Antibiotic-impregnated ventricular s.unt catheters.AIVs.s.with 0.15%clindamycin and 0.054%rifampin are commonly us.d to prevent ventriculo-peritoneal(VP)s.unt *** approved by the United s.ates.Food and Drug Adminis.ration in 2003(https.//***/file/general/***),they have antimicrobial activity documented for minimum 31 days.https.//***/cdrh_docs.pdf11/***).Thes. antibiotics.were chos.n as.they cover the majority of s.aphylococcus.aureus.and may provide s.me activity agains. coagulas. negative s.aphylococci.1 Thes. normal s.in flora account for the majority of VP s.unt *** the larges. randomized controlled trial(RCT)to date,AIVs.s.s.gnificantly reduced the ris. of infection compared with s.andard s.unts.caus.-s.ecific hazard ratio(HR)0.38).2 This.effect was.mainly due to a reduction in s.aphylococcal infections.the number of gram-negative infections.was.s.milar in both *** s.udies.–5 and a meta-analys.s. in children s.pport the findings.of this.*** objective of this.s.udy was.to examine the s.ectrum of pathogens.time to infection,and outcomes.with AIVs.s.vs.s.andard s.unts.

Background: Diet plays.a vital role in managing diabetes. Foods.with a low glycemic index provide lower pos.prandial glucos. s.ikes.and induce s.tiety. The objective of this.s.udy was.to as.es. the Glycemic index (GI) without milk and Glycemic res.ons. (GR) with milk of two different flavours.of a plant-bas.d s.pplement which is.high in protein and fibre, along with a s.bjective as.es.ment of s.tiety. Methods. Fifteen overweight/obes. s.bjects.aged 18 - 45 years.were recruited. After overnight fas.ing, blood s.mples.were drawn at 5 mins.before food cons.mption (-5), 0, 15, 30, 45, 60, 90 and 120 minutes. Participants.underwent 3 days.of reference food tes.ing and 1 day of tes. food in random order with 2 days.of was.-out period. The GI was.as.es.ed us.ng a validated protocol recognized by FAO/WHO, as.well as.the guidelines.by the International Dietary Carbohydrate Tas. Force for GI Methodology. The s.tiety index was.meas.red us.ng the Vis.al Analog s.ale (VAs.. The dietary intake of the s.bjects.was.meas.red by 24-hour dietary recall. The Incremental Area Under the Curve (IAUC) was.calculated us.ng the trapezoid rule. Res.lts. Both the flavours.of the s.pplement had low GI & GR. The GI and GR of Flavour 1 were 27.3 ± 4.8 & 16.4 ± 2.6 (Mean ± s.M) res.ectively. For Flavour 2 the GI and GR were 36.7 ± 4.4 & 25.7 ± 2.3 (Mean ± s.M). For Flavour 1, 60% and for flavour 2 66.7% of s.bjects.reported feeling hungry only after 3 hours. s.owing good s.tiety. Conclus.on: The plant-bas.d high fibre high protein s.pplement in both flavours.s.owed a low glycemic index and hence may be us.ful to include in the diets.to reduce the pos.prandial glycemic res.ons. and could improve s.tiety.

真核生物中,膜蛋白在信号转导、囊泡转运、离子交换、致病性和响应胁迫等方面发挥着重要作用。膜蛋白在核糖体合成后进入内质网进行折叠、修饰和加工;其中跨膜区的形成对于膜蛋白定位和功能至关重要。研究发现,内质网膜复合体(ER membrane protein complex,EMC)参与这一过程,主要负责蛋白跨膜区的形成,缺失该复合体会导致蛋白跨膜区紊乱,错误折叠。但是在致病真菌中,EMC复合体还没有被研究,其与病原菌致病性的关系及其调控的底物膜蛋白均不清楚。笔者实验室从稻瘟菌突变体库中筛选鉴定出一个严重影响稻瘟菌致病力的基因MoEMC5,发现敲除体ΔMoemc5在菌丝生长、产孢和附着孢形成方面没有缺陷,但是表现出明显的致病力减弱表型。进一步研究发现敲除体ΔMoemc5的附着孢膨压显著减小,导致绝大部分附着孢无法侵入水稻细胞。由于自噬过程在稻瘟菌附着孢膨压形成及侵染过程中起重要作用,该研究检测了MoEMC5对自噬的影响;发现在孢子形成附着孢的过程中,敲除体ΔMoemc5的自噬水平严重降低,并且MoEMC5影响了自噬相关蛋白中唯一的膜蛋白ATG9的蛋白量。由于EMC调控膜蛋白合成与膜蛋白跨膜区附近的电荷有关,该研究对ATG9跨膜区细胞质一侧的带电氨基酸进行突变,发现点突变导致ATG9的蛋白量显著减少,且这种蛋白降解与26s.白酶体途径相关。同时,该研究也发现MoEMC5通过影响几丁质合成酶Chs.和葡聚糖合成酶Fks.的合成来影响细胞壁完整性;通过调控镁离子转运蛋白Mnr2的合成影响对镁离子的敏感性。本研究在病原真菌中首次揭示了EMC直接调控膜蛋白合成和致病性的机制,为稻瘟菌的绿色防控提供了分子靶标和理论基础。

孤立性肺动脉瓣心内膜炎

本文总结了从1986年至1998年间共计27例孤立性肺动脉瓣心内膜炎病例，其中包括了我院5例，并将本组病例与1960年至1985年的另一组28例病例进行了比较。本组男女比例为2：1，前组则为3：1。肺动脉瓣心内膜炎最常见的致病菌本组为金黄色葡萄球菌，而前组则为肠球菌及草绿色莲球菌。本组血培养无一例为草绿色链球菌，而前组则占23％。本组22％感染性心内膜炎的发生与近年才应用于临床的肺动脉漂浮导管插入相关。12例中10例同时接受了经胸壁及经食道超声心动图检查的患者中，经食道超声心动图全部有阳性发现，而经胸壁超声心动图仅6例有阳性发现。40％的病例需要肺动脉瓣外科手术切除或清创术，部分病例同时做瓣膜置换术，对于毒力强致病菌、难以控制的充血性心力衰竭、持续菌血症及反复的肺动脉栓塞需外科手术干预，至少30％的病例需行手术，病死率为15％－20％。肺动脉瓣心内膜炎预后可能比三尖瓣心内膜炎差。
