Superior and Inferior Gastric Pouch Dilatation Post-Sleeve Surgery: Does Revision Work? A Systematic Review and Case Reports

Longitudinal sleeve gastrectomy (LSG) has become an acceptable option in the management of morbidly obese patients. Complications in the form of pouch dilatation may occur in post-sleeve surgery. However, revision sleeve gastrectomy procedures are effective in correcting such complications. The aim of this study was to systematically review all published cases that reported revision surgeries following the initial sleeve gastrectomy complicated by pouch dilatation. In addition, we are presenting two cases reported from our military/teaching hospital. A systematic literature search was conducted from English-language studies published from 2000 to 2014 from the following databases: PubMed, CINAHL, MEDLINE, EMBASE, Clinical evidence, Dara, Scopus, Web of Sciences, TRIP, Health Technology Database, Cochrane library, and Psych INFO. A total of 5340 articles were identified in the initial search and 5339 articles were excluded based on the exclusion criteria. Only one case study met the inclusion criteria for this systematic review, involving two patients. also, we are reporting two cases with revision surgery performed for pouch dilatation post-sleeve surgery from our teaching hospital.?There was only one study that reported revision surgery as a management of gastric pouch dilatation post-sleeve surgery. Authors believe that the incidence of revision surgery for the management of pouch dilatation post-sleeve surgery is higher but underreported. This assumption may be valuable, especially because some surgeons consider pouch dilatation post-sleeve surgery a complication that arises due to surgical technique rather than the procedure itself;therefore, it is unreported.

Software project outcomes heavily depend on natural language requirements,often causing diverse interpretations and issues like ambiguities and incomplete or faulty *** are exploring machine learning to predict software bugs,but a more precise and general approach is *** bug prediction is crucial for software evolution and user training,prompting an investigation into deep and ensemble learning ***,these studies are not generalized and efficient when extended to other ***,this paper proposed a hybrid approach combining multiple techniques to explore their effectiveness on bug identification *** methods involved feature selection,which is used to reduce the dimensionality and redundancy of features and select only the relevant ones;transfer learning is used to train and test the model on different datasets to analyze how much of the learning is passed to other datasets,and ensemble method is utilized to explore the increase in performance upon combining multiple classifiers in a *** National Aeronautics and Space Administration(NASA)and four Promise datasets are used in the study,showing an increase in the model’s performance by providing better Area Under the Receiver Operating Characteristic Curve(AUC-ROC)values when different classifiers were *** reveals that using an amalgam of techniques such as those used in this study,feature selection,transfer learning,and ensemble methods prove helpful in optimizing the software bug prediction models and providing high-performing,useful end mode.

Correlation between DKK-1 Level and Bone Density Status in Children on Maintenance Haemodialysis

Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and Methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DEXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BMD) with a Z-score of under -2.0. Eighteen of these patients had low BMD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BMD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-MBD.

One of the major causes of road accidents is sleepy *** accidents typically result in fatalities and financial losses and disadvantage other road *** studies have been conducted to identify the driver’s sleepiness and integrate it into a warning *** studies have examined how the mouth and eyelids ***,this limits the system’s ability to identify drowsiness ***,this study designed an Accident Detection Framework(RPK)that could be used to reduce road accidents due to sleepiness and detect the location of *** drowsiness detectionmodel used three facial parameters:Yawning,closed eyes(blinking),and an upright head *** model used a Convolutional Neural Network(CNN)consisting of two *** initial phase involves video processing and facial landmark coordinate *** second phase involves developing the extraction of frame-based features using normalization *** these phases used OpenCV and *** dataset contained 5017 images with 874 open eyes images,850 closed eyes images,723 open-mouth images,725 closed-mouth images,761 sleepy-head images,and 1084 non-sleepy head *** dataset of 5017 images was divided into the training set with 4505 images and the testing set with 512 images,with a ratio of 90:*** results showed that the RPK design could detect sleepiness by using deep learning techniques with high accuracy on all three parameters;namely 98%for eye blinking,96%for mouth yawning,and 97%for head ***,the test results have provided an overview of how the developed RPK prototype can accurately identify drowsy *** findings will have a significant impact on the improvement of road users’safety and mobility.
