AIM: To evaluate the impact of combination therapy with Lamivudine and Adefovir for treatment of chronic hepatitis B in Lamivudine-resistant patients. METHODS: Among the 110 adult chronic hepatitis B Iranian patients whom were treated with Lamivudine, for 36 months, nineteen patients (17%) with no any biochemical and viral responses to Lamivudine alone, were selected and enrolled in the study. Due to resistancy, Adefovir was added to Lamivudine, and continued for 30 months. We measured HBV_DNA viral load and serum AST, ALT in 0, 12, 24, 30 and 0, 6, 12, 18, 24, 30 months, respectively. RESULTS: Between biochemical and viral characteristics, Repeated Measure analysis identified just biochemical markers— Aspartate Aminotransferase level (AST) (P = 0.002) and Alanine Aminotransferase level (ALT) (P = 0.007) —as predictors of response to treatment, while, viral marker—HBV DNA load—was not statistically significant (P = 0.128). CONCLUSIONS: Treatment for a long time, such as 21.5 ± 8.8 months, with Lami- vudine and Adefovir, can cause liver enzymes including AST and ALT, decreasing and being normal. But, this finding is not indicative, for HBV-DNA viral load.

Objectives: This study compared the effect of Kinesio Taping (KT) with local Methyl Prednisolone injection in patients with biceps tendonitis based on visual analog scale (VAS) and range of motion (ROM). Methods: Thirty-eight patients (15 females and 23 males;mean age: 29.87 ± 6.31, years) with biceps tendonitis were participated in this study during 2014-2015. The patients were randomly assigned into two groups: 19 patients in KT group and 19 patients in injection group. In the first group, KT was used three times for 24 hours sequentially with four-day intervals;in the second group, one dose of Methyl Prednisolone (40 mg) plus 1% lidocaine was injected in the bicipital fissure around the long head of the biceps muscle. The injections and KT therapy were performed by the same physician. The patients were investigated for VAS and ROM in the first, second, seventh and twelfth days. Results: VAS and ROM indices were significantly improved in the second and seventh days (p 0.05). Conclusion: Regarding the more immediate effect of KT on ROM and VAS and the fast restoring of the patient to normal life, it could be used as a noninvasive alternative to injection and as the first line of treatment specially in patients who need the immediate effect of treatment.

屈光性角膜切削术(PRK),激光原位角膜磨镶术(LASIK)和小切口微透镜取出术(SMILE)是目前临床上屈光性手术中有效和安全的方法。这三种激光视力矫正方法虽然技术不同,但成功率大致相同。至于哪种方法是最合适的取决于多种因素,包括:屈光度、个人习惯和职业因素(如运动和生活方式)。目前,LASIK屈光手术应用最广,但SMILE也在手术医生间快速推广。后者提高了术后早期的舒适感,还有视力的快速恢复,减少了伤口的愈合反应。此外,与LASIK相比,SMILE术后干眼的发生率较低,因为小切口使前基质损伤小,这意味着前部角膜神经受到影响小。与LASIK和PRK相比,SMILE同样可以降低高阶像差(HOA)的形成。术后角膜滞后量(CH)和角膜阻力因子(CRF)与SMILE、LASIK、PRK方法无明显差异。但是,PRK仍然是中度到轻度手术量的最佳选择,特别是涉及薄角膜、频繁角膜侵蚀的病例。

To investigate the supplement of lost nerve cells in rats with traumatic brain injury by intravenous administration of allogenic bone marrow mesenchymal stem cells, this study established a Wistar rat model of traumatic brain injury by weight drop impact acceleration method and administered 3 × 106 rat bone marrow mesenchymal stem cells via the lateral tail vein. At 14 days after cell transplantation, bone marrow mesenchymal stem cells differentiated into neurons and astrocytes in injured rat cerebral cortex and rat neurological function was improved significantly. These findings suggest that intravenously administered bone marrow mesenchymal stem cells can promote nerve cell regeneration in injured cerebral cortex, which supplement the lost nerve cells.

Objective:To determine the frequency and causes of injuries from sharp instruments contaminated by blood in hospital ***:The study was cross-sectional and conducted at an educational hospital supervised by Kerman University of Medical Sciences in *** a researcher-made questionnaire,we determine the frequency and causes of injuries from sharp instruments contaminated by blood in 142 hospital ***:Of the 142 patients,68 persons (48%) did not have a history of needle stick injury,but 74 persons (52%) had suffered needle stick injury 1 to more than 5 *** those who suffered a needle stick,74.3% were trained,while 70.7% of those who did not suffer needle stick had been *** results indicated that the needle was the most common sharp instrument causing needle stick ***:The study showed a high prevalence of needle stick injury among hospital personnel specifically in ICU,orthopaedic and surgery *** measures such as improving injection practices,modification of working schedule,planning training programs targeted at using personal protective equipment,and an adequate number of safety facilities for the effective prevention of needle stick injury incidents are needed.

The main purpose of this research is to help couples that have problems in early ejection in man and veganism’s problem in woman that after 2 months has been cured by using scientific and non-drug method, successfully. This case is related to couples who live in Arak and both of them are 21 years old and their education level is Diploma. They refer to clinic center and are asking help from consulters after one year of their unconsummated marriage. Man is involved with problem of early injection and woman is involved with veganism’s problem. Treatment has been done by teaching relaxation to woman for 2 weeks and little by little along with day dreaming that is related to sexual. In the next stage, the researchers ask her in relaxing time to enter fingers in vagina. Manner therapy has been used for man that usually consists of start-stop and squeeze techniques. At last, after successful performance, first sexual relationship happens. It is noticeable that unconsummated marriage is treatable with a simple and safe method and low cost in a short time. Regarding this matter that veganism’s and early ejection are as sexual disorders and are most prevalent kind of sexual disorders, so present research concludes that unconsummated marriage is treatable by psychology and non-drug treatment which prevents marriage from failing.

The aggregation of amyloid-beta peptide and tau protein dysregulation are implicated to play key roles in Alzheimer's disease pathogenesis and are considered the main pathological hallmarks of this devastating ***,these two proteins are produced and expressed within the normal human ***,under pathological conditions,abnormal expression,posttranslational modifications,conformational changes,and truncation can make these proteins prone to aggregation,triggering specific disease-related *** studies have indicated associations between aberrant behavior of amyloid-beta and tau proteins and various neurological diseases,such as Alzheimer's disease,Parkinson's disease,and amyotrophic lateral sclerosis,as well as retinal neurodegenerative diseases like Glaucoma and age-related macular ***,these proteins have been linked to cardiovascular disease,cancer,traumatic brain injury,and diabetes,which are all leading causes of morbidity and *** this comprehensive review,we provide an overview of the connections between amyloid-beta and tau proteins and a spectrum of disorders.

In vitro maturation(IVM)is considered a potential assisted reproductive technology that is a safer and simpler alternative to conventional in vitro *** is primarily used in patients with impaired oocyte maturation and for the treatment of infertile women who are at risk of fertility *** addition,IVM is currently used mainly in polycystic ovarian syndrome patients with a high ovarian response and is stll considered an experimental option in fertility *** highly competent oocytes during IVM is considered a key step in the success of in vitro production(IVP)of *** factors,such as culture medium conditions and other supplements,have a significant impact on oocyte IVM *** of the known disruptors of oocyte developmental competence in IVP is oxidative stress(Os),which is caused by an imbalance between the production and neutralization of reactive oxygen species(ROS).In vitro conditions induce supraphysiological ROS levels due to exposure to an oxidative environment and the isolation of the oocyte from the follicle protective antioxidant *** the importance of Os in oocyte competence,the establishment of standardized antioxidant IVM systems is critical for improving the overall success of *** review focuses on the main antioxidants tested to protect oocytes against OS in IVM.

Objective:To explore the potential therapeutic impact of an alcoholic extract derived from the sea cucumber(Holothuria parva)on letrozole-induced polycystic ovary syndrome(PCOS)in adult female Sprague-Dawley ***:Sixteen female rats,aged 8 to 10 weeks,with a mean weight of(200±20)g,were randomly assigned to four groups:the control,PCOS,and two treatment groups receiving sea cucumber extract and metformin,*** was induced by administering letrozole at a dose of 1 mg/kg to initiate the treatment period at 60 days of *** study spanned four weeks,during which ovarian and uterine tissues were collected for histological examination,and blood samples were collected for hormonal ***:Significant decreases in luteinizing hormone/follicle stimulating hormone(LH/FSH)and testosterone and increases progesterone levels among groups treated with sea cucumber extract were *** no significant differences were observed in follicle-stimulating hormone(FSH)and testosterone levels,the distinctive variations in key hormones suggested a potential role for sea cucumber alcoholic extract in ameliorating PCOS ***:This research highlights the moderate efficacy of sea cucumber extract as a therapeutic intervention for PCOS,providing a novel perspective in the search for effective *** observed hormonal alterations,particularly in LH,estradiol,and progesterone,underscore the need for further exploration into the underlying mechanisms and for optimizing the application of sea cucumber extract in PCOS management.

The effect of NAFLD (non-alcoholic fatty liver disease) on long-term outcome of chronic hepatitis B in Iranian patients

Background: The influence of Non-Alcoholic Fatty Liver Disease on the outcome of chronic hepatitis B disease, including viral, biochemical and histologic characteristics, in Iranian patients is not yet fully un- derstood. Aim: To evaluate the effect of Non-Alcoholic Fatty Liver Disease (NAFLD) on long-term histology- cal, biochemical and viral outcome of chronic he- pa-tictis B in Iranian patients. Methods: We retro- spec-tively evaluated 94 “e Ag” negative chronic hepatitis B patients (with NAFLD: 44, without NAFLD: 50). Non-Alcoholic Fatty Liver Disease was diagnosed based on liver biopsy according to Kleiner classifica-tion. Liver biopsy was done for all patients. Serologi-cal and biochemical variables were evaluated with repeated measure analysis. Results: Non-Alcoholic Fat- ty Liver Disease (NAFLD) was present in 47% of the patients (44 out of 94 patients). In the NAFLD group, increase in AST, ALT, stage (P = 0.002), grade, and total score of liver biopsy were independently related to non-alcoholic fatty liver disease, while HBV-DNA viral load did not correlate with the presence of a fatty liver. Conclusion: Abnormalities of liver enzymes and liver histopathology are more prevalent in concurrent chronic hepatitis B and Non-Alcoholic Fatty Liver Disease (NAFLD).
