Intravesical bacillus Calmette-Guerin(BCG)in treating non-muscle invasive bladder cancer—analysis of adverse effects and effectiveness of two strains of BCG(Danish 1331 and Moscow-I)

Objective:To compare the differences in adverse effects and efficacy profile between bacillus Calmette-Guerin(BCG)Danish 1331 and BCG Moscow-I strain in management of non-muscle invasive bladder ***:Clinical data of 188 cases of non-muscle invasive bladder cancer treated with BCG between January 2008 and December 2018 in our institute were collected prospectively and analysed retrospectively,and 114 patients who completed a minimum of 12 months of follow-up were *** and tumor characteristics,strain of BCG,adverse effects,and tumor progression were included for *** BCG was instilled in intermediate-and high-risk *** weeks of induction BCG,followed by three weekly maintenance BCG at 3,6,12,18,and 24 months was advised in high-risk ***:Overall 68 patients received BCG Danish 1331 strain and 46 patients received Moscow-I *** and tumor characteristics were well balanced between the two *** median follow-up period was 42.5 months and 34.5 months in Danish 1331 and Moscow-I groups,*** events like dropout rate,antitubercular treatment requirement,and need of cystectomy were higher in Moscow-I group(n=31,67.4%)when compared to Danish 1331 strain(n=33,48.5%)(p=0.046).On direct comparison between Danish 1331 and Moscow-I strain,there was similar 3-year recurrence-free survival(80.0%vs.72.9%)and 3-year progression-free survival(96.5%vs.97.8%).Conclusion:Study results suggest no significant differences between Danish 1331 and Moscow-I strain in recurrence-free survival and progression-free survival,but a significantly higher incidence of moderate to severe adverse events in BCG Moscow-I strain.

Parametric Influence on Thermal Performance of Flat Plate Closed Loop Pulsating Heat Pipes

This paper presents an experimental study on a flat plate closed loop pulsating heat pipes. It consisted of total 40 channels with square cross section （2 × 2 mm^2, 165 mm long） machined directly on an aluminum plate（180×120×3 nm^2）, which was covered by a transparent plate. The working fluid employed was ethanol. As the results, the influence parameters of thermal performance were investigated, such as filling ratio, heat load and operational orientations etc. Filling ratio was found to be a critical parameter, and its effect was rather complicated. According to its values the PHP plate could have four distinct working zones with different operational characteristics and heat transfer performance. The effect of heat load on thermal performance was found to be positive, and in general, iucrcasing the heat load would improve heat transfer performance. In order to analyze the effect of gravity on thermal performance, three different heat modes and total seven tilt angles were tested and compared. Successful operation at all orientations with respect to gravity was also achieved.

Outcomes of Ahmed valve surgery for refractory glaucoma in Dhahran, Saudi Arabia

AIM: To evaluate the outcomes of Ahmed glaucoma valve(AGV) implantation surgery for refractory ***: This one-armed historical cohort study was conducted in 2011. Refractory glaucoma was defined as eyes with an intraocular pressure(IOP) greater than21 mm Hg with maximally tolerated glaucoma medications, failed surgeries, or both. For all eyes with refractory glaucoma that underwent AGV implantation,data were collected on IOP, the best corrected visual acuity(BCVA) and glaucoma medications preoperatively and 4, 6, 12, 24 and 56 wk postoperatively. Logarithm values of IOP were calculated and compared. RESULTS: The study group was comprised of 30patients(30 eyes, 16 males and 14 females) with refractory glaucoma. Mean preoperative IOP was 39.3 ±13.8 mm Hg. Postoperative mean IOP was 15.7±7.1 mm Hg,19.6 ±12.8 mm Hg and 13.9 ±14.2 mm Hg at 12, 24 and56 wk respectively. BCVA was ≥6/60 in 11 eyes preoperatively, and five eyes had BCVA ≥6/60 at 56 wk postoperatively. Preoperatively, more than four medications were used to treat glaucoma in 21 eyes. At12 wk postoperatively, no medications were required to control IOP in 20 eyes. At 56 wk postoperatively, at least one medication was required to control IOP in 10 *** the entire follow up period, four eyes were treated with yttrium aluminium garnet(YAG) laser and 14 eyes required a second surgery. The AGV was removed in four ***: AGV implantation reduced IOP and the number of medications required to control refractory glaucoma. However, there was a higher risk of decreased vision. Long-term follow up and prompt intervention are recommended.

Optics is a potential candidate in information, data, and image processing. In all-optical data and information processing, optics has been used as information carrying signal because of its inherent advantages of parallelism. Several optical methods are proposed in support of the above processing. In many algebraic, arithmetic, and image processing schemes fundamental logic and memory operations are conducted exploring all-optical devices. In this communication we report an all-optical matrix multiplication operation with non-linear material based switching circuit.

Objective To synthesize some new a-mercapto-β-substituted aryl acrylic acids, characterize them and investigate their in vitro cadmium chelating ability. Methods Six α-mercapto-β-substituted aryl acrylic acids were prepared by the alkaline hydrolysis of 5- (aryl methylene) rhodanines, obtained from the condensation of substituted aldehydes and rhodanine following the reported procedure. The new compounds were characterized by elemental analysis, infrared (IR) and nuclear magnetic resonance (NMR) spectroscopy. The liver and kidney from cadmium chloride pre-administered rats were homogenized and their nuclear mitochondrial fraction (NMF) and supernatant cytosol fraction (SCF) were separated. A measured volume of each fraction was dialyzed separately using 'dialysis sack' against buffered-KCl medium containing a compound in the final concentration of 1×10-3 mol/L for 3 h at 37℃. The whole content of 'sack' was subjected to cadmium estimation following digestion with cone. Nitric acid was detected using flame atomic absorption spectrometer. Results The in vitro screening showed that α-mercapto-β-(p-methoxyphenyl) acrylic acid (compound 2) andα-mercapto-β-(m-methoxy, p-hydroxyphenyl) acrylic acid (compound 4) were more effective than α-mercapto-β-thienyl acrylic acid (compound 1) and a-mercapto-β-(p-dimethylaminophenyl) acrylic acid (compound 3) in mobilizing cadmium as their dialyzable chelates. The presence of a methoxy group on the phenyl moiety (compounds 2 and 4) increases the metal chelating ability of mercapto acrylic acids. Conclusions Compounds 2 and 4 seem to have accessibility to the cellular system and capability of chelating-out the intracellularly bound cadmium.

Short and long-term outcomes of angle supported phakic intraocular lens implantation in high myopic eyes

This cohort study included 36 eyes of 21 patients with high myopia treated with angle supported phakic intraocular lens（p IOL）.Endothelial cell density（ECD） at baseline,6 mo and 3 y were 3017±296,2775±265 and 2558±299 cells/mm^2 *** loss at 6 mo was 7.2% and annual ECD loss was 3% over 36 *** distance visual acuity at 36 mo was 0.4 log MAR or better in 32（88.9%） *** pressure did not change（P=0.9）.No eyes developed cataract,retinal detachment or pupillary *** supported p IOL gives good visual *** cell loss should be monitored.

Two-Phase Flow Modeling in a Single Closed Loop Pulsating Heat Pipes

Mathematical modeling of pulsating heat pipes through ‘first’ principles is a contemporary problem which remains quite elusive. Simplifications and assumptions made in all the modeling approaches developed so far render them unsuitable for engineering design. In this paper, a more realistic modeling scheme is presented which provides considerable try for thought toward the next progressive step. At high enough heat flux level, closed loop pulsating heat pipes experience a bulk internal unidirectional fluid circulation. Under such a condition, conventional two-phase flow modeling in capillary tubes may be applied. This has been attempted for single-loop PHPs. A homogeneous model and a separated two-fluid flow model based on simultaneous conservation of mass, momentum and energy, have been developed for an equivalent ‘open flow’ system. The model allows prediction of two-phase flow parameters in each sub-section of the device thereby providing important insights into its operation. The concept of ‘void fraction constraint’ in pulsating heat pipe operation is introduced and its relevance to future modeling attempts is outlined.

Normal Variants of the Lower Limbs in Pediatric Orthopedics

Normal variants of lower limb development such as in-toeing, out-toeing, flat feet, bow-legs and knock knees are a common cause for parental concern and also a common source of referral to pediatric clinics. A thorough history and clinical examination is usually all that is required to make the diagnosis of a normal variant. They also help to exclude pathological conditions that may present in a similar fashion. Radiological and other investigations are not routinely required in children with normal variants of the lower limb except to exclude pathological conditions. Shoe inserts, orthoses and physical therapy are not to be encouraged as they provide no benefit. In the majority of cases the natural history is one of spontaneous resolution of the deformity. Surgery may, however, be required in a small number of patients greater than eight years in whom severe in or out-toeing is present. Parental education and reassurance is an important part of the treatment and must be re-emphasized.
