investigation of the different forms and drugs of chemotherapy and radiotherapy. New frontiers in treatment suggest the growing consideration for intraperitoneal administration of chemotherapeutics and combination of traditional drugs with new ones. Moreover, the necessity to prevent the relapse of the disease leads to the consideration of administering intraperitoneal chemotherapy earlier in the therapeutical algorithm.

目的旨在翻译Chase护理管理者胜任力测评工具(the Chase Nurse Manager Competency Instrument,CNMCI)为意大利语,并对其进行信度和效度检验。方法对英文版CNMCI进行翻译,并通过横断面研究验证其信效度。采用预翻译(建立等效)、初始翻译、预测试、审查和管理5个步骤的问卷翻译方法,将英文版工具翻译为意大利语版。于2022年8月到2023年6月,选取意大利3个地区31所医疗机构349名护理管理者进行调查。采用内容效度、结构效度评估工具的效度;采用Cronbach'sα系数、重测信度评估工具的信度。结果意大利语版CNMCI,包括“知识理解”和“实施和/或使用能力”两个分量表,每个分量表包括53个条目。工具的条目内容效度指数(I-CVI)为0.83~1.00,两个分量表的内容效度指数(S-CVI)均为0.97。经验证性因子分析显示该工具各项拟合指数表现良好。工具中“知识理解”分量表的Cronbach'sα系数为0.978,“实施和/或使用能力”分量表的Cronbach'sα系数为0.976。各个维度之间的相关系数为0.494~0.908重测信度为0.820,表明工具的一致性较好。结论意大利语版CNMCI在测量护理管理者胜任力方面显示出良好的信度和效度,并支持该工具的多维度结构。此外,研究结果支持了这些因素的层次性质,进一步支持了该工具的有效性。

Hepatitis C virus recurrence after liver transplantation:A 10-year evaluation

AIM: To evaluate the predictors of 10-year survival of patients with hepatitis C recurrence. METHODS: Data from 358 patients transplanted between 1989 and 2010 in two Italian transplant centers and with evidence of hepatitis C recurrence were analyzed. A χ2, Fisher's exact test and Kruskal Wallis' test were used for categorical and continuous variables, respectively. Survival analysis was performed at 10 years after transplant using the Kaplan-Meier method, and a log-rank test was used to compare groups. A P level less than 0.05 was considered significant for all tests. Multivariate analysis of the predictive role of different variables on 10-year survival was performed by a stepwise Cox logistic ***: The ten-year survival of the entire population was 61.2%. Five groups of patients were identified according to the virological response or lack of a response to antiviral treatment and, among those who were not treated, according to the clinical status(mild hepatitis C recurrence, "too sick to be treated" and patients with comorbidities contraindicating the treatment). While the 10-year survival of treated and untreated patients was not different(59.1% vs 64.7%, P = 0.192), patients with a sustained virological response had a higher 10-year survival rate than both the "non-responders"(84.7% vs 39.8%, P < 0.0001) and too sick to be treated(84.7% vs 0%, P < 0.0001). Sustained virological responders had a survival rate comparable to patients untreated with mild recurrence(84.7% vs 89.3%). A sustained virological response and young donor age were independent predictors of 10-year survival. CONCLUSION: Sustained virological response significantly increased long-term survival. Awaiting the interferon-free regimen global availability, antiviral treatment might be questionable in selected subjects with mild hepatitis C recurrence.

Homocysteine and oxidative stress in acute stroke

Homocysteine is a sulfhydryl-containing amino acid derived from the essential amino acid methionine. Total Hcy plasma level varies in the range of 5-15 μmol/L in the normal population. Our aim in this study was to investigate the possible correlations among homocysteine plasma levels, oxidative stress parameters and clinical evolution of stroke. Fifty patients with large-vessel ischemic stroke were studied. Biochemical determinations were performed at entry (T0) and then repeated one month after stroke (T1). Homocysteine levels were significantly increased at T0 with respect to T1 and showed a significant positive correlation with the expression of oxidative stress markers and a negative correlation with indicators of protective anti stress activity. A significant increase of antioxidant activity occurred from T0 to T1 and changes were associated with the severity of clinical conditions. In particular, the extent of homocysteine and of oxidative stress markers plasmatic levels re- duction and of the contemporary increase in anti stress biochemical activities were associated with a reduction of NIHSS scores. These findings, besides confirming an involvement of oxidative stress in in- fluencing the evolution of stroke, suggest a role for homocysteine as a potentially modifiable biochemical alteration able to modulate some mechanisms in- volved in the production of ischemic damage.
