Frequency of Emerging Parasites in HIV/AIDS and Oncological Patients Stool by Coprological and Molecular Analysis

The purpose of this study was to determine the frequency of emerging parasites in two groups of immunosuppressed patients, including individuals infected with the human immunodeficiency virus (AIDS) (HIV) or having acute lymphoblastic leukemia (ALL) with or without diarrhea. Stool samples were collected from 96 HIV and 77 ALL patients from March 2010 through December 2011. Screening for opportunistic parasites was carried out by the coproparasitoscopic Faust method, Ziehl Neelsen staining, and Polymerase Chain Reaction (PCR). Results showed that 22.9% of HIV fecal samples were positive for emerging parasites, including Cryptosporidium spp. (7.3%), Microsporidium spp. (5.2%), Isospora belli (1.0%), Giardia intestinalis (2.6%), and Cyclospora spp. (7.3%). On the other hand, 32.5% of ALL fecal samples were positive for emerging parasites, including Cryptosporidium spp. (9.1%), Microsporidium spp. (19.5%), Isospora belli (1.3%), and Giardia intestinalis (2.6%). Our results highlighted the need for specific, efficient, and reliable diagnostic methods to identify the presence of emerging parasites in immunocompromised patients susceptible to different infectious diseases or neoplastic processes and avoid the consequences for the host as an increased disease rate, alterations in the clinical manifestation of the infection or even exacerbation of its course.

Giardia duodenalis exhibits seven assemblages (A-G) that are distributed in different hosts. The A and B assemblages are commonly found in humans and several mammals, while C and D assemblages are typically found in dogs. The purpose of this study was to determine the assemblage of Giardia duodenalis present in the stool samples of ten canines using an assay based on PCR amplification, restriction analysis, and sequencing of the small subunit ribosomal DNA (SSU-rDNA), β-giardin, and triosephosphate isomerase (tpi) genes in order to establish the similarities or differences between the assemblage obtained with each gene. The results indicated that all positive isolates belonged to assemblage A, and specifically to the sub-assemblage A-I. A comparison of the SSU-rDNA gene sequence revealed the presence of three subgroups of assemblage A. These findings highlight the importance of canine transmission of Giardia in Mexico and its genetic plasticity. They also establish a method for additional and more molecularly extensive epidemiological studies to improve sanitation and hygiene in the most affected areas.

Sulfide Stress Cracking Assessment of Carbon Steel Welding with High Content of H2S and CO2at High Temperature: A Case Study

In the oil and gas industry, it has been established that for pipelines fabricated with carbon steels, their limitation is related to H2S and CO2 environments, which is 7 to 10 psia of partial pressure of CO2. Therefore, in carbon steel cracking is shown, after 7 or 10 psia of partial pressure of CO2. The experimental work was performed under static conditions in autoclaves within a pH of 3 to 3.8;partial pressures of 16 - 96 psi for H2S and 15 - 53 psi for CO2, in the temperature range of 25°C - 150°C. It was observed that the average yielding stress used in Sulfide Stress Cracking (SSC) tests decreases with temperature increment. Hydrogen Induced Cracking (HIC) evaluations showed that X52 steel, under conditions, was not susceptible to HIC. Results of SSC did not show indications of cracking after exposure to sour solutions, except for the specimen exposed to high H2S and CO2 content (96 psi of H2S and 53 psi of CO2 of the partial pressure) and high temperature (150°C). Microcracks located between the upper and lower weld beads were also observed. However, the highest average corrosion rate was 0.27 mm/year (10.6 mpy), which occurred in samples exposed to 96 psi of H2S and 53 psi of CO2 at 150°C. The dissimilar metal weld is demanding as well as the similar weld, however, dissimilar weld is more complex than similar weld due to the necessity of being applied in zones where a requirement is to improve some properties. In this work the main purpose is to know the mechanical behavior of a dissimilar weld between HSLA Steel and Superduplex Stainless Steel (SDSS) to establish if the joint is feasible or not. The alloys were welded with GTAW process using a 60-deg and 90-deg single-V groove test specimens in order to observe the effect of the weld pass. The filler metal was chosen with the aid of Schaeffler diagram. It was found that the ER 25.10.4L filler metal provided the best equilibrium between ferrite and austenite phase in the Superduplex Stainless Steel final microstructure and a band of martensite in the HSLA steel final microstructure. The dissimilar joint presented acceptable mechanical properties which are superior to the HSLA in the as-received condition, but lower than the SDSS in the as-received condition, proving that the filler metal was the adequate.

Red jumbo tilapia and M. americanum prawns were placed in 1000 L units in polyculture at three different densities of tilapia (4/5, 9/5 and 14/5 m2;tilapia/prawn) during 112 days with initial weight of? 6.5 ± 2.8 g for tilapia and 25.9 ± 8.5 g for prawn. Prawn (0/5 m2) and tilapia (4/0, 9/0 and 14/0 m2) controls were included. Tanks were maintained outdoors and under shade. Total weight gain (WG), daily individual weight gain (DWG), specific growth rate (SGR), survival, production of gross mass and food conversion ratios (FCR), ammonia, nitrite and nitrate, and chlorophyll a were measured in all units. Only prawn weight gain was significantly higher in polyculture. For tilapia, the highest survival at the end of the trial was 94% and the lowest was 75%, while prawns had final survival rates from 43% to 86%. No statistical differences were observed in weight increase among treatments at p ≤ 0.05.

Symptomatic changes in postmenopause with different methods of hormonal therapy

Objective: The diversity of opinions on the adverse effects of medications used to treat postmenopausal symptoms has prompted the use of various routes and mechanisms of action that need to be explored because bioavailability of the medications can vary. In order to select the appropriate route of administration for hormonal therapy (HT), it is necessary to determine baseline therapeutic efficacy. Design: We designed a prospective, randomized study consisting of four groups of postmenopausal wo-men: group 1 received oral conjugated estrogens, group 2 received a synthethic steroid, group 3 received estradiol nasally in spray form, and group 4 used transdermal estradiol in the form of patches. Criteria used to evaluate effectiveness was the Greene scale, which evaluate six components. These criteria were applied to each patient before hormonal intervention and then each month for 6 months. Luteinizing hormone (LH), follicle stimulating horone (FSH) and estradiol concentration were determined by chemiluminescence. Student’s t-test was used for intra-group comparisons before and after treatment. Results: There was a significant decrease in the vasomotor and sexual component (p < 0.05) with the use of four HT types. For depression, a difference was observed with synthetic steroids and oral estrogens. Upon analyzing the somatic component there was a decrease in symptoms with nasal and transdermal routes. Psychological changes were observed with the use of oral synthethic steroids and transdermal patches. Anxiety component demonstrated differences with nasal spray and oral estrogens, although all HT forms in this component showed a pattern of irregular changes. Conclusions: Changes in the response could be due each route of administration and medication used. Absorption variability may exist, which has repercussions in the control of symptoms and should be taken into consideration when selecting the appropriate route of administration for patients beginning HT.

Giant Spinal Schwannomas. Presentation of Two Cases with Interdisciplinary Surgical Resection

Purpose: To report two quite rare cases of giant schwannomas of the cervical and thoracic spine. Surgical resection is usually the treatment of choice for these neoplasms. Methods: The first patient is a 48-year-old male that came to our department, complaining about weakness and paresthesia of both lower extremities and upper left extremity, and a considerably big dorsal palpable lump. MRI and CT scan were used to diagnose giant cervical schwannoma. Because of the intensity of symptoms, he was subjected to surgery. Multidisciplinary resection involving neurosurgical and oncosurgicalteamn was performed for intravertebral and extravertebral approaches. The second patient is a 55-year-old female that came to the hospital complaining about incapacitating pain and dysesthesia involving T6, T7, and T8 dermatomes, with no other deficits. MRI and CT scan were used to diagnose giant thoracic schwannoma. Because of the intensity of the pain, surgery was decided as the treatment of choice. Video assisted thoracoscopic surgery was performed and total resection was achieved, with improvement of the symptoms. Results: Both patients showed complete remission of the symptoms, with no major deficits after surgery. Conclusion: Although infrequent, giant spinal schwannomas may be the cause of visible lumps in the back with radicular symptoms. When asymptomatic, conservative treatment could be viable, but when they present symptoms, surgery offers good results.
