A comparison of APACHE II,BISAP,Ranson’s score and modified CTSI in predicting the severity of acute pancreatitis based on the 2012 revised Atlanta Classification

Objective:Our aim was to prospectively compare the Accuracy of Acute Physiology and Chronic Health Evaluation(APACHE)II,Bedside Index of Severity in Acute Pancreatitis(BISAP),Ranson’s score and modified Computed Tomography Severity Index(CTSI)in predicting the severity of acute pancreatitis based on Atlanta 2012 definitions in a tertiary care hospital in northern ***:Fifty patients with acute pancreatitis admitted to our hospital during the period of March 2015 to September 2016 were included in the *** II,BISAP and Ranson’s score were calculated for all the *** CTSI was also determined based on a pancreatic protocol contrast enhanced computerized tomography(CT).Optimal cut-offs for these scoring systems and the area under the curve(AUC)were evaluated based on the receiver operating characteristics(ROC)curve and these scoring systems were compared ***:Of the 50 cases,14 were graded as severe acute *** necrosis was present in 15 patients,while 14 developed persistent organ failure and 14 needed intensive care unit(ICU)*** AUC for modified CTSI was consistently the highest for predicting severe acute pancreatitis(0.919),pancreatic necrosis(0.993),organ failure(0.893)and ICU admission(0.993).APACHE II was the second most accurate in predicting severe acute pancreatitis(AUC 0.834)and organ failure(0.831).APACHE II had a high sensitivity for predicting pancreatic necrosis(93.33%),organ failure(92.86%)and ICU admission(92.31%),and also had a high negative predictive value for predicting pancreatic necrosis(96.15%),organ failure(96.15%)and ICU admission(95.83%).Conclusion:APACHE II is a useful prognostic scoring system for predicting the severity of acute pancreatitis and can be a crucial aid in determining the group of patients that have a high chance of need for tertiary care during the course of their illness and therefore need early resuscitation and prompt referral,especially in resource-lim

Machine learning(ML)is increasingly applied for medical image processing with appropriate learning *** applications include analyzing images of various organs,such as the brain,lung,eye,etc.,to identify specific flaws/diseases for *** primary concern of ML applications is the precise selection of flexible image features for pattern detection and region *** of the extracted image features are irrelevant and lead to an increase in computation ***,this article uses an analytical learning paradigm to design a Congruent Feature Selection Method to select the most relevant image *** process trains the learning paradigm using similarity and correlation-based features over different textural intensities and pixel *** similarity between the pixels over the various distribution patterns with high indexes is recommended for disease ***,the correlation based on intensity and distribution is analyzed to improve the feature selection ***,the more congruent pixels are sorted in the descending order of the selection,which identifies better regions than the ***,the learning paradigm is trained using intensity and region-based similarity to maximize the chances of ***,the probability of feature selection,regardless of the textures and medical image patterns,is *** process enhances the performance of ML applications for different medical image *** proposed method improves the accuracy,precision,and training rate by 13.19%,10.69%,and 11.06%,respectively,compared to other models for the selected *** mean error and selection time is also reduced by 12.56%and 13.56%,respectively,compared to the same models and dataset.

The 18 kDa translocator protein(TSPO)located on the outer mitochondrial membrane regulates several key cellular processes including mitochondrial homeostasis,cholesterol transport,apoptosis,cell proliferation,and maintenance of mitochondrial health(Rupprecht et al.,2022,2023).TSPO is expressed in both peripheral organs and the central nervous system,with a more pronounced expression in tissues that produce *** main reason why TSPO has garnered so much attention is because it plays a key role in neurosteroidogenesis by transferring cholesterol from the outer to the inner mitochondrial membrane,which is the rate-limiting step in neurosteroid synthesis.A cholesterol-recognizing amino acid consensus domain has been identified in the cytosolic C terminus of the TSPO protein by both in vitro and site-directed mutagenesis experiments(Li et al.,2001).However,the role of TSPO in the process of neurosteroid synthesis has been challenged by several studies,particularly TSPO knockout models,which suggest that TSPO removal does not affect the phenotype or the system’s viability(Tu et al.,2014).However,ligands targeting TSPO have been shown to enhance levels of neurosteroids which suggests that neurosteroidogenesis is one of the major functional roles mediated by the TSPO protein.

Dynamically reconfigurable Field Programmable Gate Array(dr-FPGA) based electronic systems on board mission-critical systems are highly susceptible to radiation induced hazards that may lead to faults in the logic or in the configuration memory. The aim of our research is to characterize self-test and repair processes in Fault Tolerant(FT) dr-FPGA systems in the presence of environmental faults and explore their interrelationships. We develop a Continuous Time Markov Chain(CTMC) model that captures the high level fail-repair processes on a dr-FPGA with periodic online Built-In Self-Test(BIST) and scrubbing to detect and repair faults with minimum latency. Simulation results reveal that given an average fault interval of 36 s, an optimum self-test interval of 48.3 s drives the system to spend 13% of its time in self-tests, remain in safe working states for 76% of its time and face risky fault-prone states for only 7% of its time. Further, we demonstrate that a well-tuned repair strategy boosts overall system availability, minimizes the occurrence of unsafe states, and accommodates a larger range of fault rates within which the system availability remains stable within 10% of its maximum level.
