Comparative analysis of different hepaticojejunostomy techniques for treating adult type I choledochal cyst

Objective:To compare Roux-en-Y hepatico-jejunostomy with complete resection of the cyst or incomplete resection with 1-cm remnant proximal cyst wall in treating adult type I choledochal cyst(CC).Methods:The medical records of 267 adult patients with type I CC from January 1998 to December 2015 were reviewed *** them,171 underwent Roux-en-Y hepatico-jejunostomy with complete resection(PBD 0-cm group)and 96 underwent Roux-en-Y hepatico-jejunostomy with 1-cm proximal cyst wall left(PBD 1-cm group).The short-and long-termpost-operative complications were compared between the two ***:No significant difference was observed in operative time or anastomotic diameter between the two *** incidence of perioperative complications was significantly higher in the PBD 1-cm group than that in the PBD 0-cm group(28.1%vs 14.0%,p¼0.005),especially post-operative cholangitis(7.3%vs 1.2%,p¼0.021).The incidence of long-term post-operative complications was not significantly different,including anastomotic stricture,reflux cholangitis,intra-hepatic bile duct stones and bile leak(all p>0.05).Post-operative intra-pancreatic biliary malignancy occurred in one patient in the PBD 0-cm group at 25 months and one patient in the PBD 1-cm group at 5 month,*** site malignancy was observed in one patient in the PBD 1-cm group at 10 ***:Ease of performing anastomosis does not justify retaining a segment of choledochal cyst in type I CC due to its higher risk of post-operative complication and malignancy.A complete excision of the CC with anastomosis to the healthy proximal bile duct is necessary in treatment of type I CC.

Transitioning patients with inflammatory bowel disease from hospital-based to rapid home-based infliximab: A stepwise, safety and patient-orientated process towards sustainability

Effects of epidermal growth factor receptor inhibitor on proliferative cholangitis in hepatolithiasis

BACKGROUND： There is currently no effective medication to prevent stone recurrence after choledochoscopic lithotomy or to treat proliferative cholangitis（PC）, which is the pathologic basis of hepatolithiasis. This study aimed to investigate whether gefitinib, an epidermal growth factor receptor（EGFR） inhibitor, inhibited cholangio hyperplasia and lithogenesis in ***： After cholangioscopic lithotomy, indwelling catheters were placed in the diseased bile duct lumens in 94 patients with hepatolithiasis. Subsequently, 49 of the 94 patients were treated with 250 mg gefitinib solution via a catheter twice a week, and they were subjected to choledochoscopic biopsy at 6 and 12 weeks. The rest 45 hepatolithiasis patients without gefitinib treatment served as ***： The expressions of EGFR, PCNA and procollagen I were significantly reduced in the patients treated with gefitinib in 12 weeks compared with those in the control group. Patients in the gefitinib group had a much lower degree of hyperplasia of the biliary epithelium, submucosal glands and collagen fibers compared with those in the control group. Gefitinib treatment significantly decreased mucin 3 expression and β-glucuronidase ***： Postoperative gefitinib treatment could significantly inhibit PC-mediated hyperplasia and lithogenesis, which might provide a novel strategy for the prevention of biliary restenosis and stone recurrence in patients with hepatolithiasis.

BACKGROUND： Unexpected gallbladder cancer may present with acute cholecystitis-like manifestat/ons. Some authors rec- ommended that frozen section analysis should be performed during laparoscopic cholecystectomy for all cases of acute cholecystitis. Others advocate selective use of frozen section analysis based on gross examination of the specimen by the surgeon. Hepatic epithelioid hemangioendothelioma: Dilemma and challenges in the preoperative diagnosis

Hepatic epithelioid hemangioendothelioma(HEHE) is a rare category of vascular tumor with uncertain malignant potential. It commonly presents nonspecific and variable clinical manifestations, ranging from asymptomatic to hepatic failure. In addition, laboratory measurements and imaging features also lack specificity in the diagnosis of HEHE. The aim of the present study is to highlight the dilemma and challenges in the preoperative diagnosis of HEHE, and to enhance awareness of the range of hepatobiliary surgery available in patients with multiple hepatic nodular lesions on imaging. In these patients, HEHE should at least be considered in the differential diagnosis.

Malunion in displaced intracapsular fracture of femoral neck： A rare case

Intracapsular fracture of femoral neck is treated by anatomical reduction （preferably closed） and can- nulated cancellous lag screw fixation. Malunion of these fractures have been described in the coronal plane （coxa valga or coxa rata）. We reported a case of young adult patient with displaced intracapsular fracture of femoral neck that had malunited in sagittal plane with callus formation with excellent functional outcome. The radiographs revealed intracapsular fracture of femoral neck right side （Garden type 4 and Pauwel type 3）. The patient was operated and closed reduction and internal fixation with three cannulated cancellous screws was performed. The postoperative radiograph revealed a loss of reduction in the lateral view. Due to this technical error, the patient was counselled for revision fixation for which he refused. At 9 months we observed union of the fracture in the displaced position by callus formation. Harris hip score at 2 years was 96 that indicate excellent functional outcome and the ra- diographs did not reveal any evidence of avascular necrosis of femoral head. We advised revision surgery to our patient as he had increased chances of implant failure and nonunion. However he refused the revision surgery and was continued with the suboptimal reduction. However, the fracture united and that too with callus formation, which is not a described phenomenon in neck of femur fracture.

Triple Wavelength and 810 nm Diode Lasers for Hair Removal: A Clinical and in Silico Comparative Study on Indian Skin

In laser hair removal treatments on dark skin, the high concentration of melanin in the skin competes with the melanin in the hair. During standard laser procedures, with wavelengths of 755 nm or 810 nm, a high level of laser light absorption by melanin in the skin is observed. Therefore, to avoid side effects, lower fluence values are used, which further reduces hair-removal efficacy. To improve results, 810 nm diode lasers operating in dynamic mode, with high frequency and multiple passes, are typically used. The aim of this study is to compare the efficacy and safety of triple-wavelength diode lasers (810 nm, 940 nm, 1060 nm) with that of 810 nm diode lasers on Indian patients. A side-by-side comparison was performed using a triple-wavelength diode laser in stamping mode on one side, and an 810 nm diode laser in dynamic mode on the other. Three subjects with skin type IV on the Fitzpatrick scale participated in the study. Efficacy was assessed through hair counting using clinical photographs, taken before and after the treatments, and the Global Aesthetic Improvement Scale (GAIS). Additionally, comparisons related to epidermal heating and thermal damage to the hair follicle were conducted through mathematical 3D simulations using COMSOL Multiphysics® software. Side effects were also evaluated. A superior end point was observed with triple wavelength compared to the 810 nm diode laser. Hair counting showed a 27% greater hair reduction with triple wavelength. No adverse effects were observed. Thermal simulations revealed 29% higher thermal damage with the triple-wavelength laser compared to the 810 nm diode laser. To conclude, on darker skin types, the triple-wavelength diode laser has been shown to be more effective at removing hair, compared to the 810 nm diode laser, while also being a safe procedure.

Theoretical Study of Specific Heat and Density of States of MgB<sub>2</sub>Superconductor in Two Band Model

MgB2 with Tc ≈ 40 K, is a record-breaking compound among the s-p metals and alloys. It appears that this material is a rare example of the two band electronic structures, which are weakly connected with each other. Experimental results clearly reveal that boron sub-lattice conduction band is mainly responsible for superconductivity in this simple compound. Experiments such as tunneling spectroscopy, specific heat measurements, and high resolution spectroscopy show that there are two superconducting gaps. Considering a canonical two band BCS Hamiltonian containing a Fermi Surface of π- and σ-bands and following Green’s function technique and equation of motion method, we have shown that MgB2 possess two superconducting gaps. It is also pointed out that the system admits a precursor phase of Cooper pair droplets that undergoes a phase locking transition at a critical temperature below the mean field solution. Study of specific heat and density of states is also presented. The agreement between theory and experimental results for specific heat is quite convincing. The paper is organized in five sections: Introduction, Model Hamiltonian, Physical properties, Numerical calculations, Discussion and conclusions.
